The enchondroma is a benign tumor of the endosteal cartilage and it is common in young-adult patients. These tumors are usually diagnosed when a pathologic fracture occurs at the site of the lesion or when they are accidentally discovered during investigations performed for other reasons. Background and aim of the work: The incidence of long bone non-unions has been estimated to range between 5-10%. Nonunion of fracture is a delayed complication of fracture. A large bone resection, associated with Ilizarov’s osteo-distraction technique, is commonly used in these cases. The war experience was very important for dealing with these injuries. The purpose of this study is to report whether the use of Platelet Rich of Plasma(PRP) or Hyperbric Oxygen Therapy(HOT) as an adjuvant to the osteogenic distraction of Ilizarov with respect to the classical method has advantages. Methods: From 183 tibial non union, we enrolled 50 patients suffering by Type B according ASAMI non union classification. We divided the patients into two groups. The first group was a retrospective group of patient treated by Ilizarov Tecnique plus PRP. Instead the second group, patients were treated by Ilizarov Tecnique associated with HOT. The chosen criteria to evaluate the two groups during the clinical and radiological follow-up were: the complication after the surgery in the two groups; the duration of surgery; the objective quality Bone results and functional results were evaluated according to ASAMI classification while the subjective quality of life correlated with Ilizarov frame function by the Short Form 12 Health Survey (SF-12); The correlation between bone regenerate/bone healing and X-rays. The evaluation endpoint was set at 12 months from the remotion of Ilizarov’s frame for both groups. Results: In comparing the complications of the two populations, there were a significant statistically difference(p0.05. The average Time for remove Ilizarov’s Frame in months was 15.37(±7.34; range 9–32) in PRP while in HOT was15.22(± 7.83; range 9–31), p>0.05. Conclusions: From our study we can conclude that the association of HOT and PRP with the Ilizarov technique does not improve the functional outcomes but allows a more rapid healing of the regenerated bone and therefore an early removal of the device and a corresponding improvement in the quality of life. Aim Periprosthetic fracture after knee arthroplasty occurs more frequently in the supracondylar area of femur, especially after low energy trauma associated with torsional or compressive forces. Several techniques have been described for the treatment of displaced fractures. The aim of this study is the evaluation of the outcomes and bone healing of periprosthetic femoral fractures managed by standard plate fixation compared to plating with bone grafting. Methods Thirty-six periprosthetic fractures around the knee were selected. Eighteen patients underwent standard plate and screws fixation while other eighteen were treated by plating associate with a cortical strut. Knee Society Score (KSS) and Short Form 12 (SF12) with the UNION SCORE (RUS) were used for the evaluation of results. Results After a minimum follow-up of 12 months, the results showed a statistically significant difference in SF-12, KSS, and RUS in favour of plating associated to bone graft with respect to the plating alone; four cases of non-union were recorded in the group of patients treated by standard plating. Conclusions Our experience once again demonstrated that plating and bone grafting may ensure a mechanical and biological support for the healing of periprosthetic fracture of the knee more than simple plating.

To evaluate the role of quantitative digital subtraction angiography (Q-DSA) with parametric color coding (PCC) in assessing patients with type B chronic thoracic aortic dissection (TBCAD) during thoracic endovascular aortic repair (TEVAR) procedures.A total of 11 patients electively treated in our Department for a TBCAD were retrospectively enrolled. All cases were treated with TEVAR for false lumen aneurysm of the thoracic descending aorta. For digital subtraction angiography (DSA) series post-processing, a newly implemented PCC algorithm was used to turn consecutive two-dimensional images into a single color-coded picture (syngo iFLOW, Siemens AG, Forchheim, Germany). In consensus reading, two clinicians experienced in vascular imaging evaluated the DSA series in blinded assessment and compared them to the color-coded images. PCC was assessed for its accuracy in identifying the true and false lumen as well as whether it could provide improved visualization in pre-deployment stent grafting and the final evaluation of treatment.PCC facilitated the visualization of the aortic dissection angioarchitecture in terms of contemporary true and false lumen vision in 81.8% of the cases. In 72.7% of the procedures, Q-DSA was estimated to improve aorta information assessment in terms of false lumen viewing, and it was possible to identify the proximal entry tear position in 45.4% of the cases. After stent graft deployment, in 72.7% of the cases (all 8 patients in which the aortic arch false lumen was visible in pre-treatment), Q-DSA confirmed the absence of early false lumen reperfusion.Our results indicate that Q-DSA could be useful in the intraprocedural evaluation of patients with aortic dissection during TEVAR procedures without additional x-ray costs and contrast exposure.

