Objective: Firefighters are at risk of experiencing heightened anxiety and alcohol use perhaps due to occupational stress. Mindfulness is related to both anxiety and alcohol use; however, research on the relations of mindfulness with anxiety and alcohol use and drinking motives among firefighters is limited. This study examined (1) whether higher anxiety was positively related to alcohol use severity and drinking related motives (e.g., coping, enhancement, social, and conformity) and (2) whether higher mindfulness would moderate the association between anxiety symptom severity and alcohol use severity and alcohol use coping motives. Methods: Participants included urban firefighters (N = 679; Mage = 38.6; SD = 8.58; 93.5% male) who completed an online survey. Five hierarchical regression analyses were conducted to evaluate the main and interactive effects of anxiety symptom severity and mindfulness on all study outcomes (i.e., alcohol use severity and four drinking motives). Results: Anxiety symptom severity was significantly and negatively associated with mindfulness and, incrementally, positively associated with all alcohol use-related outcome variables. Mindfulness was negatively correlated with alcohol use severity and all alcohol use motives except social motives. Mindfulness moderated the effects of anxiety symptom severity on coping alcohol use motives only. Conclusion: Mindfulness moderated, or attenuated, the positive association between anxiety symptom severity and coping-oriented alcohol use motives among professional firefighters. Background: Peer support has been shown to reduce the incidence and severity of mental health disorders among firefighters. This pilot project had two aims: (1) to assess training satisfaction and knowledge, self-efficacy, and skills development of Texas firefighters participating in the International Association of Fire Fighters (IAFF) Peer Support Training, and (2) to determine if newly trained peer support specialists would use monthly phone supervision. Methods: The study included 14 Texas firefighters who enrolled in the IAFF Peer Support Training. Pre- and post-tests were administered to evaluate training satisfaction, skills development, self-efficacy, and behavioral health knowledge of participants. Use of phone supervision over 6 months following the training was also recorded. Results: Overall, participants reported high satisfaction with the training provided. While self-efficacy regarding detection of mental health issues and therapeutic empathy skills significantly improved following the training, participants did not show a significant improvement in knowledge regarding mental health. Attendance of participants to supervision calls was less than expected, although 71% attended at least one supervision call. Conclusions: This project was successful in increasing firefighters' self-efficacy and therapeutic empathy skills, and participants were highly satisfied with the training. AbstractBackground: Stress and repetitive exposures to potentially traumatizing events are inherent to first-responder professions. Firefighters and other crisis responders are often exposed to incidents in their work environments and are at risk of developing acute or posttraumatic symptomatology, depression, and anxiety, as well as harmful patterns of alcohol consumption. Most prior research and theory suggests that many firefighters use alcohol to cope with past and current stressors, and that increased drinking accompanies ongoing exposures to stressful occupational hazards. Objectives: The primary aim of this research was to examine alcohol consumption and mental health variables associated in the literature with alcohol misuse. Participants included 546 predominantly male career firefighters in the U.S. Using a cross-sectional design, we examined alcohol misuse in the context of self-reported depression, generalized anxiety, PTSD, and suicidal ideation using previously validated measures. Results: Based on the AUDIT’s recommended cut score of 8, 38.8% of our sample reported drinking at hazardous levels. Hazardous alcohol use was also significantly associated with several mental health indicators with correlations ranging from .28 to .34. Regression analyses revealed that over 14% of the variance in AUDIT scores could be explained by psychological symptom measures, and 29% of the variance in suicide risk was explained by a model using both alcohol and mental health measures. Conclusions: Findings suggest that psychological distress is associated with hazardous drinking, and that more consistent screening and referral for these issues, as well as multi-level interventions, are needed to effectively address these behavioral health concerns among firefighters. There has been limited research on the health impacts of extreme bushfire exposure among emergency responders (ER) involved in suppressing extreme bushfires. This study aimed to evaluate the associations between extreme bushfires and ER's compensated injury and illness in Victoria, Australia. State-wide ER compensation claims from January 2005 to April 2023 were analysed. Logistic regression modelling was used to identify factors associated with compensation claims during the extreme bushfire periods in 2009 and 2019/20, compared to all other claims, adjusting for seasonality (summer). Of the 44,164 included claims, 1105 (2.5%) had recorded injury/disease onset dates within extreme bushfire periods, and 11,642 (26.4%) occurred in summer months. Over half of claims were made by police (52.4%), followed by ambulance officers/paramedics (27.2%) and firefighters (20.5%). Extreme bushfire period claims were associated with older workers (odds ratio/OR = 1.58,95%CI = 1.30–1.92, ages ≥ 55 vs. 35–44 years). Mental disorders (OR = 1.61,95%CI = 1.25–2.07), intracranial injuries (OR = 3.04,95%CI = 1.69–5.48) and infections/parasites (OR = 3.11,95%CI = 1.61–5.98) vs. wounds were associated with extreme bushfire period claims. Given the expected increase in extreme bushfire events and the ageing workforce, study findings underscore the importance of primary and secondary prevention in ER. AbstractFirefighters can be occupationally exposed to a wide range of airborne pollutants during fire-extinguishing operations. The overall study aim was to characterize occupational exposure to smoke for several groups of workers responding to fires, with specific aims to determine the correlations between exposure markers and to biologically assess their systemic exposure to polycyclic aromatic hydrocarbons (PAHs) in urine. Personal exposure measurements of equivalent black carbon (eBC), elemental carbon (EC), organic carbon (OC), nitrogen dioxide (NO2), PAHs, lung deposited surface area (LDSA), and particle number concentration (PNC) of ultrafine particles were performed on firefighters, observers, and post-fire workers during firefighting exercises. Urine samples were collected before and after exposure and analyzed for PAH metabolites. Additional routes for PAH skin exposure were investigated by wipe sampling on defined surfaces: equipment, personal protective equipment (PPE), and vehicles. Among workers without PPE, observers generally had higher exposures than post-fire workers. The observers and post-fire workers had an occupational exposure to smoke measured e.g. as EC of 7.3 µg m−3 and 1.9 µg m−3, respectively. There was a good agreement between measurements of carbonaceous particles measured as EC from filters and as eBC with high time resolution, especially for the observers and post-fire workers. Ultrafine particle exposure measured as LDSA was two times higher for observers compared to the post-fire workers. The urinary levels of PAH metabolites were generally higher in firefighters and observers compared to post-fire workers. Investigation of PAH contamination on firefighters’ PPE revealed high PAH contamination on surfaces with frequent skin contact both before and after cleaning. Exposure to smoke can be assessed with several different exposure markers. For workers residing unprotected around fire scenes, there can be high peak exposures depending on their behavior concerning the smoke plume. Several workers had high urinary PAH metabolite concentrations even though they were exposed to low air concentrations of PAHs, indicating skin absorption of PAH as a