Abstract

The analysis of nuclear magnetic resonance (NMR) spectra to detect peaks and characterize their parameters, often referred to as deconvolution, is a crucial step in the quantification, elucidation, and verification of the structure of molecular systems. However, deconvolution of 1D NMR spectra is a challenge for both experts and machines. We propose a robust, expert-level quality deep learning-based deconvolution algorithm for 1D experimental NMR spectra. The algorithm is based on a neural network trained on synthetic spectra. Our customized pre-processing and labeling of the synthetic spectra enable the estimation of critical peak parameters. Furthermore, the neural network model transfers well to the experimental spectra and demonstrates low fitting errors and sparse peak lists in challenging scenarios such as crowded, high dynamic range, shoulder peak regions as well as broad peaks. We demonstrate in challenging spectra that the proposed algorithm is superior to expert results.

17. CAN A “BROKEN HEART” SUCCESSFULLY BE TRANSPLANTED? A CASE REPORT

Abstract

In January 2023 a 59–year–old woman with dilated ischemic cardiomyopathy, was admitted to our Cardiac Surgery Center for a heart transplant eligibility study. She presented symptoms of dyspnea and in the last 6 months was admitted four times to the hospital for heart failure. An echocardiogram showed a severely biventricular hypocontractility (LVEF: 22%; TAPSE 11 mm). After one month, the patient was urgently admitted to our Intensive Care Unit for cardiogenic shock. ECMO VA was implanted percutaneously. Therefore, the patient was placed on the national emergency waitlist for transplantation. A potential donation was identified ten days later. The donor was a 58–year–old woman with no cardiovascular risk factors, had died of a rupture of a cerebral aneurysm. During hospitalization, echocardiography showed LVEF of 30% with akinesia of the middle and apical wall and hyperkinesia of the basal with a mild improvement in contractility (LVEF: 40%) two days later. Coronary angiography was normal. The patient‘s condition resembled Takotsubo syndrome. Virtual and subsequently cytotoxic crossmatch was negative. We proceeded with orthotopic heart transplant. No noteworthy complications occurred during the operation. She received anti–thymocyte immunoglobulin for immunosuppression induction and prednisone, tacrolimus and mycophenolate mofetil for maintenance therapy. The echocardiogram, performed few days later, revealed preserved contractile function in both ventricles (LVEF: 58%). The patient was discharged after the first four endomyocardial biopsies displaying no signs of rejection (ISHLT’04 0R). At ten–month follow–up, the patient is in good clinical condition with no substantial changes during the follow–up echocardiogram. Brain injuries, including subarachnoid haemorrhage, traumatic brain injury and stroke, have been extensively documented in the literature to cause a surge in catecholamines, with a direct effect on the heart, resulting in a transient myocardial dysfunction commonly referred to as ‘Takotsubo cardiomyopathy‘ or ‘broken heart syndrome‘. Studies have shown that dysfunctional hearts at the start of transplant screening have normal contractile function at the time of organ retrieval have similar outcomes to those without dysfunction. This knowledge may help to increase the pool of potential donor hearts and improve the availability of hearts for transplantation, which is crucial given the shortage of suitable donor organs.

Abstract

The possible role of human papillomavirus (HPV) in the aetiology of papillary tumor of the bladder has been evaluated and a review of the literature concerning this issue was made. A group of 17 patients affected by bladder papillary tumor was analysed. Surgical specimens were collected for virological and histological analysis. The DNA of the following viruses was searched by polymerase chain reaction (PCR): Adenovirus, Herpes simplex virus type 1 (HSV-1), Herpes simplex virus type 2 (HSV-2), Human Papillomaviruses (HPV), Polyomaviruses (BKV and JCV). The results showed that 15/17 (88 %) patients with papillary bladder tumor were found negative for each viral-searched DNA; only one sample was positive for HPV (6 %) genotype 6, which is considered to convey a low risk for cancer development and only one was positive for BKV (6 %). From the results obtained there seems to be no relationship between viral infection and the presence of bladder papillary tumor. Moreover, in the examined population the association bladder carcinoma-HPV, found by others, has not been confirmed. The homogeneity of the specimens studied was such that it would not be affected by the temporal factor, as were cases of more or less advanced cancers. Nonetheless specimens from patients with advanced cancers (G III) were negative to HPV infection. The data do not appear indicative for a correlation between viral DNA presence and histological parameters. Thus, in the light of the data emerging from this investigation, no causal relationship can be established between HPV infection and papillary bladder tumors.

