Objetivos. Comprender el afrontamiento psicológico en pacientes mujeres con cáncer de mama de un hospital de Lima Metropolitana. Materiales y métodos. La investigación siguió un enfoque cualitativo con un diseño de análisis temático reflexivo. Se entrevistó a 16 mujeres con cáncer de mama entre los 35 y 65 años. El análisis de datos se realizó con apoyo del software ATLAS.ti 22. Resultados. Se presentaron tres estrategias de afrontamiento psicológico: afrontamiento emocional, el cual se encontró con mayor intensidad, y comprende el apoyo de personas significativas, el afrontamiento religioso y la concentración en las consecuencias positivas, generando una reinterpretación positiva y la aceptación progresiva de la enfermedad; el afrontamiento resolutivo, caracterizado por la acción diligente, con seguimiento a las indicaciones, y la búsqueda de apoyo profesional. Finalmente, el afrontamiento evasivo, se centró en los elementos negativos, el aplazamiento del afrontamiento y la distracción cognitiva y conductual, siendo este último de suma relevancia para equilibrar las actividades en la vida cotidiana de las pacientes. Conclusiones. Las mujeres emplearon con mayor frecuencia estrategias del estilo emocional, ya que intentan incrementar las emociones positivas, acompañado del apoyo religioso y del entorno; además, utilizaron estrategias relacionadas al estilo resolutivo, puesto que centraron sus acciones para recibir la atención y tratamiento médico, dejando de lado otras actividades; pese a ello, emplearon estrategias para desenfocarse del padecimiento y así desligarse de sus preocupaciones.

We present the case of a 25-year old man with a history of marijuana and tobacco consumption who, during a commercial flight to Cusco, presented dyspnea, loss of consciousness, and epileptic condition. He arrived in this city presenting arterial hypotension, abolished pulmonary murmur, with no recovery of consciousness. The thorax tomography revealed lung bullae and the tomography showed pneumocephalus. He was diagnosed with cerebral gas embolism. Hypoxemia associated with seizures and loss of consciousness in a young person during a flight is not a common event. Loss of cabin pressure during climb appears to be the triggering event in patients with lung disease.

La integración de los procesos en una cadena de suministro mejora su nivel de competitividad, generando productos en la cantidad, condiciones y momento requerido, a un bajo costo. Por ello el objetivo del artículo es analizar los procesos de negocios de la cadena de suministro avícola. La investigación fue de tipo analítica, con un diseño no experimental; de campo y transeccional. La población estuvo conformada por las empresas avícolas del estado Zulia, de la cual se tomó una muestra no probabilística de 4 empresas. Los resultados indican que la cadena de suministro avícola está formado por tres fases: aprovisionamiento, procesamiento y distribución, y en cada una de ellas se gestionan procesos de negocios para afianzar su integración, con sus proveedores, propietarios y clientes, dichos procesos son: gestión de la demanda, la relación con los clientes, el servicio al cliente, el flujo de producción, la logística, la relación con los proveedores y el desarrollo de nuevos productos. Se puede concluir que los procesos de negocio en la cadena de suministro avícola se trabajan de forma integrada y efectiva, lo que ha permitido que el sector permanezca en el mercado de forma de cooperación desde hace varios años, a pesar de las dificultades presentes en el país.

Flowering time is a key target trait for extending the vegetative phase to increase biomass in bioenergy crops such as perennial C4 grasses. Molecular genetic studies allow the identification of genes involved in the control of flowering in different species. Some regulatory factors of the Arabidopsis pathway are conserved in other plant species such as grasses. However, differences in the function of particular genes confer specific responses to flowering. One of the major pathways is photoperiod regulation, based on the interaction of the circadian clock and environmental light signals. Depending on their requirements for day-length plants can be classified as long-day (LD), short-day (SD), and day-neutral. The CONSTANS (CO) and Heading Date 1 (Hd1), orthologos genes, are central regulators in the flowering of Arabidopsis and rice, LD and SD plants, respectively. Additionally, Early heading date 1 (Ehd1) induces the expression of Heading date 3a (Hd3a), conferring SD promotion and controls Rice Flowering Locus T 1 (RFT1) in LD conditions, independently of Hd1. Nevertheless, the mechanisms promoting flowering in perennial bioenergy crops are poorly understood. Recent progress on the regulatory network of important gramineous crops and components involved in flowering control will be discussed.

