The United States (US) is experiencing a maternal health crisis, with high rates of maternal morbidity and mortality. The US has the highest rates of pregnancy-related mortality among industrialized nations. Maternal mortality has more than quadrupled over the last decades. Rural areas and minoritized populations are disproportionately affected. Increased pregnancy-care workforce with greater participation from family medicine, greater collaborative care, and adequate postpartum care could prevent many maternal deaths. However, more than 40% of birthing people in the US receive no postpartum care. No singular solutions can address the complex contributors to the current situation, and efforts to address the crisis must address workforce shortages and improve care during and after pregnancy. This essay explores the role family medicine (FM) can play in addressing the crisis. We discuss pregnancy care training in FM residencies as well as the threats posed by financial and medico-legal climates to the maternal health workforce. We explore how collaborative care models and comprehensive postpartum care may impact the maternal health workforce. Efforts and resources devoted to high impact solutions for which FM has considerable autonomy, including collaborative and postpartum care, are likely to have greatest impact.

Recent developments in morphological and morphometric analyses of charcoal particles have improved our ability to discern characteristics of burnt plant fuel and interpret fire-type changes. However, burning experiments linking known plants to these metrics are limited, particularly in open ecosystems. This study presents novel analyses of laboratory-produced charcoal of 22 plant species from the steppe regions of Eurasia (Romania and Russia), along with selected samples from three Holocene charcoal and pollen records from the same areas. We characterise charcoal production, morphologies and morphometrics in these grass-dominated environments, thereby enabling more robust interpretations of fuel sources and fire types for palaeofire research. Our experiments demonstrate that fire temperature can introduce biases in charcoal produced among species. Grass charcoal production was significantly lower and decreased more strongly with fire temperature compared to forbs. This suggests an underrepresentation of terrestrial graminoids in sedimentary charcoal assemblages. Morphometric analyses revealed that graminoid charcoal particles were more elongated (length-to-width ratio L/W=4) and narrower (width-to-length ratio W/L=0.38) than forbs (L/W=3.1 and W/L=0.42, respectively), in agreement with a global compilation for graminoids (L/W=4.3 for grass 5.4 grass and wetland graminoids) and forbs (L/W=2.9). However, overlapping L/W values present a challenge for establishing cut-off values for fuel type identification in charcoal assemblages with mixed fuel sources. Based on our analyses and compiled datasets from experimental burns, L/W values above 3.0 may indicate predominantly herbaceous morphologies in temperate grassland-dominated ecosystems, though values are likely to be higher for grass than forb-dominated grasslands. Notably, terrestrial grasses exhibit shorter aspect ratios (L/W=4.3) than wetland graminoids (L/W=6.4), highlighting that the aspect ratio needs tailoring to the specific environment of its application, i.e. wetland vs. terrestrial ecosystems. The long forms of graminoid charcoal particles also suggest their potential for atmospheric longer-distance transport compared to more spherical particles, meaning they likely provide insights into regional fire history. An important finding is that charcoal of herbaceous plants closely corresponded to the pollen record, highlighting a solid link between the dominant vegetation and fuel burnt in grassland-dominated environments. However, the relationship between woody charcoal and tree pollen may be more complex, as tree pollen can travel atmospherically longer distances compared to woody charcoal. Our results also highlight the complex interplay between local vegetation and charcoal composition with human fire use that needs to be considered when interpreting charcoal morphological records. A critical takeaway from this study is the importance of not assuming the universality of previous research findings and instead employing experimental approaches to characterise charcoal particles in new ecosystems prior to the application of these techniques. Furthermore, this study also highlights recommendations for further research in new geographical areas and proposes methodological adjustments to enhance the usefulness of charcoal analysis in fire research.

Pulpal pathology in young permanent teeth, caused by dental caries or trauma, can lead to disruption of root formation, leaving the tooth with an uncertain prognosis. Current therapies for such cases present a number of limitations; thus, the aim of this article is to provide an overview on the use of nanofibers in endodontics. The search was conducted on two databases and eight articles met the inclusion criteria for this systematic review. Data on nanofiber production and fiber characteristics were extracted and systematized in tables. Moreover, the ability of novel scaffolds to deliver either drugs or different therapeutic agents without interfering with the products’ characteristics is analyzed from the in vitro and in vivo data. The potential for nanofiber-based scaffolds to induce cellular differentiation and overcome the limitations of classic regenerative endodontic treatment is also discussed.

