No. of bitstreams: 0 Previous issue date: 2019-06-28 School violence has aroused the growing interest in research worldwide, not only because of the daily situations of violence that are registered in schools, but also because of the place where the aggressions occur, because the school is a place of conviviality and learning. In this scope, the experience in the victimization of violence plays an important role in the adolescent's school education, as well as its consequences and the damages caused are worrying and devastating for his health. Through situations of violence and what they can cause, projects, programs and / or tools that support coping with the problem are necessary. In this sense, the research aims to develop, validate and use a technological tool based on an application and web system that serves as a channel for recording violence practiced by students, students / staff and students / teachers, in the school environment, whether they are victims, witnesses or aggressors, in order to facilitate the process of denouncing violence and monitoring coping actions and consequently combating situations of violence. Methodological study, developed in three phases: Study and Design of an m-Health System; Technological Artifact Development Laboratory and Evaluation of an e-Health system. The development and evaluation of the Technological System were carried out at the Information Technology Application Center (NATI) of the University of Fortaleza (UNIFOR), the development of the system with the technical team and the evaluation with 11 specialists, and the field evaluation was carried out. held in the municipal schools of Fortaleza, with students and school managers. The research methodological process was carried out from January 2017 to December 2018. The data were analyzed in SPSS version 23.0, using the IVC and Cronbach's alpha; as well as median time and standard deviation for quantitative variables. Qualitative data were analyzed according to Bardin (2011). The study was approved by the Ethics Committee, under opinion 79751817.5.0000.5052. For the results, the benchmarking tool for integrative review of mobile apps was first used. There were 13 apps on the Google Play platform, 11 apps on the App Store and 05 apps on both platforms, totaling 19 apps. 10 applications were selected for comparative analysis, thus defining the functionalities of the designed application. Subsequently, an e-Health Bullying System No. was developed. Adolescents aged 11 to 18 years showed a positive assessment regarding the usability of the application. In the validation by the experts, it showed high internal reliability (Cronbach's alpha = 0.960; 0.949; 0.943) for the content (motivation, language, purpose), structure, appearance of the app and the web system and relevance of the technological system, respectively. Third, in the field assessment, students had access to the application to make reports of violence at school and, consequently, the confrontation and support of managers through the web system. 114 downloads were made, of these 17 students (14.9%) completed the registration to make complaints. Six students made complaints and in all 14 were made, that is, one student made more than one complaint. Of the 14 complaints, only seven were answered by the school coordinators. In the complaints, it was observed that verbal aggressions are the most common, marked by prejudice and discrimination. When classmates witness these types of violence, they feel bad about the situation. The lack of position and reactive conduct against violence directed at the student by the school's coordination was evident, as there was no answer or a coherent and coping response with the report. It is concluded that the Bullying System does not provide useful information on the link between the experience of violence and the effectiveness of coping, despite the weak support of the managers. These preliminary findings suggest that smartphones have great potential as valid and useful tools for tackling violence in schools. Keywords: Violence. Validation Studies. Adolescent. A violência escolar tem despertado o interesse crescente de pesquisas em âmbito mundial, não somente pelo fato das situações cotidianas de violência que são registrados nas escolas, mas pelo local em que as agressões ocorrem, pois, a escola é um lugar de convívio e aprendizado. Neste escopo, a experiência na vitimização da violência desempenha um papel importante na formação escolar do adolescente, como também as suas consequências e os danos causados são preocupantes e devastadoras para a sua saúde. Mediante as situações de violência e o que elas podem causar, se fazem necessários projetos, programas e/ou ferramentas que apoiem o enfrentamento do problema. Neste sentido, a pesquisa visa desenvolver, validar e utilizar uma ferramenta tecnológica baseada em aplicativo e sistema web que sirva de canal para registro da violência praticada entre alunos, alunos/funcionários e alunos/professores, no ambiente escolar, quer sejam vítimas, testemunhas ou agressores, a fim de facilitar o processo de denúncia da violência e acompanhamento das ações de enfrentamento e em consequência combater as situações de violências. Estudo metodológico, desenvolvido em três fases: Estudo e Concepção de um Sistema m-Health; Laboratorial de Desenvolvimento do artefato Tecnológico e Avaliação de um sistema e-Health. O desenvolvimento e a avaliação do Sistema Tecnológico foram realizados no Núcleo de Aplicação em Tecnologia da Informação (NATI) da Universidade de Fortaleza (UNIFOR), o desenvolvimento do sistema com a equipe técnica e a avaliação com 11 especialistas, e a avaliação de campo foi realizada nas escolas municipais