Background/Objective: Surgical treatment of extensive tumors of the lower lip generates important defects in its structure and functions. Over time, a multitude of reconstruction options for these defects have been imagined. Unfortunately, the majority involve the use of both local and regional flaps, which either lack labial structure or disorganize the oral commissures as nodal oral function points. We set out to design a new reconstruction method that is not burdened by any of the above disadvantages, starting from the necessity to reconstruct large lower lip defects by exclusively using local and/or regional labial flaps belonging only to the healthy upper lip and the remaining structures of the lower lip after surgical tumor removal. In this way, the tissues involved in lower lip reconstruction will have a 100% labial structure and, supplementarily, the remaining oral commissures will not be damaged. Method: This new reconstruction method is based on the original association of WY-plasty to reduce the size of the large primary defect until it becomes congruent with the Abbe–Sabattini cross-lip flap. Results: Applied in a personalized manner, impressive results were obtained in several patients with lip tumors affecting about one-half of the lower lip length and in whom oncological surgical ablation produced defects in more than two-thirds of it. Conclusions: This new method is characterized by functional oral advantages, and can be applied in a personalized way to only certain categories of patients. Background and objectives. Temporary anchoring devices (TADs) are essential for controlling unwanted reactive movements in teeth because orthodontic treatments require effective anchorage to control tooth movement. The goal of this systematic review is to collect information on the advantages, disadvantages, and applications of TADs in orthodontic treatment by addressing the variables affecting their stability and efficacy. Materials and methods. Our review is a non-Cochrane review, following the PICO format for selection criteria, analyzed studies published between 2011 and 2021. The review focused on systematic reviews, meta-analyses, RCTs, case control, and cohort studies in English, extracted from PubMed and Google Scholar databases. A total of 123 papers were assessed, with 39 included for review. Results. The review found that TAD stability is highly influenced by micro-implant design, operator experience, placement technique, bone morphology and histology, and patient variables (such as age and sex). Notably, the quality and amount of bone, the length and diameter of the implant, and the surgical method all played a significant role in achieving primary stability. Although loosening and discomfort are possible side effects, TADs have been demonstrated to provide reliable and effective anchoring during orthodontic treatment. Conclusions. Greater treatment efficacy and patient comfort are made possible by the significant advancement in orthodontic anchoring that TADs represent. The performance of TADs is largely dependent on variables like operator experience, micro-screw size, and bone quality. The mandibular site presented a higher risk of loosening, although patient factors such as age and sex did not significantly affect the loosening rate. Objectives: The objective of the present case series is to describe the histology and surface ultrastructure of augmented keratinized gingival mucosa in humans during the early healing phase after surgical placement of a xenogeneic collagen matrix. Method and Materials: Six patients underwent surgical augmentation of keratinized tissue by placement of a three-dimensional (3D) xenogeneic collagen matrix. Full-depth mucosal biopsies including original attached gingiva, augmented gingiva, and the separation zone were performed at baseline and at postoperative days 7 and 14. The specimens were stained with hematoxylin-eosin, Masson-trichrome, picrosirius red, and Papanicolaou’s trichrome. Low-vacuum scanning electron microscopy (SEM) surface analysis was correlated with histology. Results: The separation zone was clearly visible upon histologic and SEM examination at 7 days. The portions of augmented mucosa consisted of well-structured, immature gingival tissue with characteristics of per secundam healing underlying a completely detached amorphous collagenous membrane-like structure of approximately 100 μm thick. At 14 days, histologic and ultrastructural examinations showed an almost complete maturation process. There were no detectable remnants of the collagen matrix within the newly formed tissues at either time point. Conclusions: Within their limits the results suggest that the 3D collagen matrix appears to play an indirect role during the early phase of wound healing by protecting the newly formed underlying tissue and guiding the epithelialization process. Maxillary constriction can be skeletal, dental or a combination of both. The expansion of the midpalatal suture is used to treat maxillary width deficiencies; it is performed through rapid maxillary expansion, used in children and adolescents and surgically assisted rapid maxillary expansion which is an alternative for adolescents and the only option for widening the maxilla in adults. During maxillary expansion, not only skeletal effect is achieved, but also dentoalveolar tipping occurs. Various treatment modalities have been used for maxillary expansion. These protocols are generally divided into rapid maxillary expansion and slow maxillary expansion and several appliances are used as palatal expanders. Widening of the maxilla not only corrects the transverse dimension and the cross-bite but also leads to an increase of nasal width, that improving quality of sleep. Health is a right that requires responsible individual actions. Oral health corresponds to an important part of general health, even if for a large majority of people healthy teeth are equal with beautiful teeth. For children and adolescents „having an attractive smile" is synonymous with social acceptance and success. Dental decay has a high incidence in children in our country and progress in decay prevention, diagnosis and treatment is not reflected in children and adolescents' oral health. It is established by studies conducted by dentists and psychologists that dental decay can affect the quality of life by engendering specific eating behaviors, particular ways of speech or smile and unfortunately pain. The aim of this article is to emphasize the modern approach of dental decay in line with principles of non-invasive strategies. An important element to be considered - prophylactic methods - must be included in every treatment plan, in order to control individual caries risk. The time invested in prevention during childhood represents a real benefit for the future adult's oral health. Purpose: The purpose of this study was to evaluate the occurrence of peri-implant diseases and their potential risk indicators in a private practice setting., Materials and Methods: This cross-sectional study evaluated data from 390 subjects (mean age 55.8 ± 11.6 years) with implant-supported prosthetic reconstructions, who were enrolled in a maintenance program for 6.25 ± 3.36 years. Clinical evaluation included peri-implant probing pocket depth (PPD), bleeding on probing (BOP) and full-mouth plaque scores (FMPS). Radiographic evaluation was performed using retro-alveolar radiographs for each implant. Further, smoking habits, history of periodontitis, or tooth loss due to periodontal disease, presence/absence of keratinized mucosa ≥ 2 mm and the quality of the prosthetic restoration were also assessed. The prevalence of the peri-implant disease (at the subject/implant level) was determined and various potential risk indicators were evaluated by multi-level logistic regression analysis., Results: The prevalence of peri-implant diseases was 37.7% and 23.3% at the subject and implant level, respectively. 14.3% of the subjects were diagnosed with peri-implant mucositis and 8.9% were diagnosed with advanced peri-implantitis (PI). PI was statistically significantly associated with poor (FMPS > 0.45, p 0.001) or moderate oral hygiene (FMPS: 0.3-0.45, p 0.001), a history of periodontitis (p 0.001), lack of keratinized tissue ≥2 mm (p 0.001) or implant function time > 5 years (p 0.001)., Conclusion: In a private practice setting, a prevalence of peri-implant diseases of 37.7%/ 23.3% (subject/implant level) was found. Poor oral hygiene, history of periodontitis, a keratinized mucosa 2 mm and a time in function ≥ 5 years have been associated with the occurrence of peri-implant diseases.