Aim of the study Preventive orthodontic care necessitates rational treatment planning among children even in an early stage of dental and skeletal development. Material and methods The oral rehabilitation of patients with unilateral posterior crossbite in primary dentition remains a challenge, for the clinician and for the patient, due to functional, aesthetic and self-maintenance alterations, and requires early orthodontic treatment. To illustrate the treatment difficulties of such a pathology, we present a clinical case, a five years old girl diagnosed with unilateral posterior cross- bite, narrow maxilla and left mandibular shift. Results In order to fulfil the therapeutic objectives, a rapid palatal expander with a median screw welding on band cemented on 5.5, 6.5 was applied. After 3 month the transversal unilateral discrepancy was corrected, with stable occlusal contacts. Conclusions For skeletal transverse problems the appliance of choice is a rapid maxillary expander especially for growing patients.

A recent synthesis study found 54% of the high-latitude peatlands have been drying and 32% have been wetting over the past centuries, illustrating their complex ecohydrological dynamics and highly uncertain responses to a warming climate.

This review aimed at searching literature for data regarding the inflammasomes’ involvement in the pathogenesis of oral diseases (mainly periodontitis) and general pathologies, including approaches to control inflammasome-related pathogenic mechanisms. The inflammasomes are part of the innate immune response that activates inflammatory caspases by canonical and noncanonical pathways, to control the activity of Gasdermin D. Once an inflammasome is activated, pro-inflammatory cytokines, such as interleukins, are released. Thus, inflammasomes are involved in inflammatory, autoimmune and autoinflammatory diseases. The review also investigated novel therapies based on the use of phytochemicals and pharmaceutical substances for inhibiting inflammasome activity. Pharmaceutical substances can control the inflammasomes by three mechanisms: inhibiting the intracellular signaling pathways (Allopurinol and SS-31), blocking inflammasome components (VX-765, Emricasan and VX-740), and inhibiting cytokines mediated by the inflammasomes (Canakinumab, Anakinra and Rilonacept). Moreover, phytochemicals inhibit the inflammasomes by neutralizing reactive oxygen species. Biomaterials functionalized by the adsorption of therapeutic agents onto different nanomaterials could represent future research directions to facilitate multimodal and sequential treatment in oral pathologies.

Wildfire is the most common disturbance type in boreal forests and can trigger significant changes in forest composition. Waterlogging in peatlands determines the degree of tree cover and the depth of the burnt horizon associated with wildfires. However, interactions between peatland moisture, vegetation composition and flammability, and fire regime in forest and forested peatland in Eurasia remain largely unexplored, despite their huge extent in boreal regions. To address this knowledge gap, we reconstructed the Holocene fire regime, vegetation composition, and peatland hydrology at two sites located in predominantly light taiga (Pinus sylvestris Betula) with interspersed dark taiga communities (Pinus sibirica, Picea obovata, Abies sibirica) in western Siberia in the Tomsk Oblast, Russia. We found marked shifts in past water levels over the Holocene. The probability of fire occurrence and the intensification of fire frequency and severity increased at times of low water table (drier conditions), enhanced fuel dryness, and an intermediate dark-to-light taiga ratio. High water level, and thus wet peat surface conditions, prevented fires from spreading on peatland and surrounding forests. Deciduous trees (i.e. Betula) and Sphagnum were more abundant under wetter peatland conditions, and conifers and denser forests were more prevalent under drier peatland conditions. On a Holocene scale, severe fires were recorded between 7.5 and 4.5 ka with an increased proportion of dark taiga and fire avoiders (Pinus sibirica at Rybnaya and Abies sibirica at Ulukh–Chayakh) in a predominantly light taiga and fire-resister community characterised by Pinus sylvestris and lower local water level. Severe fires also occurred over the last 1.5 kyr and were associated with a declining abundance of dark taiga and fire avoiders, an expansion of fire invaders (Betula), and fluctuating water tables. These findings suggest that frequent, high-severity fires can lead to compositional and structural changes in forests when trees fail to reach reproductive maturity between fire events or where extensive forest gaps limit seed dispersal. This study also shows prolonged periods of synchronous fire activity across the sites, particularly during the early to mid-Holocene, suggesting a regional imprint of centennial- to millennial-scale Holocene climate variability on wildfire activity. Humans may have affected vegetation and fire from the Neolithic; however, increasing human presence in the region, particularly at the Ulukh–Chayakh Mire over the last 4 centuries, drastically enhanced ignitions compared to natural background levels. Frequent warm and dry spells predicted by climate change scenarios for Siberia in the future will enhance peatland drying and may convey a competitive advantage to conifer taxa. However, dry conditions will probably exacerbate the frequency and severity of wildfire, disrupt conifers' successional pathway, and accelerate shifts towards deciduous broadleaf tree cover. Furthermore, climate–disturbance–fire feedbacks will accelerate changes in the carbon balance of boreal peatlands and affect their overall future resilience to climate change.