de Fortaleza, com os alunos e gestores das escolas. O processo metodológico da pesquisa foi realizado no período de janeiro de 2017 a dezembro de 2018. Os dados foram analisados no SPSS versão 23.0, utilizando o IVC e o alpha de Cronbach; bem como tempo mediano e desvio padrão para variáveis quantitativas. Os dados qualitativos foram analisados de acordo com Bardin (2011). O estudo foi aprovado pelo Comitê de Ética, sob o parecer 79751817.5.0000.5052. Para os resultados primeiramente utilizou-se a ferramenta benchmarking para revisão integrativa de apps móveis. Verificou-se 13 apps na plataforma Google Play, 11 aplicativos no App Store e 05 apps em ambas plataformas, totalizando 19 apps. Foram selecionados 10 aplicativos para análise comparativa, assim, definindo as funcionalidades do aplicativo concebido. Posteriormente, foi desenvolvido um Sistema e-Health Bullying Não. Adolescentes de 11 a 18 anos, demonstraram a avaliação positiva em relação a usabilidade do aplicativo. Na validação pelos especialistas, evidenciou a confiabilidade interna alta (Alfa de Cronbach = 0,960; 0,949; 0,943) para o conteúdo (motivação, linguagem, finalidade), estrutura, aparência do app e do sistema web e relevância do sistema tecnológico, respectivamente. Em terceiro, na avaliação de campo, os alunos tiveram acesso ao aplicativo para fazer denúncias de violência na escola e por conseguinte o enfrentamento e apoio dos gestores pelo sistema web. Foram realizados 114 downloads, destes 17 alunos (14,9%) completaram o cadastro para realizar denúncias. Seis alunos fizeram denúncias e ao todo foram realizadas 14, ou seja, um aluno realizou mais de uma denúncia. Das 14 denúncias apenas sete foram respondidas pelos coordenadores das escolas. Nas denúncias observou-se que as agressões verbais são as mais comuns, marcadas por preconceitos e discriminações. Os colegas de sala ao testemunharem esses tipos de violência sentem-se mal com a situação. Evidenciou-se a falta de tomada de posição e conduta reativa contra a violência dirigida ao aluno por parte da coordenação da escola, pois não houve resposta ou uma resposta coerente e de enfrentamento com o relato. Conclui-se que o Sistema Bullying Não fornece informações úteis sobre a ligação entre a experiência de violências e a eficácia de enfrentamento, apesar do apoio dos gestores ter sido frágil. Essas descobertas preliminares sugerem que os smartphones têm grande potencial como ferramentas válidas e úteis para o enfrentamento da violência no espaço escolar. Palavras-chave: Violência. Identificar a prevalência e os fatores associados à sobreposição dos diferentes tipos de violência contra a mulher, notificada em serviços sentinela. Método: estudo transversal, realizado com 939 fichas de notificação de casos de violência contra a mulher, referentes ao triênio de 2006 a 2008, em Fortaleza, Ceará. Análises univariada e múltipla por regressão logística foram realizadas. Resultado: os resultados mostraram associação positiva entre a sobreposição de tipos de violência contra a mulher com a escolaridade variando de analfabeta a ensino fundamental e a agressão ter ocorrido outras vezes. Conclusão: esse conhecimento contribui para o delineamento de ações específicas que visam o enfrentamento dessa problemática, assim como gera subsídios para proposta adequada de atendimento e encaminhamento das vítimas que buscam os serviços de saúde.

Objective: To assess the perception of violence from the reports of teachers of elementary and high school in a public state school in Fortaleza, Ceará. Method: A qualitative descriptive study conducted with 26 teachers, in the period from January to May 2006. To collect the data we adopted the following procedures: the discussion on school violence during the meetings of the teacher training week; the reading of the school regulations and the distribution of an instrument with the guiding question about the concept of violence as from the school reality, among the participants of the study. Afterwards, the thematic analysis of the reports was carried out. Results: Through the reports of teachers emerged the following categories: physical aggression, verbal aggression and fights as the most cited; to hide personal belongings, cursing, shoving and disrespect for the teacher and pranks were also mentioned. Conclusion: From the reports it appears that violence is perceived in physical, dimension moral and symbolic, and these “typologies” corroborate the descriptions found in literature as bullying or incivility, which dominate the school context. Objetivo: Analisar a percepção de violência a partir dos relatos de professores do Ensino Fundamental e Médio de uma escola pública estadual, em Fortaleza, Ceará. Método: Estudo descritivo, qualitativo, realizado com 26 professores, no período de janeiro a maio de 2006. Para a coleta de dados adotaram-se os seguintes procedimentos: discussão sobre violência na escola durante as reuniões da semana pedagógica; leitura do regimento escolar e distribuição de um instrumento com questão norteadora sobre a concepção de violência, a partir da realidade escolar, entre os participantes do estudo. Posteriormente foi feita a análise temática dos relatos. Resultados: Através dos relatos dos professores emergiram as seguintes categorias: agressões físicas, verbais e brigas como as mais citadas; esconder objetos pessoais, palavrões, empurrões, desrespeito ao professor e brincadeiras de mau gosto também foram mencionados. Conclusão: A partir dos relatos conclui-se que a violência é percebida nas dimensões física, moral e simbólica e essas “tipologias” corroboram as denominações encontradas na literatura como bullying ou incivilidades, que predominam no contexto escolar.

