OBJETIVO: Compreender as possibilidades e os limites da articulação dos processos de trabalho desenvolvidos por agentes com diferentes formações para otimizar a integração e melhorar a qualidade da assistência aos pacientes com HIV/Aids. MÉTODOS: Estudo qualitativo sobre o trabalho multiprofissional em cinco centros de referência para DST/Aids do Município de São Paulo. Foram realizadas entrevistas semi-estruturadas com 26 profissionais de diferentes formações, enfocando suas relações no modo de organização da assistência prestada nesses serviços. RESULTADOS: Houve diferenças significativas do alcance da integração multiprofissional e das possibilidades de enriquecimento da assistência prestada, de acordo com as circunstâncias em que o trabalho interdisciplinar é posto em ação. CONCLUSÕES: Quando a equipe consegue trabalhar com demandas antevistas, isto é, com a formulação, por um conjunto de profissionais, de projetos assistenciais, antecipando demandas a partir de situações concretas da prática, criam-se condições favoráveis a um trabalho mais efetivamente integrado da equipe multiprofissional. Essa integração favorece intervenções que permitem um diálogo mais rico entre a aplicação do tratamento medicamentoso e outras dimensões relevantes do cuidado referentes às vivências sociais, psicológicas e emocionais dos pacientes.

O artigo tem como objetivo compreender como a Política Nacional de Atenção Integral à Saúde dos Homens (PNAISH) chega aos serviços da Atenção Básica do Sistema Único de Saúde, a partir do ponto de vista dos seus profissionais. Acompanhamos o caminho trilhado pela PNAISH com um estudo, etnográfico, de cinco casos (de cada Macro Região do país). A análise está ancorada no referencial da Antropologia e emprega a ideia de street-level bureaucreacy, de Lipsky. A formulação da PNAISH é contextualizada em termos históricos e da produção de documentos internacionais e nacionais. Reconhecendo a distância entre formulação e implementação de qualquer política, buscamos compreender como tal distância foi se construindo, na tradução da PNAISH para Planos de Ação (PA) municipais. Observou-se desconhecimento dos agentes implementadores sobre a PNAISH, a rede local de atenção à saúde do homem, as técnicas para atender às especificidades dos homens e o conceito de gênero. A implementação esbarrou na ausência de condições institucionais, como uma estrutura organizacional, uma rede consolidada de atenção – em que o usuário seja atendido por serviços com diferentes graus de complexidade dentro do sistema – e recursos em geral, especialmente humanos. The scope of this article is to see how the Brazilian Comprehensive Healthcare Policy for Men (PNAISH) has been implemented in the Unified Health System, from the standpoint of health professionals. A case study, involving five cases (each from a different macro region of Brazil) conducted using ethnographic techniques of data collection charted the progress of PNAISH implementation based on an anthropological approach using Lipsky’s idea of street-level bureaucracy. PNAISH is contextualized in historical terms with national and international documents. Acknowledging the inevitable gap between the formulation and the implementation of any policy, an attempt is made to see how this gap has evolved by analyzing the transition of PNAISH into city Action Plans (PAs). It was revealed that the implementing agents had little knowledge of PNAISH, of the local health care network for men, of the techniques required to meet men’s specific needs and of the concept of gender. It faced institutional obstacles, such as lack of an organizational structure, of a consolidated healthcare network – where the user receives services with different degrees of complexity within the system – and resources in general, especially human resources.

Estudou-se a relação do exercício das masculinidades com o cuidado em saúde para homens na atenção primária, por meio de representações e significados de usuários e trabalhadores acerca do que vem ser homem. Foram entrevistados homens usuários e profissionais de saúde de dois serviços de atenção primária. Encontrou-se uma diversidade de modelos de masculinidade que podem definir diferentes formas para pensar o cuidado de saúde dos homens. Observou-se que questões como trabalho, sexualidade, estrutura corporal, relações com as mulheres e transformações nas relações de gênero são temas importantes para os homens e devem ser consideradas nos serviços de saúde. This paper studied the relation between the exercise of masculinities and health care of men in primary health care services. It focuses on representations and meanings of primary health care service users and professional of what it is to be a man. Male service users and professionals were interviewed in two primary health care facilities. Diverse models of masculinity were found, defining various forms of reasoning upon men's health care. This study indicates that issues such as work, sexuality, body structure, relationship with women, and transformations in gender relations are important for men, and should be considered in health care services.