Objective. Low-Level Laser Therapy encourages the healing process, reduces inflammation and pain. The aim of this study is to identify the impact of Low-Level Laser Therapy on tissue regeneration with special attention to hard tissues and to compare the effect of several wave lengths in the proliferation and differentiation of cells. Methods. The keywords used were “bone regeneration”, “laser therapy”, “photobiomodulation” OR “bio-stimulation”, “Low-Level Laser therapy” OR “LLLT”, “osteoblast proliferation” AND “differentiation”. Results. The bio-stimulation with Low-Level Laser Therapy also seems to interfere with the osseous integration of implants, by increasing its adherence on the bone-implant surfaces. Evidence has shown that LowLevel Laser Therapy influences the cellular proliferation and differentiation. Conclusions. Low-Level Laser Therapy is a promising therapy in the field of regeneration, but further studies are needed in order to define the standard protocol.

Quantitative reconstructions of past land cover are necessary to determine the processes involved in climate–human–land-cover interactions. We present the first temporally continuous and most spatially extensive pollen-based land-cover reconstruction for Europe over the Holocene (last 11 700 cal yr BP). We describe how vegetation cover has been quantified from pollen records at a 1∘ × 1∘ spatial scale using the “Regional Estimates of VEgetation Abundance from Large Sites” (REVEALS) model. REVEALS calculates estimates of past regional vegetation cover in proportions or percentages. REVEALS has been applied to 1128 pollen records across Europe and part of the eastern Mediterranean–Black Sea–Caspian corridor (30–75∘ N, 25∘ W–50∘ E) to reconstruct the percentage cover of 31 plant taxa assigned to 12 plant functional types (PFTs) and 3 land-cover types (LCTs). A new synthesis of relative pollen productivities (RPPs) for European plant taxa was performed for this reconstruction. It includes multiple RPP values (≥2 values) for 39 taxa and single values for 15 taxa (total of 54 taxa). To illustrate this, we present distribution maps for five taxa (Calluna vulgaris, Cerealia type (t)., Picea abies, deciduous Quercus t. and evergreen Quercus t.) and three land-cover types (open land, OL; evergreen trees, ETs; and summer-green trees, STs) for eight selected time windows. The reliability of the REVEALS reconstructions and issues related to the interpretation of the results in terms of landscape openness and human-induced vegetation change are discussed. This is followed by a review of the current use of this reconstruction and its future potential utility and development. REVEALS data quality are primarily determined by pollen count data (pollen count and sample, pollen identification, and chronology) and site type and number (lake or bog, large or small, one site vs. multiple sites) used for REVEALS analysis (for each grid cell). A large number of sites with high-quality pollen count data will produce more reliable land-cover estimates with lower standard errors compared to a low number of sites with lower-quality pollen count data. Sedimentary charcoal records are widely used to reconstruct regional changes in fire regimes through time in the geological past. Existing global compilations are not geographically comprehensive and do not provide consistent metadata for all sites. Furthermore, the age models provided for these records are not harmonised and many are based on older calibrations of the radiocarbon ages. These issues limit the use of existing compilations for research into past fire regimes. Here, we present an expanded database of charcoal records, accompanied by new age models based on recalibration of radiocarbon ages using IntCal20 and Bayesian age-modelling software. We document the structure and contents of the database, the construction of the age models, and the quality control measures applied. We also record the expansion of geographical coverage relative to previous charcoal compilations and the expansion of metadata that can be used to inform analyses. This first version of the Reading Palaeofire Database contains 1676 records (entities) from 1480 sites worldwide. Erosion is a complex geomorphological process controlled by climate, soil and vegetation characteristics in addition to land-use. However, the interplay of these drivers is not fully understood. Here we present a 11.8 ka multi-proxy record of geochemical, mineral magnetic and grain size analyses from Lake Stiucii, in the Transylvania lowlands (CE Europe) documenting past erosional activity and the drivers of landscape change. We identify three major phases in the site's development. The first, a lacustrine phase (11.8–10.9 cal ka BP), is characterised by open water conditions with significant river inflow and depicts a period of greater erosion under a less developed catchment vegetation cover. Between 10.9 and 4.6 