3. Stationary subspace analysis based on second-order statistics

In stationary subspace analysis (SSA) one assumes that the observable p-variate time series is a linear mixture of a k-variate nonstationary time series and a (p-k)-variate stationary time series. The aim is then to estimate the unmixing matrix which transforms the observed multivariate time series onto stationary and nonstationary components. In the classical approach multivariate data are projected onto stationary and nonstationary subspaces by minimizing a Kullback-Leibler divergence between Gaussian distributions, and the method only detects nonstationarities in the first two moments. In this paper we consider SSA in a more general multivariate time series setting and propose SSA methods which are able to detect nonstationarities in mean, variance and autocorrelation, or in all of them. Simulation studies illustrate the performances of proposed methods, and it is shown that especially the method that detects all three types of nonstationarities performs well in various time series settings. The paper is concluded with an illustrative example.

Abstract The resonant single active bridge topology (R‐SAB) operated in the half‐cycle discontinuous current mode (HC‐DCM) is a very attractive solution due to its high efficiency, low complexity and fixed voltage transfer ratio (DCX). However, as expected for a series‐resonant converter (SRC), its DCX operation depends on the resonant tank circuit‐parameters, parasitic capacitive elements and output load. Specially, at light and no‐load operation, when the system is extremely underdamped, it may present a large output overvoltage due to resonance interactions. This is of prime importance for converters using medium voltage (MV) SiC‐MOSFETs, which feature significant output capacitances that can lead to voltage breakdown of the rectifier semiconductors. Therefore, the supposed fixed‐voltage transfer ratio is not entirely valid and deserves a proper understanding due its criticality. This paper reviews the subject, clarifying its root cause and its multifactorial dependencies. Moreover, it provides a simple solution based on a variable dead‐time with fixed magnetizing current experimentally verified with a 300kW/1.8kV R‐SAB prototype implemented with MV SiC‐devices.

5. Quality of reporting of adverse events in clinical trials of covid-19 drugs: systematic review

Objective To assess the quality of reporting of adverse events in clinical trials of covid-19 drugs based on the CONSORT (Consolidated Standards of Reporting Trials) harms extension and according to clinical trial design, and to examine reporting of serious adverse events in drug trials published on PubMed versus clinical trial summaries on ClinicalTrials.gov.Design Systematic review.Data sources PubMed and ClinicalTrials.gov registries were searched from 1 December 2019 to 17 February 2022.Eligibility criteria for selecting studies Randomised clinical trials evaluating the efficacy and safety of drugs used to treat covid-19 disease in participants of all ages with suspected, probable, or confirmed SARS-CoV-2 infection were included. Clinical trials were screened on title, abstract, and text by two authors independently. Only articles published in French and English were selected. The Cochrane risk of bias tool for randomised trials (RoB 2) was used to assess risk of bias.Results The search strategy identified 1962 randomised clinical trials assessing the efficacy and safety of drugs used to treat covid-19, published in the PubMed database; 1906 articles were excluded after screening and 56 clinical trials were included in the review. Among the 56 clinical trials, no study had a high score for quality of reporting of adverse events, 60.7% had a moderate score, 33.9% had a low score, and 5.4% had a very low score. All clinical trials with a very low score for quality of reporting of adverse events were randomised open label trials. For reporting of serious adverse events, journal articles published on PubMed under-reported 51% of serious adverse events compared with clinical trial summaries published on ClinicalTrials.gov.Conclusions In one in three published clinical trials on covid-19 drugs, the quality of reporting of adverse events was low or very low. Differences were found in the number of serious adverse events reported in journal articles versus clinical trial summaries. During the covid-19 pandemic, risk assessment of drugs in clinical trials of covid-19 drugs did not comply with good practice recommendations for publication of results.Systematic review registration European Network of Centres for Pharmacoepidemiology and Pharmacovigilance (ENCePP) EUPAS45959.

