Abstract

Nel contributo l'A. analizza due fondamentali profili dell'equità processuale, con particolare attenzione alle implicazioni relative al sistema di giustizia penale. Si tratta del diritto alla pubblicità dell'udienza e della sentenza e del diritto alla motivazione dei provvedimenti giurisdizionali. L'esposizione dei contenuti normativi della Convenzione europea dei diritti dell'uomo e delle libertà fondamentali viene costantemente accompagnata dall'analisi delle pronunce della Corte di Strasburgo maggiormente rilevanti nelle suddette materie. Alla luce di tale analisi, infine, vengono poste in luce le possibili interferenze e gli eventuali contrasti ipotizzabili in riferimento alla normativa processuale interna rispetto al dettato convenzionale.

Abstract

La terapia dell'orbitopatia,nella fase acuta,si basa oltre che sulla correzione della turba endocrinologica,sulla protezione della cornea e sull'uso dei corticosteroidi ad alto dosaggio o di farmaci immunosoppressori o, in alternativa,sulla radioterapia orbitaria.Per attenuare la sintomatologia dolorosa,frequente nella fase acuta e che può inficiare la qualità della vita,data la componente flogistica,abbiamo voluto valutare gli effetti di una terapia antalgica mediante appliocazione sulla superficie palpebrale di un gel chirurgico criogenico decongestinante ( a contenuto di estratto di ananas più acido betaglicirretico),ad oggi utilizzato con risultati soddisfacenti in soggetti sottoposti a trattamenti chirurgici degli annessi oculari.Sono stati studiati in doppio cieco 50 soggetti ( 16 uomini e 34 donne, range età 35-46,5 anni) con orbitopatia basedowiana ( OB) di grado 3,in fase dinamica,secondo la classificazione dell'American Thyroid Association.Tutti i pazienti,egualmente distribuiti in due gruppi,erano in terapia tireostatica con MMI.Nessun effetto collaterale è stato riscontrato.All'esame clinico-strumentale-oftalmologico (visus,motilità oculare,fundus,esoftalmometria e tonometria) non si sono apprezzate variazioni significative pre e post trattamento in nessuno dei due gruppi.2 soggetti del gruppo placebo hanno riferito un miglioramento complessivo della sintomatologia, mentre nel gruppo trattato 24 soggetti hanno riferito un sensibile miglioramento come da questionario validato.Nel nostro studio la terapia con Hydrogel si è rivelata una valida strategia antalgica da associare ai farmaci ad azione sistemica.

Abstract

Summary: This study evaluates serum creatine kinase isoenzyme activity in children with osteogenesis imperfecta to determine its usefulness as a biochemical marker during treatment with bisphosphonate. The changes of creatine kinase (CK) isoenzyme activity during and after discontinuation therapy were observed. These results could be useful in addressing over-treatment risk prevention. Introduction: The brain isoenzyme of creatine kinase (CKbb) is highly expressed in mature osteoclasts during osteoclastogenesis, thus plays an important role in bone resorption. We previously identified high serum CKbb levels in 18 children with osteogenesis imperfect (OI) type 1 treated for 1 year with bisphosphonate (neridronate). In the present study, serum CK isoenzymes were evaluated in the same children with continuous versus discontinued neridronate treatment over a further 2-year follow-up period. Methods: This study included 18 children with OI type 1, 12 with continued (group A) and 6 with ceased (group B) neridronate treatment. Auxological data, serum biochemical markers of bone metabolism, bone mineral density z-score, and serum total CK and isoenzyme activities were determined in both groups. Results: Serum CKbb was progressively and significantly increased in group A ( p < 0.004) but rapidly decreased to undetectable levels in group B. In both groups, the cardiac muscle creatine kinase isoenzyme (CKmb) showed a marked decrease, while serum C-terminal telopeptide (CTx) levels were almost unchanged. Conclusions: This study provides evidence of the cumulative effect of neridronate administration in increasing serum CKbb levels and the reversible effect after its discontinuation. This approach could be employed for verifying the usefulness of serum CKbb as a biochemical marker in patients receiving prolonged bisphosphonate treatment. 34. MOCVD of Lanthanum Oxides from La(tmhd)3and La(tmod)3Precursors: A Thermal and Kinetic Investigation

Abstract

Metal‐organic (MO)CVD processes using two different precursors, (La(tmhd)3and La(tmod)3), have been investigated. In‐situ Fourier transform infrared (FTIR) spectroscopy investigations and thermal analysis have provided information on their thermal robustness during the sublimation processes and mass‐transport properties during MOCVD experiments. Both precursors can be efficiently vaporized in the temperature range 160–230 °C even though La(tmod)3exhibits higher volatility. Lanthanum oxides have been efficiently deposited using a direct liquid injection (DLI) equipped MOCVD reactor in the temperature range 350–500 °C for both precursors. The effect of the addition of tetraglyme to the precursor solution has been evaluated. Films mainly consist of different phases of La2O3and LaO(OH), while the carbon‐containing La2O2CO3phase was observed only on the surface, with no relevant carbon contamination being present in the bulk of the film.