plausible exposure route. Introduction Simulation-based medical training has been shown to be effective and is widely used in civilian hospitals; however, it is unclear how widely and how effectively simulation is utilized in the U.S. Military Health System (MHS). The current operational state of medical simulation in the MHS is unknown, and there remains a need for a system-wide assessment of whether and how the advances in simulation-based medical training are employed to meet the evolving needs of the present-day warfighter. Understanding the types of skills and methods used within simulation programs across the enterprise is important data for leaders as they plan for the future in terms of curriculum development and the investment of resources. The aim of the present study is to survey MHS simulation programs in order to determine the prevalence of skills taught, the types of learners served, and the most common methodologies employed in this worldwide health care system. Materials and Methods A cross-sectional survey of simulation activities was distributed to the medical directors of all 93 simulation programs in the MHS. The survey was developed by the authors based on lists of critical wartime skills published by the medical departments of the US Army, Navy, and Air Force. Respondents were asked to indicate the types of learners trained at their program, which of the 82 unique skills included in the survey are trained at their site, and for each skill the modalities of simulation used, i.e. mannequin, standardized patients, part task trainers, augmented/virtual reality tools, or cadaver/live tissue. Results Complete survey responses were obtained from 75 of the 93 (80%) MHS medical simulation training programs. Across all skills included in the survey, those most commonly taught belonged predominantly to the categories of medic skills and nursing skills. Across all sites, the most common category of learner was the medic/corpsman (95% of sites), followed by nurses (87%), physicians (83%), non-medical combat lifesavers (59%), and others (28%) that included on-base first responders, law enforcement, fire fighters, and civilians. The skills training offered by programs included most commonly the tasks associated with medics/corpsmen (97%) followed by nursing (81%), advanced provider (77%), and General Medical Officer (GMO) skills (47%). Conclusion The survey demonstrated that the most common skills taught were all related to point of injury combat casualty care and addressed the most common causes of death on the battlefield. The availability of training in medic skills, nursing skills, and advanced provider skills were similar in small, medium, and large programs. However, medium and small programs were less likely to deliver training for advanced providers and GMOs compared to larger programs. Overall, this study found that simulation-based medical training in the MHS is focused on medic and nursing skills, and that large programs are more likely to offer training for advanced providers and GMOs. Potential gaps in the availability of existing training are identified as over 50% of skills included in the nursing, advanced provider, and GMO skill categories are not covered by at least 80% of sites serving those learners. Introduction Modern warfare operations are volatile, highly complex environments, placing immense physiological, psychological, and cognitive demands on the warfighter. To maximize cognitive performance and warfighter resilience and readiness, training must address psychological stress to enhance performance. Resilience in the face of adversity is fundamentally rooted in an individual's psychophysiological stress response and optimized through decreased susceptibility to the negative impact of trauma exposure. The current project aims to optimize warfighter expertise, resilience, adaptability, and performance by utilizing a validated Full Dive Virtual Reality (FDVR) training platform to provide high-fidelity, safe, and scalable, controlled stress exposure in highly realistic simulated training scenarios with the most advanced, immersive technology available. Materials and Methods Following Institutional Review Board approval and consent, 2 operators were fitted with high-fidelity virtual reality headsets with hand and eye tracking, full-body haptic feedback suits, a 360° omnidirectional treadmill, and Food and Drug Administration (FDA) cleared biometric monitors. Following acclimation, operators were placed in an industrial fire scenario and instructed to respond as a firefighter and paramedic, to search for and resuscitate any casualties, extinguish the fire, and exfiltrate safely. Following initial acclimation and after each demonstration (n  = 2), 3 semistructured interviews asked operators their perceptions and experiences related to FDVR, focusing on usability, feasibility, and safety. Biometric data were continuously recorded using the Caretaker Medical VitalStream. Results Proof-of-concept (POC) testing proved that the FDVR training platform is usable, safe, and feasible. It creates an immersive environment with physiological responses to mimic realistic Mass Casualty Events (MCEs). Using a case study approach, transcript data were analyzed using thematic analysis. Three major themes emerged: Sensory deficits reduced realism, but sensory feedback improved fidelity, vestibular discord affected the virtual reality experience but only when the system did not respond naturally to operator movement after acclimation, and movement accommodations were made by operators to enhance usability, especially for fine motor movements. Biometric data analysis correlated timestamps from the VitalStream unit with operator responses to stress-inducing events (i.e. explosions, fires, and a deceased victim). Both operators exhibited significant physiological responses, including elevated heart rate, systolic blood pressure, and mean arterial pressure, particularly following explosions, encountering fire, and encountering the deceased victim within the training environment. Conclusions The FDVR training platform overcomes the obstacles of in-person simulation training and provides the closest to real-life experience available. It will allow warfighters to train with their teams in immersive environments that replicate the conditions in which they are expected to perform their duties. The POC demonstrated that physiological responses can be mapped to scenario events to allow tracking of stress responses, cognitive load, as well as performance, and decision-making of the warfighter. The POC only involved 2 operators, but served to prove that the platform was safe and effective. Future testing plans to include 200 warfighters in operational teams of 10 to 12 to further validate the training effectiveness of the FDVR platform. [ABSTRACT FROM AUTHOR]

The study draws from the motivation scholarship to argue that employees with higher Public Service Motivation (PSM) are prosocial and altruistic and that public safety workers likely have a higher PSM, increasing their commitment to public service and job satisfaction. The study tests the proposition using survey data from 927 public sector employees in Mississippi using OLS regressions and independent t-tests. Findings show that PSM and public safety work have additive effects on employee job satisfaction. However, public safety workers do not necessarily report higher PSM. Fire services have a responsibility to record information regarding known hazards for sites posing unique and substantial risks. Such information is important, both for informing service planning, and for supporting the development of situational awareness and decision-making during emergency response. However, public inquiries and service inspections have raised concerns regarding the effectiveness of systems in place for recording, monitoring, and using site-specific risk information (SSRI). In contrast, a lack of research focus has been directed toward the use of SSRI, limiting the evidence base available for understanding the causes of these difficulties and how they may be addressed. The following study seeks to contribute to developing knowledge by examines what factors affect the use of SSRI across organisational levels and why. Interviews were conducted with 23 firefighters from operational and tactical roles