15. Working lives of GPs in Scotland and England: cross-sectional analysis of national surveys

Abstract

OBJECTIVES: The UK faces major problems in retaining general practitioners (GPs). Scotland introduced a new GP contract in April 2018, intended to better support GPs. This study compares the career intentions and working lives of GPs in Scotland with GPs in England, shortly after the new Scotland contract was introduced. DESIGN AND SETTING: Comparison of cross-sectional analysis of survey responses of GPs in England and Scotland in 2017 and 2018, respectively, using linear regression to adjust the differences for gender, age, ethnicity, urbanicity and deprivation. PARTICIPANTS: 2048 GPs in Scotland and 879 GPs in England. MAIN OUTCOME MEASURES: Four intentions to reduce work participation (5-point scales: 1='none', 5='high'): reducing working hours; leaving medical work entirely; leaving direct patient care; or continuing medical work but outside the UK. Four domains of working life: job satisfaction (7-point scale: 1='extremely dissatisfied', 7='extremely satisfied'); job stressors (5-point-scale: 1='no pressure', 5='high pressure); positive and negative job attributes (5-point scales: 1='strongly disagree', 5='strongly agree'). RESULTS: Compared with England, GPs in Scotland had lower intention to reduce work participation, including a lower likelihood of reducing work hours (2.78 vs 3.54; adjusted difference=-0.52; 95% CI -0.64 to -0.41), a lower likelihood of leaving medical work entirely (2.11 vs 2.76; adjusted difference=-0.32; 95% CI -0.42 to -0.22), a lower likelihood of leaving direct patient care (2.23 vs 2.93; adjusted difference=-0.37; 95% CI -0.47 to -0.27), and a lower likelihood of continuing medical work but outside of the UK (1.41 vs 1.61; adjusted difference=-0.2; 95% CI -0.28 to -0.12). GPs in Scotland reported higher job satisfaction, lower job stressors, similar positive job attributes and lower negative job attributes. Abstract

Few studies assess the effects of recreational fishing in isolation from commercial fishing. We used meta-analysis to synthesise 4444 samples from 30 years (1987–2017) of fish surveys inside and outside a large network of highly protected reserves in the Ningaloo Marine Park, Western Australia, where the major fishing activity is recreational. Data were collected by different agencies, using varied survey designs and sampling methods. We contrasted the relative abundance and biomass of target and non-target fish groups between fished and reserve locations. We considered the influence of, and possible interactions between, seven additional variables: age and size of reserve, one of two reserve network configurations, reef habitat type, recreational fishing activity, shore-based fishing regulations and survey method. Taxa responded differently: the abundance and biomass inside reserves relative to outside was higher for targeted lethrinids, while other targeted (and non-targeted) fish groups were indistinguishable. Reef habitat was important for explaining lethrinid response to protection, and this factor interacted with reserve size, such that larger reserves were demonstrably more effective in the back reef and lagoon habitats. There was little evidence of changes in relative abundance and biomass of fishes with reserve age, or after rezoning and expansion of the reserve network. Our study demonstrates the complexities in quantifying fishing effects, highlighting some of the key factors and interactions that likely underlie the varied results in reserve assessments that should be considered in future reserve design and assessment.

Abstract

Basal-like breast cancer (BLBC) is a poorly characterised, heterogeneous disease. Patients are diagnosed with aggressive, high-grade tumours and often relapse with chemotherapy resistance. Detailed understanding of the molecular underpinnings of this disease is essential to the development of personalised therapeutic strategies. Inhibitor of Differentiation 4 (ID4) is a helix-loop-helix transcriptional regulator required for mammary gland development. ID4 is overexpressed in a subset of BLBC patients, associating with a stem-like poor prognosis phenotype, and is necessary for the growth of cell line models of BLBC, through unknown mechanisms. Here, we have defined a molecular mechanism of action for ID4 in BLBC and the related disease highgrade serous ovarian cancer (HGSOV), by combining RIME proteomic analysis and ChIP-Seq mapping of genomic binding sites. Remarkably, these studies have revealed novel interactions with DNA damage response proteins, in particular, mediator of DNA damage checkpoint protein 1 (MDC1). Through MDC1, ID4 interacts with other DNA repair proteins (γH2AX and BRCA1) at fragile chromatin sites. ID4 does not affect transcription at these sites, instead binding to chromatin following DNA damage and regulating DNA damage signalling. Clinical analysis demonstrates that ID4 is amplified and overexpressed at a higher frequency inBRCA1-mutant BLBC compared with sporadic BLBC, providing genetic evidence for an interaction between ID4 and DNA damage repair pathways. These data link the interactions of ID4 with MDC1 to DNA damage repair in the aetiology of BLBC and HGSOV.