cal ka BP, the lake became a wetland characterised by shallow water with a reduced drainage network under warmer climate and/or reduced rainfall. From 4.6 cal ka BP, a second lacustrine phase developed driven by enhanced stream flow and sediment input subsequently accelerated by land use change. On a millennial scale, the two significant changes in the lake-catchment system and erosion regime at 10.9 and 4.6 cal ka BP may reflect a response to fluctuations in large-scale atmospheric circulation patterns. The eight short-term erosional events identified at 10.6, 7.8 cal ka BP, between 4.4–4.1, 3.5, 3.0–2.5, 1.9, 1.5, 1.1–0.8 cal ka BP, overlap well-known climate events. However, from 4.6 cal ka BP the climate-landscape relationship is amplified by rising anthropogenic impacts. Our data provide critical information on the paleoenvironmental evolution of CE Europe's lowlands and facilitate discussion of the effects of millennial and centennial climatic shifts as expressed in the local environment. Introduction. Burning mouth syndrome (BMS) is a clinical condition characterized by the presence of chronic pain in absence of clinically visible lesions of the oral mucosa. The etiology is uncertain and the therapeutic strategies still controversial. The objective of this prospective study is to analyze the efficacy of essential oils-based mouthwashes in the therapy of BMS. Material and method. This study included 16 patients affected by BMS who were treated with essential oils-based mouthwashes and glucose solution on alternated days for 30 days. Symptomatology was evaluated after 15, 30 and 90 days. Results and discussions. A the end of the treatment, most of the patients (67%) referred an improvement of symptoms up to complete remission in 90 days. Conclusions. Based on this study, essential oils-based mouthwashes could represent a valid aid in the treatment of BMS. Further studies are necessary in order to identify effective and standardized therapeutic protocols.

The analysis of charcoal fragments in peat and lake sediments is the most widely used approach to reconstruct past biomass burning. With a few exceptions, this method typically relies on the quantification of the total charcoal content of the sediment. To enhance charcoal analyses for the reconstruction of past fire regimes and make the method more relevant to studies of both plant evolution and fire management, the extraction of more information from charcoal particles is critical. Here, I used a muffle oven to burn seven fuel types comprising 17 species from boreal Siberia (near Teguldet village), which are also commonly found in the Northern Hemisphere, and built on published schemes to develop morphometric and finer diagnostic classifications of the experimentally charred particles. I then combined these results with those from fossil charcoal from a peat core taken from the same location (Ulukh-Chayakh mire) in order to demonstrate the relevance of these experiments to the fossil charcoal records. Results show that graminoids, Sphagnum, and wood (trunk) lose the most mass at low burn temperatures ( ∘C), whereas heathland shrub leaves, brown moss, and ferns lose the most mass at high burn temperatures. This suggests that species with low mass retention in high-temperature fires are likely to be under-represented in the fossil charcoal record. The charcoal particle aspect ratio appeared to be the strongest indicator of the fuel type burnt. Graminoid charcoal particles are the most elongate (6.7–11.5), with a threshold above 6 that may be indicative of wetland graminoids; leaves are the shortest and bulkiest (2.1–3.5); and twigs and wood are intermediate (2.0–5.2). Further, the use of fine diagnostic features was more successful in separating wood, graminoids, and leaves, but it was difficult to further differentiate these fuel types due to overlapping features. High-aspect-ratio particles, dominated by graminoid and Sphagnum morphologies, may be robust indicators of low-temperature surface fires, whereas abundant wood and leaf morphologies as well as low-aspect-ratio particles are indicative of higher-temperature fires. However, the overlapping morphologies of leaves and wood from trees and shrubs make it hard to distinguish between high-intensity surface fires, combusting living shrubs and dead wood and leaves, and high-intensity crown fires that have burnt living trees. Distinct particle shape may also influence charcoal transportation, with elongated particles (graminoids) potentially having a more heterogeneous distribution and being deposited farther away from the origin of fire than the rounder, polygonal leaf particles. Despite these limitations, the combined use of charred-particle aspect ratios and fuel morphotypes can aid in the more robust interpretation of fuel source and fire-type changes. Lastly, I highlight the further investigations needed to refine the histories of past wildfires.