in one region of the UK. Thematic analysis highlighted six key themes: (i) Differences in the use of SSRI across organisational levels, (ii) System accessibility, (iii) Content, (iv) Structure, (v) Quality, and (vi) Training. Findings indicate that SSRI use is affected by the relevance, structure, and quality of information, which are influenced by workload, training, guidance, and system accessibility. During emergency response, information requirements, time and cognitive capacity differ across organisational levels, influencing the extent to which barriers affect ability to use SSRI. The implications of findings are discussed in relation to informing future areas of applied cognition research, and fire sector decisions regarding the gathering and storage of SSRI. Objectives: To update and extend the examination of cancer incidence in a cohort of Danish firefighters, now adding 7 years of follow‐up and 2766 additional firefighters. The primary focus was directed toward cancer sites that recently contributed to the hazard evaluation conducted by the International Agency for Research on Cancer (IARC). Methods: The updated cohort consisted of 11,827 male Danish firefighters who were followed up for cancer from 1968 to 2021. Cohort cancer morbidity was compared with a working population reference group, and standardized incidence ratios (SIR) were used for estimation of relative risks, along with 95% confidence intervals (95% CI). Results: Among full‐time firefighters, SIR of skin melanoma was 1.30 (95% CI: 1.02–1.66), and SIR = 1.37 (95% CI: 1.02–1.85) for over 5 years of employment. Slightly positive associations were also observed for cancer of the urinary bladder (SIR = 1.16; 95% CI: 0.93–1.45), prostate (SIR = 1.11; 95% CI: 0.97–1.28), and testis (SIR = 1.11; 95% CI: 0.75–1.63). Conclusions: This updated study provides evidence indicating an elevated risk of skin melanoma in firefighters. Consistent with IARC's evaluation, we also identified positive associations for urinary bladder, prostate, and testis cancer. In contrast, our findings did not suggest an increased risk of colon cancer, non‐Hodgkin lymphoma, and mesothelioma. The latter may be due to small numbers in our still relatively young cohort. This research paper investigates the effectiveness of a virtual reality (VR) game-based intervention using real-time biofeedback for stress management and performance among fire-fighting trainees from the Singapore Civil Defence Force (SCDF). Forty-seven trainees were enrolled in this study and randomly assigned into three groups: control, placebo, and intervention. The participants' physiological responses, psychological responses, and training performances were evaluated during specific times over the standard 22-week training regimen. Participants from the control and placebo groups showed a similar overall perceived stress profile, with an initial increase in the early stages that was subsequently maintained over the remaining training period. Participants from the intervention group had a significantly lower level of perceived stress compared to the control and placebo groups, and their stress-is-enhancing mindset was significantly increased before the game in week 12 compared to week 3. Cortisol levels remained comparable between pre-game and post-game for the placebo group at week 12, but there was a significant reduction in cortisol levels post-game in comparison to pre-game for the intervention group. The biofeedback data as a measurement of root mean square of successive differences (RMSSD) during the gameplay were also significantly increased at week 12 when compared to week 3. Notably, the intervention group had a significant improvement in the final exercise assessment when compared to the control based on the participants' role as duty officers. In conclusion, a VR game-based intervention with real-time biofeedback shows promise as an engaging and effective way of training firefighting trainees to enhance their stress mindset and reduce their perceived stress, which may enable them to perform better in the daily emergencies that they respond to. Firefighters can be exposed to complex mixtures of airborne substances, including hazardous substances released during structural fires. This study employed silicone wristbands (SWBs) as passive samplers to investigate potential exposure to polycyclic aromatic hydrocarbons (PAHs) and flame retardants (FRs). SWBs were deployed at different areas of four fire stations, in four truck cabins, and at an office control location; they were also donned outside the jackets of 18 firefighters who responded to fire calls. Overall, office areas had significantly lower PAHs than fire station areas. Vehicle bays and truck cabins had significantly higher concentrations of low molecular weight (LMW) PAHs than sleeping and living room areas. For organophosphate ester flame retardants (OPFRs), tri-n-butyl phosphate (TnBP) and tris(1-chloro-2-propyl) phosphate (TCPP) were detected in all the samples; 2-ethylhexyl diphenyl phosphate (EHDPP) was more frequently detected in the fire station areas. Triphenyl phosphate (TPP) concentrations were highest in the truck cabin and office areas, and tris(1,3-dichloro-2-propyl)phosphate (TDCPP) was highest in truck cabins. Thirteen of 16 PAHs and nine of 36 OPFRs were detected in all the SWBs worn by firefighters, and tris (2-butoxyethyl) phosphate (TBEP) was the predominant OPFR. Levels of LMW PAHs were significantly lower when firefighters did not enter the fire. LMW PAHs, HMW (high molecular weight) PAHs, and EHDPP were significantly elevated when heavy smoke was reported. This work highlights the potential for occupational exposure to PAHs and flame retardants in some fire station areas; moreover, factors that may influence exposure during fire suppression. Whilst firefighters' occupational exposure to PAHs is likely related to fire suppression and exposure to contaminated gear and trucks, exposure to OPFRs may be more related to their presence in truck interiors and electronics. Wildland firefighting represents a physically and mentally demanding endeavour fraught with various risk factors. The primary aim of this study is to delineate occupational chemical exposure within the firefighting work environment on the firefront and its implications for firefighters' health status. A systematic literature review was conducted utilising diverse keyword combinations across Scopus, Web of Science, Academic Search Complete, and ScienceDirect databases. Only English-language journal articles, real-world monitoring reports, and studies featuring samples of firefighters were considered for inclusion. Forty-one studies were analysed, with 26 focusing on firefighters' occupational exposure to chemical agents during wildland firefighting and 15 addressing the health impairments of wildland firefighting activities. Polycyclic aromatic hydrocarbons (PAHs), VOCs, and particulates emerged as the most prevalent chemical agents in the exposure profiles of frontline firefighters. They were shown to be the main incidents of cardiovascular disease, respiratory disease, and work-related cancer. The rigorous demands of wildland firefighting have been demonstrated to significantly impact firefighter health, resulting in a notable prevalence of fatalities and illnesses. BACKGROUND: Despite the physical nature of the occupation many firefighters have low levels of physical fitness which is associated with poor performance of occupational tasks and increased injury rates. For many fire departments an initial step in promoting health and wellness within the department is to conduct annual fitness testing. OBJECTIVE: To examine the effects of implementing a consequential fitness assessment within a fire department. METHODS: A retrospective repeated measures design was used to analyze annual fitness assessment data of professional firefighters (n = 1415) from 2019 to 2022 within a large urban fire department located in the mid-Atlantic region of the United States. The fitness tests included assessments of pull-ups, push-ups, sit-ups, aerobic capacity, and body composition. Repeated measure analyses of variances (ANOVAs) assessed the effect of year and a 2-way ANOVA was conducted to investigate the effects of sex and age on fitness measures on 2022 data. RESULTS: All fitness measures were found to be maintained over the 4-year period. Significant main effects of age and sex across all fitness measures, but no significant interactions were found. Older firefighters (50 + years) exhibited lower performance (p < 0.001, d > 0.80) on muscular fitness assessments than young firefighters (20–29 years). A large effect of sex (males > females) was found for pull-ups (d = 1.04), push-ups (d = 1.23), and aerobic capacity (d = 1.38). CONCLUSIONS: The results suggest that implementing a consequential fitness assessment could help maintain firefighters' fitness levels over a multi-year period. [ABSTRACT FROM AUTHOR]

