27. Bacteria-macrophage interactions in the pathogenesis of Crohn's disease

Abstract

Crohn's disease (CD) is associated with defective innate immunity, including impaired neutrophil chemotaxis, and mucosal invasion by bacteria, particularly E. coli that replicate inside macrophage phagolysosomes. In this thesis several hypotheses were tested: (i) that CD macrophages might be defective at killing and or responding to Gram-negative bacteria, particularly E. coli; (ii) that killing of phagocytosed bacteria within macrophages might be enhanced by drugs such as hydroxychloroquine (HCQ) (previously shown to raise intravacuolar pH), and vitamin D (previously shown to enhance macrophage function). I assessed CD peripheral blood monocyte-derived macrophages (MDM) for their abilities to kill E. coli, with and without HCQ and vitamin D, and to generate neutrophil chemoattractants. MDM from patients with CD were similar to those from healthy controls (HC) in allowing replication of phagocytosed CD-derived E. coli: HM605 [CD N=10, mean fold replication in 3h: 1.08, (95% confidence interval (CI) 0.39-1.78); HC N=9, 1.50, (95%CI 1.02-1.97); P=0.15] and also in generation of neutrophil chemoattractants in response to E. coli (mean fold chemotaxis relative to control: CD 2.55, 95%CI 2.31-2.80; HC 2.65, 95%CI 2.46-2.85, P=0.42). The only possible exception was reduced bacterial killing in macrophages from the relatively rare patients homozygous for ATG16L1 polymorphisms, reported previously and confirmed in the single example in this series. HCQ and 1,25 OH2-vitamin D3 both caused dose-dependent inhibition of intra-macrophage E .coli replication 3h post-infection, HCQ: 73.9% inhibition (P < 0.001) at 1μg/mL, accompanied by raised intra-phagosomal pH, and 1,25OH2-Vitamin D3: 80.7% inhibition (P < 0.05) at 80nM. HCQ had synergistic effects with doxycycline and ciprofloxacin on killing of phagocytosed E. coli. Thus: CD and HC macrophages generally perform similarly in allowing replication of phagocytosed E. coli and generating neutrophil chemoattractants. Replication of phagocytosed E. coli was substantially decreased by HCQ and vitamin D. These warrant further therapeutic trials in CD in combination with relevant antibiotics.

33. A cross-linguistic investigation of the order of attributive adjectives

Abstract

This thesis examines the order in which attributive adjectives are placed when appearing in a string modifying the same head noun. Noun phrases featuring more than one adjective are examined in six languages, all of which have modification patterns which exhibit distinctive patterns of syntax and morphology. Northern Sotho is a Bantu language with postnominal adjectives, agglutinative morphology and qualificative particles which link modifier and head; Welsh also has predominantly postnominal adjectives but less complex adjectival morphology. Polish and English adjectives typically appear before the noun, and the order in which they are sequenced is compared with Chinese, in which all modification appears before the noun, including relative clauses. I also examine the syntax of adjective strings in Tagalog, an Austronesian language in which adjectives can appear both before and after the noun, and in which the nature of lexical categories is particularly complex. The universality of the adjective class has generated considerable debate among linguists, with much discussion in the last decade with regard to whether adjectives constitute a independent lexical category across all languages. Chinese, Tagalog and Northern Sotho are all languages in which the nature of the adjectival category has been questioned, and this comparative analysis of a syntactic phenomenon which is an essential characteristic of adjectives adds a new dimension to the debate surrounding the universality of the adjective class. Based on a combination of corpus data and field-based methods, I analyse the patterns which appear across the languages in my sample. I evaluate the various explanations of the different factors which affect the order in which English adjectives are placed ahead of a noun, and relate my findings to equivalent structures in each of my focus languages, before proposing some conventions which appear to be consistent across a representative sample of languages.

38. Unpacking Ideas of Sexuality in Childhood: What Do Primary Teachers and Parents Say?

Abstract

Children who engage in perceived sexual actions face possible marginalization, isolation and exclusion in schools. The author's counselling practice included numerous examples where effects of adults' understanding have led to over reactive and punitive responses on children. This article complements a political ethic of social justice and supporting children's agency--that is, children as actors and childhood as being and becoming. Teachers and parents of primary school children were interviewed as part of a current doctoral project on discourses of childhood sexuality in Aotearoa, New Zealand. In focus groups and individual interviews, six teachers and seven parents of children in one primary school responded to vignettes on children's actions designed from counselling and anecdotal evidence of children's experiences in New Zealand primary schools. Participants' thoughts, ideas and reflections, including personal stories, were stimulated by the vignettes. Their understandings and perceptions of sexuality in childhood are explored, and discursive positionings for children in both the vignettes and participants' responses are examined. The crisis for children is when school policy constrains understandings and experiences of childhood due to adult-centric constructions of sexuality without regard to multiple positions shaped by culture, community values, personal histories and ideas of childhood and sexuality.

41. Ethical Review and Reflexivity in Research of Children's Sexuality

Abstract

Research in the area of children's sexuality is largely based on observational and retrospective studies. Childhood studies literature increasingly calls upon the inclusion of children's voices, yet with sensitive topics ethical positions often close research possibilities in the territories of children's worlds. Children are perceived as a vulnerable group, especially when the investigation focuses on their sexual development and activity--and it is perceived that this research area is too sensitive and potentially harmful to children. Within the context of beginning a qualitative study on children's sexuality in New Zealand (including interviews with children), this paper reviews a number of studies of childhood research. These studies provide a glimpse at how research focusing on children has been conducted, and explores ethical issues arising in such research. The significance of researcher reflexivity is acknowledged for ethical research practice. The paper concludes that in research on children's sexuality a process of ethical review is limited, and that researcher competence in sensitive investigations is required. Among other difficulties for this researcher (with a professional background in child and family therapy) is the vulnerability of being a man choosing to research children. (Contains 1 note.)
