Abstract

The diagnosis and classification of lymphoma has evolved into a complex, multimodal process that requires rigorous attention to and quality assurance of numerous preanalytical, analytical, and postanalytical details. After exclusion [...], Context.--The diagnostic workup of lymphoma continues to evolve rapidly as experience and discovery led to the addition of new clinicopathologic entities and techniques to differentiate them. The optimal clinically effective, efficient, and cost-effective approach to diagnosis that is safe for patients can be elusive, in both community-based and academic practice. Studies suggest that there is variation in practice in both settings. Objective.--To develop an evidence-based guideline for the preanalytic phase of testing, focusing on specimen requirements for the diagnostic evaluation of lymphoma. Design.--The American Society for Clinical Pathology, the College of American Pathologists, and the American Society of Hematology convened a panel of experts in the laboratory workup of lymphoma to develop evidence-based recommendations. The panel conducted a systematic review of literature to address key questions. Using the Grading of Recommendations Assessment, Development, and Evaluation approach, recommendations were derived based on the available evidence, strength of that evidence, and key judgements as defined in the Grading of Recommendations Assessment, Development, and Evaluation Evidence to Decision framework. Results.--Thirteen guideline statements were established to optimize specimen selection, ancillary diagnostic testing, and appropriate follow-up for safe and accurate diagnosis of indolent and aggressive lymphoma. Conclusions.--Primary diagnosis and classification of lymphoma can be achieved with a variety of specimens. Application of the recommendations can guide decisions on specimen suitability, diagnostic capabilities, and correct use of ancillary testing. Disease prevalence in patient populations, availability of ancillary testing, and diagnostic goals should be incorporated into algorithms tailored to each practice environment. Abstract

A single degree of freedom model is presented for calculating the free-field seismic response of bridge embankments due to horizontal ground shaking using equivalent linear analysis and a design response spectrum. The shear wave velocity profile, base flexibility, 2D shape, and damping ratio of the embankment are accounted for in the model. A step-by-step procedure is presented for calculating the effective cyclic shear strain of the embankment, equivalent homogeneous shear modulus and damping ratio, fundamental period of vibration, peak crest acceleration, peak shear stress profile, peak shear strain profile, equivalent linear shear modulus profile, and peak relative displacement profile. Model calibration and verification of the proposed procedure is carried out with linear, equivalent linear, and nonlinear finite element analysis for embankments with fundamental periods of vibration between 0.1 and 1.0 s. The proposed model is simple, rational, and suitable for practical implementation using spreadsheets for a preliminary design phase of bridge embankments. On presente un modele a un seul degre de liberte pour le calcul de la reponse sismique en champ libre des remblais de ponts en raison des secousses horizontales du sol a l'aide d'une analyse lineaire equivalente et d'un spectre de reponse de conception. Le modele tient compte du profil de vitesse des ondes de cisaillement, de la flexibilite de la base, de la forme 2D et du facteur d'amortissement du remblai. C'est une procedure etape par etape, qui permet de calculer la contrainte cyclique effective de cisaillement du remblai, le module de cisaillement homogene equivalent et le facteur d'amortissement, mais aussi la periode fondamentale de vibration, l'acceleration de crete maximale, le profil de contrainte de cisaillement maximale, le profil de contrainte de cisaillement maximale, le profil de module de cisaillement lineaire equivalent et le profil de deplacement relatif maximal. La realisation de l'etalonnage et la verification du modele de la procedure proposee est faite grace a une analyse par elements finis lineaires, lineaires equivalents et non lineaires pour les remblais dont les periodes fondamentales de vibration sont comprises entre 0,1 et 1,0 s. Abstract

Flow cytometry is a technology that rapidly measures antigen-based markers associated to cells in a cell population. Although analysis of flow cytometry data has traditionally considered one or two markers at a time, there has been increasing interest in multidimensional analysis. However, flow cytometers are limited in the number of markers they can jointly observe, which is typically a fraction of the number of markers of interest. For this reason, practitioners often perform multiple assays based on different, overlapping combinations of markers. In this paper, we address the challenge of imputing the high dimensional jointly distributed values of marker attributes based on overlapping marginal observations. We show that simple nearest neighbor based imputation can lead to spurious subpopulations in the imputed data, and introduce an alternative approach based on nearest neighbor imputation restricted to a cell's subpopulation. This requires us to perform clustering with missing data, which we address with a mixture model approach and novel EM algorithm. Since mixture model fitting may be ill-posed, we also develop techniques to initialize the EM algorithm using domain knowledge. We demonstrate our approach on real flow cytometry data. more...