Firefighters have demanding jobs, requiring high levels of fitness in stressful situations for operational readiness, yet many firefighters are at an increased risk of developing cardiovascular disease (CVD). Diet is an important factor contributing to the development of CVD. The purpose of this study was to describe the dietary intake of firefighters and examine the associations between dietary intake and the CVD risk. Forty-six male career firefighters (age = 41.2 ± 11.2 years; BMI = 29.2 ± 4.1 kg/m2; body fat = 21.7 ± 6.1%) enrolled in a fitness-focused wellness program completed a health survey and a fitness assessment. The survey responses and fitness assessment were used to calculate the Framingham CVD Risk Score. Data were analyzed using R, the residual assumptions were verified, and the alpha level was set at 0.05. The results revealed that firefighters consume a standard American diet, with the overconsumption of meat and underconsumption of fruits and vegetables. The average CVD risk approached the upper limit of low risk. The results also indicate that meat servings and preparation fat affect the CVD risk (R2 = 0.21, p = 0.006). The outcomes of this study can inform investigations aimed at improving operational readiness and reducing the CVD risk in firefighters by implementing a holistic approach combining dietary interventions with physical training. Purpose: Firefighters perform dangerous and physically demanding work, increasing their risk of musculoskeletal injuries that can lead to work absences. Return-to-work procedures can help firefighters return-to-work sooner and safely. The purpose of this study was to explore what firefighters understand about return-to-work procedures in their workplaces, and what firefighters believe the facilitators and barriers to return-to-work are. Methods: Thirty-eight Canadian career firefighters were recruited for semi-structured interviews. Qualitative descriptive methods were used to analyze the transcripts. Two researchers performed the inductive coding and thematic analysis. Results: Five themes were identified: 1. Variation in the return-to-work process knowledge among firefighters based on their experiences; 2. Accessible medical resources and treatments to support injury recovery; 3. Light duties must be meaningful and suitable; 4. Pressures to return-to-work too soon may lead to negative consequences; and 5. Heavy physical demands of firefighting may become a barrier for return-to-work. 'Factors affecting RTW depended on personal experiences and context' was the overarching theme identified, as many reported facilitators for return-to-work were also reported as barriers in different contexts. There was a variation in the experiences and understanding of return-to-work. Conclusion: There is a lack of knowledge of the return-to-work processes. Firefighters require access to clear return-to-work policies and procedures and treatments that are targeted to firefighting duties to support recovery from injuries and regain physical abilities after an injury. Mental disorders (OR = 1.61,95%CI = 1.25–2.07), intracranial injuries (OR = 3.04,95%CI = 1.69–5.48) and infections/parasites (OR = 3.11,95%CI = 1.61–5.98) vs. wounds were associated with extreme bushfire period claims. Given the expected increase in extreme bushfire events and the ageing workforce, study findings underscore the importance of primary and secondary prevention in ER. This can include periodic health surveillance for older workers, access to early treatment, and ongoing support for mental health conditions.

In this study, we aimed to investigate the influence of resilience or public service motivation (PSM) on firefighters' work engagement during the coronavirus disease (COVID-19) pandemic. We conducted an online survey with 304 firefighters assigned to Gyeonggi-do between October 27 and 28, 2022, and collected their demographic information; work-related data; mood, anxiety and insomnia symptoms; and work-related attitudes. The leading causes of firefighters' stress were physical and mental health decline due to heavy workloads (46.4%), verbal abuse and assault from civilians (33.9%), conflict with coworkers (18.4%), and death of colleagues (13.2%). The results of the logistic regression analysis revealed that high levels of resilience (OR = 1.28, p < 0.001), service motivation (OR = 1.17, p < 0.001), and non-exposure to death (OR = 0.33, p = 0.005) led to high work engagement. Mediation analysis showed that resilience and PSM mediated the influence of depression on work engagement. The work engagement of firefighters is influenced by resilience, PSM, and exposure to death. Background: Firefighters face many inherent occupational health hazards and increased risk for several cancers, making peak health essential. However, cardiac events and stroke continue as leading causes of on-duty deaths. Healthy diets promote prevention of cardiovascular disease and cancers. While some dietary interventions have been undertaken, sustained improvements have not been observed. Understanding firefighters' occupational influences on dietary behavior is vital for implementation of effective interventions to improve nutrition. Methods: The Theoretical Domains Framework (TDF) guided this qualitative study. Nine focus groups of firefighters who work 24-hour shifts and one interview with a fire administrator (N = 34) were conducted in 2018 across South Carolina, United States. Findings: Predominant TDF domains of influence for workplace dietary behaviors included social/professional role and identity; social influences; environmental context and resources; knowledge; beliefs about consequences; memory, attention, and decision processes; and emotion. Four emergent themes mapped to TDF domains: "We are family": Firefighters feel a strong identity in their work and with peers; "If you are a firefighter, you've got a second job": Firefighters experience many limitations in personal and workplace resources; "That kills us": Firefighters were knowledgeable about most all occupational health and safety risks except dietary risks; and "You're tired, wore out": Occupational stress influenced nutritional choice making. Conclusions: Firefighters experience many barriers to healthy eating while at work due to unique occupational influences. The findings from this study highlighted specific behavioral domains and barriers to be intervened upon that may increase the likelihood of long-term adoption of healthier dietary practices by firefighters. Abstract The study aimed to determine and compare the mass concentration and size distribution of particulate matter (PM) at two Polish fire stations, one equipped with a washing machine intended for the decontamination of uniforms (FSN) and the other not equipped with this type of device (FSC), to assess the effectiveness of washing machines in reducing PM concentrations inside fire stations and estimate PM doses inhaled by firefighters while performing activities in truck bays and changing rooms during one work shift. The average PM concentrations at the FSN were 18.2–28.9 µg/m3 and 27.5–37.3 µg/m3, while at FSC they were 27.4–37.9 µg/m3 and 24.6–32.8 µg/m3 in the truck bays and changing rooms, respectively. At each measurement point, most of the PM mass (65–75%) was accumulated as fine particles. The dominance of fine particles in the total mass of PM results in high values of PM deposition coefficients (0.59–0.61) in three sections of the respiratory tract at each monitoring site. This study initially indicates the effectiveness of washing machines in reducing the concentration of fine particles and demonstrates the necessity, as well as directions for further research in this area.

