Resumo: Com a pandemia da covid-19, muitos setores produtivos foram impactados e tiveram de realizar mudanças para sobreviver no contexto de incertezas, incluindo a agricultura orgânica. Dessa forma, o objetivo da pesquisa foi evidenciar as mudanças na cadeia produtiva do algodão orgânico oriundas da pandemia da covid-19. A pesquisa se caracteriza pela abordagem qualitativa, a partir de estudo de caso na cadeia produtiva da cooperativa Justa Trama, que possui elos de produção em diferentes regiões do Brasil. A coleta de dados foi realizada a partir de entrevistas semiestruturadas com seis pessoas, uma de cada elo. Os dados foram interpretados utilizando a análise de conteúdo por categorias de análise, conforme palavraschave predefinidas. Foi concluído que a pandemia alterou a forma de trabalho nos elos, porém não houve prejuízos à Justa Trama. Além disso, a cadeia produtiva do algodão orgânico está sujeita a alguns desafios, como a seca, falta de assistência técnica, pouco incentivo para aderir à produção orgânica e a dificuldade em encontrar mão de obra qualificada. Esses são pontos relevantes encontrados na pesquisa, que indicam a necessidade de acesso às políticas públicas e aos financiamentos pelos trabalhadores da cadeia produtiva. Abstract: With the COVID-19 pandemic, many productive sectors were impacted and had to make changes to survive in the context of uncertainties, including organic agriculture. Thus, the objective of the research was to highlight the changes in the organic cotton production chain arising from the COVID-19 pandemic. The research characterizes by a qualitative approach based on a case study on the production chain of the Justa Trama cooperative, which has production links in different regions of Brazil. Data collection was carried out through semi-structured interviews with six people, one from each link. Data were interpreted using content analysis by analysis categories, according to pre-defined keywords. It was concluded that the pandemic changed the way of working in the links, but there was no damage to Justa Trama. In addition, the organic cotton production chain is subject to some challenges, such as drought, lack of technical assistance, little incentive to adhere to organic producton, and difficulty finding qualified labor. These are relevant points found in the research, which indicate the need for workers in the production chain to access public policies and funding. Así, el objetivo de la investigación fue evidenciar los cambios en la cadena productiva del algodón orgánico derivados de la pandemia del COVID-19. La investigación se caracteriza por un enfoque cualitativo basado en un estudio de caso en la cadena productiva de la cooperativa Justa Trama, que tiene vínculos productivos en diferentes regiones de Brasil. La recolección de datos se realizó a través de entrevistas semiestructuradas con seis personas, una de cada enlace. Los datos fueron interpretados mediante análisis de contenido por categorías de análisis, según palabras clave predefinidas. Se concluyó que la pandemia cambió la forma de trabajar en los enlaces, pero no hubo daños en Justa Trama. Además, la cadena productiva del algodón orgánico está sujeta a algunos desafíos, como sequía, falta de asistencia técnica, poco incentivo para adherirse a la producción orgánica y la dificultad para encontrar mano de obra calificada. Son puntos relevantes encontrados en la investigación, que apuntan a la necesidad de que los trabajadores de la cadena productiva tengan acceso a políticas públicas y financiamiento.