Resumen Introducción y Objetivo: Las técnicas de liposucción han sufrido diversas variaciones en el transcurrir del tiempo; sin embargo, observamos que los resultados estéticos en cuanto a naturalidad en el abdomen en ocasiones no son los mejores a pesar del apoyo tecnológico y de las nuevas técnicas quirúrgicas, pues el abdomen, al ser un área con características específicas en sus diferentes áreas, requiere un tratamiento especial por zonas. Nos proponemos demostrar, mediante el estudio de imágenes, el diferente grosor del tejido celular subcutáneo en el abdomen así como proponer, de acuerdo a los hallazgos, subunidades estéticas del mismo como base al planeamiento prequirúrgico de la liposucción abdominal. Material y método: Evaluamos 14 pacientes mediante ecografía abdominal de partes blandas para medir el grosor del tejido celular subcutáneo en 3 puntos de la zona lateral y 3 puntos de la zona medial del abdomen previamente establecidos, a fin de demostrar si existen diferencias estadísticamente significativas entre ellos. Resultados: El análisis estadístico demostró que existen diferencias estadísticamente significativas respecto al grosor del tejido celular subcutáneo entre la zona medial del abdomen (puntos 4, 5 y 6) con respecto a la zona lateral (puntos 1, 2 y 3) encontrando una relación 2:1; así como diferencias de grosor entre los puntos 1 y 2 en comparación con los puntos 4 y 5; de los puntos 1 y 2 con el punto 3; de los puntos 4 y 5 con el punto 6; y del punto 3 con el punto 6. En base a esas diferencias proponemos dividir el abdomen en una zona medial y otra lateral con subunidades estéticas de acuerdo a sus características morfológicas (grosor del tejido celular subcutáneo). También obtuvimos, de acuerdo a la correlación de Pearson, que dicha diferencia entre la zona medial y lateral del abdomen no está en relación al peso ni a la edad del paciente, pues siempre existe diferencia de grosor del tejido adiposo entre la zona lateral y la medial del abdomen. Conclusiones: Existe diferente grosor del tejido celular subcutáneo abdominal en las diferentes zona señaladas, por lo que decidimos usar este parámetro para agrupar subunidades estéticas en el abdomen. Inferimos que esto podría ayudar a un planteamiento prequirúrgico para la liposucción abdominal, a fin de conseguir resultados estéticos favorables y conservar los relieves naturales del abdomen.

Abstract Background Making timely moral decisions can save a life. However, literature on how moral decisions are made under time pressure reports conflicting results. Moreover, it is unclear whether and how moral choices under time pressure may be influenced by personality traits like impulsivity and sensitivity to reward and punishment. Methods To address these gaps, in this study we employed a moral dilemma task, manipulating decision time between participants: one group (N = 25) was subjected to time pressure (TP), with 8 s maximum time for response (including the reading time), the other (N = 28) was left free to take all the time to respond (noTP). We measured type of choice (utilitarian vs. non-utilitarian), decision times, self-reported unpleasantness and arousal during decision-making, and participants’ impulsivity and BIS-BAS sensitivity. Results We found no group effect on the type of choice, suggesting that time pressure per se did not influence moral decisions. However, impulsivity affected the impact of time pressure, in that individuals with higher cognitive instability showed slower response times under no time constraint. In addition, higher sensitivity to reward predicted a higher proportion of utilitarian choices regardless of the time available for decision. Conclusions Results are discussed within the dual-process theory of moral judgement, revealing that the impact of time pressure on moral decision-making might be more complex and multifaceted than expected, potentially interacting with a specific facet of attentional impulsivity.

Objective. To understand the psychological coping strategies in female patients with breast cancer from a hospital in Metropolitan Lima. Materials and methods. This was a qualitative research with a reflexive thematic analysis design. Sixteen women with breast cancer between 35 and 65 years of age were interviewed. Data was analyzed with the ATLAS.ti 22 software. Results. Three psychological coping strategies were described: emotional coping, which was found more frequently, includes the support of important people, religious coping, and focusing on positive consequences, which leads to a positive reinterpretation and progressive acceptance of the disease; active coping, characterized by diligent action, following indications, and seeking professional support. Finally, avoidance coping, which focuses on negative elements, postponement of the coping process and cognitive and behavioral distractions, the latter being of utmost relevance to balance the activities in the patients' daily lives. Conclusions. Participants used emotional coping strategies more frequently, since they tried to increase positive emotions, accompanied by religious and environmental support. In addition, they also used active coping strategies, since they focused their actions to receive medical attention and treatment, leaving aside other activities; in spite of this, they used strategies to take their focus off the condition and thus detach themselves from their worries. Abstract The aim of this research was to determine the impact of the architecture of digital technologies in university teacher training as a catalyst for change and educational innovation. Methodologically, it is framed in a quantitative approach with a pre-experimental design and an explanatory level, with a sample of 269 teachers out of a total population of 450 at the university. The results show a significant impact of the architecture of digital technologies as a catalyst for change in university teacher training, with an increase of 22.88% from pre-test to post-test. The statistical significance of the results is supported by a P-value of 0.000, which means that it is less than the established significance level (α = 0.05), leading to the acceptance of hypothesis H1: The architecture of digital technologies as a catalyst for change in university teacher education has an impact. In conclusion, the architecture of digital technologies, consisting of the two academic pillar systems: the academic management system and the virtual learning system, contribute to the development of digital skills in key areas such as digital literacy, communication and collaboration, digital content creation and problem solving in virtual environments.