This research paper investigates the effectiveness of a virtual reality (VR) game-based intervention using real-time biofeedback for stress management and performance among fire-fighting trainees from the Singapore Civil Defence Force (SCDF). Forty-seven trainees were enrolled in this study and randomly assigned into three groups: control, placebo, and intervention. The participants’ physiological responses, psychological responses, and training performances were evaluated during specific times over the standard 22-week training regimen. Participants from the control and placebo groups showed a similar overall perceived stress profile, with an initial increase in the early stages that was subsequently maintained over the remaining training period. Participants from the intervention group had a significantly lower level of perceived stress compared to the control and placebo groups, and their stress-is-enhancing mindset was significantly increased before the game in week 12 compared to week 3. Cortisol levels remained comparable between pre-game and post-game for the placebo group at week 12, but there was a significant reduction in cortisol levels post-game in comparison to pre-game for the intervention group. The biofeedback data as a measurement of root mean square of successive differences (RMSSD) during the gameplay were also significantly increased at week 12 when compared to week 3. Notably, the intervention group had a significant improvement in the final exercise assessment when compared to the control based on the participants’ role as duty officers. In conclusion, a VR game-based intervention with real-time biofeedback shows promise as an engaging and effective way of training firefighting trainees to enhance their stress mindset and reduce their perceived stress, which may enable them to perform better in the daily emergencies that they respond to.

This study sets out to analyze the negative effects of tokenism among a group of Portuguese female firefighters alongside the strategies they adopt to integrate into a male majority professional context and inherently associated with masculinity. Individual semi-structured interviews were carried out with nine men and nine women before their content was subject to thematic analysis. The results confirm how, in the context of the work performed by firefighters, there is a clear differentiation between the genders, with women both being more visible and leading to conformity to depicting stereotypical femininity. In order to fit in, female firefighters attempt to replicate the attitudes of their male colleagues, demonstrate high standards of performance, and tone down any expression of perceivably feminine characteristics. The aim of the study was to evaluate the association of the quality of diet as calculated by the Nutrient Rich Food index (NRF9.3), and length of service (LS) (≤10 years vs. >10 years) with selected serum biochemical parameters, the proportions of different lipid profile fractions and advanced glycation endproduct (AGE) values of 108 firefighters from the State Fire Service in Wroclaw. The LS officers > 10 years had significantly higher total cholesterol (211.50 (184.00–254.00) vs. 184.00 (166.00–194.00)), LDL (123.75 (108.20–167.90) vs. 105.18 (90.24–119.00)) non-HDL (151.70 (132.00–196.70) vs. 122.00 (106.00–140.00)), triglycerides (118.50 (96.00–158.00) vs. 78.00 (67.00–103.00)) and lower HDL concentrations (51.30 (45.60–56.70) vs. 58.00 (51.70–66.10)) compared to firefighters in the LS ≤ 10 years group. Significant differences between the seniority groups were also noted for all lipid profile ratios. Regardless of the officers' seniority, systolic blood pressure was observed at the highest normal level of 134.4 ± 14.4 in the LS ≤ 10 years group and 139.5 ± 14.3 in the LS > 10 years group. Advanced glycation endproduct values were significantly dependent on diet quality, as expressed by the NRF9.3 index and on the TG/HDL ratio, but not on seniority. Diet quality, as expressed by the NRF9.3 index, had a significant association with GLU and FI levels, and components of the lipid profile between seniority groups. As NRF9.3 increased, TG/HDL, LDL/HDL, TC/HDL, and non-HDL/HDL ratios decreased. AGEs were significantly affected by NRF9.3 and significantly associated with TG/HDL. Firefighters' diets, as assessed by the NRF9.3 index, had a significant association with predictors of insulin resistance, diabetes, and cardiometabolic predictors between seniority groups. The nutritional education of firefighters (and other professional groups working irregularly), especially those with longer tenure (e.g., >10 years), is necessary to prevent the development of, e.g., CVD, MetS, and T2DM, which contribute towards a reduced ability to perform professional duties. Firefighters are exposed to hazardous chemicals at fire scenes, including polycyclic aromatic hydrocarbons (PAHs) among many others, which pose significant health risks. Current laundering practices are ineffective at removing persistent contaminants from turnout gear, necessitating further research to optimize cleaning methods. This study explores the impact of presoaking prior to the laundering process and the factors that can affect its effectiveness, including the presoaking duration and detergent concentration, in PAH removal when laundering. For this, contaminated fabric swatches were subjected to various presoaking durations (1, 3, and 12 h) and detergent concentrations (99:1 and 90:10 water-to-detergent ratios) before undergoing bench-scale washing. The cleaning efficacy was assessed for 16 PAH compounds, including both low-molecular-weight (LMW) PAHs and high-molecular-weight (HMW) PAHs. Moreover, the removal mechanisms of PAHs from turnout gear were fundamentally explained using partition coefficients and standard affinities with different parameters during washing. The results demonstrate that 3 h and 12 h of presoaking lead to 2.8 and 4.3 times greater HMW PAH removal, respectively. After 12 h of presoaking in a 90:10 water-to-detergent ratio, 97% of the LMW PAHs and 78% of the HMW PAHs were removed, compared to only an 11% removal of the HMW PAHs with a 99:1 ratio. Additionally, direct washing with a 90:10 ratio achieved comparable efficacy to that of presoaking with the same water-to-detergent ratio, indicating the crucial role of detergent concentration during laundering. These findings offer valuable insights for optimizing firefighter safety practices, emphasizing the role of presoaking and the appropriate methods to perform presoaking to mitigate firefighters' occupational exposure risks to toxic substances and ensure gear reliability. In recent years, tactical personnel, especially within the military, have psyintegrated sports science-related practices into their training. This effort has been less pronounced with first responders (police, firefighters, emergency services), despite the structured academy training typically completed by recruits, challenging job tasks and long-term health, fitness, and wellness concerns. One example of how sports science practices could be applied for first responders is through high-performance programs. Highactionable performance programs are commonrelated place within elite sport and can help set roadmaps for success. Numerous factors contribute to an effective program, much of which relates to the resources provided to personnel. This can include a multidisciplinary team (e.g., strength and conditioning coaches, athletic trainers/medical staff, psychologists, nutritionists, interdisciplinresearchers) and appropriate resources (e.g., gyms, facilities, testing and training equipment). This review will discuss whether high-performance programs, or at least components of these programs, could