Abstract

The years of ever-increasing revenue from international students appear to have come to an end. Data show that the number of international students and the revenue they generated rose annually throughout the first years of the past decade; however, enrolment and associated fee revenue for school districts reached its highest level in 2007-2008, and has declined in the two years since then. Revenue information is gathered from Schedule 2 of school-district audited financial statements as of June 30 of each year. Student numbers are drawn from ministry Student Statistics reports, which show that the headcount of non-resident students enrolled in British Columbia (BC) public schools in September 2009 was 9,014. Not surprisingly, reduced district revenue from fees followed the pattern of downturn in the global economy. Total provincial international-student-fee revenue of $121,019,957 for 2009-2010 was down $8,087,998 from the high of $129,107,955 in 2007-2008. International students can enrich the education of BC students by producing more diversity and rich experiences in classrooms. However, concerns have been raised by teachers. In some cases, students are simply assigned to classes, adding to the class size without additional supports. These practices can leave the international students without the ESL support they need and presumably are expecting. When the teacher does respond to their particular needs, this may reduce the attention they can provide to all the other students in the class. Districts that generate significant revenues from international-student enrolments are better able to offset the budget shortfalls currently plaguing school districts across the province. Districts without these additional revenues are left to further cut core services and close schools to balance district budgets. In other words, the current funding practices of the provincial government are creating pressures within the public system that are reinforcing systemic inequalities between upper/middle/low-income communities as well as urban/rural communities. 6. Fiscal Fitness for Non-Profits: Project Puts Chicago After-School Programs and Funders through a Financial Workout. Stories from the Field

Abstract

Strengthening after-school programming for city youngsters has long been an objective of The Wallace Foundation, a national philanthropy based in New York City. In its work over the years, Wallace has found that weak financial management of the nonprofits running many high-quality programs hampers their ability to improve and expand. In 2009, Wallace introduced an initiative in one city, Chicago, to see if a combination of financial training and consulting for executives at after-school nonprofits and the introduction of friendlier government reimbursement policies could change that picture. The foundation funded Fiscal Management Associates (FMA) to provide financial management training to the CEO's and CFO's of 25 Chicago-area nonprofits that provide after-school services. Participating organizations were also given financial incentives that varied depending on the training they received: $40,000 or $115,000 for initial financial management improvements, and, for those completing the program, $25,000 for administrative infrastructure or $125,000 for operating reserves. Wallace also supported a forum bringing together representatives of government, advocacy groups, private funders, academics and service providers to recommend new contracting practices. The results of the Strengthening Financial Management initiative thus far suggest that, with sufficient grounding in the basics of financial analysis, nonprofits that run after-school programs can learn to navigate difficult challenges to their organizations more adeptly, even in the face of a troubled economy. The forum (in which several Wallace grantees participate) has led to a closer partnership among government, nonprofits and funders--an achievement in itself given the state's chaotic funding landscape--and early steps toward streamlining contracting practices with nonprofits. Abstract

Recent advances indicate that, in various chronic inflammatory disorders, the activation of the immune system is triggered locally rather than in lymphoid organs. In this study, we have evaluated whether the humoral alloimmune response involved in chronic rejection is elicited within the graft. We used the rat aortic interposition model and microdissected the adventitia of the graft. Over time, the T cell infiltrate shifted toward a B helper phenotype. B lymphocyte clusters were detected and were the site of intense proliferation and apoptosis. Simultaneously, adventitial vascular endothelium acquired a high endothelial venule phenotype. Similar features were evidenced in the interstitium of chronically allografts (hearts and kidneys). Strikingly, ganocultured graft interstitial tissue was found to be the site of production of antibodies directed against donor MHC-I molecules. These findings, therefore, document the appearance of germinal centers in chronically rejected tissues. This lymphoid neogenesis implies that the graft is not only the target of the alloimmune response but also a site where this response actually develops, so as to optimize the communication between the targeted tissue and the immune effectors. chronic rejection | transplantation | B cells | germinal centers | adventitia

Abstract

Summary Background Heterogeneity is a major obstacle to developing effective treatments for patients with primary Sjogren's syndrome. We aimed to develop a robust method for stratification, exploiting heterogeneity in patient-reported symptoms, and to relate these differences to pathobiology and therapeutic response. Methods We did hierarchical cluster analysis using five common symptoms associated with primary Sjogren's syndrome (pain, fatigue, dryness, anxiety, and depression), followed by multinomial logistic regression to identify subgroups in the UK Primary Sjogren's Syndrome Registry (UKPSSR). We assessed clinical and biological differences between these subgroups, including transcriptional differences in peripheral blood. Patients from two independent validation cohorts in Norway and France were used to confirm patient stratification. Data from two phase 3 clinical trials were similarly stratified to assess the differences between subgroups in treatment response to hydroxychloroquine and rituximab. Findings In the UKPSSR cohort (n=608), we identified four subgroups: Low symptom burden (LSB), high symptom burden (HSB), dryness dominant with fatigue (DDF), and pain dominant with fatigue (PDF). Significant differences in peripheral blood lymphocyte counts, anti-SSA and anti-SSB antibody positivity, as well as serum IgG, κ-free light chain, β2-microglobulin, and CXCL13 concentrations were observed between these subgroups, along with differentially expressed transcriptomic modules in peripheral blood. Similar findings were observed in the independent validation cohorts (n=396). Reanalysis of trial data stratifying patients into these subgroups suggested a treatment effect with hydroxychloroquine in the HSB subgroup and with rituximab in the DDF subgroup compared with placebo. Interpretation Stratification on the basis of patient-reported symptoms of patients with primary Sjogren's syndrome revealed distinct pathobiological endotypes with distinct responses to immunomodulatory treatments. Our data have important implications for clinical management, trial design, and therapeutic development. Similar stratification approaches might be useful for patients with other chronic immune-mediated diseases.