Abstract Background The V Brazilian Consensus for determination of autoantibodies against cellular constituents on HEp-2 cells, held in Brasilia (DF, Brazil) on August 27, 2016, discussed the harmonization between the Brazilian Consensus on ANA (BCA) guidelines and the International Consensus on ANA Patterns (ICAP) recommendations (www.anapatterns.org). Initial guidelines were formulated by the group of Brazilian experts with the purpose of guiding and enabling Brazilian clinical laboratories to adopt recommendations and to provide a common standard for national and international consensuses. Mainbody Twenty Brazilian researchers and experts from universities and clinical laboratories representing the various geographical regions of the country participated in the meeting. Three main topics were discussed, namely the harmonization between the BCA guidelines and latest recommendations of the ICAP initiative, the adjustment of the terminology and report on HEp-2 patterns, and a reassessment of quality assurance parameters. For the three topics, our aim was to establish specific guidelines. All recommendations were based on consensus among participants. There was concrete progress in the adjustment of the BCA guidelines to match the ICAP guidelines. To a certain extent, this derives from the fact that ICAP recommendations were largely based on the algorithm and recommendations of the IV Brazilian ANA Consensus, as consistently recognized in the ICAP publications and presentations. However, although there is great overlap between the two Consensuses, there are some point divergences. These specific items were individually and extensively discussed, and it was acknowledged that in several points ICAP improved recommendations previously issued by the Brazilian ANA Consensus and these changes were readily implemented. Regarding some specific topics, the BCA panel of experts felt that the previously issued recommendations remained relevant and possibly will require further discussion with ICAP. The term anti-cell antibodies was adopted as the recommended designation, recognizing that the assay addresses antibodies against antigens in the nucleus and in other cell compartments. However, the acronym ANA HEp-2 was maintained due to historical and regulatory reasons. It was also signalized that the latest trend in ICAP is to adopt the term Indirect Immunofluorescent Assay on HEp-2 cell substrate (HEp-2 IIFA). In addition, the quality assurance strategies previously presented were ratified and emphasized. Conclusion The V BCA edition was successful in establishing an overall harmonization with the ICAP recommendations for interpretation of the HEp-2 IIFA test, pinpointing the perspectives in filling the remaining gaps between both initiatives.

O modelo de cuidado da Estratégia de Saúde da Família pautado no papel do Agente Comunitário de Saúde (ACS), desafia refletir sobre as condições de trabalho e a saúde deste trabalhador. Destacam-se as diversas fontes de desgaste e sofrimento no trabalho e a insuficiência de métodos de apoio a esse trabalhador no seu cotidiano. Considerando esta fragilidade, investigamos o trabalho de 8 agentes comunitários de saúde a partir da Técnica de Grupo Focal, buscando conhecer e analisar as fontes e tipos de sobrecargas provenientes da atividade laboral, as condições de trabalho, o vínculo empregatício, a formação e o papel chave deste trabalhador na Atenção Básica. O corpus de análise consistiu da transcrição de seis sessões de grupo focal. Como produto destacamos sete núcleos de sentidos organizados a partir de quatro eixos temáticos: Vitaminas (motivações para exercer seu papel profissional), Espinhos (dificuldades para exercer o trabalho), Frutos (resultados observados e valorizados decorrentes do trabalho) e Ferramentas (instrumentais utilizados). Destacaram-se a relevância da estabilidade; o maior reconhecimento do uso de tecnologias leves (vínculo afetivo) do que o êxito técnico do trabalho, que está vinculado a uma ação secundária do ACS originada no procedimento médico; o duplo vínculo com a comunidade (morador e profissional de referência) tensiona a identidade do ACS. A identificação das fontes de sofrimento possibilitou problematizar localmente os efeitos para a saúde mental do trabalhador e para a qualidade do trabalho realizado. Ratificamos as potencialidades do Grupo Focal como estratégia de investigação e intervenção na atenção básica.

