Abstract

Human papillomavirus (HPV) genotype 52 is commonly found in Asian cases of cervical cancer but is rare elsewhere. Analysis of 611 isolates collected worldwide revealed a remarkable geographical distribution, with lineage B predominating in Asia (89.0% vs 0%-5.5%; P(corrected) < .001), whereas lineage A predominated in Africa, the Americas, and Europe. We propose that the name "Asian lineage" be used to denote lineage B, to signify this feature. Preliminary analysis suggested a higher disease risk for lineage B, although ethnogeographical confounders could not be excluded. Further studies are warranted to verify whether the reported high attribution of disease to HPV52 in Asia is due to the high prevalence of lineage B.

Abstract

Human papillomavirus (HPV) 58 accounts for a notable proportion of cervical cancers in East Asia and parts of Latin America, but it is uncommon elsewhere. The reason for such ethnogeographical predilection is unknown. In our study, nucleotide sequences of E6 and E7 genes of 401 HPV58 isolates collected from 15 countries/cities across four continents were examined. Phylogenetic relationship, geographical distribution and risk association of nucleotide sequence variations were analyzed. We found that the E6 genes of HPV58 variants were more conserved than E7. Thus, E6 is a more appropriate target for type-specific detection, whereas E7 is more appropriate for strain differentiation. The frequency of sequence variation varied geographically. Africa had significantly more isolates with E6-367A (D86E) but significantly less isolates with E6-203G, -245G, -367C (prototype-like) than other regions (p ≤ 0.003). E7-632T, -760A (T20I, G63S) was more frequently found in Asia, and E7-793G (T74A) was more frequent in Africa (p < 0.001). Variants with T20I and G63S substitutions at E7 conferred a significantly higher risk for cervical intraepithelial neoplasia grade III and invasive cervical cancer compared to other HPV58 variants (odds ratio = 4.44, p = 0.007). In conclusion, T20I and/or G63S substitution(s) at E7 of HPV58 is/are associated with a higher risk for cervical neoplasia. These substitutions are more commonly found in Asia and the Americas, which may account for the higher disease attribution of HPV58 in these areas.

Abstract

Half a million new cases of cervical cancer are diagnosed annually World-wide; with optimal management the five year survival is 58%. In Developing countries cervical carcinoma is the commonest female malignancy with a lifetime risk approaching 1 in 15. In the absence of adequate health care facilities, incidence and mortality figures equate. Alternate treatment modalities are therefore urgently awaited. There is a close association between Human Papilloma Virus infection (HPV) and cervical carcinogenesis; the HPV E6 and E7 proteins interacting with the cellular regulatory proteins, p53 and Rb respectively. E6 and E7 are consistently expressed by neoplastic HPV-infected cells, serving as tumour-specific markers and as targets for immunotherapy. The importance of immunological control in HPV-associated disease is inferred from histological evidence of an immune response around regressing lesions and by the increased incidence of HPV-related disease in immunosuppressed individuals. However, there are few reports of HPV-specific T cell responses in the literature. Evidence of HPV-specific cytotoxic lymphocyte (CTL) activity presented here, in patients with cervical intraepithelial neoplasia. Eight patients with late stage invasive disease were immunised with a recombinant vaccinia virus encoding HPV16 & 18 E6 and E7 genes. All patients mounted a serological reaction to the vector while three patients showed a definite anti-HPV serological response. Of three evaluable patients one showed an HPV-specific CTL response nine weeks after vaccination. No serious adverse events occurred and environmental release was contained. During the vaccine study evidence of immune compromise was noted amongst patients with cervical carcinoma. Systematic investigation for different disease stages revealed a pan T cell deficit, related to tumour load. This has implications for the safety and efficacy of future immunotherapeutic intervention.

45. Recurrence of vulval intraepithelial neoplasia following treatment with cidofovir or imiquimod: results from a multi-centre, randomised, phase II trial (RT3VIN)

Abstract

Objective\ud \ud To compare the recurrence rates after complete response to topical treatment with either cidofovir or imiquimod for vulval intraepithelial neoplasia (VIN) 3. \ud \ud \ud Design\ud \ud A prospective, open, randomised multicentre trial.\ud \ud \ud Setting\ud \ud 32 general hospitals located in Wales and England.\ud \ud \ud Population or Sample\ud \ud 180 patients were randomised consecutively between 21 October 2009 and 11 January 2013, 89 to cidofoovir (of whom 41 completely responded to treatment) and 91 to imiquimod (of whom 42 completely responded to treatment). \ud \ud Methods\ud \ud After 24 weeks of treatment, complete responders were followed up at 6‐monthly intervals for 24 months. At each visit, the Common Terminology Criteria for Adverse Events (CTCAE) v3.0 was assessed and any new lesions were biopsied for histology.\ud \ud \ud Main outcome measures\ud \ud Time to histologically confirmed disease recurrence (any grade of VIN).\ud \ud \ud Results\ud \ud The median length of follow up was 18.4 months. At 18 months, more participants were VIN‐free in the cidofovir arm: 94% (95% CI 78.2–98.5) versus 71.6% (95% CI 52.0–84.3) [univariable hazard ratio (HR) 3.46, 95% CI 0.95–12.60, P = 0.059; multivariable HR 3.53, 95% CI 0.96–12.98, P = 0.057). The number of grade 2+ events was similar between treatment arms (imiquimod: 24/42 (57%) versus cidofovir: 27/41 (66%), χ2 = 0.665, P = 0.415), with no grade 4+. \ud \ud \ud Conclusions\ud \ud Long‐term data indicates a trend towards response being maintained for longer following treatment with cidofovir than with imiquimod, with similar low rates of adverse events for each drug. Adverse event rates indicated acceptable safety of both drugs \ud \ud \ud Tweetable abstract\ud \ud Long‐term follow up in the RT3VIN trial suggests cidofovir may maintain response for longer than imiquimod.