Ever since, the government organs and the no-government ones look for forms for yours to face. The study (i) it analyzed the violence against the woman starting from the Records Notified in services sentry, in Fortaleza - CE, in the period from 2006 to 2008; (ii) it described the women's social and demographic profile in sexual situation of violence, maidservant and/or other violence among people; (iii) he/she told the social profile and the aggressor's behavior; (iv) to investigate the association among the characteristics social and demographic of the women, the aggressor's behavior, of the occurrence of the aggression, with the typology of the violence and (v) it investigated the actions among sectorial accomplished with those women in the services sentry investigated. I study of traverse cut, accomplished in three services surveillance sentry in violence against the woman, reference in domestic and sexual violence of the Municipal General office of Health of Fortaleza, Ceará. The population of the study was represented by the women that suffered violence with age same or larger of 12 years. 939 records of violence notification were collected against the woman. Were the data organized, done codify and done submit to the descriptive statistical analysis and the test Pearson Chi-Square (x2) with value of p = 0,05. The research was approved by the Committee of Ethics and Research of the University of Fortaleza, to seem no. 123/2009. Of the 939 analyzed records, the psychological violence happened in 84,7% cases, following by the physics (74,8%), negligence or abandonment (18,2%), sexual (13,3%), financial (4,0%) and torture (2,7%). The number of women with violence occurrence increases in the age between 25 and 39 years. To be married showed association to suffer physical violence (p=0,036) and torture (p=0,042). The paper aims at analyzing the health care team's reports on educational practices to road traffic victims during the period of hospitalization and rehabilitation, trying to reduce traumatic orthopedic sequelae. The study was carried out with 10 professionals from the health care team who work in a public emergency hospital in the city of Fortaleza, State of Ceará, Brazil, in 2006. The semi-structured interview was the technique for collecting data and these were submitted to thematic analysis and then discussed in the light of Health Education and Health Promotion. To the participants, the conception of educational practice converged to a preventive, informational, and normative approach; although the health care team did not accomplish with integrality and humanization the therapeutic support to individuals, they developed a timid educational practice, which demystifies Health Promotion as the focus of primary care. Thus, the reorientation of the health care team's practice, based on an interdisciplinary approach to road traffic victims, can help minimize the incidence of sequelae. O objetivo deste estudo foi analisar os relatos da equipe de saúde quanto às práticas educativas ao vitimado no trânsito durante a hospitalização/reabilitação, visando à minimização da instalação de sequelas traumato-ortopédicas. Este estudo foi realizado com 10 profissionais da equipe de saúde que trabalham em um hospital público de emergência em Fortaleza, Ceará, Brasil, em 2006. A entrevista semiestruturada foi a técnica utilizada para a coleta de dados, os quais foram submetidos à análise temática e discutidos à luz da Educação em Saúde e Promoção da Saúde. Para os participantes, a concepção sobre a prática educativa convergiu para o caráter preventivo, informacional e normativo; a equipe de saúde, apesar de não exercer o processo de assistência terapêutica integral e humanizada, alguns profissionais da equipe de saúde desenvolvem de forma tímida a prática educativa, desmitificando a Promoção da Saúde como foco da atenção primária. Desse modo, a reorientação da prática da equipe de saúde, com enfoque interdisciplinar à vítima de acidente de trânsito, pode ser um diferencial para minimizar a instalação de sequelas.