10. Decentralized Phase Shedding with Low Power Mode for Multiphase Converter

With a multiphase converter, the phase-shedding function dedicated to maximizing the power efficiency, in a manner that is dependent on the load current, is always provided by a centralized controller that induces a Single Point of Failure (SPOF). The objective of this study is to obtain a decentralized control approach to implement this function by removing any SPOF. The method consists of using identical local controllers, each associated with a converter phase, that communicate with each other in a daisy-chain structure. Instead of measuring the global output current to determine the optimal number of active phases required, each local controller measures its own leg current and takes a local decision based on threshold crossing management and inter-controller communications. Functional simulations are carried out on a 5-leg 12 V/1.2 V 60 W multiphase converter supplying a modern microcontroller. They demonstrate that the number of active phases is well adjusted, in a dynamic manner, depending on the load current level. Specific events such as load current inrush or the start-up sequence are analyzed to guarantee optimal transient responses. A maximum power efficiency tracking ability is also demonstrated. Finally, it is shown that this control strategy allows phase shedding to be implemented using as many phases as desired, in a modular manner, thereby avoiding any centralized processing.

- Abstract
Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. Abstract Background Lung cancer is the most frequently diagnosed cancer and the leading cause of cancer-related deaths worldwide. Up to 80% of cancer patients are classified as non-small-cell lung cancer (NSCLC) and cisplatin remains as the gold standard chemotherapy treatment, despite its limited efficacy due to both intrinsic and acquired resistance. The CK2 is a Ser/Thr kinase overexpressed in various types of cancer, including lung cancer. CIGB-300 is an antitumor peptide with a novel mechanism of action, since it binds to CK2 substrates thus preventing the enzyme activity. The aim of this work was to analyze the effects of CIGB-300 treatment targeting CK2-dependent signaling pathways in NSCLC cell lines and whether it may help improve current chemotherapy treatment. Methods The human NSCLC cell lines NCI-H125 and NIH-A549 were used. Tumor spheroids were obtained through the hanging-drop method. A cisplatin resistant A549 cell line was obtained by chronic administration of cisplatin. Cell viability, apoptosis, immunoblotting, immunofluorescence and luciferase reporter assays were used to assess CIGB-300 effects. A luminescent assay was used to monitor proteasome activity. Results We demonstrated that CIGB-300 induces an anti-proliferative response both in monolayer- and three-dimensional NSCLC models, presenting rapid and complete peptide uptake. This effect was accompanied by the inhibition of the CK2-dependent canonical NF-κB pathway, evidenced by reduced RelA/p65 nuclear levels and NF-κB protein targets modulation in both lung cancer cell lines, as well as conditionally reduced NF-κB transcriptional activity. In addition, NF-κB modulation was associated with enhanced proteasome activity, possibly through its α7/C8 subunit. Neither the peptide nor a classical CK2 inhibitor affected cytoplasmic β-CATENIN basal levels. Given that NF-κB activation has been linked to cisplatin-induced resistance, we explored whether CIGB-300 could bring additional therapeutic benefits to the standard cisplatin treatment. We established a resistant cell line that showed higher p65 nuclear levels after cisplatin treatment as compared with the parental cell line. Remarkably, the cisplatin-resistant cell line became more sensitive to CIGB-300 treatment. Conclusions Our data provide new insights into CIGB-300 mechanism of action and suggest clinical potential on current NSCLC therapy.

14. Transfer of Visual Learning Between a Virtual and a Real Environment in Honey Bees: The Role of Active Vision

To study visual learning in honey bees, we developed a virtual reality (VR) system in which the movements of a tethered bee walking stationary on a spherical treadmill update the visual panorama presented in front of it (closed-loop conditions), thus creating an experience of immersion within a virtual environment. In parallel, we developed a small Y-maze with interchangeable end-boxes, which allowed replacing repeatedly a freely walking bee into the starting point of the maze for repeated decision recording. Using conditioning and transfer experiments between the VR setup and the Y-maze, we studied the extent to which movement freedom and active vision are crucial for learning a simple color discrimination. Approximately 57% of the bees learned the visual discrimination in both conditions. Transfer from VR to the maze improved significantly the bees’ performances: 75% of bees having chosen the CS+ continued doing so and 100% of bees having chosen the CS− reverted their choice in favor of the CS+. In contrast, no improvement was seen for these two groups of bees during the reciprocal transfer from the Y-maze to VR. In this case, bees exhibited inconsistent choices in the VR setup. The asymmetric transfer between contexts indicates that the information learned in each environment may be different despite the similar learning success. Moreover, it shows that reducing the possibility of active vision and movement freedom in the passage from the maze to the VR impairs the expression of visual learning while increasing them in the reciprocal transfer improves it. Our results underline the active nature of visual processing in bees and allow discussing the developments required for immersive VR experiences in insects.

