Abstract

An increasing number of women with urea cycle disorders (UCDs) are reaching child‐bearing age and becoming pregnant. Improved diagnostics and increased awareness of inherited metabolic diseases has also led to more previously undetected women being diagnosed with a UCD during or shortly after pregnancy. Pregnancy increases the risk of acute metabolic decompensation with hyperammonemia—which can occur in any trimester, and/or the postpartum period, and may lead to encephalopathy, psychosis, coma, and even death, if not diagnosed promptly and treated appropriately. There are also (theoretical) concerns that a maternal UCD, or its treatment, may cause potential risks for the unborn child. Currently evidence on management and outcome of pregnancies in UCDs is limited to case reports and there are no clear guidelines. In order to inform management and investigate outcomes of pregnancies in women with a UCD, we performed a retrospective review of published cases and analyzed data collected from an international online survey. We conclude that, although risk during the intra‐ and postpartum period exists, multidisciplinary management by an experienced team and a prospective plan usually result in successful pregnancy, labor, delivery, and postpartum period. No deaths were reported in mothers managed accordingly. With the exception of male neonates with Ornithine Transcarbamylase deficiency, the clinical outcome of children born to mothers with UCDs appears positive, although follow‐up is limited. The outcome for women presenting with a first acute metabolic decompensation during pregnancy or postpartum is less favorable. Deaths were associated with diagnostic delay/late management of hyperammonemia in previously undiagnosed women. [ABSTRACT FROM AUTHOR]

Abstract

This article, winner of the 2022 Marna Feldt Graduate Publication Award, explores the concept of postmemory in relation to Sweden’s cultural memory of World War II. Through an analysis of Göran Rosenberg’s memoir "Ett kort uppehåll på vägen från Auschwitz" (2012), translated as "A Brief Stop on the Road from Auschwitz," this article investigates how the representation of postmigrant identity and belonging relates to revisionist historiography regarding Sweden’s positionality during World War II. Furthermore, this article illuminates how exploring the postmemory trauma of the children of Holocaust survivors is relevant to the current discourse in Sweden’s contemporary transcultural society., Cet article, gagnant du Prix Marna Feldt de publication pour diplomé [graduate], explore le concept de post-mémoire dans la mémoire culturelle de la Seconde Guerre mondiale en Suède. Abstract

Research was conducted to examine the hypothesis that pituitary adenylate cyclase-activating polypeptide (PACAP) stimulates insulin secretion, insulin sensitivity and glucose disposal after an intravenous glucose challenge in anesthetized mice. Seven samples were administered with an intravenous glucose tolerance test to assess the minimal model of glucose disappearance for glucose and insulin data analysis. Results indicate that PACAP inhibits insulin sensitivity in mice in addition to stimulating insulin secretion.

Bakgrund: Reumatoid artrit, RA, är en systemisk, autoimmun och livslång sjukdom. Runt 2 000 personer nydiagnostiseras i Sverige varje år. Kvinnor drabbas i högre grad än män. Syfte: Syftet var att beskriva personers upplevelse av att leva med reumatoid artrit. Metod:En litteraturöversikt baserad på 14 artiklar med kvalitativ ansats. Resultat:RA påverkar hela livet, fysiskt, psykiskt och socialt. Den är högst individuell, orsakar smärta, förlust och sorg. Vardagsliv och relationer förändras och personen med RA tvingas till anpassning och förändring. Många behöver hjälp av familjen och samhället. RA innebär också en livslång behandling och kontakt med vården. Slutsatser:Vid omvårdnaden av personer med RA, kan sjuksköterskan genom ett personcentrerat förhållningssätt, fånga upp varje persons unika upplevelse av livet och sjukdomen. Det är också viktigt att vara medveten om att upplevelsen av RA kan variera från en dag till en annan och att ta personen på allvar. Sjuksköterskan kan också ha en viktig roll i att upptäcka sjukdomen. Many people need support from family and society. RA also means a life-long treatment and contact with the healthcare system. Conclusion:With a person-centered care approach, the nurse is able to capture every person's unique experience of life and the disease, when caring for a person with RA. It is also important to be aware that the experience of RA can vary from one day to another and to take the persons own experience seriously. To detect RA, the nurse can play an important role. 21. PACAP contributes to insulin secretion after gastric glucose gavage in mice

Am J Physiol Regulatory Integrative Comp Physiol 279: R424-R432, 2000.--Pituitary adenylate cyclase-activating polypeptide (PACAP) is localized to pancreatic ganglia governing the parasympathetic nerves, which contribute to prandial insulin secretion. We hypothesized that this contribution involves PACAP and show here that the PACAP receptor antagonist PACAP-(6--27) (1.5 nmol/kg iv) reduces the 15-min insulin response to gastric glucose (150 mg/ mouse) by 18% in anesthetized mice (P = 0.041). The reduced insulinemia was not due to inhibited release of the incretin factor glucagon-like peptide 1 (GLP-1) because PACAP-(6--27) enhanced the GLP-1 response to gastric glucose. Furthermore, the GLP-1 antagonist exendin-3-(9--39) (30 nmol/kg) exerted additive inhibitory effect on the insulin response when combined with PACAP-(6--27). The PACAP antagonism was specific because intravenous PACAP-(6--27) inhibited the insulin response to intravenous PACAP-27 plus glucose without affecting the insulin response to intravenous glucose alone (1 g/kg) or glucose together with other insulin secretagogues of potential incretin relevance of intestinal (GLP-1, gastric inhibitory polypeptide, cholecystokinin) and neural (vasoactive intestinal peptide, gastrin-releasing peptide, cholinergic agonism) origin. 32. Evidence for Contribution by Increased Cytoplasmic Na+to the Insulinotropic Action of PACAP38 in HIT-T15 Cells*

Abstract

Pituitary adenylate cyclase-activating polypeptide (PACAP) is localized to pancreatic nerve terminals and stimulates insulin secretion. The insulinotropic effect of PACAP38 in insulin-producing HIT-T15 cells is accompanied by increases in cellular cAMP and cytoplasmic Ca2+([Ca2+]cyt). As also intracellular Na+is important for insulin secretion after glucose and other cAMP forming peptides, we examined the Na+dependence of the insulinotropic effect of PACAP38 in HIT-T15 cells. We found that PACAP38 (100 nm)-induced insulin secretion was diminished by approximately 50% by removal of extracellular Na+(replaced by equimolar N-methyl-d-glucamine). In contrast, removal of Na+did not diminish the formation of cellular cAMP (measured by radioimmunoassay) or the increase in [Ca2+]cyt(measured in FURA-2AM-loaded cell suspensions) induced by PACAP38. Furthermore, PACAP-38 increased the cytoplasmic Na+([Na+]cyt) in single HIT-T15 cells as measured by the fluorophore sodium-binding benzofran isophthalate. This increase was reduced by removal of extracellular Na+and by inhibition of protein kinase A by H-89. We conclude that the insulinotropic action of PACAP38 is Na+-dependent. We propose that PACAP38 opens plasma membrane Na+channels by an action partially mediated by cAMP and protein kinase A, and the subsequent raise in [Na+]cytelicits insulin secretion by an as yet unsolved mechanism.
