21. Optimisation of anti-platelet therapy in acute coronary syndromes

Abstract

Acute coronary syndromes (ACS) are a major cause of morbidity and mortality worldwide. Platelets are central to the underlying pathology of ACS, and anti-platelet drugs, such as aspirin, clopidogrel and ticagrelor, form a cornerstone of its treatment. However, response to anti-platelet drugs is not uniform, with a substantial proportion of patients being nonresponsive to their effects, leading to an increase in risk of adverse cardiovascular events. Several mechanisms underlie this observed non-response, including clinical risk factors, genetic polymorphisms, drug interactions, medication adherence and inflammation. However, the data investigating these mechanisms are often contradictory with no consensus on how non-response should be detected or treated. This thesis sought to investigate easily detectable and potentially modifiable causes for anti-platelet nonresponse, which may have potential clinical utility. Through comprehensive meta-analyses of the published literature, we demonstrated a consistent association between carriage of the CYP2C19 loss-of-function polymorphisms, higher platelet reactivity and an increased risk of adverse cardiovascular events in clopidogrel treated patients. Importantly, we failed to demonstrate the effect of other genes such as ABCB1, CYP3A5 and PON1, which had been previously suggested as important in determining response to clopidogrel. We failed to detect a clear association between genes in aspirin's pharmacokinetic and pharmacodynamic pathway and aspirin response, defined by two platelet function tests, in a cohort of patients with ACS. However, weak associations were detected between poor aspirin response and two polymorphisms in the UGT1A6 and TBXA2R genes prior to correction for multiple testing, which may deserve further investigation in larger numbers of patients. Given the importance of lipid oxidation in the context of vascular inflammation and the pathogenesis of atherosclerosis, we investigated the role of the OxLDL-ß2GPI complex in clinical outcomes, platelet reactivity and lipid profiles in ACS patients. We demonstrated an association between higher levels of OxLDL-ß2GPI and a lower risk of adverse cardiovascular events, which is not consistent with published clinical studies but is supported by in vitro and non-human data. We also detected a significant association between aspirin non-response and raised HDL to cholesterol ratios in patients, although there was no association between OxLDL-ß2GPI levels and platelet reactivity demonstrated. Finally, we investigated whether H. Pylori which has been associated with adverse cardiovascular outcomes due to increased inflammation may be responsible for the welldocumented interaction between clopidogrel and proton pump inhibitors, which are often prescribed in patients with non-specific gastrointestinal symptoms. In a cohort of patients with ACS, we failed to demonstrate any association between H. Pylori serology and clinical outcome although an association between clinical outcome and carriage of the CYP2C19*2 allele was observed, but only in H. Pylori negative patients. The underlying cause for this finding is unclear but may represent an interaction between PPI use and changes in the gastric flora induced by a higher gastric pH. In summary, this thesis has identified the CYP2C19 gene as a critical determinant of clopidogrel response which could be used as a biomarker for stratification of ADP receptor antagonists. In addition, we have identified putative biomarkers for aspirin response including the TBXA2R gene and clinical risk factors such as hyperlipidaemia. These data suggest that stratification and personalisation of anti-platelets is possible by using genetic and clinical biomarkers, and further, well-powered, clinical outcome studies are necessary.

Abstract

We introduce here the concept of establishing Parity-time symmetry through a gauge transformation involving a shift of the mirror plane for the Parity operation. The corresponding unitary transformation on the system's constitutive matrix allows us to generate and explore a family of equivalent Parity-time symmetric systems. We further derive that unidirectional zero reflection can always be associated with a gauged PT-symmetry and demonstrate this experimentally using a microstrip transmission-line with magnetoelectric coupling. Abstract

This study examines the 2016 Diba Expedition to Facebook, a mass collective organized campaign directed at independence-leaning Taiwanese individuals and institutions, as an example of cyber nationalism through a highly organized meme war between anti- and pro-Taiwan independence users on Facebook. Drawing upon a social semiotic multimodal discourse analysis of the nationalist campaign, this study examines the ways Diba members deployed multimodal elements of political propaganda and popular culture in the preparation and mobilization of the campaign as well as the playful memetic interaction between the two camps on the battlefield. The analysis and discussion underscore the playful and carnivalesque ecology of Chinese social media that users deployed in the 2016 expedition while also demonstrating the dilemma of undertaking a Chinese nationalism campaign beyond the Great Firewall, where those based in the Mainland needed to overcome the state's regulations of online security to defend the motherland. This study contributes to the growing research on cyber nationalism in China and adds a further dimension to the study of Chinese social media. [ABSTRACT FROM AUTHOR]

