1. Use of Telemedicine for Chronic Liver Disease at a Single Care Center During the COVID-19 Pandemic: Prospective Observational Study

Abstract

BackgroundThe COVID-19 outbreak has overwhelmed and altered health care systems worldwide, with a substantial impact on patients with chronic diseases. The response strategy has involved implementing measures like social distancing, and care delivery modalities like telemedicine have been promoted to reduce the risk of transmission. ObjectiveThe aim of this study was to analyze the benefits of using telemedicine services for patients with chronic liver disease (CLD) at a tertiary care center in Italy during the COVID-19–mandated lockdown. MethodsFrom March 9 to May 3, 2020, a prospective observational study was conducted in the Liver Unit of the University Hospital of Naples Federico II to evaluate the impact of (1) a fully implemented telemedicine program, partially restructured in response to COVID-19 to include video consultations; (2) extended hours of operation for helpline services; and (3) smart-working from home to facilitate follow-up visits for patients with CLD while adhering to social distancing regulations. ResultsDuring the lockdown in Italy, almost 400 visits were conducted using telemedicine; only patients requiring urgent care were admitted to a non–COVID-19 ward of our hospital. Telemedicine services were implemented not only for follow-up visits but also to screen patients prior to hospital admission and to provide urgent evaluations during complications. Of the nearly 1700 patients with CLD who attended a follow-up visit at our Liver Unit, none contracted COVID-19, and there was no need to alter treatment schedules. ConclusionsTelemedicine was a useful tool for following up patients with CLD and for reducing the impact of the COVID-19 pandemic. This system of health care delivery was appreciated by patients since it gave them the opportunity to be in contact with physicians while respecting social distancing rules.

Abstract

Maritime spatial planning, management of marine resources, environmental assessments and forecasting all require good seabed maps. Similarly there is a need to support the objectives to achieve Good Environmental Status in Europe's seas by 2020, set up by the European Commission's Marine Strategy Framework Directive. Hence the European Commission established the European Marine Observation and Data Network (EMODnet) programme in 2009, which is now in its fourth phase (2019–21). The programme is designed to assemble existing, but fragmented and partly inaccessible, marine data and to create contiguous and publicly available information layers which are interoperable and free of restrictions on use, and which encompass whole marine basins. The EMODnet Geology project is delivering integrated geological map products that include seabed substrates, sedimentation rates, seafloor geology, Quaternary geology, geomorphology, coastal behaviour, geological events such as submarine landslides and earthquakes, and marine mineral occurrences. Additionally, as a new product during the ongoing and preceding phase of the project, map products on submerged landscapes of the European continental shelf have been compiled at various time frames. All new map products have a resolution of 1:100 000, although finer resolution is presented where the underlying data permit. A multi-scale approach is adopted whenever possible. Numerous national seabed mapping programmes worldwide have demonstrated the necessity for proper knowledge of the seafloor. Acting on this, the European Commission established the European Marine Observation and Data Network (EMODnet) programme in 2009. The national geological survey organizations of Europe have a strong network of marine geological teams through the Marine Geology Expert Group of the association of European geological surveys (Eurogeosurveys). 22. Lessons Learnt from Operating the First Cubesat Mission Equipped with a Hall Thruster

Abstract

Only a few electric propulsion solutions on-board small satellites have been operated in space, even if propulsion is nowadays seen as one of the most important subsystems for the future of small satellites. Exotrail demonstrated its Hall Effect Thruster propulsion solution in space at the end of 2020. It was the first Hall thruster ever flown onboard a satellite weighing below 100kg of mass, while also being the only known permanent magnet Hall Thruster demonstrated in space. Operations of small satellites with propulsion is a novel field and the behavior of such thrusters in space still brings new challenges. Exotrail developed its ExoMG™ propulsion system demonstrator between November 2018 and September 2019, when the flight model was delivered to the small satellite manufacturer NanoAvionics. The satellite was eventually launched in November 2020 and following the commissioning of the platform, the thruster was conditioned and subsequently fired. These two last steps will be detailed. The propulsion system consists of the propellant storage and regulation system, the control and power electronics, the actual thruster consisting of an anode and a cathode and a mechanical and thermal interface. It is operated through Exotrail’s ExoOPS™ - Operations software. The different subsystems will be described as well as their interaction. Each of them has many observables that will be detailed and discussed –typically temperatures, voltages. The operations software enables the monitoring of the orbital parameters, the maneuver generation, the housekeeping of the propulsion unit and the preparation of the maneuvers. It will be briefly described as it is the main software tool to understand the behavior of the thruster. The commissioning operations of the propulsion unit are firstly focused on health check and controllability of the environmental parameters. Then a health check of the active parts of the propulsion is performed. Finally, the firing sequence is initiated. The different steps, the expected and actual results will be presented and discussed. Finally, we will present the performance estimation of our propulsion unit. Thanks to our on-board GPS data, we perform maneuver restitution and we compare the in-flight performance with ground tests.

