Abstract

Abstract Introduction Contouring has become an increasingly important aspect of radiation therapy due to inverse planning, and yet is extremely time‐consuming. To improve contouring efficiency and reduce potential inter‐observer variation, the atlas‐based auto‐segmentation (ABAS) function in Velocity was introduced to ICON cancer centres (ICC) throughout Australia as a solution for automatic contouring. Methods This paper described the implementation process of the ABAS function and the construction of user‐defined atlas sets and compared the contouring efficiency before and after the introduction of ABAS. Results The results indicate that the main limitation to the ABAS performance was Velocity's sub‐optimal atlas selection method. Three user‐defined atlas sets were constructed. Abstract

BACKGROUND & AIMS: The Primary Biliary Cholangitis (PBC) Obeticholic Acid (OCA) International Study of Efficacy (POISE) randomized, double-blind, placebo-controlled trial demonstrated that OCA reduced biomarkers associated with adverse clinical outcomes (ie, alkaline phosphatase, bilirubin, aspartate aminotransferase, and alanine aminotransferase) in patients with PBC. The objective of this study was to evaluate time to first occurrence of liver transplantation or death in patients with OCA in the POISE trial and open-label extension vs comparable non-OCA-treated external controls. METHODS: Propensity scores were generated for external control patients meeting POISE eligibility criteria from 2 registry studies (Global PBC and UK-PBC) using an index date selected randomly between the first and last date (inclusive) on which eligibility criteria were met. Cox proportional hazards models weighted by inverse probability of treatment assessed time to death or liver transplantation. Additional analyses (Global PBC only) added hepatic decompensation to the composite end point and assessed efficacy in patients with or without cirrhosis. RESULTS: During the 6-year follow-up, there were 5 deaths or liver transplantations in 209 subjects in the POISE cohort (2.4%), 135 of 1381 patients in the Global PBC control (10.0%), and 281 of 2135 patients in the UK-PBC control (13.2%). The hazard ratios (HRs) for the primary outcome were 0.29 (95% CI, 0.10-0.83) for POISE vs Global PBC and 0.30 (95% CI, 0.12-0.75) for POISE vs UK-PBC. In the Global PBC study, HR was 0.20 (95% CI, 0.03-1.22) for patients with cirrhosis and 0.31 (95% CI, 0.09-1.04) for those without cirrhosis; HR was 0.42 (95% CI, 0.21-0.85) including hepatic decompensation. Abstract

BackgroundThe BSG-BASL decompensated cirrhosis care bundle seeks to optimise patient management during the first 24 hours of presentation. The pandemic has impacted on junior doctors’ hepatology training, by reducing access to face-to-face teaching. We sought to redress this by assessing how a remote education programme could affect bundle implementation and awareness, shortly after the UK’ s first COVID-19 wave.MethodsA retrospective snapshot case-note audit was undertaken for 4 weeks (August 2020) at Bristol Royal Infirmary (BRI). Decompensated cirrhosis admissions were identified from coding and bundle usage and compliance with ascitic tap guidance were quantified. An online survey was conducted of BRI junior doctors (medical) to assess knowledge of the bundle and awareness of appropriate ascitic fluid tests. Subsequent quality improvement interventions were developed, with repeat audits in May-June 2021.ResultsThe baseline audit identified appropriate bundle completion in 50% (n=8/16) decompensated cirrhosis patients, none by foundation doctors, with 2 by hepatology consultants. Ascitic tap was attempted in only 50% of patients with detectable ascites (4/8), with recommended tests requested in 2 of 3 successful taps.The online survey (40 participants 42% response) revealed good knowledge of when to use the bundle (85%) and how to access it (90%). For patients with new ascites, 46% of participants identified appropriate tests, however 39/40 also selected unnecessary options. Knowledge was poorer for patients with known ascites;20% selected correct investigations and no participants knew the exact combination to request. Procedural skills varied, 49% of participants had performed an ascitic tap unsupervised, but 33% had never attempted one.After these audits, remote teaching sessions and posters were used to improve prompting of bundle completion;paper copies were distributed to admission areas. Re-audit (May 2020) identified 15 hepatic decompensation admissions, of whom 53% (n=8) had a completed bundle. 9 ascitic taps were attempted in 13 patients with ascites (69%);correct investigations were requested in 5 of 7 taps (71%). At a repeat online survey (32% response rate) 10% (3/30) selected only the necessary investigations for new ascites and 13% (4/30) for recurrent ascites. This indicates that knowledge has not significantly improved.ConclusionThere remained low usage of the BSG-BASL decompensated cirrhosis bundle despite an education campaign. Ascitic tap performance improved but test requesting remained suboptimal. Systematic changes such as IT-system alerts and integrated guidance on key ascites tests within bundle documentation may be more sustainable ways of improving compliance (data will be presented).

