28. Idiopathic acute transverse myelitis in children: an analysis and discussion of MRI findings.

Abstract

Background: There is lack of reported magnetic resonance imaging (MRI) studies of idiopathic acute transverse myelitis (ATM) in children.Objective: To describe the imaging features of idiopathic ATM in children.Methods: We retrospectively analyzed the spinal MRI findings of children diagnosed with ATM. The anatomic regions, vertebral segmental length, gray or white matter involvement, cord expansion and gadolinium enhancement were examined.Results: A total of 27 children were diagnosed with isolated monophasic ATM with a mean follow-up of 5.2 years. Two children later diagnosed with neuromyelitis optica were excluded from the pediatric ATM cohort. None of the patients had a subsequent diagnosis of multiple sclerosis. The mean age of onset was 9.5 years (0.5—16.9 years). Spinal MRIs were abnormal in 21 (78%). The mean interval between symptom onset and the MRI was 1.7 days (0—19 days). Central cord hyperintensity involving gray matter was seen in all patients. A majority (67%) of the patients demonstrated long segment lesions with a mean segment length of 6.4.Conclusions: We conclude that central cord inflammation extending over three or more segments is the most common finding of idiopathic monophasic transverse myelitis in children. The risk of multiple sclerosis in children who experience isolated transverse myelitis as a first demyelinating event is low. [ABSTRACT FROM AUTHOR]

38. CT Diagnosis and ShortTerm Prognosis of Intracranial Hemorrhage and HypoxicIschemic Brain Damage in Neonates

Abstract

A group of 476 neonates who had suffered perinatal asphyxia were studied with computed tomography (CT), and 341 were found to have some form of intracranial hemorrhage. The different hemorrhages were identified and classified. While the presence and severity of intraventricular hemorrhage in the premature neonate clearly increase neonatal mortality, subependymal and subarachnoid hemorrhages seem to have very little influence on whether the neonate survives or succumbs. Decreased brain tissue attenuation seen to some extent in all premature neonates depends on the maturity of the neonate and thus probably represents the normal appearance of the immature brain on CT. It should be noted that essentially all the CT examinations included in this study were carried out with an Ohio-Nuclear Delta 50 scanner. Findings with newer scanners may or may not corroborate the validity of some aspects of our observations.

40. Tuberous Sclerosis Complex in Children

Abstract

• Of 62 children seen at The Hospital for Sick Children, Toronto, who had tuberous sclerosis (TS), 58 had a history of seizures (developing during the first year of life in 37) and mental retardation was diagnosed in 51. Classic congenital hypopigmented patches were present in 42 and adenoma sebaceum occurred in 26 of the patients. Computed tomography (CT) is an essential diagnostic tool. We conclude that TS is more common than has been believed. Seizures are the most common manifestation of the disease and the hypopigmented patches, its first cutaneous manifestation, are present at an early age. When a positive diagnosis has been made, parents and siblings should be examined for signs of TS, keeping in mind the wide spectrum of findings in this condition; if no stigmata are present on examination, then a CT scan should be considered for parents of reproductive age so that proper genetic counseling can be given.(Am J Dis Child 1981;135:912-917)

43. The Intelligence of Hydrocephalic Children

Abstract

• In a group of 78 children with hydrocephalus in the first months of life, the level and pattern of intelligence were considered in relation to various parameters and symptoms of their condition. These included demography (age, sex, handedness); early developmental status; symptoms (visual, motor, and seizure); formative pathology; type of hydrocephalus; site of CSF obstruction; extent and configuration of cortical thinning; and shunt treatment. The common outcome of early hydrocephalus is an uneven growth of intelligence during childhood, with nonverbal intelligence developing less well than verbal intelligence. The origin of this selective cognitive deficit is in neither the hydrocephalic condition itself nor its treatment, but rather in the developmental brain anomalies and symptoms to which the hydrocephalic child is prone: in children with aqueduct blocks and intraventricular hydrocephalus, a selectively thin vertex and occipital lobe; in any hydrocephalic child, ocular abnormalities, motor deficits, and seizures.

Abstract

Phenylbutazone has been associated with bioavailability problems and has shown nonclassical behavior in phase-transport studies. This nonclassical behavior has been attributed, in part, to the fact that phenylbutazone, as a carbon acid, undergoes noninstantaneous ionization kinetics. Instantaneous reaction is an assumption made in many diffusion-limited transport models involving a simultaneous ionization reaction. The ionization kinetics of phenylbutazone were determined at an ionic strength of 0.1 and 25 degrees using a stopped-flow spectrophotometer. A log kobs versus pH profile for the approach to the ionization equilibrium was determined, and a mechanism consistent with the profile was postulated. The percent enol versus the diketo form of phenylbutazone acid as well as pKaenol and pKadiketo was kinetically calculated. The protonation reaction was highly catalyzed by general acids while the deprotonation reaction was highly catalyzed by general bases. The general acid, water, was a poor proton donor to the anionic form (the so-called mesomericanion) of phenylbutazone.