Objectives: Sustained monomorphic ventricular tachycardia (SMVT) is a life-threatening condition that is often observed in patients with structural heart disease. Catheter ablation (CA) ablation is an effective and well-established treatment for the scar-related ventricular tachycardias (VTs). Sometimes, due to patient fragility or contraindications to CA, a noninvasive procedure is required. In these cases, VT ablation with stereotactic arrhythmia radioablation (STAR) for SMVTs supported by the CardioInsight mapping system seems to be a promising and effective noninvasive approach. Methods and results: We report a case of a 55-year-old male smoker and heavy alcohol consumer who developed ischemic heart disease and frequent refractory SMVT relative to antiarrhythmic drugs. Catheter ablation was not practicable due to the presence of an apical thrombosis in the left ventricle. The CardioInsightTM system (Cardioinsight Technologies Inc., Cleveland, OH, USA) was useful for noninvasively mapping the VTs, identifying two target areas on the septum and anterior wall of the left ventricle. A personalized STAR treatment plan was carefully designed, and it was delivered in a few minutes. During follow-up, a significant reduction in the arrhythmia burden was documented. Conclusions: Stereotactic arrhythmia radioablation supported by the CardioInsight system could be an alternative treatment for VTs when catheter ablation is not possible. Larger studies are needed to investigate this technique.

Giant cell myocarditis (GCM) is a rare and often fulminant autoimmune disease. The immune-mediated pathogenesis of GCM is also supported by animal models, association with other immunological diseases and therapeutic efficacy of immunosuppressive drugs. The diagnosis of GCM is based on endomyocardial biopsy. GCM is an orphan disease. Heart transplantation is effective but up to 25% of transplanted patients experience disease recurrence. Immunosuppressive drugs have been shown to be potential therapeutic agents for GCM. In the present case report, the prescription of cyclosporine, azathioprine and prednisone resulted in a rapid and prolonged remission in support of the role of a combined immunosuppressive regimen in improving the long-term prognosis of this cardiac pathology.

Abstract The aromatic amino acid L-tryptophan (Trp) is essentially metabolized along the host and microbial pathways. While much is known about the role played by downstream metabolites of each pathways in intestinal homeostasis, their role in lung immune homeostasis is underappreciated. Here we have examined the role played by the Trp hydroxylase/5-hydroxytryptamine (5-HT) pathway in calibrating host and microbial Trp metabolism during Aspergillus fumigatus pneumonia. We found that 5-HT produced by mast cells essentially contributed to pathogen clearance and immune homeostasis in infection by promoting the host protective indoleamine-2,3-dioxygenase 1/kynurenine pathway and limiting the microbial activation of the indole/aryl hydrocarbon receptor pathway. This occurred via regulation of lung and intestinal microbiota and signaling pathways. 5-HT was deficient in the sputa of patients with Cystic fibrosis, while 5-HT supplementation restored the dysregulated Trp partitioning in murine disease. These findings suggest that 5-HT, by bridging host-microbiota Trp partitioning, may have clinical effects beyond its mood regulatory function in respiratory pathologies with an inflammatory component.

Abstract Background B‐cell chronic lymphocytic leukemia (B‐CLL) is characterized by the expansion of CD5+ malignant B lymphocytes. Recent discoveries have shown that double‐negative T (DNT) cells, double‐positive T (DPT) cells, and natural killer T (NKT)‐cells may be involved in tumor surveillance. Methods A detailed immunophenotypic analysis of the peripheral blood T‐cell compartment of 50 patients with B‐CLL (classified in three prognostic groups) and 38 healthy donors (as controls) matched for age was performed. The samples were analyzed by flow cytometry using a stain‐lyse‐no wash technique and a comprehensive six‐color antibody panels. Results Our data confirmed a reduction in percentage values and an increase in absolute values of T lymphocytes in patients with B‐CLL, as already reported. In particular, DNT, DPT, and NKT‐like percentages were significantly lower than in the controls, except for NKT‐like in the low‐risk prognostic group. Moreover, a significant rise in the absolute counts of DNT cells in each prognostic group and in the low‐risk prognostic group of NKT‐like cells was found. A significant correlation of the absolute values of NKT‐like cells in the intermediate‐risk prognostic group versus B cells was observed. Furthermore, we analyzed whether the increase in T cells was related to the subpopulations of interest. Only DNT cells were positively correlated with the increase in CD3+ T lymphocytes, regardless of the stage of the disease, supporting the hypothesis that this T‐cell subset plays a key role in the immune T response in B‐CLL. Conclusion These early results supported that DNT, DPT, and NKT‐like subsets may be related to disease progression and should encourage further studies aimed at identifying the potential immune surveillance role of these minority T subpopulations.