Abstract

A homo-dimeric enzyme, thymidylate synthase (TS), has been a long-standing molecular target in chemotherapy. To further elucidate properties and interactions with ligands of wild-type mouse thymidylate synthase (mTS) and its two single mutants, H190A and W103G, spectroscopic and theoretical investigations have been employed. In these mutants, histidine at position 190 and tryptophan at position 103 are substituted with alanine and glycine, respectively. Several emission-based spectroscopy methods used in the paper demonstrate an especially important role for Trp 103 in TS ligands binding. In addition, the Advanced Poisson–Boltzmann Solver (APBS) results show considerable differences in the distribution of electrostatic potential around Trp 103, as compared to distributions observed for all remaining Trp residues in the mTS family of structures. Together, spectroscopic and APBS results reveal a possible interplay between Trp 103 and His190, which contributes to a reduction in enzymatic activity in the case of H190A mutation. Comparison of electrostatic potential for mTS complexes, and their mutants, with the substrate, dUMP, and inhibitors, FdUMP and N4-OH-dCMP, suggests its weaker influence on the enzyme–ligand interactions in N4OH-dCMP-mTS compared to dUMP-mTS and FdUMP-mTS complexes. This difference may be crucial for the explanation of the ”abortive reaction” inhibitory mechanism of N4OH-dCMP towards TS. In addition, based on structural analyses and the H190A mutant capacity to form a denaturation-resistant complex with N4-OH-dCMP in the mTHF-dependent reaction, His190 is apparently responsible for a strong preference of the enzyme active center for the anti rotamer of the imino inhibitor form.

Abstract

Background Förster Resonance Energy Transfer (FRET) is widely used to study the structure and dynamics of biomolecular systems and also causes the non-linear fluorescence response observed in multi-fluorophore proteins. Accurate FRET analysis, in terms of measuring changes in donor and acceptor spectra and energy transfer efficiency is therefore critical. Methods We demonstrate a novel quantitative FRET analysis using anisotropy resolved multidimensional emission spectroscopy (ARMES) in a Human Serum Albumin (HSA) and 1,8-anilinonaphathalene sulfonate (ANS) model. ARMES combines 4D measurement of polarized excitation emission matrices (pEEM) with multivariate data analysis to spectrally resolve contributing fluorophores. Multivariate analysis (Parallel Factor, PARAFAC and restricted Tucker3) was used to resolve fluorophore contributions and for modelling the quenching of HSA emission and the HSA-ANS interactions. Results pEEM spectra were modelled using Tucker3 which accommodates non-linearities introduced by FRET and a priori chemical knowledge was used to optimise the solution, thus resolving three components: HSA emission, ANS emission from indirect FRET excitation, and ANS emission from direct excitation. Perpendicular emission measurements were more sensitive to indirectly excited acceptor emission. PARAFAC modelling of HSA, donor emission, separated ANS FRET interacting (Tryptophan) and non-interacting (Tyrosine) components. This enabled a new way of calculating quenching constants using the multi-dimensional emission of individual donor fluorophores. Conclusions FRET efficiency could be calculated using the multi-dimensional, resolved emission of the interacting donor fluorophores only which yielded higher ET efficiencies compared to conventional methods. General significance Shows the potential of multidimensional fluorescence measurements and data analysis for more accurate FRET modelling in proteins. Abstract