This review presents a comparative analysis of gelation properties in plant-based versus animal-based proteins, emphasizing key factors such as pH, ionic environment, temperature, and anti-nutritional factors. Gelation, a crucial process in food texture formation, is influenced by these factors in varying ways for plant and animal proteins. Animal proteins, like casein, whey, meat, and egg, generally show stable gelation properties, responding predictably to pH, temperature, and ionic changes. In contrast, plant proteins such as soy, pea, wheat, and oilseed show more variable gelation, often requiring specific conditions, like the presence of NaCl or optimal pH, to form effective gels. Animal proteins tend to gel more reliably, while plant proteins require precise environmental adjustments for similar results. Understanding these factors is crucial for selecting and processing proteins to achieve desired textures and functionalities in food products. Abstract

AWZ1066S has been developed as a potential treatment for the neglected tropical diseases lymphatic filariasis and onchocerciasis. AWZ1066S targets the Wolbachia bacterial endosymbiont present in the causative nematode parasites. This phase 1, first‐in‐human study aimed to assess the safety and pharmacokinetics of AWZ1066S in healthy human participants. In a randomized double‐blind, placebo‐controlled, single ascending dose study, healthy adults received a single oral dose of AWZ1066S (or placebo) and were followed up for 10 days. The planned single doses of AWZ1066S ranged from 100 to 1600 mg, and each dose was administered to a cohort of 8 participants (6 AWZ1066S and 2 placebo). In total 30 people participated, 18 (60%) female, median age 30.0 years (minimum 20, maximum 61). The cohorts administered 100, 200, 300, and 400 mg of AWZ1066S progressed unremarkably. After single 700‐mg doses all 4 participants developed symptoms of acute gastritis and transient increases in liver enzymes. The severity of these adverse events ranged from mild to severe, with 1 participant needing hospital admission. Pharmacokinetic analysis indicated that AWZ1066S is rapidly absorbed with predictable pharmacokinetics. In conclusion, safety concerns prevented this study from reaching the human exposures needed for AWZ1066S to be clinically effective against lymphatic filariasis and onchocerciasis. 36. The Work to Make an Experiment Work

Abstract

Contrary to its typical presentation in scientific publications as a certain and linear process, in reality, the experimental method, not least the design aspect of it, requires a great deal of trial-and-error and ad hoc decision-making on the part of the researchers. This uncertain and contingent aspect of research, although little known outside of the circle of experts, has important implications for our understanding of the nature of science and scientific findings. This paper offers a backstage perspective to experiment design, where the uncertain and contingent nature of experimental research is at its starkest. It draws on insights from the sociological perspective of ethnomethodology through the auto-ethnographic first-hand experience of the first author's own social psychology experiments. Based on detailed lab notes and planning documents on how and why design changes were made, the analysis focuses on the evolution of the experiment design, particularly the researcher's "in situ" practical reasoning for how to make the experiment work. From this, we show how ethno-methods shape experiment results and highlight the inseparability of social science experimentation from "in situ" practical reasoning.

Abstract

The marine environment is an excellent source for many physiologically active compounds due to its extensive biodiversity. Among these, fish proteins stand out for their unique qualities, making them valuable in a variety of applications due to their diverse compositional and functional properties. Utilizing fish and fish coproducts for the production of protein hydrolysates and bioactive peptides not only enhances their economic value but also reduces their potential environmental harm, if left unutilized. Fish protein hydrolysates (FPHs), known for their excellent nutritional value, favorable amino acid profiles, and beneficial biological activities, have generated significant interest for their potential health benefits. These hydrolysates contain bioactive peptides which are peptide sequences known for their beneficial physiological effects. These biologically active peptides play a role in metabolic regulation/modulation and are increasingly seen as promising ingredients in functional foods, nutraceuticals and pharmaceuticals, with potential to improve human health and prevent disease. This review aims to summarize the current in vitro, cell model (in situ) and in vivoresearch on the antioxidant, glycaemic management and muscle health enhancement properties of FPHs and their peptides.