Abstract

The maintenance of normal body weight is disrupted in patients with anorexia nervosa (AN) for prolonged periods of time. Prior to the onset of AN, premorbid body mass index (BMI) spans the entire range from underweight to obese. After recovery, patients have reduced rates of overweight and obesity. As such, loci involved in body weight regulation may also be relevant for AN and vice versa. Our primary analysis comprised a cross-trait analysis of the 1000 single-nucleotide polymorphisms (SNPs) with the lowest P-values in a genome-wide association meta-analysis (GWAMA) of AN (GCAN) for evidence of association in the largest published GWAMA for BMI (GIANT). Subsequently we performed sex-stratified analyses for these 1000 SNPs. Functional ex vivo studies on four genes ensued. Abstract

The phenotypic effect of some single nucleotide polymorphisms (SNPs) depends on their parental origin. We present a novel approach to detect parent-of-origin effects (POEs) in genome-wide genotype data of unrelated individuals. The method exploits increased phenotypic variance in the heterozygous genotype group relative to the homozygous groups. We applied the method to >56,000 unrelated individuals to search for POEs influencing body mass index (BMI). Six lead SNPs were carried forward for replication in five family-based studies (of ∼4,000 trios). Two SNPs replicated: the paternal rs2471083-C allele (located near the imprinted KCNK9 gene) and the paternal rs3091869-T allele (located near the SLC2A10 gene) increased BMI equally (beta = 0.11 (SD), P, Author Summary Large genetic association studies have revealed many genetic factors influencing common traits, such as body mass index (BMI). These studies assume that the effect of genetic variants is the same regardless of whether they are inherited from the mother or the father. In our study, we have developed a new approach that allows us to investigate variants whose impact depends on their parental origin (parent-of-origin effects), in unrelated samples when the parental origin cannot be inferred. This is feasible because at genetic markers at which such effects occur there is increased variability of the trait among individuals who inherited different genetic codes from their mother and their father compared to individuals who inherited the same genetic code from both parents. We applied this methodology to discover genetic markers with parent-of-origin effects (POEs) on BMI. This resulted in six candidate markers showing strong POE association. We then attempted to replicate the POE effects of these markers in family studies (where one can infer the parental origin of the inherited variants). Two of our candidates showed significant association in the family studies, the paternal and maternal effects of these markers were in the opposite direction.

Abstract

Since the introduction of the endosseous concept to North America in 1982, there have been new permutations of the original ad modum Branemark design to meet the unique demands of treating the edentulous maxilla with an implant restoration. While there is a growing body of clinical evidence to assist the student, faculty, and private practitioner in the algorithms for design selection, confusion persists because of difficulty in assessing the external and internal validity of the relevant studies. The purpose of this article is to review clinician- and patient-mediated factors for implant restoration of the edentulous maxilla in light of the hierarchical level of available evidence, with the aim of elucidating the benefit/risk calculus of various treatment modalities.

Abstract

In recent years, a class of adaptive schemes has been developed for suppressing periodic disturbance signals with unknown frequencies, phases, and amplitudes. The stability and robustness of these schemes with respect to inevitable unmodeled dynamics and noise disturbances in the absence of persistently exciting signals has not been established despite successful simulation results and implementations. The purpose of this technical note is to propose a robust adaptive scheme for rejection of unknown periodic components of the disturbance and analyze its stability and performance properties. First, we consider the ideal case (non-adaptive) when complete information about the characteristics of the disturbance is available. We show that the rejection of periodic terms may lead to amplification of output noise and in some cases lead to a worse output performance. The way to avoid such undesirable noise amplification is to increase the size of the feedback control filter in order to have the flexibility to achieve rejection of the periodic disturbance terms while minimizing the effect of the noise on the output. The increased filter order leads to an over-parameterized scheme where persistence of excitation is no longer possible, and this shortcoming makes the use of robust adaptation essential. With this important insight in mind, the coefficients of the feedback filter whose size is over parameterized are adapted using a robust adaptive law. We show analytically that the proposed robust adaptive control scheme guarantees stability, performance and robustness with respect to unmodeled dynamics and bounded broadband noise disturbances. Abstract

Fisheries are complex social-ecological systems, where managers struggle to balance the socio-economic interests of fishing communities with the biology and ecology of fisheries species. Spatial closures are a popular measure to address conservation and fisheries management goals, including the protection of shark populations. However, very little research has been published on the effectiveness of shark-specific closures to protect sharks, or their impacts on fisher behavior. Situated within the global center of tropical marine biodiversity, Indonesia's shark fishery contributes more to the international shark fin trade than any other nation. Here we evaluate the effect of shark-specific closures on sharks and other species of interest, as well as shark fishers' responses to losing access to their former fishing grounds. We assessed shark diversity and abundance in an open access zone (OAZ) and two No-Take Zones (NTZs) of a Marine Protected Area within the recently established shark sanctuary in Raja Ampat, Indonesia, where sharks have high monetary value as a tourism attraction. Shark abundance was significantly higher in the privately managed NTZs than in the OAZ. Across all management zones, neither zone size, depth nor reef complexity explained variations in shark abundance, suggesting that governance is the main driver of successful shark conservation areas. These trends were also reflected in species targeted by small-scale reef fisheries, including snappers, emperor, groupers, tunas, mackerels, and large-bodied wrasse and parrotfish. Interviews with shark fishers who lost access to their primary fishing grounds when the shark sanctuary was established showed that while most fishers (88%) knew that sharks were protected in Raja Ampat, many were unsure about the purpose of the sanctuary. This paper studies the robustness and performance of an adaptive scheme for suppression of laser beam jitter in the presence of unmodeled dynamics in a continuoustime formulation. To demonstrate the level of performance that can be achieved in the case of known disturbances, at first we consider the ideal case (non-adaptive) when a complete information about characteristics of the noise-corrupted periodic disturbance is available. Then the adaptive case when the disturbance is unknown with time-varying characteristics is investigated. The results show under what conditions jitter suppression can be achieved and what parameters contribute to the performance of the adaptive control scheme.