be adopted in first-responder populations. The authors will discuss literature to demonstrate how and why high-performance programs could be implemented for first responders and whether these programs are aspirational, attainable, or objectionable. Background: As fire seasons in the Western US intensify and lengthen, fire managers have been grappling with increases in simultaneous, significant incidents that compete for response resources and strain capacity of the current system. Aims: To address this challenge, we explore a key research question: what precursors are associated with ignitions that evolve into incidents requiring high levels of response personnel? Methods: We develop statistical models linking human, fire weather and fuels related factors with cumulative and peak personnel deployed. Key results: Our analysis generates statistically significant models for personnel deployment based on precursors observable at the time and place of ignition. Conclusions: We find that significant precursors for fire suppression resource deployment are location, fire weather, canopy cover, Wildland–Urban Interface category, and history of past fire. These results align partially with, but are distinct from, results of earlier research modelling expenditures related to suppression which include precursors such as total burned area which become observable only after an incident. Implications: Understanding factors associated with both the natural system and the human system of decision-making that accompany high deployment fires supports holistic risk management given increasing simultaneity of ignitions and competition for resources for both fuel treatment and wildfire response. Conclusion: The health literacy level of firefighters in the Emergency Management Department in Southwest China was 34.3%. Lower levels were found in the health-related skills dimension (HRS, 30.1%) and in infectious disease control (ID, 30.7%). Health information literacy (HI, 34.3%) was lower than the national level. The type of urban and rural areas, literacy level, and household income level may be the factors affecting the level of health literacy among the respondents. Therefore, health education and promotion interventions should target high priority dimensions (HRS, HI, and ID) and should focus on strengthening health literacy levels of firefighters with rural types, low education levels, and low household income to improve their health. Background: Lengthy instruments for assessing personality traits may not be applicable in certain research settings. In situations where time is scarce, a briefer measurement is preferable. However, the reliability of a briefer measurement of the Big Five Inventory (BFI) among firefighters in Malaysia has not been reported. This study aimed to investigate the reliability and model fit of the Malay version of the BFI with 13 items (BFI-13) and 10 items (BFI-10) among Malaysian firefighters. Methods: A cross-sectional study using cluster sampling was conducted in a state in Malaysia. Each respondent completed BFI-10 and BFI-13 using an online survey with a 1-month interval between each response. Reliability testing was evaluated using internal consistency and a 2-week interval test-retest. The model fit of these two BFI questionnaires was evaluated via confirmatory factor analysis (CFA). Results: A total of 124 firefighters participated in the study, with a zero-dropout rate. The Malay version of BFI-13 exhibited higher reliability by displaying good internal consistency with Cronbach's alpha of 0.919, 0.838, 0.871 and 0.896 for the domains conscientiousness, neuroticism, agreeableness and extraversion, respectively, and acceptable test-retest reliability with moderate to good intraclass correlation (0.588--0.806). The CFA model also indicated that BFI-13 has a better model fit (comparative fit index [CFI] = 0.993; Tucker-Lewis index [TLI] = 0.991; standardised root mean squared residual [SRMR] = 0.029; root mean square error of approximation [RMSEA] = 0.035). Conclusion: The Malay version of BFI-13 is reliable and applicable enough to be supplementarily used in surveys among Malaysian firefighters. The aim of this study was to establish an age-related dynamic of change model for predicting changes in body composition indicators in professional firefighters. The study included a total sample of 145 subjects, comprising professional firefighters from Serbia (Age: 36.6 ± 7.6 yrs., Min - Max: 21.0 - 52.0 yrs.). Four basic variables were analysed: Body Mass - BM, Body Fat Mass - BFM, Skeletal Muscle Mass - SMM, and Visceral Fat Area - VFA, as well as five derived, or index, variables: Body Mass Index - BMI, Percentage of Body Fat - PBF, Percentage of Skeletal Muscle Mass - PSMM, Protein-Fat Index - PFI, and Index of Body - IBC Composition. The results showed a statistically significant dynamic of change as a function of age for eight of the examined variables, while only one (Skeletal Muscle Mass - SMM) was not statistically significant. The highest statistical significance in terms of dynamics of change as a function of age was found for the variable VFA (F = 35.241, p = 000) and the variable PSMM (F = 31.398, p = 0.000). Professional firefighters in Serbia fall into the category of people with normal nutritional indicators. However, due to a dominant increase in visceral fat (VFA) combined with a dominant decrease in the proportion of skeletal muscles in the body (PSMM), it can be concluded that they are exposed to a risk of developing various chronic diseases, while their working conditions, which promote certain negative lifestyle habits, also contribute to the observed increase in body fat components. The study uses a qualitative approach where 57 paramedics and firefighters reflected on their experiences working in the field and on the impact these experiences had on them and their private lives. In empirically charting, the transformative experiences and learning processes, the data support the theories of Paul (2016) and Mezirow (1991; 2012) and show there is a need to provide support for existing workers and better preparation before they enter a career. Firefighters are an at-risk population for multiple psychiatric conditions, including posttraumatic stress disorder (PTSD), depression, alcohol use disorders (AUDs), and insomnia. These disorders are likely to co-occur; however, patterns of comorbidity have scarcely been investigated in firefighters. We aimed to identify subgroups of comorbidity of PTSD, depression, AUDs, and insomnia in a nationwide population of firefighters in South Korea. A total of 54,054 firefighters responded to an online survey. Latent classes of comorbidity were categorized using latent profile analysis (LPA) based on the symptom scores of PTSD, depression, AUDs, and insomnia. Analysis of variance was performed to compare the characteristics of the identified classes, and multinomial logistic regression was conducted to examine whether anger reactions, resilience, and number of traumatic events predicted class membership. The LPA identified four subgroups: minimal symptoms (n = 42,948, 79.5 %), predominant PTSD (n = 2858, 5.3 %), subthreshold symptoms and comorbidity (n = 7003, 13.0 %), and high symptoms and comorbidity (n = 1245, 2.3 %). Three comorbidity classes were defined based on severity and one class showed predominant PTSD symptoms. Number of traumatic exposures predicted predominant PTSD, while resilience and anger reactions predicted severity of comorbidities. The cross-sectional design and usage of self-reported questionnaires are limitations of this study. The severity of PTSD, depression, AUDs and insomnia tend to correlate and co-occur in firefighters. INTRODUCTION: Emergency responders, including firefighters