2. Retos para los sistemas sanitarios de Latinoamérica: ¿qué puede aprenderse de la experiencia europea?

Abstract

Este artículo compara los retos de los sistemas sanitarios latinoamericanos y la experiencia en Europa. El marco conceptual se centra en cuatro funciones: a) generar recursos; b) producir intervenciones; c) financiar, y d) «ejercer rectoría». Es a este nivel donde los actores pueden influir sobre el desempeño del sistema. Se identifican cinco retos para Latinoamérica: a) extender (prepago y solidaridad) la protección financiera; b) estabilizar en el tiempo dicha protección para épocas de crisis; c) equilibrar los recursos coherentemente con la capacidad de financiar servicios, d) aumentar la eficiencia ubicativa y técnica al producir servicios, y e) mejorar la función de rectoría de las demás funciones en los sectores público y privado (el más difícil y más importante reto hoy para los sistemas latinoamericanos). Se analiza luego la experiencia de reforma en Europa, presentando: a) experiencias sobre protección financiera en los sistemas tipo Beveridge y Bismarck; b) estabilidad en tiempos de crisis refrendada recientemente (Oeste) y con graves obstáculos (Este); c) el esfuerzo por equilibrar camas hospitalarias y profesionales sanitarios combinando regulación e incentivos; d) un aumento de la eficiencia en la producción de servicios priorizando más expresamente, dando voz a los pacientes, descentralizando la gestión y con incentivos de mercado, y e) una mejora de la rectoría no regulando menos sino mejor (y en algunos casos, más). Tres áreas de la experiencia europea sobresalen: a) combinar solidaridad con sostenibilidad financiera; b) introducir mesuradamente incentivos de mercado pero manteniendo un claro papel rector del Estado, y c) adoptar innovaciones en la organizacíon y producción de servicios. Pese a las dificultades metodológicas, la convergencia de los retos y las «soluciones» adoptadas justifica este análisis, pero las enseñanzas deben contemplarse desde cada contexto nacional. Un futuro artículo abordará las lecciones ofrecidas por las reformas de los sistemas latinoamericanos a las reformas europeas.

Abstract

In this paper, we present and illustrate a general methodology to apply KAM theory in particular problems, based on an a posteriori approach. We focus on the existence of real analytic quasi-periodic Lagrangian invariant tori for symplectic maps. The purpose is to verify the hypotheses of a KAM theorem in an a posteriori format: Given a parameterization of an approximately invariant torus, we have to check non-resonance (Diophantine) conditions, non-degeneracy conditions and certain inequalities to hold. To check such inequalities, we require to control the analytic norm of some functions that depend on the map, the ambient structure and the parameterization. To this end, we propose an efficient computer-assisted methodology, using fast Fourier transform, having the same asymptotic cost of using the parameterization method for obtaining numerical approximations of invariant tori. We illustrate our methodology by proving the existence of invariant curves for the standard map (up to [Formula omitted]), meandering curves for the non-twist standard map and 2-dimensional tori for the Froeschlé map., Author(s): J.-Ll. Abstract

Background: Major surgery is a term frequently used but poorly defined. The aim of the present study was to reach a consensus in the definition of major surgery within a panel of expert surgeons from the European Surgical Association (ESA). Methods: A 3-round Delphi process was performed. All ESA members were invited to participate in the expert panel. In round 1, experts were inquired by open- and closed-ended questions on potential criteria to define major surgery. Results were analyzed and presented back anonymously to the panel within next rounds. Closed-ended questions in round 2 and 3 were either binary or statements to be rated on a Likert scale ranging from 1 (strong disagreement) to 5 (strong agreement). Participants were sent 3 reminders at 2-week intervals for each round. 70% of agreement was considered to indicate consensus. Results: Out of 305 ESA members, 67 (22%) answered all the 3 rounds. Significant comorbidities were the only preoperative factor retained to define major surgery (78%). Vascular clampage or organ ischemia (92%), high intraoperative blood loss (90%), high noradrenalin requirements (77%), long operative time (73%) and perioperative blood transfusion (70%) were procedure-related factors that reached consensus. Regarding postoperative factors, systemic inflammatory response (76%) and the need for intensive or intermediate care (88%) reached consensus. Consequences of major surgery were high morbidity (>30% overall) and mortality (>2%). Conclusion: ESA experts defined major surgery according to extent and complexity of the procedure, its pathophysiological consequences and consecutive clinical outcomes.

Abstract

International audience; In order to take action against global warming and ensure a greater energy independence, countries around the world are expected to drastically increase the proportion of renewable energy in their energy mix. However, the intermittent production of energy explains why energy supply and demand do not match. In this context, biomethanation, coupled with anaerobic digestion, could be an interesting approach to transform the extra amount of produced electricity by converting hydrogen (produced by electrolysis) and carbon dioxide (present in biogas) into methane. In this review, we have summarized several recently published results which involve biological methanation processes performed by mixed cultures, with an emphasis on microbiological as well as process aspects. In particular, the different microorganisms involved in the process, as well as the used metabolic pathways, along with their kinetic and thermodynamic specificities, are described. Furthermore, the influence of process parameters such as the type of reactor, the type of diffuser and the choice of H-2 injection (in situ or ex situ) or the different operating conditions are presented. Explanations of the different performances observed in literature are assumed, technical bottlenecks are listed, and possible solutions to overcome these issues are presented. Finally, the current commercial deployment of this technology is discussed through the example of three companies offering different biomethanation solutions.

