4. Critiquing the Computer-Aided Design of Dental Prostheses.

Abstract

This paper describes RaPiD, a computer-aided assistant for the design of dental prostheses called removable partial dentures. The user manipulates icons directly to indicate the desired design solution to a given clinical situation. A developing design is represented as a logic database of components in a design; expert rules are applied as integrity constraints governing valid database transactions/design alterations. RaPiD has two modes of operation: manual mode designed for educational use (at either "student" or "expert" level), and automatic, which is intended for dentists in practice. In automatic mode, the dentist enters key clinical information, after which the system takes over and completes the design. Contravention of design rules is presented to the user in a critiquing style. The critiquing style strategies form the basis for the system's use in undergraduate and graduate dental education. Critiquing strategies used include: (1) a critique is issued only when the user has completed the proposed alteration; (2) a critique is issued immediately upon the user's radical misuse of a tool; (3) critiquing dynamically without negotiation with the user; (4) critiquing requested by the user upon completion of a design session, or at certain other stages in the design process; and (5) optional critiquing, requested by the user who wishes to compare his design with that which would have been produced independently by the system. An assessment of the educational effectiveness is planned. Expanding the range of design rules in RaPiD so that it becomes comprehensive is a current priority, as is the introduction of critiquing strategies (4) and (5), as well as completion of the automatic mode of operation. The extensive use and testing of RaPID, already carried out, show it to be a versatile and robust knowledge-based system. (Contains 14 references.) (Author/MAS)

Abstract

Antibiotics are modifiable risk factors for Clostridioides dffficile infection (CDI), driving pathogenesis via gut microbiome disruption. The ma " ement of patients with CDI prescribed concomitant non-CDI antibiotics is problematic and influences CDI outcome and recurrence risk. Though an assessment of the ongoing requirement for concomitant antibiotics is essential, discontinuation is often not possible. Antibiotics for other reasons might also need to be commenced during CDI therapy. Attempts to minimise the number and duration of antibiotics with a change to a low-risk class are recommended. Fidaxomicin might be preferable to vancomycin due to it having less effect on the gut microbiome; however, vancomycin is also acceptable. Metronidazole should be avoided and proton pump inhibitors discontinued. Access to fidaxomicin might be limited; hence, it should be prioritised for patients at high risk of recurrence. There is insufficient evidence to support extending anti-CDI therapy duration and concerns regarding microbiome effect remain. The addition of bezlotoxumab might be considered if multiple additional risk factors for recurrent CDI exist, though the amount of evidence is low. Investigational approaches to reduce the effect of concomitant antibiotics on the gut microbiome could further optimise CDI treatment in the presence of concomitant antibiotic use in the future.

Abstract

COVID study groups - PORTUGAL: Portuguese Laboratory Network for the Diagnosis of COVID-19 and Public Health Department of the Health Administrative Regions, Physicians that provided data and samples from suspected cases and SARS-CoV-2 genetic characterization. INSA laboratory team for the diagnosis of SARS-CoV-2. Algarve Biomedical Center and Unilabs. We compared 19,207 cases of SARS-CoV-2 variant B.1.1.7/S gene target failure (SGTF), 436 B.1.351 and 352 P.1 to non-variant cases reported by seven European countries. COVID-19 cases with these variants had significantly higher adjusted odds ratios for hospitalisation (B.1.1.7/SGTF: 1.7, 95% confidence interval (CI): 1.0-2.9; B.1.351: 3.6, 95% CI: 2.1-6.2; P.1: 2.6, 95% CI: 1.4-4.8) and B.1.1.7/SGTF and P.1 cases also for intensive care admission (B.1.1.7/SGTF: 2.3, 95% CI: 1.4-3.5; P.1: 2.2, 95% CI: 1.7-2.8).