Studying the interaction between plasma proteins and liposomes is critical, particularly for their use as drug delivery systems. Here, the efficacy of anisotropy resolved multidimensional emission spectroscopy (ARMES) for investigating the interaction of human serum albumin (HSA) with liposomes was explored and compared to conventional spectroscopic techniques. Dynamic Light Scattering (DLS) and absorbance spectroscopy (with Multivariate Curve Resolution (MCR) modeling) indicated that the highest degree of liposome rupturing, and aggregation occurred in water, with less in ammonium bicarbonate buffer (ABC) and phosphate buffered saline (PBS). Fluorescence emission spectra of HSA-liposome mixtures revealed significant hypsochromic shifts for water and ABC, but much less in PBS, where the data suggests a non-penetrating protein layer was formed. Average fluorescence lifetimes decreased upon liposome interaction in water (6.2¿5.2 ns) and ABC buffer (6.3¿5.6 ns) but increased slightly for PBS (5.6¿5.8 ns). ARMES using polarized Total Synchronous Fluorescence Scan measurements with parallel factor (PARAFAC) analysis resolved intrinsic HSA fluorescence into two components for interactions in water and ABC buffer, but only one component for PBS. These components, in water and ABC buffer, corresponded to two different HSA populations, one blue-shifted and penetrating the liposomes (¿ex/em = ~ 280/320 nm) and a second, similar to free HSA in solution (¿ex/em = ~ 282/356 nm). PARAFAC scores for water and ABC buffer suggested that a large proportion of HSA interacted in an end on configuration. ARMES provides a new way for investigating protein-liposome interactions that exploits the full intrinsic emission space of the protein and thus avoids the use of extrinsic labels. The use of multivariate data analysis provided a comprehensive and structured framework to extract a variety of useful information (resolving different fluorescent species, quantifying their signal contribution, and extracting light scatter signals) all of which can be used to discriminate between interaction mechanisms. Abstract

Background: Treatment guidelines recommend a stepwise approach to primary biliary cholangitis: all patients begin treatment with ursodeoxycholic acid (UDCA) monotherapy and those with an inadequate biochemical response after 12 months are subsequently considered for second-line therapies. However, as a result, patients at the highest risk can wait the longest for effective treatment. We determined whether UDCA response can be accurately predicted using pretreatment clinical parameters. Methods: We did logistic regression analysis of pretreatment variables in a discovery cohort of patients in the UK with primary biliary cholangitis to derive the best-fitting model of UDCA response, defined as alkaline phosphatase less than 1·67 times the upper limit of normal (ULN), measured after 12 months of treatment with UDCA. We validated the model in an external cohort of patients with primary biliary cholangitis and treated with UDCA in Italy. Additionally, we assessed correlations between model predictions and key histological features, such as biliary injury and fibrosis, on liver biopsy samples. Findings: 2703 participants diagnosed with primary biliary cholangitis between Jan 1, 1998, and May 31, 2015, were included in the UK-PBC cohort for derivation of the model. Abstract

Previous studies have documented links between sub-clinical narcissism and the active pursuit of short-term mating strategies (e.g., unrestricted sociosexuality, marital infidelity, mate poaching). Nearly all of these investigations have relied solely on samples from Western cultures. In the current study, responsesfrom a cross-cultural survey of 30,470 people across 53 nations spanning 11 world regions (North America, Central/South America, Northern Europe, Western Europe, Eastern Europe, Southern Europe, Middle East, Africa, Oceania, Southeast Asia, and East Asia) were used to evaluate whether narcissism (as measured by the Narcissistic Personality Inventory; NPI) was universally associated with short-term mating. Results revealed narcissism scores (including two broad factors and seven traditional facets as measured by the NPI) were functionally equivalent across cultures, reliably associating with key sexual outcomes (e.g., more active pursuit of shortterm mating, intimate partner violence, and sexual aggression) and sex-related personality traits (e.g., higher extraversion and openness to experience). Whereas some features of personality (e.g., subjective well-being) were universally associated with socially adaptive facets of Narcissism (e.g., self-sufficiency), most indicators of short-term mating (e.g., unrestricted sociosexuality and marital infidelity) were universally associated with the socially maladaptive facets of narcissism (e.g., exploitativeness). Discussion addresses limitations of these cross-culturally universal findings and presents suggestions for future research into revealing the precise psychological features of narcissism that facilitate the strategic pursuit of short-term mating. Abstract

In a series of two studies, we explored people's selection of self-promotion or competitor derogation when intrasexually competing for mates, as influenced by sex and romantic relationship involvement. In Study 1, student participants completed a forced-choice survey outlining six hypothetical competitive tactics. The findings indicated that self-promotion was chosen more often than competitor derogation, regardless of sex and current relationship involvement. In Study 2, we relied upon a community sample that completed a continuous measure that expanded upon the survey of Study 1. We found that women reported significantly more self-promotion than men, and men reported significantly more competitor derogation. Individuals who were romantically uninvolved and those who were dating reported higher levels of self-promotion and competitor derogation than those who were married or in a common-law relationship. Abstract