To assess whether angiotensin-converting enzyme inhibitors and third-generation dihydropyridine calcium channel blockers ameliorate diabetic complications, we compared glomerular filtration rate (GFR; primary outcome), cardiovascular events, retinopathy, and neuropathy in 380 hypertensive type 2 diabetics with albuminuria 2 (IQR: 0.16–0.50 mL/min per 1.73 m 2 ) on combined therapy, 0.36 mL/min per 1.73 m 2 (IQR: 0.18–0.53 mL/min per 1.73 m 2 ) on delapril, and 0.30 mL/min per 1.73 m 2 (IQR: 0.12–0.50 mL/min per 1.73 m 2 ) on placebo ( P =0.87 and P =0.53 versus combined therapy or delapril, respectively). Similar findings were observed when baseline GFR values were not considered for slope analyses. Albuminuria was stable in the 3 treatment groups. The hazard ratio (95% CI) for major cardiovascular events between combined therapy and placebo was 0.17 (0.04–0.78; P =0.023). Among 192 subjects without retinopathy at inclusion, the hazard ratio for developing retinopathy between combined therapy and placebo was 0.27 (0.07–0.99; P =0.048). Among 200 subjects with centralized neurological evaluation, the odds ratios for peripheral neuropathy at 3 years between combined therapy or delapril and placebo were 0.45 (0.24–0.87; P =0.017) and 0.52 (0.27–0.99; P =0.048), respectively. Glucose disposal rate decreased from 5.8±2.4 to 5.3±1.9 mg/kg per min on placebo ( P =0.03) but did not change on combined or delapril therapy. Treatment was well tolerated. In hypertensive type 2 diabetic patients, combined manidipine and delapril therapy failed to slow GFR decline but safely ameliorated cardiovascular disease, retinopathy, and neuropathy and stabilized insulin sensitivity.

The optimization of optical properties of buildings' envelope and urban paving represents an important research field for reducing the urban heat island effect. The overheating of a surface exposed to sunlight can be reduced by improving solar reflectance. In this sense, several studies have demonstrated the positive effect of cool materials on UHI mitigation.In addition to traditional cool materials, retroreflective (RR) materials have been recently proposed for this application. The present paper aims at the assessment of angular reflectance of RR films for several inclination angles of solar radiation. To reproduce variation of solar radiation's inclination during the daytime, an ad hoc experimental setup was designed and used. Characterization of RR materials when hit by solar radiation with different inclinations allows to assess their behaviour on daytime if used as novel urban coatings for mitigation of the UHI phenomenon.Measurement results are used as input for an original algorithm which allows to quantify cooling potential of RR materials in terms of energy reflected and sent beyond the urban canyon. The experimental characterization and energy evaluations showed that RR materials could be effectively applied as coatings on urban paving and building envelope, in order to reduce the circulating energy into the canyon. © 2015 Elsevier Ltd.

The climate of high-density urban areas is often affected by the air temperature increase with respect to the neighbouring country-side. This phenomenon, known as the urban heat island (UHI) effect, is strongly influenced by the solar reflectance of building envelope and coating materials, and it is enhanced in the presence of built patterns that trap the solar and anthropogenic energy, usually referred to as urban heat canyons. An original method to quantify the urban heat canyon effect as a function of meteorological conditions, geometry, and surface properties is proposed. The goal is to provide a reliable tool to estimate the effect of the reflective properties of the canyon surfaces on the urban environment, in order to guide the choice of effective solution towards the UHI mitigation. An energy-balanced analytic model, specifically set-up to predict surface temperatures inside an urban canyon, is applied to a scale test facility located at the University of Perugia, Italy. The test facility is made of two twin arrays resembling urban canyons with different aspect ratios. Each canyon can be equipped with reflective films to quantify the radiative exchange variation. Preliminary results from the experimental facility monitoring and the analytic model validation are presented. © 2013 Taylor & Francis.

Canine anal melanoma is an aggressive neoplasm that rapidly leads to constipation in dogs, thus mimicking the behavior of their human counterpart. In this paper, the successful local palliation of this neoplasm is described using cisplatin selectively driven within the tumor cells by trains of biphasic pulses. The dog experienced tumor reduction with restoration of normal defecation for three months, then experienced massive dissemination to the sublumbar lymph nodes that led to intestinal obstruction and euthanasia. Electrochemotherapy (ECT) is a safe palliative therapy for such neoplasm and warrants further investigations in dogs as well humans.

This book chapter presents the main key innovations being devised, implemented and validated in the scope of Anastacia H2020 EU research project, to meet the cybersecurity challenge of protecting dynamically heterogenous IoT scenarios, endowed with SDN/NFV capabilities, which face evolving kind of cyber-attacks. The key innovations encompasses, among others, policy-based security management in IoT networks, trusted and dynamic security orchestration of virtual networks security functions using SDN/NFV technologies, security monitoring and cognitive reaction to countering cyber-treats, behavioural analysis, anomaly detection and automated testing for the detection of known and unknown vulnerabilities in both physical and virtual environments as well as secured and authenticated dynamic seal system as a service.