OBJETIVO: Compreender as percepções das equipes de farmácia sobre seu papel nos desafios assistenciais e adesão aos anti-retrovirais de usuários de drogas injetáveis vivendo com HIV/Aids. MÉTODOS: Estudo qualitativo por grupos focais e análise temática das falas produzidas com farmacêuticos, técnicos e auxiliares com experiência superior a seis meses na dispensação de medicamentos, de 15 unidades assistenciais de DST/Aids do município de São Paulo, em 2002. RESULTADOS: Formaram-se três grupos, totalizando 29 participantes, provenientes de 12 dos 15 serviços existentes, com 12 universitários e 17 profissionais de nível médio. Os grupos concluíram que a farmácia tem importante papel na dispensação de anti-retrovirais, o que se reflete na adesão ao tratamento, pois por meio de seus procedimentos podem ser construídas relações de confiança. Avaliam, porém, que tal construção não ocorra pela excessiva burocratização de suas atividades. Isso repercute negativamente para todos os pacientes, em especial sobre usuários de drogas injetáveis, concebidos como "pessoas difíceis". Tal concepção essencializa seus comportamentos: seriam confusos, com limites de compreensão e sem possibilidades de adesão ao tratamento. Os profissionais, porém, afirmam que tratam esses pacientes tal como os demais, igualando-os. Não percebem neste proceder, uma invisibilização das necessidades específicas dos usuários de drogas injetáveis no serviço, bem como a possibilidade de gerarem estereótipos estigmatizantes com barreira adicional ao trabalho da adesão. CONCLUSÕES: Embora a farmácia seja indicada como espaço potencialmente favorável para escuta e construção de vínculos com os usuários, os resultados mostram obstáculos objetivos e subjetivos para torná-la propícia para trabalhar a adesão. OBJECTIVE: To understand the perceptions of pharmacy teams about their role in the healthcare assistance challenges and adherence to antiretroviral therapy by injecting drug users living with HIV/AIDS. METHODS: Qualitative study through focus groups and thematic discourse analysis of pharmacists, technicians and assistants with more than six months of experience with medication supply, in 15 assisting units for STD/AIDS in the city of São Paulo, in 2002. RESULTS: Three groups were formed, totaling 29 participants, originating from 12 out of the 15 existing services, and including 12 university level professionals and 17 high-school level professionals. The groups concluded that the pharmacy has an important role in the antiretroviral drug supply, which is reflected in the treatment adherence, because trust-based relationships can be built up through their procedures. In spite of this, they pointed out that such building-up does not take place through excessively bureaucratic activities. This has negative repercussions for all patients, especially for injecting drug users, considered "difficult people". Such concept sums up their behavior: they are supposed to be confused and incapable to adhere to treatment, and have limited understanding. Staff members, however, affirm they treat these patients equally. They do not realize that, by this acting, the specific needs of injecting drug users may become invisible in the service. There is also the possibility that stigmatizing stereotypes may be created, resulting in yet another barrier to the work on adherence. CONCLUSIONS: Although the pharmacy is recommended as a potentially favorable place to listen to and form bonds with users, the results show objective and subjective obstacles to render it suitable for the work on adherence.

OBJETIVO: Descrever a influência das concepções dos profissionais de saúde sobre o cuidado prestado a pacientes usuários de drogas vivendo com HIV/Aids. MÉTODOS: Estudo qualitativo baseado em entrevistas semi-estruturadas com 22 profissionais de diferentes categorias, pertencentes a dois serviços especializados em DST/Aids da rede municipal de São Paulo, em 2002. As entrevistas foram gravadas e submetidas à análise temática. RESULTADOS: Os profissionais relataram dificuldades em identificar usuários de drogas entre seus pacientes, indicando a invisibilidade da questão. Acham os usuários de drogas pacientes mais difíceis de tratar, por tumultuarem o serviço e/ou não aderirem ao tratamento. Embora reconheçam necessidades especiais dos pacientes usuários e que lidar com o uso de drogas seja importante, os profissionais de saúde acreditam que essas questões fogem de suas atribuições. Os profissionais mostraram limites pessoais e técnicos para o manejo desses casos, indicando sua falta de capacitação específica como importante. Assim, recomendam a criação de serviços especializados para esse atendimento, reconhecendo os serviços em que atuam como inadequados e, embora conhecessem o projeto de redução de danos, pouco participavam dele. CONCLUSÕES: Elementos técnicos, ideológicos e pessoais, tais como crenças, valores e dimensões afetivo-emocionais, mostraram-se relevantes para ampliar ou recusar vínculos mais específicos com o paciente usuário de drogas. As concepções sobre o uso de drogas podem interferir no desenvolvimento de uma assistência melhor e da eqüidade no cuidado em saúde. OBJECTIVE: To describe the influence of conceptions of health professionals on the care given to HIV/AIDS patients using drugs. METHODS: Qualitative study based on semi-structured interviews with 22 professionals of different levels from two specialized STD/AIDS public services of the city of São Paulo was conducted in 2002. The interviews were recorded and submitted to a thematic analysis. RESULTS: Professionals reported difficulties in identifying drug users among their patients, indicating the invisibility of the issue. They find drug users more difficult to treat, because they disturb the service and do not comply with treatment. Although they acknowledge the special needs of users, and that it is important to deal with drug use, health professionals believe that these issues are not their responsibility. Professionals showed personal and technical limits in handling these cases, showing the importance of their lack of specific capacity building. Thus, they recommend the creation of specialized services for this care, recognizing their own services as inappropriate. Although they were aware of the harm reduction project, there was a little participation in it. CONCLUSIONS: Technical, ideological and personal elements such as beliefs, values and affective/emotional dimensions were relevant to enhance or refuse to develop more specific bonds with drug user patients. The conceptions on drug use may interfere in the development of a better care and equity in health care.