Language production has often been described as impaired in psychiatric diseases such as in psychosis. Nevertheless, little is known about the characteristics of linguistic difficulties and their relation with other cognitive domains in patients with a first episode of psychosis (FEP), either affective or non-affective. To deepen our comprehension of linguistic profile in FEP, 133 patients with FEP (95 non-affective, FEP-NA; 38 affective, FEP-A) and 133 healthy controls (HC) were assessed with a narrative discourse task. Speech samples were systematically analyzed with a well-established multilevel procedure investigating both micro- (lexicon, morphology, syntax) and macro-linguistic (discourse coherence, pragmatics) levels of linguistic processing. Executive functioning and IQ were also evaluated. Both linguistic and neuropsychological measures were secondarily implemented with a machine learning approach in order to explore their predictive accuracy in classifying participants as FEP or HC. Compared to HC, FEP patients showed language production difficulty at both micro- and macro-linguistic levels. As for the former, FEP produced shorter and simpler sentences and fewer words per minute, along with a reduced number of lexical fillers, compared to HC. At the macro-linguistic level, FEP performance was impaired in local coherence, which was paired with a higher percentage of utterances with semantic errors. Linguistic measures were not correlated with any neuropsychological variables. No significant differences emerged between FEP-NA and FEP-A (p≥0.02, after Bonferroni correction). Machine learning analysis showed an accuracy of group prediction of 76.36% using language features only, with semantic variables being the most impactful. Such a percentage was enhanced when paired with clinical and neuropsychological variables. Results confirm the presence of language production deficits already at the first episode of the illness, being such impairment not related to other cognitive domains. The high accuracy obtained by the linguistic set of features in classifying groups support the use of machine learning methods in neuroscience investigations.

This paper presents a numerical approach for the analysis of hydrodynamic radial journal bearings. The effect of shaft and housing elastic deformation on pressure distribution within oil film is investigated. An iterative algorithm that couples Reynolds equation with a plane finite elements structural model is solved. Temperature and pressure effects on viscosity are also included with the Vogel-Barus model. The deformed lubrication gap and the overall stress state were calculated. Numerical results are presented with reference to a typical journal bearing configuration at two different inlet oil temperatures. Obtained results show the great influence of elastic deformation of bearing components on oil pressure distribution, compared with results for ideally rigid components obtained by Raimondi and Boyd solution.

OBJETIVO: Realizar avaliação audiológica em crianças e adolescentes acompanhados em um ambulatório de referência para doenças autoimunes, independentemente do diagnóstico específico. MÉTODOS: Foi realizado um estudo cego simples do tipo caso-controle. Foram incluídos 48 pacientes com idades entre 5 e 19 anos e tempo de seguimento de um a 151 meses, divididos em três grupos: 15 pacientes controle com diagnóstico de dor em membros e exclusão de doença autoimune, 23 pacientes com Artrite Idiopática Juvenil, e dez pacientes com outras doenças autoimunes. Os voluntários foram submetidos a avaliações clínica, otológica e audiológica (timpanometria, pesquisa dos reflexos acústicos, audiometria, índice de reconhecimento de fala, emissões otoacústicas e potenciais evocados auditivos de tronco encefálico com estímulo click). RESULTADOS: O grupo com outras doenças autoimunes teve maior número de pacientes sintomáticos e maior número de orelhas alteradas no teste de emissões otoacústicas em comparação com o grupo controle e com o grupo com Artrite Idiopática Juvenil. Ainda no grupo com outras doenças autoimunes, 50% dos sujeitos assintomáticos apresentaram alterações na pesquisa dos reflexos, na audiometria e nas emissões otoacústicas. Na audiometria, o grupo com Artrite Idiopática Juvenil apresentou mais alterações nas frequências altas, e o grupo com outras doenças autoimunes, nas frequências baixas. CONCLUSÃO: Houve maior número de sintomas relacionados à perda auditiva e a alterações audiológicas em crianças e adolescentes com Artrite Idiopática Juvenil e outras doenças autoimunes. As alterações auditivas ocorreram também em pacientes assintomáticos, justificando-se a avaliação audiológica como rotina clínica desses pacientes.