This article describes risk factors for domestic and collective violence against women, these women's understanding of domestic and collective violence and the administration of the Calgary Family Assessment Model (CFAM) to community residents in the city of Fortaleza, state of Ceará. It was a case study conducted in 2005 with 20 families. Four families were selected because they showed high potential for violence against women. The results showed that the women know about some risk factors for violence; however, they apparently do not know how to act with the objective of avoiding it. Unfavorable housing and family conditions are perceived as risk factors. Violence is perceived by the woman as something common in the daily life of the couple. Alcohol, illicit drugs, unemployment and low schooling constitute aggravating factors in the occurrence of violence among the participant families. It is concluded that the women face, in their daily lives, different modes of violence. They live in a social and political isolation system that may contribute to the reproduction of more complex mechanisms of violence, preventing them from expressing themselves with autonomy. Este trabalho descreve os fatores de risco para violência doméstica e coletiva contra a mulher, a compreensão dessas mulheres sobre a violência doméstica e coletiva e a aplicação do Modelo Calgary de Avaliação em Famílias (MCAF), em moradores de uma comunidade, em Fortaleza, Ceará. Foi um estudo de caso com 20 famílias, e, entre estas, foram escolhidas quatro que apresentaram maior potencial para violência contra a mulher, em 2005. Os resultados mostraram que as mulheres conhecem alguns fatores de risco para violência, porém aparentam não saber agir no sentido de evitá-la; as condições desfavoráveis de habitação e entorno familiar são percebidos como risco. A violência é percebida, principalmente pela mulher, como "algo comum" no cotidiano do casal. O álcool, o uso da droga ilícita, o desemprego e a baixa escolaridade também agravam a ocorrência da violência entre as famílias participantes. Conclui-se que as mulheres enfrentam, em seu cotidiano, diferentes modos de violência. Elas vivem em um sistema de isolamento social e político que pode contribuir para a reprodução de mecanismos mais complexos de violência, impedindo-as de manifestarem-se de forma mais autônoma.

O vírus linfotrópico de células T humanas 1 (HTLV-1) é o agente etiológico da leucemia de células T do adulto (ATL) e mielopatia associada ao HTLV-1/paraparesia espástica tropical (HAM/TSP), além de outras doenças inflamatórias. O presente estudo tem como objetivo investigar os aspectos soroepidemiológicos e moleculares da infecção pelo HTLV-1/2 em primodoadores de sangue e seus familiares no Estado do Piauí. No período de agosto/2011 a julho/2012, 37.306 primodoadores de sangue foram triados para detecção de anticorpos anti-HTLV-1/2 por ensaio imunoenzimático (ELISA), no Centro de Hematologia e Hemoterapia do Estado do Piauí (HEMOPI). Os indivíduos reagentes foram recrutados para segunda coleta de sangue e entrevista, ocasião que seus familiares foram convidados para participar da investigação. Os soros foram testados por ELISA, e a reatividade confirmada pelo line immunoassay (LIA). As amostras de sangue total dos indivíduos anti-HTLV-1/2 reagentes foram submetidas à detecção do DNA proviral por reação em cadeia pela polimerase (PCR) e sequenciamento, seguido da análise filogenética. Dos 37.306 primodoadores de sangue triados para anti-HTLV-1/2, 48 foram reagentes pelo ELISA. Ao testar a segunda amostra desses indivíduos também por ELISA, 47 (0,13%) permaneceram como reagentes. Destas, 22 (0,06%) foram anti-HTLV-1 e 14 (0,04%) anti-HTLV-2 positivas pelo LIA. A maioria dos doadores infectados pelo HTLV-1 ou 2 relatou características de risco relacionadas à transmissão vertical e sexual. A caracterização genética demonstrou que os 22 isolados do HTLV-1 foram pertencentes ao subtipo Cosmopolita (HTLV-1a) do subgrupo Transcontinental (A). Para o HTLV-2, 11/14 (78,6%) amostras anti-HTLV-2 positivas foram amplificadas e, posteriormente, caracterizadas como do subtipo a (HTLV-2a/c). De 21 famílias dos primodoadores de sangue HTLV-1/2 soropositivos contactadas, cinco concordaram em participar do estudo. Três famílias apresentaram, além do caso índice (doador), pelo menos dois outros membros anti-HTLV-1 ou 2 soropositivos (ELISA/LIA). Combinando os dados epidemiológicos dos membros da primeira família com a análise molecular de seus isolados, foi evidenciada a ocorrência de transmissão intrafamiliar/vertical do HTLV-1 e, na segunda e terceira famílias, do HTLV-2. Adicionalmente, nesta última, a transmissão sexual do HTLV-2 pode ser considerada. A prevalência encontrada para o HTLV- 1/2 no presente estudo (0,13%; IC 95%: 0,09-0,17) é concordante com as estimadas em doadores de sangue no Brasil, e os subtipos do HTLV-1 e 2 identificados nesta investigação corroboram a predominância desses no País. Este estudo fornece, ainda, evidência de transmissão intrafamiliar do HTLV-1 e 2. Human T-lymphotropic virus type (HTLV-1) is the etiological agent of adult T-cell leukemia/lymphoma (ATL), HTLV-1-associated myelopathy/tropical spastic paraparesis (HAM/TSP), and other inflammatory diseases. HTLV-2 has been associated with a syndrome similar to HAM/TSP and other clinical manifestations. The present study aimed to investigate the seroepidemiologic and molecular aspects of HTLV-1/2 infection in the first-time blood donors and their family members in the state of Piauí. Between August 2011 and July 2012, 37,306 first-time blood donors were screened by enzyme-linked immunosorbent assay (ELISA) for the presence of anti-HTLV-1/2 antibodies in the Centro de Hematologia e Hemoterapia do Estado do Piauí (HEMOPI). The positive individuals were recruited for a second blood collection and interview, when their family members were invited to participate to this investigation. Sera were tested by ELISA, and the positivity confirmed by line immunoassay (LIA). Whole-blood samples of anti-HTLV seropositive individuals were submitted to proviral DNA detection by polymerase chain reaction (PCR) and nucleotide sequencing, followed by phylogenetic analysis. Of 37,306 first-time blood donors, 48 were anti-HTLV-1/2 reactive by ELISA. By testing the second serum samples, 47 (0.13%) were also reactive by ELISA. Among these, 22 (0.06%) were anti-HTLV-1 and 14 (0.04%) anti-HTLV-2 positive by LIA. Most of HTLV-1 or 2 infected patients reported risk characteristics related to vertical and sexual transmission. Genetic characterization demonstrated that all 22 HTLV-1 isolates belonged to the Cosmopolitan subtype HTLV-1a Transcontinental subgroup A. For HTLV-2, 11/14 (78.6%) anti-HTLV-2 positive samples were amplified and then identified as subtype a (HTLV-2 a/c). Of 21 families invited to take part in the study, five agreed to participate. Three had, in addition to the index case (blood donor), two or more anti-HTLV-1 or 2-positive members (ELISA/LIA). By combining epidemiological data of the first family members with molecular analysis of their isolates, intrafamilial/vertical transmission of HTLV-1 was suggested, as well as the transmission of HTLV-2 among the second and the third families. Additionally, HTLV-2 sexual transmission could be considered in the last family. The prevalence of 0.13% (CI 95%: 0.11-0.17) for HTLV-1/2 found in this study is similar to those observed in blood donors in Brazil, and HTLV-1 and 2 subtypes identified in this investigation were consistent with those circulating in Brazil. - Abstract
Against the dimension of the problem on violence against child and adolescent for the public health, this article analyzes the process of notification of violence in children and adolescents for doctors, nurses and dentists by the family health teams in Fortaleza, Ceará State, in the exercise of professional practice. It is treated as a study of cut-traverse with the participation of 359 professionals, registered as Family Health Teams (FHT). The questionnaire was applied, and the data collected were organized, codified, tabulated and submitted to a descriptive statistical analysis and calculations of statistics measures significant, through the Qui-square Test of Pearson (chi2) with respective value of p = 0.05, in the program Statistical Social Package Sciences-SPSS, version 13.0 to Windows. It was observed that 52% of the professionals don't know the notification record and 69% never participated on training in the area. The time of formation is significant statistics in the notification of cases (p=0.002). The conclusion is that the notification of child abuse for professionals of FHT happens on a punctual way instead of a systematic, in that sense, the development of programs of continuous training and the amplification of the nets of professional support they can reduce the insecurity degree and to increase the number of notifications of cases of child abuse.

19. [Impact of violence on the health of families at Fortaleza, Ceará State].

- Abstract
The study describes the impact of the violence on the quality of life of the families, which were attended at the Relatives and Friends Violence Victims Association (ARFVV) in Fortaleza, Ceará State. The case study was accomplished among five mothers that were involved in a NGO on the battle against the violence. The semi-structured interview, the participation at the institution's meetings and the documental research were the data collecting techniques, being these last ones, submitted to the thematic analysis and discussed according to the literature and Letters of Health Promotion. The impacts caused by the violence comprise the physical, emotional and social family's health, cause changes on behavior between the members, and lead to the increasing of smoking, drinking, social isolation and exacerbate revolt feelings, vengeance and pessimism. The NGO's work has been important to support the families on the conflicts overcoming, health familiar restoration, redeeming the self-expectation, the hope on the justice and social mobility. The health of families' victims from violence is affected on the biological, emotional and social aspects, favoring the process of sickness. These have appealed to the support from NGOs to transform the 'victim condition' - violence hostage to the 'responsible citizenship' - guardians of peace on the battle against the urban violence.