Não recebi financiamento The present study aimed to investigate how the accessibility to oral health services occurs in two municipalities in the interior of the São Paulo State (SP), using the therapeutic itineraries (called as ITs) as theoretical-methodological reference. Municipalities of Ibaté and São Carlos were chosen, since both are part of the same Regional Management Collegiate (called as CGR Coração), being that, in the process of health regionalization, the Center of Dental Specialties of São Carlos city is a reference for the Ibaté city. Twelve health units participated of this study, being six Basic Health Units (called as UBS) and six Family Health Units (called as USF). Twelve users were interviewed. The data collection was made from a semi-structured interview, a field diary and a non-participant observation. Users of different age groups were interviewed, and in all ITs, the trigger for care search was the perception of an installed problem. However, there was a variation in this trigger according to age, i.e., the dental pain was the trigger for the younger persons, whereas the need for prosthesis was the trigger for middle-aged adults and elderly persons. Five users had systemic diseases: one renal transplant, one with Parkinson’s disease and three with diabetes mellitus and hypertension. In addition, two users were pregnant. No interviewed user correlated their general health with oral health, even with all the efforts of Dentistry to prove the influence of oral diseases on the systemic diseases. The accessibility barriers found in this study were related to those of an organizational, economic and sociocultural nature. The obstacles originated in the entrance of the health services were the scheduling of the first dental consultation and emergency care, both were through the spontaneous demand attendance with the criterion of order of arrival in all units evaluated and with small differences between UBS and USF. On the other hand, the post-entry obstacles were the consultation scheduling in the dental prosthesis specialty, the difficulty in returning to the treatment after a lack and specialized scheduling only in a private clinic. The economic barriers referred to the financial difficulty to access the treatments in private clinics and the impossibility to perform the treatment in the health unit to avoid losing the work day, whereas the sociocultural barriers consisted in the understanding of the public health system as a place for the treatment of people who do not have money to pay for the private, as a favor of the State and, therefore, as a system that cannot receive complaints and improvements. Thus, despite the considerable increase in the supply of oral health services provided by National Oral Health Policy in order for the population has accessibility to these services, it is necessary the reorganization of the work processes developed in the health units so that the integrality to health is the north of oral health practices, besides of the need for humanized care. O presente estudo objetivou investigar como ocorre a acessibilidade aos serviços de saúde bucal em dois municípios do interior do estado de São Paulo (SP), utilizando os itinerários terapêuticos (ITs) como referencial teórico-metodológico. Optou-se por escolher os municípios de Ibaté e São Carlos, pois ambos fazem parte do mesmo Colegiado de Gestão Regional (CGR Coração), sendo que no processo de regionalização da saúde, o Centro de Especialidades Odontológicas (CEO) do município de São Carlos constitui-se em referência para o município de Ibaté. Participaram deste estudo doze unidades de saúde, sendo seis Unidades Básicas de Saúde (UBS) e seis Unidades de Saúde da Família (UFS). Foram entrevistados doze usuários. A coleta de dados se deu por intermédio de entrevista semiestruturada, diário de campo e a observação não-participante. Foram entrevistados usuários de diferentes faixas etárias, sendo que em todos os ITs o disparador para a busca por cuidado foi a percepção de um problema instalado. No entanto, houve variação neste disparador de acordo com a idade, ou seja, para os mais jovens foi a dor dentária, enquanto para os adultos de meia-idade e idosos, a necessidade de prótese. Cinco usuários possuíam doenças sistêmicas: uma transplantada renal, uma com Doença de Parkinson e três com diabetes mellitus e hipertensão arterial. Além disso, duas usuárias estavam grávidas. Nenhum usuário entrevistado correlacionou sua saúde geral com a saúde bucal, mesmo com todos os esforços da Odontologia em comprovar a influência das doenças bucais sobre as enfermidades sistêmicas. As barreiras de acessibilidade encontradas neste estudo foram relacionadas com aquelas de ordem organizacional, econômica e sociocultural. Os obstáculos originados na entrada dos serviços de saúde foram o agendamento da primeira consulta odontológica e o atendimento de urgência, sendo que ambos se deram por meio do atendimento da demanda espontânea com o critério de ordem de chegada em todas as unidades avaliadas e com pequenas diferenças entre UBS e USF. Já os obstáculos da pós-entrada foram o agendamento da consulta na especialidade de prótese dentária, a dificuldade em retornar ao tratamento após uma falta e o atendimento especializado apenas em consultório particular. As barreiras de ordem econômica referiram-se à dificuldade financeira para acessar os tratamentos em consultórios particulares e a impossibilidade de realizar o tratamento na unidade de saúde para não perder o dia de trabalho, enquanto que as barreias de ordem sociocultural consistiram no entendimento do sistema público de saúde como um local para o tratamento de pessoas que não têm dinheiro para pagar pelo privado, como um favor do Estado e, portanto, como um sistema não passível de reclamações e melhorias. Assim, apesar do considerável aumento da oferta de serviços de saúde bucal proporcionado pela Política Nacional de Saúde Bucal para que a população tenha acessibilidade a estes serviços, faz-se necessária a reorganização dos processos de trabalho desenvolvidos nas unidades de saúde, de forma que a integralidade à saúde seja o norte das práticas em saúde bucal, além da necessidade de atenção humanizada.