Primary biliary cirrhosis (PBC) is a classical autoimmune liver disease for which effective immunomodulatory therapy is lacking. Here we perform meta-analyses of discovery data sets from genome-wide association studies of European subjects (n=2,764 cases and 10,475 controls) followed by validation genotyping in an independent cohort (n=3,716 cases and 4,261 controls). We discover and validate six previously unknown risk loci for PBC (Pcombined, Primary biliary cirrhosis is an autoimmune liver disease with poor therapeutic options. Here Cordell et al. a perform meta-analysis of European genome-wide association studies identifying six novel risk loci and a number of potential therapeutic pathways.

Abstract

Trainees' hours are under review once again, due to the changes incurred by the latest pay deal and the likely enforcement of the European Working Time Directive. Until now, specialist registrars (SpRs) training in general internal medicine (GIM) have been accustomed to residential on-call rotas or partial shift schemes as a means of covering emergency work. Some NHS trusts have suggested that GIM SpRs should now work full shifts to meet the restrictions of European Working Time Directive. This article outlines the views of the Royal College of Physicians' Trainees Committee on the potential impact of full shift work for GIM SpRs on medical services. Impaired continuity of inpatient care and reduced staffing levels for outpatient services were felt to be the most serious possible consequences. Some short-term solutions are suggested, but in the long term, increased numbers of both consultants and SpRs will be vital to maintain and improve standards in acute medicine.

Abstract

The ground covered by recent studies on Brazil between 1930 and 1945 has fallen into two main schools of interest: one has concentrated on the political history of this period; the other on the background to Brazilian industrialization. With the exception of coffee and rice, Brazilian agriculture has remained relatively neglected as an area of research. Yet the importance of Brazilian agriculture both as a source for domestic consumption requirements and also as an export earner has constantly been presented as the explanation for the political evolution of Brazil in these years and the concomitant increased interest shown in the development of Brazilian manufacturing industries. This paper will concentrate on the Brazilian agricultural sector and will consider the nature of Vargas's agriculturalautarquias— that is, the semi-autonomous administrative units Vargas established to supervise and direct the activities of selected areas of the economy and of society. It is hoped that, in so doing, it will contribute to a deeper understanding of how Getúlio Vargas governed Brazil between 1930 and 1945.

Abstract

It has been known for more than a century that the surfaces of cells bordering tissue air spaces are often ornamented with beads or elongated protuberances of material apparently containing pectin, as judged by their reactivity with ruthenium red (reviews by Kisser, 1928; Carlquist, 1956; Carr and Carr, 1975; Butterfield, Meylan and Exley, 1981). Pectic beads occur in all major groups of vascular plants and on cells from a wide variety of tissue types (Kisser, 1928). They have been observed by SEM in fern mesophyll by Carr and Carr, (1975) and in the ground parenchyma of Cocos nucifera stems by Butterfield et al., (1981) who reported that the beads stained with ruthenium red, although other tests for pectins were inconclusive. Davies and Lewis (1981) reported pectic projections on callus cells of Daucus carota consisting of more than one compartment, the outer one often containing oil droplets. They proposed that this oil may spread over the cell surface to provide a waterproof barrier, while Lipetz (1970) suggested it may be a prerequisite for wound healing. Nevertheless the function of pectic beads is still almost entirely obscure.

Abstract

Originally published in 1984, at a time when international commodity control was brought from the periphery to the centre of international trade policy, this book provided a new and more comprehensive approach to, and an analytical appraisal of, international commodity controls, from their origins in the 1920s to their widespread acceptance as an important element in international trade policy in the 1970s. The first part establishes the economic and institutional background against which controls were introduced and includes sections on a wide range of issues such as the changing structure of world commodity trade and the roles of GATT, UNCTAD and the former EEC. Part 2 considers the principal control mechanisms which have been used at the international level and review the national counterparts and alternatives. Part 3 assesses on a commodity-by-commodity basis how the control worked in practice. It covers all the international commodity agreements to 1982 and also considers examples of raw material cartels.
