The main goal of this study was to compare and describe the non-COVID-19 patient’s disease severity presentation during the pandemic with its pre-pandemic severity.Methods: We conducted a retrospective observational study. We selected two samples of visits: one during the first COVID-19 wave of 2020 (pandemic period, PP) and the other during the same months of 2019 (control period, CP). The primary endpoints were the comparison of severity and distribution of the Emergency Severity Index (ESI). Secondary endpoints were comparisons of specific patient characteristics (age, sex, length of the symptoms before the visits, spontaneous visits or not, return home or not).Results: The mean ESI of the visits during the PP (3.19) was statistically significantly lower (P = 0.001) than it was in the CP (3.43). These changes were more pronounced during the months of March (3.03 versus 3.33, P = 0.037) and April (2.96 versus 3.48, P < 0.001). The change in ESI was mainly due to an increase in the proportion of visits by patients with an ESI score of 3 (42% versus 28%, P < 0.001). There were no differences in the characteristics of patients except a decline in patients whose symptoms had a duration of more than 30 days (2% during PP versus 4% during CP, P = 0.03).Conclusion: The COVID-19 pandemic caused a change in the pattern of non-COVID-19 visits, with proportionally more severe presentations based on the ESI. To our knowledge, this is the first description of changes in behaviour in ED visits by specifically non-COVID-19 patients.Keywords: COVID-19, emergency visits, epidemiology, Emergency Severity Index

Abstract

La sordera afecta no sólo a las capacidades de percepción auditiva y de comunicación, sino también a la calidad de vida y a las interacciones sociales. En la actualidad, se ha establecido que debe considerarse el factor de riesgo modificable más importante de la aparición de una demencia. Desde finales de la década de 1980, se ha demostrado la eficacia de la rehabilitación de la audición con el implante coclear, con resultados que han mejorado gracias a la evolución de la técnica quirúrgica, de las tecnologías de procesamiento de la señal y de las prácticas clínicas y a la mejor definición de las indicaciones. En Francia, la actividad de implantación coclear está regulada por la Haute Autorité de Santé y se basa en equipos pluridisciplinarios sometidos a la aprobación de las agencias regionales de salud. En este artículo, se detalla el funcionamiento, las indicaciones y los resultados actuales del implante coclear en adultos.

La prise en charge des teignes des enfants <10kg est difficile et sans consensus. Suite à l’arrêt en 2021 de la commercialisation de la griséofulvine, seule AMM en pédiatrie dans cette indication, le Centre de preuve en dermatologie (CPD) de la Société française de dermatologie (SFD) a élaboré un algorithme de prise en charge pour l’enfant ≥10kg. L’objectif de notre étude était de décrire les modalités de prise en charge thérapeutique des teignes chez les enfants ≤10kg.

Les traitements systémiques de la pelade sévère de l’enfant sont prescrits hors AMM en France. Aucune étude descriptive nationale de l’utilisation de ces traitements n’a été publiée. Nos objectifs sont de constituer une cohorte nationale française d’enfants et adolescents ayant une pelade traités par au moins une ligne de traitement systémique et d’évaluer le profil de ces traitements par la mesure de leur durée et les facteurs associés au maintien d’un traitement.

Abstract

Background:Cannabis use is on the rise. Several casesof cannabinoid syndrome, secondary to chronic cannabis intoxication, have been described worldwide, but few cases have described this entity in pregnant women. Case presentation: We describe a 29-year-old pregnant patient that had consumed cannabis and experienced uncontrolled vomiting. The use of hot baths, the rapid improvement in symptoms, and results of complementary examinations suggested a diagnosis of cannabinoid syndrome. The patient could return home and she continued her pregnancy and childbirth without peculiarities. Conclusion:Cannabinoid syndrome should be considered in the differential diagnosis of vomiting in pregnancy. Consumption of cannabis must be systematically included in the anamnesis. However, it seems to be somewhatunacceptable socially or medically. Consumption must be stopped to manage symptoms.

Abstract

We discuss the treatment of spin-orbit coupling (SOC) in time-reversal-symmetry-broken periodic systems for relativistic electronic structure calculations of materials within the generalized noncollinear Kohn-Sham density functional theory (GKSDFT). We treat SOC self-consistently and express the GKS orbitals in a two-component spinor basis. Crucially, we present a methodology (and its corresponding implementation) for the simultaneous self-consistent treatment of SOC and exact nonlocal Fock exchange operators. The many advantages of the inclusion of nonlocal Fock exchange in the self-consistent treatment of SOC, as practically done in hybrid exchange-correlation functionals, are both formally derived and illustrated through numerical examples: (i) it imparts a local magnetic torque (i.e., the ability of the two-electron potential to locally rotate the magnetization with respect to a starting guess configuration) that is key to converge to the right solution in noncollinear DFT regardless of the initial guess for the magnetization; (ii) because of the local magnetic torque, it improves the rotational invariance of noncollinear formulations of the DFT; (iii) it introduces the dependence on specific pieces of the spinors (i.e., those mapped onto otherwise missing spin blocks of the complex density matrix) into the two-electron potential, which are key to the correct description of the orbital- and spin-current densities and their coupling with the magnetization; and (iv) when space-inversion symmetry is broken, it allows for the full breaking of time-reversal symmetry in momentum space, which would otherwise be constrained by a sum rule linking the electronic band structure at opposite points in the Brillouin zone ($\underline{\mathbf{k}}$ and $\ensuremath{-}\underline{\mathbf{k}}$). The presented methodology is implemented in a developmental version of the public crystal code. Numerical tests are performed on the model system of an infinite radical chain of ${\mathrm{Ge}}_{2}\mathrm{H}$ with both space-inversion and time-reversal symmetries broken, which allows to highlight all the above-mentioned effects.