Background Multi-element interventions for first-episode psychosis (FEP) are promising, but have mostly been conducted in non-epidemiologically representative samples, thereby raising the risk of underestimating the complexities involved in treating FEP in ‘real-world’ services. Methods/Design The Psychosis early Intervention and Assessment of Needs and Outcome (PIANO) trial is part of a larger research program (Genetics, Endophenotypes and Treatment: Understanding early Psychosis - GET UP) which aims to compare, at 9 months, the effectiveness of a multi-component psychosocial intervention versus treatment as usual (TAU) in a large epidemiologically based cohort of patients with FEP and their family members recruited from all public community mental health centers (CMHCs) located in two entire regions of Italy (Veneto and Emilia Romagna), and in the cities of Florence, Milan and Bolzano. The GET UP PIANO trial has a pragmatic cluster randomized controlled design. The randomized units (clusters) are the CMHCs, and the units of observation are the centers’ patients and their family members. Patients in the experimental group will receive TAU plus: 1) cognitive behavioral therapy sessions, 2) psycho-educational sessions for family members, and 3) case management. Patient enrolment will take place over a 1-year period. Several psychopathological, psychological, functioning, and service use variables will be assessed at baseline and follow-up. The primary outcomes are: 1) change from baseline to follow-up in positive and negative symptoms’ severity and subjective appraisal; 2) relapse occurrences between baseline and follow-up, that is, episodes resulting in admission and/or any case-note records of re-emergence of positive psychotic symptoms. The expected number of recruited patients is about 400, and that of relatives about 300. Owing to the implementation of the intervention at the CMHC level, the blinding of patients, clinicians, and raters is not possible, but every effort will be made to preserve the independency of the raters. We expect that this study will generate evidence on the best treatments for FEP, and will identify barriers that may hinder its feasibility in ‘real-world’ clinical settings, patient/family conditions that may render this intervention ineffective or inappropriate, and clinical, psychological, environmental, and service organization predictors of treatment effectiveness, compliance, and service satisfaction. Le présent article étudie le travail de composition effectué par Giovanni Verga dans l'élaboration de son dernier recueil de nouvelles, le Don Candeloro e C.i.(1894). La confrontation des étapes de publications des textes, l'une sur périodique, l'autre dans l'édition princeps, et le recours aux traditions théoriques italiennes, anglo-saxonnes et francophones étudiant le modèle du recueil permettent d'analyser et d'identifier une dynamique nouvelle à l'œuvre entre les récits brefs lorsqu'ils sont réunis dans un volume unique. Les résultats de cette analyse rendent alors possible, d'une part, d'interroger de manière plus large les spécificités de la forme « recueil » et, d'autre part, d'identifier la nouveauté et l'importance poétique du Don Candeloro e C.i.dans la production de l'auteur sicilien.

24. PP10 Évaluation d’un programme d’activité physique au sein d’une structure d’hospitalisation de semaine de diabétologie.