Abstract

To analyze the sexuality of Italian menopausal women.Cross-sectional study.Menopausal women consecutively observed during the study period in menopause clinics.Women were interviewed about their current and premenopausal sexual activity: sexual intercourse frequency and self-rated sexual desire, capacity for orgasm and sexual satisfaction were recorded. Women were defined as having poor sexual functioning if they had one or less sexual intercourses per week or answered 'absent/poor' to the questions about the sexual domains.Oral hormone therapy (HT) use (odds ratio (OR) 0.43 for desire, 0.54 for orgasm and 0.56 for overall sexual satisfaction, all p0.001) and transdermal HT (OR 0.38, 0.53 and 0.53, respectively, all p0.001) were significantly associated with lower risk of poor sexual functioning. Higher physical and mental component scores (PCS and MCS, range 0-100) of the Short Form-12 are inversely related to poor sexual functioning (OR by point 0.96, 0.95, 0.95 for PCS and 0.96, 0.96 and 0.95, for MCS, respectively, all p0.001). Pain during and symptoms after sexual intercourse were significantly related to desire (OR 1.96 and 1.78, respectively), orgasm (OR 2.22 and 2.06, respectively) and sexual satisfaction (OR 2.02 and 1.79, respectively). The partner's health problems were associated with low sexual intercourse frequency (OR 4.18, p0.001) and absent/poor overall satisfaction (OR 2.61, p0.001).This study shows that, in menopausal Italian women attending menopause clinics, sexual function is associated with the quality of sexual life in reproductive age, partner's health status, current quality of life, HT and occurrence of pain during and symptoms after sexual intercourse.

To analyze risk factors for type 2 diabetes among women attending menopause clinics in Italy for counselling about the menopause.Women attending a network of first-level outpatient menopause clinics in Italy for general counselling about menopause or treatment of menopausal symptoms.Cross-sectional study with no exclusion criteria. Type 2 diabetes was defined according to National Diabetes Data Groups Indications and the fasting blood glucose at an oral glucose tolerance test within the previous year.Out of the 44 694 considered in this analysis, 808 had a diagnosis of diabetes type 2 (1.8%). In comparison with women aged50 years, the multivariate odds ratios (OR) of type 2 diabetes were 1.31 (95% confidence interval (CI), 0.99-1.74) for women aged 50-52 years, 1.66 (95% CI, 1.27-2.17) at 53-56 years and 2.84 (95% CI, 2.20-3.67) in women agedor = 57 years. Type 2 diabetes was less frequently reported in more educated women (OR high school/university vs. primary school = 0.44 (95% CI, 0.36-0.55)). Being overweight was associated with an increased risk of type 2 diabetes. In comparison with women reporting a low level of physical activity, the multivariate OR of type 2 diabetes was 0.67 (95% CI, 0.54-0.84) for women reporting regular physical activity. In comparison with premenopausal women, the multivariate OR of type 2 diabetes was 1.38 (95% CI, 1.03-1.84) in women with natural menopause. This finding was present also after allowing for the potential confounding effect of age. The multivariate OR of diabetes for users of hormonal replacement therapy was 0.58 (95% CI, 0.46-0.73).This large cross-sectional study suggests that postmenopausal women are at higher risk of type 2 diabetes after allowance for the effect of age. Other main determinants of risk of type 2 diabetes in women around menopause were low socioeconomic status and being overweight. Diabetes was found less frequently in those taking hormone replacement therapy.

Abstract

OBJECTIVE To obtain data on correlates of climacteric symptoms in women around menopause attending menopause clinics in Italy. METHODS Since 1997 a large cross sectional study has been conducted on the characteristics of women around menopause attending a network of first level menopause outpatient's clinics in Italy. A total of 66,501 (mean age 54.4 years) women are considered in the present paper. RESULTS The odds ratios of moderate and severe hot flashes/night sweats were lower in more educated women and (for severe symptoms only) in women reporting regular physical activity. Depression, difficulty to sleep, forgetfulness and irritability tended to be less frequent in more educated women and (depression only) in women reporting regular physical activity. Parous women reported more frequently these symptoms. CONCLUSIONS This large study confirms in Southern European population that low education, body mass index and low physical activity are associated with climacteric symptoms. Parous women are at greater risk of psychological symptoms.

Abstract

Urethral diverticula is a rare pathology with an incidence varying between 0.3 and 6%. It is difficult to diagnose owing to the aspecificity of its clinical symptoms. The aim of this study was to evaluate the imaging techniques now available for its diagnosis.The sample consisted of 19 female patients aged between 20 and 53 years old undergoing diverticolectomy owing to urethral diverticula between 1980 and 1996 at the 4th Division of the Department of Urology at "La Sapienza" University of Rome. All patients underwent preoperative X-ray examinations (micturitional cystourethrography and P positive urethrography). A retrospective study was performed in order to evaluate the accuracy of the individual methods.Micturitional urethrocystography showed a sensitivity equivalent to 77% of cases. P positive urethrography showed a 85.7% accuracy rate. Lastly, transvaginal ultrasonography, which always showed the diverticular sac in all patients in which it was used, also highlighting multiple and divided diverticuli which were not visible using traditional radiology. The latter method is easy to use and well tolerated by patients; moreover, it shows the spatial relations of the diverticulum and allows the characteristics of periurethral tissues to be evaluated.The authors affirm that transvaginal ultrasonography is the first method of choice for the diagnosis of urethral diverticula.
