Abstract

Rationale: A relatively important proportion of patients with severe asthma end up using a daily dose of oral corticosteroids (OCS) in addition to their high dose of inhaled corticosteroids (ICS). We have determined whether oral CS-dependent severe asthma is a distinct phenotype from those only on ICS. Methods: Participants were recruited from adult pulmonary clinics across Europe and severe asthma diagnosed according to IMI-criteria (Bel et al. Thorax 2011). They had either uncontrolled asthma as defined by the GINA guidelines AND/OR two or more severe exacerbations in the past 12 months. Asthma diagnosis was confirmed by a history of typical symptoms and either reversibility in FEV1 of >=12% and 200ml after 400 T-test, Mann Whitney U test and Chi-square test were used. Results: In the 374 severe asthma participants, 167 (44%) were on oral prednisolone. The oral prednisolone group was older but had similar FEV1 (% predicted) and BMI. There was a lower rate of cigarette smoking with lower proportion of current smokers and never-smokers, but a higher percentage with nasal polyps. There was no difference in atopy as measured by skin prick tests or RAST IgE. A higher proportion of patients had 2 or more exacerbations per year (68.3% vs 56.4%; p=0.02). A greater proportion of patients were using MDI and nebulised short-acting b-agonists and theophylline. Exhaled NO was raised (31 ppb vs 22 ppb; p=3% and neutrophils>=65%, there was a greater proportion of eosinophilic (38% vs 30%) and of mixed granulocytic (23% vs 5%), at the expense of lower paucigranulocytic (14% vs 41%)(Chi-square: p

Abstract

These studies use starch synthesis mutants to quantify the contribution of assimilatory starch to whole plant growth and form. Arabidopsis thaliana (L.) Heynh plants were used with null plastid phosphoglucomutase (T Caspar, SC Huber, CR Sommerville, [1986] Plant Physiol 79; 1-7) or 7% of wild-type ADP-glucose pyrophosphorylase (T-P Lin, T Caspar, CR Sommerville, J Preiss [1988] Plant Physiol 88; 1175-1179). The daily turnover of starch and the rate of biomass increase in the mutants and the wild type were investigated during growth in a 14 hour light/10 hour dark cycle in high irradiance (600 micromoles per square meter per second) and nitrogen (6 millimolar NH(4)NO(3)), in high irradiance and low nitrogen (0.1 millimolar NH(4)NO(3)) or in low irradiance (80 micromoles per square meter per second) and high nitrogen. There is some variability in the data, but the following conclusions can be drawn. Growth was slow in the absence of starch turnover. In high nitrogen conditions, about 1 mole of carbon per gram dry weight per day was incorporated additionally into structural biomass for every one mole of carbon turned over as starch per gram dry weight per day. In low nitrogen, the gain was much lower. This indicates that temporary storage of photosynthate is important for rapid growth in high nitrogen, but not in low nitrogen when carbohydrate is in excess. Starch-deficient plants showed the usual decrease of the shoot/root ratio in low nitrogen and increase of the ratio in low light. This shows that adjustment of plant form to nitrogen nutrition and irradiance is not mediated via regulation of photosynthate partitioning in the leaf. Starch deficient plants had lower shoot/root ratios than the wild type and the nitrogen concentration in their leaves was increased. It is discussed how interactions between carbohydrate allocation, respiration and growth at the organ and whole plant level generate these changes. We conclude that mutants with a decreased capacity to carry out a particular partial process provide a powerful tool to disect complex mutually interacting systems, and define and quantify causal interactions at the level of whole plant growth.

12. Gabapentin for the treatment of painful diabetic neuropathy: dosing to achieve optimal clinical response.

ObjectiveTo determine whether gabapentin titrated to achieve clinical effect (≥ 50% reduction in pain; 900—3,600 mg/day) provides superior efficacy to a commonly prescribed fixed-dose (900 mg/day) in subjects with PDN.MethodsIn Latin America, an open-label trial randomised 339 subjects with PDN to gabapentin, 900 mg/day, for seven weeks (n=170), or to 900—3,600 mg/day titrated over four weeks to achieve clinical effect, followed by three weeks at stable dose (n=169).ResultsGabapentin produced a significantly greater reduction in final weekly mean pain scores from baseline when titrated to clinical effect than when administered as a fixed-dose regimen (53.6% vs. 43.3%; p=0.009). Responder rate was significantly increased (64.5% vs. 47.5%; p=0.002), mean VAS scores significantly decreased, final weekly sleep interference scores significantly decreased (57% C vs. 37.2%; p=0.013), and trends favouring improvement in global functioning and QOL were seen in the titration to clinical effect group (p<0.001). Both regimens were well-tolerated.ConclusionsTitration to clinical effect offered superior efficacy in treating PDN compared to a low fixed-dose treatment.