Não recebi financiamento Introduction: The demand for homecare (AD) is increasing due to the accelerated demographic and epidemiological transition throughout Brazil. The AD is part of the practice of various services within the network of Attention to Health. While health model should produce caution and completeness in the various levels of intensity of care. Objective: To understand the contribution of AD as a device in the structuring of the Health Care Network in the city of Araraquara, situated in the interior of the state of São Paulo. Methods: a case study of a methodological proposal for a qualitative approach. We used the interviews to 14 health workers of the municipality's primary network, between August 2016 and April 2017. Used the content analysis by means of the separation of empirical categories. The categories were analyzed: Network of Attention to Health; Homecare; Basic Care and homecare and finally, the Homecare in the network of attention. This study was submitted to the Ethics and Research Committee of Universidade Federal de São Carlos (CEP/UFSCar) and approved under the opinion number 1,680.558/2016. Results and Discussion: The health workers interviewed reported the homecare as a practice of network services, but also as a modality of care. Reinforced its development by basic health units, especially those with family health. And also, the actions developed by the Service Homecare (SAD). Scored that the actions that are being developed currently does not correspond to the demands of care. Pointed out the need for articulation of the health network of a more dynamic and alive, so that the user is assisted in its entirety. Highlighted care for caregivers in the home. Conclusion: The homecare develops at different spaces, but mainly by transforming a new space of caution: the household. For the realization of the care is integral to consideration of culture and subjectivity of people who reside in that space is essential for the formation of the bond between workers and caregivers and thus be continuity of care. Looking through this practice to corresponsabilização of care and the autonomy of the family. The AD can be a device for structuring of RAS, to give visibility to the pallets of attention. Introdução: A demanda por atenção domiciliar (AD) vem aumentando devido à acelerada transição demográfica e epidemiológica em todo Brasil. A AD faz parte da prática de diversos serviços inseridos na Rede de Atenção á Saúde. Enquanto modelo de saúde deve produzir cuidado e integralidade nos diversos níveis de intensidade de cuidado. Objetivo: Compreender a contribuição da AD como dispositivo na estruturação da Rede de Atenção à Saúde no município de Araraquara, situado no interior do estado de São Paulo. Métodos: Estudo de caso com uma proposta metodológica de abordagem qualitativa. Utilizou-se da entrevista semiestruturada a 14 trabalhadores de saúde da rede básica do município, entre agosto de 2016 e abril de 2017. Utilizado a análise de conteúdo com definição de categorias empíricas. As categorias analisadas foram: Rede de Atenção á Saúde; Atenção Domiciliar; Atenção Básica e Atenção Domiciliar e por fim, a Atenção Domiciliar na rede de atenção. Resultados e Discussão: Os trabalhadores de saúde entrevistados relataram a atenção domiciliar como uma prática dos serviços da rede, mas também como uma modalidade de atenção. Reforçaram o seu desenvolvimento por diferentes unidades de saúde, e especial, pelas unidades básicas de saúde vinculadas a estratégia saúde da família. E também, as ações desenvolvidas pelo Serviço de Atenção Domiciliar (SAD). Pontuaram que as ações que estão sendo desenvolvidas atualmente não correspondem às demandas dos atendimentos. Apontaram a necessidade da articulação da Rede de Saúde de forma mais dinâmica e viva, para que o usuário seja assistido em sua integralidade. Destacaram o cuidado ao cuidador no domicílio. Conclusão: A atenção domiciliar se desenvolve em diversos espaços, mas principalmente transformando um novo espaço de cuidado: o domicilio. Para que a efetivação do cuidado seja integral a consideração da cultura e subjetividade das pessoas que residem naquele espaço é essencial para a formação do vínculo entre trabalhadores e cuidadores e assim dar continuidade ao cuidado, buscando através dessa prática a corresponsabilização do cuidado e a autonomia da família. A AD pode ser um dispositivo de estruturação da RAS, para dar visibilidade aos vazios da atenção. O SAD pode se constituir num formador/capacitador da rede para a realização do cuidado domiciliar. A atenção domiciliar parece ter um papel importante como uma estratégia que assegura o atendimento em sua amplitude, capacitando os trabalhadores das unidades básicas de saúde sob a luz das diretrizes da Politica Nacional de Humanização (PNH), gerando um movimento para a transformação da rede viva de cuidados.

Orientadores: Marta Fuentes-Rojas, Álvaro de Oliveira D'Antona Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Aplicadas Resumo: Atualmente a infecção pelo HIV representa um dos maiores problemas sociais e de saúde pública do Brasil e do mundo que, desde seu surgimento, tem causado grande impacto à humanidade. Nesse sentido, o cuidado às PVHA é um dos principais desafios para a saúde pública, tendo em vista que não exige somente cuidados biomédicos, mas de outras dimensões, pois viver com HIV/aids envolve questões sociais, culturais, organizacionais, biológicas, políticas e de diversas áreas de conhecimento. A tecnologia possibilitou todo um aparato científico para o cuidado das PVHA. Com os exames CD4 e CVP e o tratamento antirretroviral é possível monitorar a doença, oferecer melhores condições de vida e aumentar a sobrevida. Todavia, o atendimento às PVHA, por meio desse aparato, colocou em evidencia a doença, o vírus, e fragmentou o sujeito, não considerando o HIV/aids em sua multidimensionalidade. Este estudo teve como objetivo refletir sobre o cuidado às PVHA, a partir da percepção dos profissionais de saúde. Para tanto, realizou-se um estudo empírico, fundamentado na abordagem da Pesquisa Social, utilizando como estratégia a triangulação de métodos, para explorar e compreender a percepção dos profissionais de saúde acerca do cuidado às PVHA. Participaram 45 profissionais de saúde e gestores de três unidades de Serviço de Assistência Especializada ¿ SAE da Diretoria Regional de Saúde DRS-X. Para a coleta de dados foram utilizados três instrumentos: questionários, entrevistas semiestruturadas e grupos de discussão. A análise dos dados ocorreu por meio de análises estatística-descritiva e análise de conteúdo de Bardin. A partir dos resultados e análise dos dados, emergiram cinco classes temáticas: (1) o viver com HIV/aids hoje, (2) Identificação, formação e perfil profissional, (3) O trabalho em equipe e o cuidado integral, (4) O cuidado às PVHA e (5) As dificuldades e as invisibilidades do cuidado. Nesse sentido, verificou-se que os profissionais de saúde criam um conceito sobre o viver com HIV/aids hoje, sendo que esse conceito, que está sempre em um processo de reconstrução, guia as ações nos serviços de saúde. Quanto à formação e capacitação profissional, foi possível constatar que é necessário pensá-la em um modelo de formação permanente e continuada, construída no cotidiano dos serviços de saúde. Além disso, faz-se necessário também avaliar as novas tecnologias educativas que estão sendo utilizadas. O acolhimento, a escuta ativa, o vínculo entre profissional e usuário e o trabalho em equipe surgem, neste estudo, como a base para um atendimento integral para às PVHA, superando um modelo fragmentado de cuidado em saúde. Constatou-se também que as dificuldades vivenciadas pelos profissionais de saúde e a dimensão emocional do trabalho de cuidar em saúde, muitas vezes, são aspectos invisibilizados, não sendo abordados nas discussões de cuidado, embora exerçam grande influência na produção do cuidado. Assim, conclui-se este estudo destacando que o cuidado integral acontece quando o uso das novas tecnologias no tratamento e cuidado às PVHA é incluído em um contexto muito maior que é o do cuidado humanizado, não sendo essas tecnologias compreendidas como seu oposto, mas como mais uma ferramenta, entre várias outras, que engloba todo o universo do cuidado integral Abstract: Nowadays, infection by the HIV virus represents one of the greatest social and public health problems not only for Brazil but the world, and has heavily impacted humanity since its appearance. Therefore, care for PVHA is a big challenge for public health, since not only biomedical cares are in question, for living with the HIV virus involves other dimensions, such as social, cultural, organizational, biological, political and other areas of knowledge. Technology has provided a whole scientific apparatus for treating the PVHA. With the C4, CVP and antiretroviral exams it is possible to monitor the disease, offer better life conditions and increase life expectancy. However, such treatment of the PVHA by means of this apparatus, has placed the virus in evidence, emphasizing the disease and fragmenting the individual, therefore not considering the HIV/AIDS in its multidimensionality. This study's objective is to think about care of PVHA from the health professional's perspective. An empirical study was developed, based on the Social Research approach and using method triangulation as strategy. 45 health professionals and 3 Specialized Service Unit (SAE) managers, from the Regional Health Board of Directors DRS-X, participated in the research. Three instruments were used for data collection: questionnaires, semi-structured interviews and focus groups. Analysis was conducted by means of Bardin's statistic-descriptive content analysys. From the results 5 thematic classes came up: (1) living with HIV/AIDS today, (2) identification, education and profiling of professionals, (3) team work and integral care, (4) care for the PVHA and (5) difficulties and unviability of care. In this sense, it was verified that health professionals create their own concept of living with HIV/AIDS, and such concept, which is always in process of reconstructing itself, guides the actions performed on health services. Regarding training and education it was possible to verify the need for a permanent and continued education model, based on the daily work of health services. Besides that it is necessary to evaluate technological education methods currently in use. Welcoming, active listening, bonding between user and professional and team work were presented, in this study, as the basis for an integral care of the PVHA, surpassing the currently fragmented model of healthcare. It was also noticed that the difficulties experienced by health professionals and the emotional dimension of working with care in health are, in many occasions, factors of unviability, which are not approached when discussing care, though they exert great influence over its production. 26. The health care of invisible homeless people: health professional concepts and social assistance in São Carlos, SP, Brazil

