Production of 19 word-initial two-segment consonant clusters was examined in 12 children (ages 6-16) with cochlear implants, including 6 using oral communication and 6 using total communication. Results showed that 48% of the clusters were correctly produced (75% correct for oral communication users and 21% correct for total communication users). (Contains references.) (Author/CR)

An important consideration in cement plants is the pliability of rubber belts. Such flexibility depends on intensive and continuous maintenance to prevent unplanned interruptions to cement production. Mechanical conveyor-belt splicing usually takes the form of clips which are normally raised above the the belt and present problems when cleaning. A solution is simply to lower fastener edges by removing a strip of top rubber from the belt, so extending the life of cleaner blades and belt splices.

Production of 19 word-initial two-segment consonant clusters was examined in 12 children who have used cochlear implants for at least 5 years, including 6 children who used oral communication (OC) and 6 who used Total Communication (TC). Results showed that 48% of the clusters were correctly produced by the children (75% correct for oral communication users and 21% correct for Total Communication users). Incorrect productions were limited to realizations with one or two segments, null onsets not being produced by any of the children. One-segment realizations exhorting simple reduction generally retained the less sonorous element of the target cluster, with the exception that some Total Communication users retained the more sonorous element if the less sonorous segment was a velar stop. Two-segment realizations, in which one or both segments could be incorrect, generally reflected the singleton realization patterns of the constituent elements. Approximately 7% of target clusters were realized with epenthetic vowels, indicating awareness of multiple segments but lack of ability to implement the phonotactic requirements of a cluster. These results indicate that children with cochlear implants use similar strategies to children with normal hearing in their realization of word-initial consonant clusters. [ABSTRACT FROM AUTHOR]

The article explores the client benefits of using local replication in Domino R5 and Notes/Domino 6 (ND6). There are many reasons to replicate the mail file and other Notes databases to one's workstation. First, there is the advantage of response time. One can also have the advantages of working offline. Another great advantage of replication of one's mail file to one's workstation is that any response time problems in accessing mail file are solved. Many companies disallow or discourage having a replica of the Notes mail file or other Notes databases on a laptop because of security concerns. However, one can resolve the security issues by encrypting mail and other databases on laptops. International Business Machines Corp. ran some tests to determine the impact of local replication on network traffic. The results for each user and each test were similar in that network traffic was reduced when using local mode as compared server mode. The ND6 replication architecture is much more efficient that in previous versions of Notes. Several enhancements to the replication process result in greater efficiency, namely, multitasking and memory use, streaming replication, dynamic builds of folders and views, documents replicated in ascending size, among others.

The article focuses on conveyor belts, considered as the lifeline for effective, profitable production. The goals of effective belt-conveyor maintenance include the prevention of surprise belt failures and keeping maintenance downtime to a minimum. It states that component selection requires a thorough knowledge of the composition of fasteners, the cleaners and the belt itself. It is inferred that proper cleaner/fastener interface requires receiving and dissipating kinetic energy without damaging the splice or the cleaner.

© 2019 Elsevier Ltd. All rights reserved., Background: Non-alcoholic steatohepatitis (NASH) is a common type of chronic liver disease that can lead to cirrhosis. Obeticholic acid, a farnesoid X receptor agonist, has been shown to improve the histological features of NASH. Here we report results from a planned interim analysis of an ongoing, phase 3 study of obeticholic acid for NASH. Methods: In this multicentre, randomised, double-blind, placebo-controlled study, adult patients with definite NASH, non-alcoholic fatty liver disease (NAFLD) activity score of at least 4, and fibrosis stages F2-F3, or F1 with at least one accompanying comorbidity, were randomly assigned using an interactive web response system in a 1:1:1 ratio to receive oral placebo, obeticholic acid 10 mg, or obeticholic acid 25 mg daily. Patients were excluded if cirrhosis, other chronic liver disease, elevated alcohol consumption, or confounding conditions were present. The primary endpoints for the month-18 interim analysis were fibrosis improvement (≥1 stage) with no worsening of NASH, or NASH resolution with no worsening of fibrosis, with the study considered successful if either primary endpoint was met. Primary analyses were done by intention to treat, in patients with fibrosis stage F2-F3 who received at least one dose of treatment and reached, or would have reached, the month 18 visit by the prespecified interim analysis cutoff date. The study also evaluated other histological and biochemical markers of NASH and fibrosis, and safety. This study is ongoing, and registered with ClinicalTrials.gov, NCT02548351, and EudraCT, 20150-025601-6. Findings: Between Dec 9, 2015, and Oct 26, 2018, 1968 patients with stage F1-F3 fibrosis were enrolled and received at least one dose of study treatment; 931 patients with stage F2-F3 fibrosis were included in the primary analysis (311 in the placebo group, 312 in the obeticholic acid 10 mg group, and 308 in the obeticholic acid 25 mg group). The fibrosis improvement endpoint was achieved by 37 (12%) patients in the placebo group, 55 (18%) in the obeticholic acid 10 mg group (p=0·045), and 71 (23%) in the obeticholic acid 25 mg group (p=0·0002). The NASH resolution endpoint was not met (25 [8%] patients in the placebo group, 35 [11%] in the obeticholic acid 10 mg group [p=0·18], and 36 [12%] in the obeticholic acid 25 mg group [p=0·13]). In the safety population (1968 patients with fibrosis stages F1-F3), the most common adverse event was pruritus (123 [19%] in the placebo group, 183 [28%] in the obeticholic acid 10 mg group, and 336 [51%] in the obeticholic acid 25 mg group); incidence was generally mild to moderate in severity. The overall safety profile was similar to that in previous studies, and incidence of serious adverse events was similar across treatment groups (75 [11%] patients in the placebo group, 72 [11%] in the obeticholic acid 10 mg group, and 93 [14%] in the obeticholic acid 25 mg group). Interpretation: Obeticholic acid 25 mg significantly improved fibrosis and key components of NASH disease activity among patients with NASH. The results from this planned interim analysis show clinically significant histological improvement that is reasonably likely to predict clinical benefit. This study is ongoing to assess clinical outcomes.