Peatlands are a significant landscape component in temperate mountain ranges and serve multiple functions. However, many peatlands were damaged, altered, or drained over the past centuries, and knowledge about their development is needed for their protection and sustainable management. In our study, we analysed two peat cores from a bog site located in the Black Forest (Southwest Germany) using high-resolution multi-proxy palaeoecological analysis, including biological indicators (plant macrofossils, pollen, testate amoebae), macro-and micro-charcoal as well as geochemical analyses (peat stoichiometry and stable isotopes (δ13C, δ15N)) supported by radiocarbon dating. We tracked the development of the peatland and forest ecosystem to (i) determine their current ecological state, and to (ii) assess the role of different agents of change at the catchment scale on peatland development. The overall objective was to determine natural reference conditions as a basis for the restoration of degraded mountain bogs in the Black Forest Mountains and other temperate mountain ranges. Our results revealed that: i) the bog developed on mineral bedrock of slight slope through paludification around ca. 10,000 cal. BP; ii) the pristine plant populations dominated by Sphagnum fuscum occurring from 6200 cal. BP were replaced by Sphagnum medium/divinum and Eriophorum vaginatum since ca. 3300 cal. BP, most likely due to hydrological disturbances and possible dust deposition on the bog; iii) peat formation at the study site seems still active despite relatively deep water levels (ca. 22–26 cm below surface) and was active over the last 8000 years also when WTD varied between 12 and 26 cm; iv) the self-recovery of peat formation at this mountain bog is possible; v) the protection and restoration of mountain bog ecosystems should also consider land cover changes in the surrounding catchment, serving as further indicators or drivers of mountain bog development.

Forest steppes are dynamic ecosystems, highly susceptible to changes in climate, disturbances and land use. Here we examine the Holocene history of the European forest steppe ecotone in the lower Danube Plain to better understand its sensitivity to climate fluctuations, fire and human impact, and the timing of its transition into a cultural forest steppe. We used multi-proxy analyses (pollen, n-alkanes, coprophilous fungi, charcoal and geochemistry) of a 6000-year sequence from Lake Oltina (southeastern Romania) combined with a REVEALS (Regional Estimates of Vegetation Abundance from Large Sites) model of quantitative vegetation cover. We found a greater tree cover, composed of xerothermic (Carpinus orientalis and Quercus) and temperate (Carpinus betulus, Tilia, Ulmus and Fraxinus) tree taxa, between 6000 and 2500 cal yr BP. Maximum tree cover ( ∼  50 %), dominated by C. orientalis occurred between 4200 and 2500 cal yr BP at a time of wetter climatic conditions and moderate fire activity. Compared to other European forest steppe areas, the dominance of C. orientalis represents the most distinct feature of the woodland's composition at this time. Tree loss was underway by 2500 yr BP (Iron Age), with the REVEALS model indicating a fall to ∼  20 % tree cover from the Late Holocene forest maximum, linked to clearance for agriculture, while climate conditions remained wet. Biomass burning increased markedly at 2500 cal yr BP, suggesting that fire was regularly used as a management tool until 1000 cal yr BP when woody vegetation became scarce. A sparse tree cover, with only weak signs of forest recovery, then became a permanent characteristic of the lower Danube Plain, highlighting more or less continuous anthropogenic pressure. The timing of anthropogenic ecosystem transformation here (2500 cal yr BP) falls between that in central-eastern (between 3700 and 3000 cal yr BP) and eastern (after 2000 cal yr BP) Europe. Our study is the first quantitative land cover estimate at the forest steppe ecotone in southeastern Europe spanning 6000 years. It provides critical empirical evidence that, at a broad spatial scale, the present-day forest steppe and woodlands reflect the potential natural vegetation in this region under current climate conditions. However, the extent of tree cover and its composition have been neither stable in time nor shaped solely by the climate. Consequently, vegetation change must be seen as dynamic and reflecting wider changes in environmental conditions including natural disturbances and human impact.