Não recebi financiamento The enhance of the Unified Health System (SUS) in this country is still a challenge for the Brazilian public health, in order to allow the health services access for the whole population, with a good care quality, which includes the difficulties that homeless people have to be cared at the health services in this national territory. Despite that aforementioned population is numerically greater in the metropolis, nowadays even at medium-sized cities, such as São Carlos, there are people in this condition, justifying this city’s choice as the research field. This work aimed to analyze the caring practices and the conceptions of the homeless population in the city of São Carlos, from the speech of health professionals that work with that. Thereunto, social research methodology has been utilized, having as a tool the semi-structured interview, the focal groups with sixteen health services professionals from SUS who work with the health care for the homeless, and a representative of the specialized service for the homeless population from the Unified System of Social Assistance. For analyzing the data, the Content Analysis techinique has been used. In the light of the vulnerability concept and articles about the SUS and the homeless population care, five major content categories were discussed, extracted from the interview/focal groups. These were: Conceptions about the homeless population; Conceptions about the homeless population’s health care; Care provided to homeless population’s health; Knowledge about the network and intersectoral approach; Training for professionals to take care of the homeless. It has been argued that there is a need to occur a permanent education process for the workers, about the multiple intrinsic aspects of the street population caring. And that the municipal, state and federal managements enable the adequate working conditions, so that this work may happen with quality. Ainda é um desafio para a saúde pública brasileira a potencialização do seu Sistema Único de Saúde (SUS) de forma a possibilitar a toda população o acesso aos serviços de saúde, e o cuidado ser realizado com qualidade. O que inclui a dificuldade da população em situação de rua em acessar os serviços de saúde neste território nacional. Apesar dessa população ser numericamente maior em metrópoles, atualmente, mesmo em cidades de médio porte, tal como São Carlos, existem pessoas nessa condição, justificando-se a escolha do município enquanto campo de pesquisa. O presente trabalho teve como objetivo analisar as práticas de cuidado e concepções sobre a população em situação de rua no município de São Carlos, a partir da fala dos profissionais de saúde que a realizam. Tratou-se de uma pesquisa social, tendo como instrumento a entrevista semi-estruturada e os grupos focais com 16 profissionais de serviços de saúde do SUS que praticam o cuidado as pessoas em situação de rua e um representante do serviço especializado para População de Rua do Sistema Único de Assistência Social. Para analisar os dados, foi utilizada a técnica da análise de conteúdos. À luz do conceito de vulnerabilidade e artigos a respeito do SUS e o cuidado à População em Situação de Rua discutiu-se cinco grandes categorias de conteúdos extraídos da entrevista/grupos focais. Foram essas: concepções sobre a População em Situação de Rua; concepções sobre o cuidado à saúde da População em Situação de Rua; cuidado realizado à saúde da População de Rua; conhecimento sobre a rede e intersetorialidade; formação para os profissionais cuidarem da População em Situação de Rua. Nos comentários finais, discutiu-se que existe a necessidade de ocorrerem processos de educação permanente para os trabalhadores sobre os diversos aspectos intrínsecos ao cuidado à população de rua. E que as gestões municipais, estaduais e federais possibilitem as condições adequadas de trabalho para que este trabalho possa ocorrer com qualidade.