Abstract

Metal‐organic (MO)CVD processes using three different precursors (Bi(tmhd)3, Bi(p‐tol)3, and Bi(o‐tol)3) have been investigated. Combined, thermal, and mass spectroscopic investigations have provided information on their thermal robustness during sublimation processes. In‐situ Fourier‐transform infrared (FTIR) measurements have allowed the monitoring of mass‐transported precursors during MOCVD experiments. Temperature windows of 190–277 °C, 170–270 °C, and 150–220 °C have proved suitable for the efficient vaporization of Bi(tmhd)3, Bi(p‐tol)3, and Bi(o‐tol)3, respectively, even though aryl precursors have proved to be more stable than β‐diketonate during the sublimation and transport processes.Above 350 °C, decomposition during the MOCVD processes has been observed for all the precursors. In the case of Bi(tmhd)3and Bi(o‐tol)3it involves the ligand fragmentation, while for Bi(p‐tol)3, dissociation of the intact aryl ring seems to occur.

Abstract

BACKGROUND: Insulin glargine is a once-daily basal insulin analog with prolonged duration of action and absence of an evident peak. Glargine is associated with reduced frequency of hypoglycemic episodes (mostly nocturnal) as well as effective glycemic control. Maintenance of good metabolic control before conception and throughout pregnancy is essential to lower the risk of fetal malformations. Glargine might be a valuable alternative in the management of pregnancies complicated by diabetes mellitus. However, because its clinical utility has not been established, the use of glargine is not currently recommended during pregnancy. OBJECTIVE: The aim of this study was to retrospectively evaluate (years 2004-2007) the effectiveness and safety of insulin glargine compared with neutral protamine Hagedorn (NPH) in women affected by type 1 diabetes mellitus (T1DM) during pregnancy. METHODS: The study comprised pregnant women affected by T1DM who were followed up in the Diabetes and Pregnancy Outpatient Clinic at the University of Palermo, Palermo, Italy, within 8 +/- 3.4 weeks subsequent to a positive pregnancy test. All patients with T1DM were treated with conventional basal-bolus insulin therapy (aspart or lispro analogs at the 3 main meals plus glargine or NPH at bedtime). Healthy pregnant women were used as controls for fetal and neonatal parameters. Patients were consecutively enrolled. In all women, metabolic status was determined daily by mean glycemic values (2-hour postprandial blood glucose) and glycosylated hemoglobin (HbA1c) values (at 3-month intervals). Fetal measurements (90th centiles of the head circumference, abdomen circumference, and femoral length) were evaluated by ultrasound at second and third trimesters. Weight and femoral length were assessed at birth, and neonates were classified according to the fetal growth curve for the Italian population (90th centile = large for gestational age (LGA). RESULTS: A total of 73 pregnant women (30 with T1DM and 43 healthy [control]) were included in the study. Of the 30 diabetic pregnant women included in the study, 15 (mean [SD] age, 27.4 [5.2] years; mean pregravidic weight, 59.7 [11.7] kg) maintained their preconception therapy with glargine, and 15 (mean age, 30.1 [2.4] years; mean pregravidic weight, 60.7 [8.7] kg) with NPH. No significant difference was observed between the glargine-treated group and the NPH-treated group with regard to pregravidic hypertension, third-trimester preeclampsia, maternal complications and/or their progression during pregnancy (diabetic retinopathy, micro- or macroalbuminuria) and episodes of mild hypoglycemia, severe hypoglycemia, and ketosis. There were no significant between group differences in insulin requirements (IU/kg of body weight) and glycemic profile, with the exception of better fasting and 2 hours after breakfast glycemic values in the glargine group during the first (P = 0.008 and P < 0.001, respectively) and the second (P = 0.015 and P = 0.016) trimesters, confirmed by the lower HbA1c levels in the first trimester (P = 0.037). Abstract

The physics objectives of SuperB, an asymmetric electron-positron collider with a luminosity above 10^36/cm^2/s are described, together with the conceptual design of a novel low emittance design that achieves this performance with wallplug power comparable to that of the current B Factories, and an upgraded detector capable of doing the physics in the SuperB environment.