Reliable quantitative vegetation reconstructions for Europe during the Holocene are crucial to improving our understanding of landscape dynamics, making it possible to assess the past effects of environmental variables and land-use change on ecosystems and biodiversity, and mitigating their effects in the future. We present here the most spatially extensive and temporally continuous pollen-based reconstructions of plant cover in Europe (at a spatial resolution of 1° × 1°) over the Holocene (last 11.7 ka BP) using the ‘Regional Estimates of VEgetation Abundance from Large Sites’ (REVEALS) model. This study has three main aims. First, to present the most accurate and reliable generation of REVEALS reconstructions across Europe so far. This has been achieved by including a larger number of pollen records compared to former analyses, in particular from the Mediterranean area. Second, to discuss methodological issues in the quantification of past land cover by using alternative datasets of relative pollen productivities (RPPs), one of the key input parameters of REVEALS, to test model sensitivity. Finally, to validate our reconstructions with the global forest change dataset. The results suggest that the RPPs.st1 (31 taxa) dataset is best suited to producing regional vegetation cover estimates for Europe. These reconstructions offer a long-term perspective providing unique possibilities to explore spatial-temporal changes in past land cover and biodiversity.

The Eurasian (née European) Modern Pollen Database (EMPD) was established in 2013 to provide a public database of high-quality modern pollen surface samples to help support studies of past climate, land cover, and land use using fossil pollen. The EMPD is part of, and complementary to, the European Pollen Database (EPD) which contains data on fossil pollen found in Late Quaternary sedimentary archives throughout the Eurasian region. The EPD is in turn part of the rapidly growing Neotoma database, which is now the primary home for global palaeoecological data. This paper describes version 2 of the EMPD in which the number of samples held in the database has been increased by 60 % from 4826 to 8134. Much of the improvement in data coverage has come from northern Asia, and the database has consequently been renamed the Eurasian Modern Pollen Database to reflect this geographical enlargement. The EMPD can be viewed online using a dedicated map-based viewer at https://empd2.github.io and downloaded in a variety of file formats at https://doi.pangaea.de/10.1594/PANGAEA.909130 (Chevalier et al., 2019).

Wildfire occurrence is influenced by climate, vegetation and human activities. A key challenge for understanding the risk of fires is quantifying the mediating effect of vegetation on fire regimes. Here, we explore the relative importance of Holocene land cover, land use, dominant functional forest type, and climate dynamics on biomass burning in temperate and boreo-nemoral regions of central and eastern Europe over the past 12 kyr. We used an extensive data set of Holocene pollen and sedimentary charcoal records, in combination with climate simulations and statistical modelling. Biomass burning was highest during the early Holocene and lowest during the mid-Holocene in all three ecoregions (Atlantic, continental and boreo-nemoral) but was more spatially variable over the past 3–4 kyr. Although climate explained a significant variance in biomass burning during the early Holocene, tree cover was consistently the highest predictor of past biomass burning over the past 8 kyr. In temperate forests, biomass burning was high at ∼45 % tree cover and decreased to a minimum at between 60 % and 70 % tree cover. In needleleaf-dominated forests, biomass burning was highest at ∼ 60 %–65 % tree cover and steeply declined at >65 % tree cover. Biomass burning also increased when arable lands and grasslands reached ∼ 15 %–20 %, although this relationship was variable depending on land use practice via ignition sources, fuel type and quantities. Higher tree cover reduced the amount of solar radiation reaching the forest floor and could provide moister, more wind-protected microclimates underneath canopies, thereby decreasing fuel flammability. Tree cover at which biomass burning increased appears to be driven by warmer and drier summer conditions during the early Holocene and by increasing human influence on land cover during the late Holocene. We suggest that long-term fire hazard may be effectively reduced through land cover management, given that land cover has controlled fire regimes under the dynamic climates of the Holocene.