27. Discutindo sobre a violência perpetrada por parceiro íntimo na gestação : desfechos relacionados à saúde materno-infantil

Não recebi financiamento Violence is a current topic, and violence perpetrated by intimate partners is growing concern of social, legal, and health policy. The subject is already configured a public health problem, considering the physical and emotional harm caused to the victim, and study data refer to women victims of domestic violence seeking up to six times more for the health service. Pregnancy can be an increase of the risk of violence against women according to some studies, can change the default for the frequency and severity during this period or even be initiated at this stage of a woman's life. Still, this type of violence ends up in invisibility in health services, and can cause risks to maternal and fetal health. Objective: To discuss the scientific production in Brazil in the period 2010 to 2015 regarding the children's maternal outcomes during gestation and postpartum women victims of violence perpetrated by an intimate partner. Methods: We used the scientific literature integrative review with cover articles in Portuguese, English and Spanish available in CINAHL databases, Scielo and BVS published in the periods 2010-2015 through descriptors in Portuguese and English "violence against women and pregnancy "," domestic violence and pregnancy. " Results: There was negative repercussions resulting from VPI, the analyzed studies bring these can affect both the pregnancy, such as puerperal, with higher occurrence of risk among women with unplanned pregnancy, abortion practice, presenting also high rates of gestational and postpartum depression, at high risk of suicide, poor quality in prenatal care, Fetal death (neo and postnatal) and increased risk of low birth weight. Conclusion: The knowledge of these data IPV during pregnancy by health professionals can help prevent negative outcomes for women's health and fetal development, and must become concrete action of primary care and specialized in prenatal care. A violência é um tema atual, e a violência perpetrada pelo parceiro íntimo uma preocupação crescente das esferas social, jurídica, política e de saúde. O assunto já é configurado um problema de saúde pública, considerando os danos físicos e emocionais ocasionados à vítima, e dados de estudos remete que mulheres vítimas de violência doméstica procuram até seis vezes mais pelo serviço de saúde. A gravidez pode ser um incremento ao risco de violência às mulheres de acordo com alguns estudos, podendo alterar o padrão quanto à frequência e gravidade nesse período ou mesmo ser iniciada nessa etapa da vida da mulher. Ainda assim, este tipo de violência acaba ficando na invisibilidade nos serviços de saúde, e pode propiciar riscos à saúde materna-fetal. Objetivo: Discutir a produção científica produzida no Brasil no período de 2010 a 2015 referentes aos desfechos maternos infantis no período gravídico e puerperal de mulheres vítimas de Violência perpetrada por parceiro íntimo. Metodologia: Utilizou-se a revisão integrativa de literatura científica com inclusão dos artigos em português, inglês e espanhol disponíveis nas bases de dados Cinahl, Scielo e BVS publicados nos períodos de 2010 a 2015, através de descritores em português e inglês “violência contra mulher and gravidez”, “violência doméstica and gravidez”. Resultados: Verificou-se repercussões negativas decorrentes da VPI, os trabalhos analisados trazem que estas podem afetar tanto o período gestacional, como o puerperal, tendo maiores riscos de ocorrência entre mulheres com gravidez não planejada, prática de aborto induzido, apresentando também altos índices de depressão gestacional e pós-parto, com risco elevado de suicídio, má qualidade no acompanhamento pré-natal, Óbito fetal (neo e pós-natal) e maior risco de baixo peso ao nascer. Conclusão: O conhecimento destes dados de VPI na gestação por profissionais de saúde podem auxiliar na prevenção de desfechos negativos para saúde da mulher e fetal, e deve tornar-se ação concreta da atenção primária e especializada na atenção pré-natal.

