Abstract

Background: Age-related macular degeneration (AMD) is a leading cause of vision loss in older individuals, driven by a multifactorial etiology involving genetic, environmental, and dietary factors. Nutritional genomics, which studies gene-nutrient interactions, has emerged as a promising field for AMD prevention and management. Genetic predispositions, such as variants in CFH, C3, C2/CFB, APOE, and oxidative stress pathways, significantly affect the risk and progression of AMD. Methods: This narrative review synthesizes findings from randomized controlled trials and recent advances in nutritional genomics research. It examines the interplay between genetic predispositions and dietary interventions, exploring how personalized nutritional strategies can optimize AMD management. Results and Discussion: The AREDS and AREDS2 trials demonstrated that supplements, including vitamins C, E, zinc, copper, lutein, and zeaxanthin, can reduce the progression to advanced AMD. Nutritional interventions tailored to genetic profiles show promise: CFH risk alleles may enhance zinc supplementation's anti-inflammatory effects, while APOE variants influence the response to omega-3 fatty acids. Adjusting carotenoid intake, such as lutein and zeaxanthin, based on genetic susceptibility exemplifies emerging precision nutritional approaches. Ongoing research seeks to integrate nutrigenomic testing into clinical settings, enabling clinicians to tailor interventions to individual genetic profiles. Conclusions: Further studies are needed to assess the long-term effects of personalized interventions, investigate additional genetic variants, and develop tools for clinical implementation of nutrigenomics. Abstract

Definition: The stress response is a natural physiological reaction of the organism, elicited to maintain the internal environment and evoke adaptive behaviors, ultimately leading to survival. However, at the turn of the century, stress-related disorders gained increasing significance. The aim of entry is to explore the fundamental question of when the stress system changes from a beneficial to a detrimental system, contributing to a higher risk of the development of disorders and/or diseases. To develop context, here, we explore the different concepts of stress and reveal the complexities, perspectives, and multiple relationships between the neurochemistry, cerebral functional network, and associated pathologies. According to the literature, the stress response affects nearly every biological system through the close interactions between the physiological, nervous, endocrine, and immune systems when faced with a real or perceived threat. Considering today's challenging times, where people are facing multiple unavoidable adversities in their lives and a level of uncertainty never before seen, this review emphasizes the importance of understanding the potential consequences of being unable to cope with stressful events. Susceptibility and resilience to stress have gained recognition as important areas of study. The literature presented here enhances our understanding and identifies the causes of various psychopathologies, mental health conditions, disabilities, and even mortality that are closely linked to vulnerability to stress. Experimental studies from recent decades have demonstrated the many factors affecting our ability to cope with stress, including differences between individuals due to their genetic background, epigenetic regulation, gender, and early-life experiences. Finally, there is an urgent need to change the paradigm of modern lifestyles as a potential strategy to prevent the spread of the "health epidemic of the 21st century", which is stress. Abstract

Purpose: Patellofemoral instability (PFI) is a common condition that can be caused from multiple factors, including lower limb rotational malalignments. Determining precise criteria for performing corrective torsional osteotomy can be a daunting task due to the lack of consensus on normal and excessive values and the limited evidence‐based data in the postoperative results. The purpose was to assess the clinical, functional and imaging outcomes following derotational distal femoral osteotomy (DDFO) in patients with PFI and/or anterior knee pain (AKP) associated with lower limb rotational malalignments. Methods: Searches were conducted on PubMed, EMBASE and Web of Science databases up to October 2023. Studies reporting outcomes after DDFO in patients with PFI and/or AKP were eligible for the systematic review. The primary outcome was imaging metrics, especially femoral anteversion. Secondary outcomes included the patient‐reported outcome measures (PROMs) (clinical and functional). Quantitative synthesis involved the use of weighted averages to calculate pre‐ to postoperative mean differences (MD) and compare them against the minimal clinically important difference (MCID). Results: Ten studies (309 knees) were included with a mean follow‐up of 36.1 ± 11.7 months. Imaging outcomes consistently indicated the correction of femoral anteversion (MD = −19.4 degrees, 95% confidence interval: −20.1 to −18.7) following DDFO. PROMs showed significant improvements in most studies, exceeding the MCID. Patient satisfaction with the DDFO was high (93.3%). Conclusions: The DDFO was an effective treatment option for correcting excessive femoral anteversion in patients with PFI associated with clinically relevant functional and clinical improvement and a high satisfaction rate. Abstract

As feridas complexas representam ameaça a viabilidade de um membro ou a vida por acometerem extensas áreas corpóreas, necessitando de métodos especiais para a sua resolução. Objetivou-se identificar a eficácia da terapia por pressão negativa no tratamento de lesões complexas. Trata-se de uma revisão integrativa realizada em abril de 2021, nas bases de dados LILACS, Web of Science, Medline e SciELO. Cruzaram-se os descritores MeSH “Wounds and injuries” and Therapeutics and “Negative-Pressure Wound Therapy” e após aplicação dos critérios de elegibilidade selecionaram-se 11 artigos. Constatou-se que as potencialidades relacionadas ao uso da terapia por pressão negativa sobressaem-se as fragilidades existentes, sendo esta considerada eficaz no tratamento de lesões complexas, todavia, pouco utilizada por profissionais da rede pública de saúde, fazendo-se necessária sua apropriação pelos profissionais de saúde a fim de conferir tratamentos alternativos e de maior eficácia àqueles que possuem feridas complexas. Negative pressure therapy in complex injuries: an integrative review Abstract: Complex wounds represent a threat to the viability of a limb or to life as they affect large areas of the body, requiring special methods for their resolution. The objective was to identify the effectiveness of negative pressure therapy in the treatment of complex lesions. This is an integrative review carried out in April 2021, in the LILACS, Web of Science, Medline and SciELO databases. The MeSH descriptors "Wounds and injuries" and Therapeutics and "Negative-Pressure Wound Therapy" were crossed and after applying the eligibility criteria, 11 articles were selected. It was found that the potentialities related to the use of negative pressure therapy stand out from the existing weaknesses, which is considered effective in the treatment of complex injuries, however, little used by professionals in the public health network, requiring its appropriation by health professionals in order to provide alternative and more effective treatments for those with complex wounds.