Abstract

Flow cytometry is often used to characterize the malignant cells in leukemia and lymphoma patients, traced to the level of the individual cell. Typically, flow cytometric data analysis is performed through a series of 2-dimensional projections onto the axes of the data set. Through the years, clinicians have determined combinations of different fluorescent markers which generate relatively known expression patterns for specific subtypes of leukemia and lymphoma -- cancers of the hematopoietic system. By only viewing a series of 2-dimensional projections, the high-dimensional nature of the data is rarely exploited. In this paper we present a means of determining a low-dimensional projection which maintains the high-dimensional relationships (i.e. information) between differing oncological data sets. By using machine learning techniques, we allow clinicians to visualize data in a low dimension defined by a linear combination of all of the available markers, rather than just 2 at a time. This provides an aid in diagnosing similar forms of cancer, as well as a means for variable selection in exploratory flow cytometric research. Abstract

We consider the problems of clustering, classification, and visualization of high-dimensional data when no straightforward Euclidean representation exists. Typically, these tasks are performed by first reducing the high-dimensional data to some lower dimensional Euclidean space, as many manifold learning methods have been developed for this task. In many practical problems however, the assumption of a Euclidean manifold cannot be justified. In these cases, a more appropriate assumption would be that the data lies on a statistical manifold, or a manifold of probability density functions (PDFs). In this paper we propose using the properties of information geometry in order to define similarities between data sets using the Fisher information metric. We will show this metric can be approximated using entirely non-parametric methods, as the parameterization of the manifold is generally unknown. Furthermore, by using multi-dimensional scaling methods, we are able to embed the corresponding PDFs into a low-dimensional Euclidean space. This not only allows for classification of the data, but also visualization of the manifold. Abstract

Although glycine prevents renal tubular cell injury in vitro, its effect in vivo is not clear. The purpose of this study was to investigate whether a bolus injection of glycine given before reperfusion plus continuous dietary supplementation afterward would reduce renal injury caused by ischemia-reperfusion. Female Sprague-Dawley rats received a semisynthetic powdered diet containing 5% glycine and 15% casein (glycine group) or 20% casein (control group). Two days later, renal ischemia was produced by cross-clamping the left renal vessels for 15 min, followed by reperfusion. The right kidney was removed before reperfusion. The postischemic glomerular filtration rate (GFR) showed that renal function was less impaired and recovered more quickly in rats receiving glycine. For example, at day 7, GFR in controls (0.31 [+ or -] 0.03 ml * [min.sup.-1] * 100 [g.sup.-1]) was about one-half that of glycine-treated rats (0.61 [+ or -] 0.06 ml * [min.sup.-1] * 100 [g.sup.-1], P < 0.05). Furthermore, tubular injury and cast formation observed in controls was minimized by glycine (pathology score, 3.2 [+ or -] 0.4 vs. 1.0 [+ or -] 0.4, P < 0.05). Urinary lactate dehydrogenase (LDH) concentration was elevated by ischemia-reperfusion in the control group (260 [+ or -] 22 U/l), but values were significantly lower by about fourfold (60 [+ or -] 30 U/l) in glycine-fed rats. Similarly, free radical production in urine was significantly lower in glycine-treated animals. Importantly, on postischemic day 1, binding of pimonidazole, an in vivo hypoxia marker, was increased in the outer medulla in controls; however, this phenomenon was prevented by glycine. Two weeks later, mild leukocyte infiltration and interstitial fibrosis were still observed in controls, but not in kidneys from glycine-treated rats. Abstract

Abstract Background Interleukin-33 is a member of the IL-1 cytokine family whose functions are mediated and modulated by the ST2 receptor. IL-33-ST2 expression and interactions have been explored in mouse macrophages but little is known about the effect of IL-33 on human macrophages. The expression of ST2 transcript and protein levels, and IL-33-mediated effects on M1 (i.e. classical activation) and M2 (i.e. alternative activation) chemokine marker expression in human bone marrow-derived macrophages were examined. Results Human macrophages constitutively expressed the membrane-associated (i.e. ST2L) and the soluble (i.e. sST2) ST2 receptors. M2 (IL-4 + IL-13) skewing stimuli markedly increased the expression of ST2L, but neither polarizing cytokine treatment promoted the release of sST2 from these cells. When added to naïve macrophages alone, IL-33 directly enhanced the expression of CCL3. In combination with LPS, IL-33 blocked the expression of the M2 chemokine marker CCL18, but did not alter CCL3 expression in these naive cells. The addition of IL-33 to M1 macrophages markedly increased the expression of CCL18 above that detected in untreated M1 macrophages. Similarly, alternatively activated human macrophages treated with IL-33 exhibited enhanced expression of CCL18 and the M2 marker mannose receptor above that detected in M2 macrophages alone. Conclusions Together, these data suggest that primary responses to IL-33 in bone marrow derived human macrophages favors M1 chemokine generation while its addition to polarized human macrophages promotes or amplifies M2 chemokine expression. more...