Coordenação de Aperfeiçoamento de Pessoal de Nível Superior (CAPES) This study adopted the Family Health Strategy (FHS) as the locus of research. The FHS is the Brazilian government's response to the organization and execution of Basic Health Care. Analyzes the FHS as a strategical dispositive in the art of governing the population wich adopts the family as a central instrument for developing epidemiological changes, based on the concept of Governamentality, an exercise of power inserted in the relationship between security, population and government. Some effects of this power are normalization of conduct, medicalization of life, imposing lifestyles considered healthy from political technologies that investing on the body, health, the right ways to eat, to live and interact, in sume an entire space of existence. This work has taken as its object FHS care practices, analyzing how they can determine the ways of life and subjectivity of families who receive assistance, for it adopted the theoretical framework of analytical power of Michel Foucaut. From the perspective of Social Constructionism, were analyzed official documents from the Ministry of Health and observed the daily work of a family health team, analyzing the discursive practices in these different fields of research. The process of this investigation resulted in the possibility to analyze and relate the meanings of three discursive practices: Families, Power and Care. In the documentary analysis, three groups of meaning were discussed: (1) as the FHS healthcare model approaches of a Governmentality dispositive; (2) the meanings of family treated in the documents, the highlight occurs in the created norms on families, and as this is an important strategy for the good government of the population; (3) the strategic position that discursive practices of Care and Humanization occupy in the documents and the changes they propel to care practices. In participant observation, we report three situations experienced in the field, it is argued from these reports the main meanings of team care concrete practices and the effects of power in families. In the final considerations discusses the three discursive practices that made up this work: Power, Families and Care, tracing possible ways that contribute to the movement of Clinical and Care Management, and indicating FHS care practices in order to enable a ethical and humane meeting between worker-user which generates autonomy, emancipation growth for these different actors. Este trabalho adotou a Estratégia Saúde da Família (ESF) como lócus de pesquisa. A ESF é a resposta do Estado brasileiro para a organização e efetivação da Atenção Básica. Analisa-se a ESF como um dispositivo estratégico na arte de governar a população que adota a família como instrumento central para desenvolver mudanças epidemiológicas, a partir do conceito de Governamentalidade, um exercício de poder inserido na relação entre segurança, população e governo. Alguns efeitos desse poder são normalização de condutas, medicalização da vida, imposição de estilos de vida considerados saudáveis a partir de tecnologias políticas, que investem sobre o corpo, a saúde, as maneiras corretas de se alimentar, de morar, de viver e se relacionar, enfim de todo um espaço de existência. Este trabalho tomou como objeto de estudo as práticas de cuidado da ESF, analisando como estas podem determinar os modos de vida e subjetivação das famílias que são atendidas, para isso adotou-se o referencial teórico da analitica de poder de Michel Foucaut. A partir da perspectiva do Construcionismo Social, foram analisados documentos oficiais do Ministério da Saúde e observado o cotidiano de trabalho de uma equipe de saúde da família, analisando as práticas discursivas presentes nestes diferentes campos de pesquisa. O processo dessa investigação resultou na possibilidade de analisar e relacionar os sentidos de três práticas discursivas : Famílias, Poder e Cuidado. Na analise documental três núcleos de sentidos foram discutidos: (1) como o modelo de atenção da ESF a aproxima de um dispositivo de governamentalidade ; (2) os sentidos de família tratados nos documentos, o destaque se dá nas normas criadas sobre famílias, e como esta é uma importante estratégia para o bom governo da população ; (3) o lugar estratégico que ocupam as práticas discursivas sobre Cuidado e Humanização presentes nos documentos e as mudanças que elas impulsionam às práticas de cuidado. Na observação participante, relata-se 3 situações vivenciadas no campo, discute-se a partir destes relatos os principais sentidos das praticas concretas de cuidado da equipe e os efeitos de poder sob as famílias. Nas considerações finais discute-se as três práticas discursivas que compuseram este trabalho : Poder, Famílias e Cuidado, delineando possíveis caminhos que contribuam para o movimento da Gestão da Clinica e do Cuidado, e indicando práticas de cuidado da ESF de forma a possibilitar um encontro ético e humanizado entre trabalhador e usuário que gere autonomia, emancipação a crescimento para esses diferentes atores.

Orientador: Maria Cecília Goi Porto Alves Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciências Médicas Resumo: Por muito tempo as mulheres têm sido o foco de estudos sobre saúde relacionada ao gênero, enquanto poucas pesquisas investigam as condições de saúde dos homens. A população masculina apresenta altas taxas de mortalidade, principalmente nas idades mais precoces, além de sofrerem mais de condições graves e crônicas de saúde e adotarem mais comportamentos prejudiciais à saúde. Nesse contexto, é importante conhecer o perfil dessa população em relação à saúde, considerando-se sua condição socioeconômica e de que forma a estrutura social influencia sobre seu estado de saúde. O presente estudo tem por objetivo avaliar desigualdades sociais em saúde, segundo escolaridade, entre homens de 20 a 59 anos residentes no município de Campinas. Os dados foram coletados por meio do Inquérito Domiciliar de Saúde, um estudo transversal, de base populacional, com amostra por conglomerados em dois estágios, realizado no município de Campinas em 2008-2009. Dentre as variáveis incluídas neste estudo estão as sociodemográficas, as de comportamentos relacionados à saúde, as de estado de saúde e as de uso de serviços de saúde. A escolaridade, variável independente principal, foi categorizada em 0 a 8 anos de estudo e 9 ou mais anos de estudo. Para estimar as prevalências e as associações, foi utilizado o teste qui-quadrado, com nível de significância de 5% e, para obtenção das razões de prevalência (RP) e seus respectivos intervalos de confiança, foi feita a regressão de Poisson. Esta análise foi realizada por meio do software STATA versão 11, que considera as ponderações relativas ao desenho amostral Abstract: For long time women have been the focus of health studies related to gender, while little research investigating the health conditions of men. The male population has high rates of mortality, especially in younger ages and suffer more severe and chronic conditions of health and adopting more unhealthy behaviors. In this context, it is important to know the profile of this population in relation to health, considering their socioeconomic and how social structure influences on their health. The present study aims to assess social inequalities in health, schooling, men 20 to 59 years living in the city of Campinas. Data were collected through the Household Health Survey, a cross-sectional study of populationbased cluster sampling in two stages, held in Campinas in 2008-2009. Among the variables included in this study are the demographic characteristics, the behaviors related to health, the health status and use of health services. Schooling, the main independent variable, was categorized as 0-8 years of study and nine or more years of study. To estimate the prevalence and associations, we used the chi-square test, with significance level of 5% and to obtain the prevalence ratios (PR) and their respective confidence intervals, was performed by Poisson regression. - Abstract
The scope of this article is to see how the Brazilian Comprehensive Healthcare Policy for Men (PNAISH) has been implemented in the Unified Health System, from the standpoint of health professionals. A case study, involving five cases (each from a different macro region of Brazil) conducted using ethnographic techniques of data collection charted the progress of PNAISH implementation based on an anthropological approach using Lipsky's idea of street-level bureaucracy. PNAISH is contextualized in historical terms with national and international documents. Acknowledging the inevitable gap between the formulation and the implementation of any policy, an attempt is made to see how this gap has evolved by analyzing the transition of PNAISH into city Action Plans (PAs). It was revealed that the implementing agents had little knowledge of PNAISH, of the local health care network for men, of the techniques required to meet men's specific needs and of the concept of gender. It faced institutional obstacles, such as lack of an organizational structure, of a consolidated healthcare network - where the user receives services with different degrees of complexity within the system - and resources in general, especially human resources.