Abstract

The poor access to data on the marine environment is a handicap to government decision-making, a barrier to scientific understanding and an obstacle to economic growth. In this light, the European Commission initiated the European Marine Observation and Data Network (EMODnet) in 2009 to assemble and disseminate hitherto dispersed marine data. In the ten years since then, EMODnet has become a key producer of publicly available, harmonised datasets covering broad areas. This paper describes the methodologies applied in EMODnet Geology project to produce fully populated GIS layers of seabed substrate distribution for the European marine areas. We describe steps involved in translating national seabed substrate data, conforming to various standards, into a uniform EMODnet substrate classification scheme (i.e., the Folk sediment classification). Rock and boulders form an additional substrate class. Seabed substrate data products at scales of 1:250,000 and 1:1 million, compiled using descriptions and analyses of seabed samples as well as interpreted acoustic images, cover about 20% and 65% of the European maritime areas, respectively. A simple confidence assessment, based on sample and acoustic coverage, is helpful in identifying data gaps. The harmonised seabed substrate maps are particularly useful in supraregional, transnational and pan-European marine spatial planning. © 2019 by the authors. Licensee MDPI, Basel, Switzerland.

Abstract

Background and aims: Coeliac disease (CD) is considered a high‐risk condition for developing non‐alcoholic fatty liver disease (NAFLD) and other related metabolic disorders, particularly after commencing gluten‐free diet (GFD). Recently, a new concept of metabolic‐associated fatty liver disease (MAFLD) has been proposed to overcome the limitations of NAFLD definition. This study aimed at exploring the prevalence of NAFLD and MAFLD in CD patients at the time of CD diagnosis and after 2 years of GFD. Furthermore, we evaluated the role of PNPLA3 rs738409 in the development of NAFLD and MAFLD in the same population. Methods: We retrospectively enrolled all newly diagnosed CD patients who underwent clinical, laboratory and ultrasonography investigations both at diagnosis and after 2 years of follow‐up. Moreover, a PNPLA3 rs738409 genotyping assay was performed. Results: Of 221 newly diagnosed CD patients, 65 (29.4%) presented NAFLD at CD diagnosis, while 32 (14.5%) met the criteria for MAFLD (k = 0.57). There were no significant differences between NAFLD and MAFLD, except for the higher rate of insulin resistance (IR) of MAFLD patients (75% vs 33.8%, P <.001). At 2 years of follow‐up, 46.6% of patients developed NAFLD while 32.6% had MAFLD (k = 0.71). MAFLD subjects had higher transaminases (P =.03), LDL‐cholesterol (P =.04), BMI and waist circumference and higher IR than NAFLD patients. MAFLD patients showed higher non‐invasive liver fibrosis scores than NAFLD subjects (APRI = 1.43 ± 0.56 vs 0.91 ± 0.62, P <.001; NFS=−1.72 ± 1.31 vs −2.18 ± 1.41, P =.03; FIB‐4 = 1.27 ± 0.77 vs 1.04 ± 0.74, P =.04). About PNPLA3 polymorphisms, at 2 years follow‐up, NAFLD subjects presented a higher rate of heterozygosis (40.8%) and homozygosis (18.4%) polymorphisms than non‐NAFLD (26.3% and 7.6%, respectively, P =.03 and 0.02), while no correlation between PNPLA3 polymorphisms and MAFLD was seen. Conclusions: The new MAFLD definition better reflects the metabolic alterations following GFD in CD population. This new classification could be able to identify patients at higher risk of worse metabolic outcome, who need a close multidisciplinary approach for their multisystemic disease.