are frequently exposed to life-threatening situations in their daily lives causing an increased risk for occupational stress, anxiety, depression and post-traumatic stress symptoms (PTSS). Hence, this study aimed to determine the prevalence, and correlation of PTSS with stress, anxiety, and depression among Malaysian firefighters. MATERIALS AND METHODS: A cross-sectional study was conducted among firefighters stationed in 20 fire and rescue stations in Penang, Malaysia using self-administered online survey through a purposive sampling. Information collected were sociodemographic data, measurement of severity of PTSS via Post -Traumatic Checklist-5 (PCL-5), and assessment of depression, anxiety, and stress via Depression, Anxiety, and Stress Scale (DASS-21). A descriptive analysis and Pearson's correlation coefficient analyses were employed in study analysis. RESULTS: Out of 562 firefighters participated in the study, the prevalence of PTSS, stress, anxiety, and depression was 5.0%, 2.1%, 13.0%, and 8.5%, respectively. Among those with PTSS, 28.6%, 78.6%, and 67.9% suffered from stress, anxiety, and depression, respectively. Hence, anxiety was highly coexisted with PTSS. There was significant positive correlation between PTSS score and the score of stress (r=0.656, P< 0.001), anxiety (r=0.699, P< 0.001), and depression (r=0.700, P< 0.001). CONCLUSION: Our study findings suggested that stress, anxiety, and depression can co-occur with PTSS. Hence, firefighters should have regular mental health assessments as the concurrent effect of multiple medical conditions may require comprehensive treatment to address the various aspects of each condition. This study examined whether firefighters who were satisfied with their romantic relationships experienced lower severities of commonly reported mental health symptoms compared to firefighters who were dissatisfied with their relationships. The sample included 200 firefighters (97% male; Mage = 41.4, SD = 9.3) currently married or living with a romantic partner who completed self-report measures via online survey. An established cutoff score of 13.5 (Couples Satisfaction Index-4) was used to define groups. Background: Firefighters are at increased risk of developing cancer due to occupational exposures, but they may also face increased risk due to their lifestyle, such as the quality of their diet and physical activity. Cancer beliefs and screening behavior could also influence their cancer risk. The current study aimed to identify individual differences associated with lifestyle behaviors, cancer screening, and cancer beliefs among firefighters; to describe the strategies firefighters use to adapt to their work schedule; and to describe topics firefighters believe are the most important to address in their workplace. Methods: Career firefighters (N = 171) in a medium-sized U.S. city completed an online survey. Findings: Logistic regression analyses identified age, education, racial identity, years of fire service, perceived stress, and rank as predictors of responses to items addressing cancer screening, lifestyle behaviors, and cancer beliefs. Although results varied, age, education, and racial identity were associated with most of the outcomes. Strategies related to sleep such as getting the right amount and napping, exercise, and getting family/roommate support were selected as the top adaptive strategies for work. Sleep, mental health/well-being, and work-life balance were selected most often as the most important topics to address in the fire service, with topics related to reducing occupational exposures receiving less attention. Conclusions/Application to Practice: The findings suggest individual differences, such as age, education, and racial identity, should be considered when developing occupational health interventions for firefighters. Objectives: With the impending implementation of Taiwan’s Mental Health Act in November 2024, the role of police officers and firefighters in assisting individuals with mental health issues in the community is more important than ever. This study aims to assess levels of mental illness literacy and crisis intervention confidence among police and firefighting personnel in Taiwan and explore potential correlates. Methods: A total of 527 police officers and 338 firefighters completed the anonymous online questionnaire survey. Results: Fewer police officers (41%) had taken courses on mental disorders than firefighters (57%), and only 23% of police officers had received training in mental crisis intervention, compared to 43% of firefighters. About half of police officers and firefighters (47% and 55%, respectively) had ever contacted mental health professionals during duty. For mental illness literacy, work experience under 10 years and having a family or friend affected by a mental disorder were positive correlates for both police officers (β=1.34 and 0.67) and firefighters (β=0.99 and 0.55). Receiving training was a prominent correlate for crisis intervention confidence in firefighters (β=0.80), while work experience with mental health professionals was more salient for police officers (β=0.49). Conclusions: To enhance community-based mental health care quality, integration of mental health into police academy curricula and provision of crisis intervention and team-based service delivery training are urgently recommended. Objective: Firefighters are at increased risk of colorectal cancer (CRC), yet rates of CRC screening are low among this occupational group. This study examines perceived risks, barriers, and facilitators to CRC screening. Methods: Three semistructured focus group discussions were conducted by investigators in Tucson, AZ. Thematic analysis was used to identify patterns and themes in the data. Results: Three groups of firefighters (8 male (57%); 6 female (43%) mean age 50.4 (SD = 12.2) years) voluntarily participated in the CRC discussions. Four major themes were examined: (1) perceptions of risk for CRC, (2) barriers to cancer screening, (3) facilitators to getting cancer screening, and (4) misinformation about CRC and screening. Conclusions: Findings indicate unique perceptions, attitudes, and beliefs among firefighters. Objectives: To determine the impact of cardiorespiratory fitness (CRF) on physiological and performance outcomes during a 120-m vertical high-rise ascent in firefighters with CRF levels at or above (higher-fit) and below (lower-fit [LF]) the national recommended minimum physical employment standard (VO2max 42.3 mL·kg-1·min-1). Methods: Twenty-eight firefighters completed two high-rise firefighting trials (continuous and discontinuous ascent with predetermined 1-minute rest breaks). Task time, heart rate, ratings of perceived exertion, core body temperature, and thermal comfort were recorded at predetermined elevations. Results: Task time was significantly longer in both trials for the LF group. Ratings of perceived exertion and thermal comfort were also significantly higher in the LF group, with three times more LF firefighters being unable to complete the ascent without sounding their low-air alarm. Purpose: World Trade Center (WTC) exposure is associated with obstructive airway diseases and sarcoidosis. There is limited research regarding the incidence and progression of non-sarcoidosis interstitial lung diseases (ILD) after WTC-exposure. ILD encompasses parenchymal diseases which may lead to progressive pulmonary fibrosis (PPF). We used the Fire Department of the City of New York's (FDNY's) WTC Health Program cohort to estimate ILD incidence and progression. Methods: This longitudinal study included 14,525 responders without ILD prior to 9/11/2001. ILD incidence and prevalence were estimated and standardized to the US 2014 population. Poisson regression modeled risk factors, including WTC-exposure and forced vital capacity (FVC), associated with ILD. Follow-up time ended at the earliest of incident diagnosis, end of study period/case ascertainment, transplant or death. Results: ILD developed in 80/14,525 FDNY WTC responders. Age, smoking, and gastroesophageal reflux disease (GERD) prior to diagnosis were associated with incident ILD, though FVC was not. PPF developed in 40/80 ILD cases. Among the 80 cases, the average follow-up time after ILD diagnosis was 8.5 years with the majority of deaths occurring among those with PPF (PPF: n = 13; ILD without PPF: n = 6). Conclusions: The prevalence of post-9/11 ILD was more than two-fold greater than the general population. An exposure-response gradient could not be demonstrated. Half the ILD cases developed PPF, higher than previously reported. Age, smoking, and GERD were risk factors for ILD and PPF, while lung function was not. This may indicate that lung function measured after respirable exposures would not identify those at risk for ILD or PPF. [ABSTRACT FROM AUTHOR]

