Abstract

Rare information is available on clinical Enterococcus faecium encountered in Sardinia, Italy. This study investigated the antimicrobial susceptibility profiles and genotypic characteristics of E. faecium isolated at the University Hospital of Sassari, Italy, using the Vitek2 system and PCR, MLST, or WGS. Vitek2 revealed two VanB-type vancomycin-resistant Enterococcus faecium (VREfm) isolates (MICs mg/L = 8 and ≥32) but failed to detect vancomycin resistance in one isolate (MIC mg/L ≤ 1) despite positive genotypic confirmation of vanB gene, which proved to be vancomycin resistant by additional phenotypic methods (MICs mg/L = 8). This vanB isolate was able to increase its vancomycin MIC after exposure to vancomycin, unlike the "classic" occult vanB-carrying E. faecium, becoming detectable by Vitek 2 (MICs mg/L ≥ 32). All three E. faecium had highly mutated vanB2 operons, as part of a chromosomally integrated Tn1549 transposon, with common missense mutations in VanH and VanB2 resistance proteins and specific missense mutations in the VanW accessory protein. There were additional missense mutations in VanS, VanH, and VanB proteins in the vanB2-carrying VREfm isolates compared to Vitek2. The molecular typing revealed a polyclonal hospital-associated E. faecium population from Clade A1, and that vanB2-VREfm, and nearly half of vancomycin-susceptible E. faecium (VSEfm) analyzed, belonged to ST117. Based on core genome-MLST, ST117 strains had different clonal types (CT), excluding nosocomial transmission of specific CT. Detecting vanB2-carrying VREfm isolates by Vitek2 may be problematic, and alternative methods are needed to prevent therapeutic failure and spread. [ABSTRACT FROM AUTHOR]

Abstract

Enterobacterspp. are important nosocomial pathogens responsible of a wide variety of infections, mainly due to Extended Spectrum β-Lactamase (ESBL) producing isolates, constituting a global public health issue in terms of clinical treatment and infection control, especially in low-income countries, where last-line treatment is often unavailable and there is weak nosocomial surveillance. In this study, we conducted a phenotypic and molecular characterization of 8 clinical Enterobacterspp. strains, isolated from patient’s blood in three hospitals in Mozambique. Isolates were identified by MALDI-TOF and antimicrobial Susceptibility Testing was performed by VITEK 2 system. Half of isolates were analyzed by PCR for β-lactamases genes, other isolates by Whole Genome Sequencing. We identified all isolates as Enterobacter cloacaecomplex (ECC), those from Maputo Central Hospital were polyclonal, multidrug resistant (5/8), and ESBL producers (50%), carrying blaCTX-M-15and different assortment of blaSHV-12, blaTEM-1Band blaOXA-1, and AmpCs blaCMH-3, blaACT-7and blaACT-9genes. Resistance determinants linked to fluoroquinolone (aac(6')Ib-crand qnrB1) and others antimicrobials were also found. Notably, one isolate showed phenotypically resistance to colistin, while another colistin susceptible isolate carried a silent mcr-9gene. ECC nosocomial surveillance is urgently needed to contain and prevent the dissemination of ESBLs producing clones, and mcr-9spread to other Enterobacteriaceae.

Abstract

Objective: The current study explored changes in trend of anaemia and BMI among currently pregnant nullipara adolescent women against socio-economic determinants in India from 2005 through 2015. The association between anaemia in currently pregnant nullipara adolescent women v. currently pregnant nullipara older women of reproductive age was also explored.Design: We used the 2005 and the 2015 nationally representative Indian Demographic and Health Surveys (DHS). The outcomes of interest, anaemia and BMI, were measured based on the DHS methodology following WHO standards and indicators. Place of residence, educational attainment and wealth quintiles were used as determinants in the analysis.Setting: India.Participants: In total, 696 adolescent girls from the India 2005 DHS and 3041 adolescent girls from the India 2015 DHS.Results: The 10-year transition from 2005 to 2015 showed differences between the least and most wealthy sections of society, with heaviest gains in anaemia reduction over time among the latter (from 50·0 to < 40·0 %). The odds of anaemia were significantly higher among the adolescent population when compared with adult women both in 2005 and in 2015 (OR = 1·2).Conclusions: Despite an overall improvement in the prevalence of both BMI < 18·5 and anaemia among adolescents nullipara in India, the adjusted risk of anaemia in the latter category was still significantly higher as compared with their adult counterparts. Abstract