Abstract

International audience; In a circular economy approach, heterogeneous wastes can be converted to syngas by pyro-gasification. The produced syngas can then be upgraded to methane to make use of the existing natural gas infrastructures. In this work, we report the performance of a biomethanation process to produce methane from syngas at high pressure and high syngas load. Experiments were performed in a 10L continuous stirred tank reactor at 4 bars and 55°C. A syngas mixture (40% CO, 40% H2 and 20% CO2) was injected continuously. The inlet flow rate was gradually increased in steps of 20% from 7.5 NL/h to 56.5 NL/h. Methane productivity increased from 6.2 mmolCH4/Lreactor/h to 38.5 mmolCH4/Lreactor/h, which is to our knowledge the highest methane productivity obtained with syngas iomethanation. The conversion efficiencies dropped accordingly from 95% to 82% for CO and from 97% to 89% for H2. This drop in conversion efficiencies is attributed to mass transfer limitations. The 20% increase strategy allowed for biomass concentration to increase gradually and methane production was not interrupted when at each flow increase. These results are very encouraging for syngas biomethanation, since methane productivity as high as 1 LCH4/Lreactor/h was obtained with our reactor which was not optimized in terms of mass transfer. This shows great promise for the future of the technology and is a remarkable step towards the industrialization of the process.

Abstract

OBJECTIVE: To elucidate minor hepatectomy (MiH) outcomes. SUMMARY BACKGROUND DATA: Liver surgery has moved toward a parenchyma-sparing approach, favoring MiHs over major resections. MiHs encompass a wide range of procedures. METHODS: We retrospectively evaluated consecutive patients who underwent open liver resections in 17 high-volume centers. EXCLUSION CRITERIA: cirrhosis and associated digestive/biliary resections. Resections were classified as (Brisbane nomenclature): limited resections (LR); (mono)segmentectomies/bisegmentectomies (Segm/Bisegm); right anterior and right posterior sectionectomies (RightAnteriorSect/RightPosteriorSect). Additionally, we defined: complex LRs (ComplexLR = LRs with exposed vessels); postero-superior segmentectomies (PosteroSuperiorSegm = segment (Sg)7, Sg8, and Sg7+Sg8 segmentectomies); and complex core hepatectomies (ComplexCoreHeps = Sg1 segmentectomies and combined resections of Sg4s+Sg8+Sg1). Left lateral sectionectomies (LLSs, n = 442) and right hepatectomies (RHs, n = 1042) were reference standards. Outcomes were adjusted for potential confounders. RESULTS: Four thousand four hundred seventy-one MiHs were analyzed. Compared with RHs, MiHs had lower 90-day mortality (0.5%/2.2%), severe morbidity (8.6%/14.4%), and liver failure rates (2.4%/11.6%, P < 0.001), but similar bile leak rates. LR and LLS had similar outcomes. ComplexLR and Segm/Bisegm of anterolateral segments had higher bile leak rates than LLS rates (OR = 2.35 and OR = 3.24), but similar severe morbidity rates. ComplexCoreHeps had higher bile leak rates than RH rates (OR = 1.94); the severe morbidity rate approached that of RH. PosteroSuperiorSegm, RightAnteriorSect, and RightPosteriorSect had severe morbidity and bile leak rates similar to RH rates. MiHs had low liver failure rates, except RightAnteriorSect (vs LLS OR = 4.02). CONCLUSIONS: MiHs had heterogeneous outcomes. Mortality was low, but MiHs could be stratified according to severe morbidity, bile leak, and liver failure rates. Some MiHs had postoperative outcomes similar to RH.

Abstract

International audience; Dans une logique d'économie circulaire, la gazéification permet de convertir des déchets organiques hétérogènes en énergie sous la forme de syngas (CO, H2, CO2 en majeurs). Transformer ensuite le syngas en méthane permet d'améliorer le pouvoir calorifique du gaz et d'utiliser les infrastructures déjà existantes de transport et stockage du gaz naturel. La biométhanation, moins chère et plus facile à mettre en oeuvre que la méthanation catalytique, est une solution intéressante faisant l'objet de plus en plus de recherche (Grimalt-Alemany et al., 2020; Li et al., 2020). Cependant, aucune étude n'a encore expérimenté à notre connaissance la biométhanation du syngas en continu et sous pression (Grimalt-Alemany et al., 2018).Dans le cadre de ce travail, un pilote de 12L à 4 bars fonctionnant en continu a été mis en œuvre afin de traiter un mélange type syngas (40% CO, 40% H2, 20% CO2). Le transfert gaz-liquide, enjeu limitant de la biométhanation, est assuré par agitation et a été caractérisé en eau claire. Après une période d’adaptation et d’ajustement des nutriments nécessaires au bon fonctionnement des micro-organismes, une conversion stable et performante de 99% de l’H2 et de 97% du CO a été atteinte (Fig. 1). Le gaz ainsi obtenu est proche des proportions stœchiométriques correspondant à 33% de CH4 et 66% de CO2 (Eq. 1 et 2). L’arrêt de la méthanogenèse (Eq. 2) par déficience en nutriments (jour 45) laisse apparaitre la réaction intermédiaire de conversion du CO en H2 et CO2 (Eq. 1).CO + H2O → H2 + CO2 (1)4H2 + CO2 → CH4 + 2H2O (2)