- Abstract
In 2009, the Brazilian Comprehensive Healthcare Policy for Men (PNAISH) was launched in Brazil, seeking to reduce morbidity and mortality in this population group. This article strives to analyze the conceptions that health professionals have about the specific demands and behaviors of the male population served by the healthcare services. The data analyzed are part of a larger research project, the objective of which was to evaluate the initial actions of the implementation of PNAISH. Ethnographic observations in 11 health services and semi-structured interviews were conducted with 21 health professionals. From the perspective of health professionals, the presence of men in the healthcare services is still limited. According to them, it is comprised of two types of clients: workers and the elderly. The male behavior characteristics - haste, objectivity, fear and resistance - and the difficulty faced by health services in receiving this population are the main factors that drive men away from health services. Although the concept of gender is central to PNAISH, it is only triggered by healthcare professionals in order to justify the social standards expected in terms of men's behavior. The attribution of men's behavior to cultural factors ultimately obscures the relations of power that underlie gender relations.

- Abstract
This paper studied the relation between the exercise of masculinities and health care of men in primary health care services. It focuses on representations and meanings of primary health care service users and professional of what it is to be a man. Male service users and professionals were interviewed in two primary health care facilities. Diverse models of masculinity were found, defining various forms of reasoning upon men's health care. This study indicates that issues such as work, sexuality, body structure, relationship with women, and transformations in gender relations are important for men, and should be considered in health care services.

- Abstract
This study deals with the relations between masculinities and health care, approaching the recognition of health needs among male users of primary health care and the responses by the services. The study is part of a larger research project in four Brazilian States, with a convenience sample of eight health services. Ethnographic observation was compared with semi-structured interviews with 182 health care users from 15 to 65 years of age and 72 health professionals. Thematic analysis of the ethnographic records and interviews was based on gender references and studies on health work. The findings show how medicalization of health needs affects users, professionals, and services, disguising issues related to masculinity. Primary care focuses mainly on women, thereby reproducing gender inequalities in health services operations and professional performance, with women receiving disciplined care and men receiving insufficient attention and care.

- Abstract
Objective: To understand the perceptions of pharmacy teams about their role in the healthcare assistance challenges and adherence to antiretroviral therapy by injecting drug users living with HIV/AIDS., Methods: Qualitative study through focus groups and thematic discourse analysis of pharmacists, technicians and assistants with more than six months of experience with medication supply, in 15 assisting units for STD/AIDS in the city of São Paulo, in 2002., Results: Three groups were formed, totaling 29 participants, originating from 12 out of the 15 existing services, and including 12 university level professionals and 17 high-school level professionals. The groups concluded that the pharmacy has an important role in the antiretroviral drug supply, which is reflected in the treatment adherence, because trust-based relationships can be built up through their procedures. In spite of this, they pointed out that such building-up does not take place through excessively bureaucratic activities. This has negative repercussions for all patients, especially for injecting drug users, considered "difficult people". Such concept sums up their behavior: they are supposed to be confused and incapable to adhere to treatment, and have limited understanding. Staff members, however, affirm they treat these patients equally. They do not realize that, by this acting, the specific needs of injecting drug users may become invisible in the service. There is also the possibility that stigmatizing stereotypes may be created, resulting in yet another barrier to the work on adherence., Conclusions: Although the pharmacy is recommended as a potentially favorable place to listen to and form bonds with users, the results show objective and subjective obstacles to render it suitable for the work on adherence.

- Abstract
Objective: To describe the influence of conceptions of health professionals on the care given to HIV/AIDS patients using drugs., Methods: Qualitative study based on semi-structured interviews with 22 professionals of different levels from two specialized STD/AIDS public services of the city of São Paulo was conducted in 2002. The interviews were recorded and submitted to a thematic analysis., Results: Professionals reported difficulties in identifying drug users among their patients, indicating the invisibility of the issue. They find drug users more difficult to treat, because they disturb the service and do not comply with treatment. Although they acknowledge the special needs of users, and that it is important to deal with drug use, health professionals believe that these issues are not their responsibility. Professionals showed personal and technical limits in handling these cases, showing the importance of their lack of specific capacity building. Thus, they recommend the creation of specialized services for this care, recognizing their own services as inappropriate. Although they were aware of the harm reduction project, there was a little participation in it., Conclusions: Technical, ideological and personal elements such as beliefs, values and affective/emotional dimensions were relevant to enhance or refuse to develop more specific bonds with drug user patients. The conceptions on drug use may interfere in the development of a better care and equity in health care.

- Abstract
Objective: To understand the possibilities and limitations of developing coordinated work among professionals of different background in order to promote work collaboration and improve the quality of care of HIV/AIDS patients., Methods: A qualitative study on multiprofessional work was carried out in five STD/AIDS reference centers in the city of São Paulo, Brazil. Semi-structured interviews were applied to 26 professionals from different background, focusing on how they position their practices in a multiprofessional setting., Results: Significant differences were observed as to the outreach of multiprofessional work collaboration and possibilities of improving health care are related to the circumstances the multiprofessional work is done., Conclusions: When the multiprofessional team has to deal with predictable circumstances and the planning of long-term care projects where demands are anticipated from situations found in their practices, this creates better working conditions for effective collaboration. This collaboration favors interventions that promote a more productive interaction between the medical treatment and other relevant dimensions of care, such as social, psychological, and emotional needs of the patients.