Intercontinental dust transport represents one of the mechanisms of microbial dispersion, contributing to biodiversity of ecosystems and impacting on human health when of pathogenic nature. Studies on airborne dust microbiology have just begun reporting variable densities of microorganisms depending on seasons and environmental conditions, in particular a low number of culturable microrganisms, only 1%. In this study, a combined culture-dependent and independent DNA-based analyses via NGS of 16S genes was applied for the first time to investigate the airborne dust microbiology in air sampled during dust events at two opposite coastal sites of italian Island Sardinia, which represents the crossroad of wind circulation from Africa towards Europe. The results of culturable method showed that microrganisms possessing adaptive strategies to persist in the atmosphere during dust transportation survived, as Gram positive Bacillus spp and filamentous fungi. By using NGS analysis, we observed distinct differences in bacterial community composition. A major conserved core microbiome was evidenced but increases in species richness and presence of specific taxa were observed in relation to each wind regime and seasons. Taxa which can feature strains with clinical implications were also detected. Data suggested the existence of a main pattern facilitating airborne bacteria dispersion and local scale migration after the dry seasons, which offers a refined interpretive understanding of overall environmental microbiology dynamics.

Abstract

An NGS-based taxonomic analysis was carried out on airborne bacteria sampled at ground level in two periods (May and September) and two opposite localities on the North-South axis of the Sardinia Island. Located in a central position of the Mediterranean basin, Sardinia constitutes a suitable outpost to reveal possible immigration of bacterial taxa during transcontinental particle discharge between Africa and Europe. With the aim of verifying relative effects of dust outbreaks, sampling period and sampling site, on the airborne bacterial community composition, we compared air collected during dust-carrying meteorological events to that coming from wind regimes not associated to long-distance particle lifting. Results indicated that: (a) a higher microbial diversity (118 orders vs 65) and increased community evenness were observed in the campaign carried out in September in comparison to the one in May, irrespective of the place of collection and of the presence or absence of dust outbreaks. b) During the period of standard wind regimes without transcontinental outbreaks a synchronous, concerted succession of bacterial communities across distant locations of the same island, accompanied as mentioned by a parallel rise in bacterial diversity and community evenness appears to have occurred. (c) changes in wind provenance could transiently change community composition in the locality placed on the coast facing the incoming wind, but not in the one located at the opposite side of the island; for this reason the community changes brought from dust outbreaks of African origin are observed only in the sampling station exposed to south; (d) the same winds, once proceeding over land appear to uplift bacteria belonging to a common core already present over the region, which dilute or replace those that were associated with the air coming from the sea or conveyed by the dust particulate, explaining the two prior points. Abstract

Aerial wind-mediated dust transportation represents one of the mechanisms determining microbial dispersion. Particles and the associated microflora, can be scattered across the Earth's surface via dry and wet depositions, affecting biodiversity and function of aquatic and terrestrial ecosystems and impacting on human health when of pathogenic nature. The aim of D.U.S.T. (Desert Upon Sardinian Territory) is to define the identities of microorganisms carried by Saharan dust to Sardinia (Italy) through culture-based and cultivation-independent methods. Data comparison and integration could provide interesting clues concerning the contribution of this mechanism of geographical dispersion to the biodiversity modulation and its possible effects on ecosystems and human health and activities.

Abstract

Introduction: Candida spp. are responsible for infections ranging from local to systemic, and resistance to antifungal first-line therapy is increasing in non-albicans Candida species. We aimed to determine the etiology of candidiasis and the antifungal resistance of Candida spp. isolated in Hue hospitals, Central-Vietnam. Methods: Species identification was performed by matrix-assisted laser desorption/ionization-time of flight mass spectrometry supported by fungal internal-transcribed-spacer amplification and sequencing. Antifungal susceptibility testing was performed by disk diffusion method and minimum inhibitory concentrations of azoles, caspofungin, and amphotericin B against C. tropicalis were determined by broth microdilution. Polymorphism of erg11 gene associated with fluconazole resistance was carried out by polymerase chain reaction and sequencing. Multilocus sequence typing (MLST) was used for typing selected C. albicans isolates. Results: Overall, 196 Candida isolates were detected, mostly C. albicans (48%), followed by C. tropicalis (16%), C. parapsilosis (11%), C. glabrata (9%), C. orthopsilosis (6%) and to a lesser extent another eight species. High rates of resistance to fluconazole and voriconazole (18.8%) were observed in C. tropicalis with five isolates co-resistant to both agents. Y132F and S154F missense mutations in the ERG11 protein were associated with fluconazole-resistance in C. tropicalis (67.7%). Resistance to caspofungin was found in one isolate of C. albicans. MLST identified a polyclonal population of C. albicans with multiple diploid sequence types, and with few lineages showing potential nosocomial spread. Conclusions: Resistance to triazole agents should be considered in C. tropicalis infections in the studied hospitals, and surveillance measures taken to avoid Candida diffusion.