- Abstract
Introduction Pour promouvoir l’activité physique dans la prise en charge du diabète de type 2, le service de diabétologie du groupe hospitalier Paris Saint-Joseph a mis en place deux ateliers réalisés chaque semaine dans l’unité d’hospitalisation de semaine. Une évaluation a 3 mois des modifications comportementales des patients est réalisée a l’aide questionnaire. Patients et méthodes Sont proposés chaque semaine – une réunion de groupe le mardi après-midi, atelier théorique avec animation à l’aide d’un jeu de cartes créé dans le service, qui sensibilise sur les freins concernant l’activité – le prêt d’un podomètre du mardi au vendredi à tous les volontaires, qui mesure le nombre de pas réalisés chaque jour et les incite à marcher. Le nombre de pas quotidien est noté sur les feuilles de glycémie, comme les constantes à surveiller – après sélection des patients sans contre-indication, une activité marche le mercredi pendant 30 à 40 minutes avec contrôle des glycémies capillaires avant et après. 35 patients diabétiques de type 2 volontaires consécutifs ont été colligés sur une période de 1 mois et ont répondu à un questionnaire lors de l’hospitalisation et 3 mois après (sondage téléphonique). Le score IPAQ ( International Physical Activity Questionnaire ) a été utilisé pour ces recueils qui identifient 3 niveaux d’activité: bas, moyen élevé. Résultats Sur les 35 patients, l’activité initiale est basse pour 14 patients, modérée pour 13 et élevée pour 8 selon le score. Après 3 mois les chiffres s’élèvent respectivement à 9, 13 et 13 montrant une amélioration globale du niveau d’activité. Conclusion La réalisation pratique d’ateliers d’activité physique est possible dans la plupart de nos environnements hospitaliers. L’impact de cette activité peut être facilement évalué à l’aide de scores validés tels que le score IPAQ. Dans cette enquête déclarative, les résultats attestent de la progression du score, d’une amélioration des performances de marche. Dans le cadre d’un programme d’éducation thérapeutique, cette évaluation permet de prouver la pertinence de cette action au moins a court terme. L’évaluation à plus long terme reste à faire. Objectif L’effet du Ramadan sur l’équilibre glycémique des patients diabétiques de type 2 reste incertain néanmoins l’utilisation récente du CGMS (Glucose Continuous Monitoring System) semble montrer une variabilité glycémique accrue. Nous avons cherché à analyser avec le CGMS l’impact de l’éducation thérapeutique sur cette variabilité. Patients et méthodes Cette étude pilote randomisée a inclus des diabétiques de type 2 non insulino-traités, ayant décidé de jeûner pendant le Ramadan 2014, répartis en 2 groupes. Une séance d’éducation spécifique au Ramadan avec un médecin, une infirmière d’éducation et une diététicienne a été réalisée la semaine précédant le Ramadan pour les patients du groupe interventionnel (A), ceux du groupe contrôle (B) n’ayant reçu que les conseils de leur diabétologue habituel. Aucun traitement médicamenteux n’a été modifié. Un CGMS sur 5 jours a été réalisé pour chacun des patients pendant le jeûne avec le système Freestyle Navigator 2. Résultats Sur 22 patients randomisés, 16 (7 du groupe A et 9 du groupe B) ont réalisé un enregistrement analysable. Les HbA1c étaient de 6,8 % vs 7,3 % ( p = 0,244). Les patients étaient traités par Metformine (6/7 vs 9/9 patients, p = 0,896), sulfamides hypoglycémiants (4/7 vs 6/9, p = 0,696) ou inhibiteurs de la DPP4 (3/7 vs 2/9, p = 0,377) respectivement dans les groupes A et B. La glycémie moyenne différait peu entre les deux groupes (112 vs 117 mg/dl, p = 0,791). Toutefois la variabilité glycémique semblait moins importante dans le groupe A que dans le B: l’écart type des glycémies était de 23 vs 33 mg/dl ( p = 0,204), leur amplitude sur le nycthémère de 149 vs 168 mg/dl ( p = 0,244) et l’augmentation de la glycémie à la rupture de jeûne de 105 vs 162 mg/dl ( p = 0,057) respectivement. Conclusion Malgré le faible effectif, ces tendances laissent supposer un impact bénéfique de l’éducation thérapeutique sur la variabilité glycémique pendant le Ramadan.