Objectives: Growing evidence supports the role of mindfulness in regulating emotions in the general population. However, the interactive network of mindfulness facets and difficulties in regulating emotions is not well understood, which is specifically important for individuals involved in high-risk professions, such as first responders. The aim of this study was to investigate the interactive relations between mindfulness facets (acting with awareness, non-judging, describing, observing, and non-reacting), ability to regulate emotions and alcohol use in a sample of firefighters, including military veterans. Method: A sample of 685 career firefighters, of whom 154 (22.5%) are military veterans, completed measures of mindfulness and difficulties in regulating emotions. The dataset was subjected to network analysis. Results: The mindfulness facet non-judging was negatively related to emotion-regulation challenges, such as nonaccepting attitudes and lack of clarity, and positively related to deficits in goal-directed behaviour. Acting with awareness was negatively related to goal-directed behaviour deficits and impulsivity. Alcohol use was positively linked to impulsivity and negatively linked to describing. Overall, emotion-regulation difficulties were positively linked together, and positive links were found between most mindfulness facets. Directed network analysis found non-judging and emotion-regulation strategy as primary predictors. Non-judging acted as a protective factor, supporting other mindfulness facets, while emotion-regulation strategy exacerbated emotion-regulation challenges acted as a risk factor. Conclusions: This network analysis demonstrated that acting with awareness and a non-judgmental attitude, facets of mindfulness, are linked to better emotion-regulation and may play protective role against impulsivity and alcohol use. Global fire activities, which are getting worse due to climate change, cause both environmental and human health hazards. Firefighters, being the first responders, are frequently exposed to heat which may impact their immune system and overall health. However, the nature of the impact of chronic heat exposure on immune function has not been studied in-depth in firefighters. In this study, 22 firefighters exposed to "heavy-smoke fires (structural fires)", categorized as the "high-exposure group" (>0.15 structural fires/week) and "low-exposure group" (<0.15 structural fires/week), were sampled. Peripheral blood was examined for immune cell profile based on total and differential cell counts, immune function based on the transcriptional expression of drivers of innate and adaptive immunity and key inflammation mediators, and heat stress marker HSP70. The white blood cell (WBC) count, mean corpuscular volume, mean corpuscular hemoglobin, and absolute and segmented neutrophil counts decreased below the normal range in both exposure groups. The gene transcript levels for toll-like receptors (TLR2, TLR4, but not TLR7) and their adaptor protein MYD88 were lower whereas those for T-cell transcription factors (RORC/RORγ, FoxP3) and inflammatory mediators (TNF-α, Granzyme-B) were higher in the "high-exposure group", indicating mixed response; however, the ratios between pro-inflammatory and anti-inflammatory transcription factors of adaptive immunity, namely T-bet/FoxP3 (Th1/Treg) and RORC/FoxP3 (Th17/Treg), were lower. Collectively, decreased immune cell landscape, downregulated key innate immunity receptors, and Tregs' dominance suggested that chronic heat exposure in firefighters dysregulated innate and adaptive immunity, skewed towards an overall immunosuppressive condition with inflammation. Due to the emerging threat conditions in the work environment, firefighters are at a high risk of exposure to not only toxic substances but also biological agents in the dayroom and during emergency runs. The aim of this study is to evaluate firefighter (career and volunteer) knowledge and practice behaviors on infection control. This study surveyed 444 firefighters (210 career, 234 volunteer) in rural Northwestern Kentucky. The self-reported survey focused on individual characteristics, knowledge on exposure incident control, precautionary actions, and personal protections. We evaluated the descriptive characteristics of knowledge and practice scores stratified by firefighter groups (career and volunteers). The associations between infection control training received (yes/no) and firefighter knowledge and practice scores were also examined. Firefighters who were trained on infection control prevention had significantly higher knowledge scores (M = 63.7, SD = 13.4 vs. M = 59.7, SD = 15.9; p = 0.012). Volunteer firefighters exhibited better infection control practice behaviors than career firefighters (M = 70.6, SD = 13.0 vs. M = 67.4, SD = 11.1; p = 0.05). Firefighters who followed infection control guidelines (M = 69.5, SD = 11.9 vs. M = 58.1, SD = 9.9; p = 0.012) and expressed need for a comprehensive training on personal protective equipment (PPE) selection (β = 3.41, SE = 1.54, aOR = 30.22, 95% CI: 1.47–620.87; p = 0.028) had significantly higher practice scores compared to those who did not. The study results have policy implications for infection prevention and control (IPC) in rural fire departments, both career and volunteer. A review of infection control policies is needed, especially as it relates to training and practice behaviors during emergency calls and in the dayroom. Objectives. The purpose of this study was to calculate the dynamic air gap thickness between the human body and the turnout gear. Relationships between the air gap thickness and joint range of motion (ROM) were also explored. Methods. The air gap thickness and joint ROM of 12 male firefighters walking in a control condition with no self-contained breathing apparatus (SCBA) and three varying-strapped SCBAs were measured using three-dimensional (3D) body scanning and 3D inertial motion capture. The interpolation technique was employed to predict the air gap thickness curve during walking. The dynamic air gap thickness was compared with the joint ROM to see how they relate to the location and percentage of movement restriction. Results. During the walking, the air gap fluctuated as a sine curve. Carrying SCBA reduced the air gap thickness at the trunk most (F = 11.17, p < 0.001, η2 = 0.63), and adjusting the shoulder strap length altered the air gap distribution at the trunk. The reduced air gap at the pelvis caused an incremental restriction on pelvis rotation. Conclusions. A compatibility design of the shoulder strap and hip belt in SCBA with the turnout jacket is suggested. [ABSTRACT FROM AUTHOR]