Systemic lupus erythematosus (SLE), antiphospholipid syndrome (APS), and Sjögren's syndrome (SS) are heterogeneous autoimmune diseases. Severe manifestations and refractory/intolerance to conventional immunosuppressants demand other options, namely biological drugs, and small molecules. We aimed to define evidence and practice-based guidance for the off-label use of biologics in SLE, APS, and SS. Recommendations were made by an independent expert panel, following a comprehensive literature review and two consensus rounds. The panel included 17 internal medicine experts with recognized practice in autoimmune disease management. The literature review was systematic from 2014 until 2019 and later updated by cross-reference checking and experts' input until 2021. Preliminary recommendations were drafted by working groups for each disease. A revision meeting with all experts anticipated the consensus meeting held in June 2021. All experts voted (agree, disagree, neither agree nor disagree) during two rounds, and recommendations with at least 75% agreement were approved. A total of 32 final recommendations (20 for SLE treatment, 5 for APS, and 7 for SS) were approved by the experts. These recommendations consider organ involvement, manifestations, severity, and response to previous treatments. In these three autoimmune diseases, most recommendations refer to rituximab, which aligns with the higher number of studies and clinical experience with this biological agent. Belimumab sequential treatment after rituximab may also be used in severe cases of SLE and SS. Second-line therapy with baricitinib, bortezomib, eculizumab, secukinumab, or tocilizumab can be considered in SLE-specific manifestations. These evidence and practice-based recommendations may support treatment decision and, ultimately, improve the outcome of patients living with SLE, APS, or SS. publishersversion published

Protocols to support safe discontinuation are still needed, especially in countries with high utilization rates. Objectives: To evaluate the feasibility, effectiveness, and safety of a benzodiazepine discontinuation protocol in primary care setting. Methods: Nonrandomized, single-arm interventional study, at primary care units. Family physicians (FPs) recruited patients (18-85 years-old) with benzodiazepine dependence and chronic daily use ≥3 months. Patients with daily dosages ≥30 mg diazepam-equivalent, taking zolpidem, with a history of other substance abuse or major psychiatric disease were excluded. After the switch to diazepam, the dosage was gradually tapered according to a standardized protocol. Primary endpoint was the percentage of patients who stopped benzodiazepine at the intervention last visit. Dosage reduction, withdrawal symptoms, patients' and FPs' satisfaction with the protocol were evaluated. Results: From 66 enrolled patients (74% female; 66.7% aged >64 years; median time of benzodiazepine use was 120 months), 2 withdrew due to medical reasons and 3 presented protocol deviations. Overall, 59.4% of participants successfully stopped benzodiazepine (60.7% when excluding protocol deviations). Men had higher probability of success (relative risk = 0.51, P = 0.001). A total of 31 patients reported at least 1 withdrawal symptom, most frequently insomnia and anxiety. Most of participating FP considered the clinical protocol useful and feasible in daily practice. Among patients completing the protocol, 77% were satisfied. For the patients who reduced dosage, 85% kept without benzodiazepines after 12 months. Conclusion: The discontinuation protocol with standardized dosage reduction was feasible at primary care and showed long-term effectiveness. publishersversion publishersversion published

Guidance about treatment-resistant depression (TRD) in Portugal is very limited, even though depression prevalence is among the highest in European countries. A questionnaire was conducted, followed by two advisory boards with seven Portuguese psychiatry experts, to characterize and discuss MDD and TRD epidemiology, diagnosis, patient journey, treatment options, and unmet clinical needs. Consensus was reached on the main issues. In daily practice, TRD can be defined as moderate to severe MDD episodes with insufficient clinical improvement after two antidepressant treatments, taken in adequate doses and duration. TRD diagnosis and treatment are mostly decided by psychiatrists at public hospitals. Treatment type and duration must be adjusted to characteristics of the patient and the depressive episode, including symptoms, number of previous episodes, comorbidities, and previous treatment response and side effects. The most relevant objectives of TRD treatment are reaching response and remission, prevention of suicide, and improvement of quality of life, functionality, and wellbeing. Regarding pharmacotherapy, antidepressant switch occurs more frequently with non-response, while optimization, combination, and augmentation are considered for patients with partial response. Psychotherapy should be considered in parallel to pharmacological treatment. Brain stimulation techniques are underused. Lifelong treatment is required for recurrent or more chronic TRD episodes, but patient adherence is also poorer in these cases. In Portugal, TRD management is limited by lack of access to specialist care and to many treatment options. These aspects highlight that conventional pharmacotherapy does not lead to remission in many patients and that optimization strategies are frequently necessary to achieve satisfactory treatment outcomes. publishersversion published

O livro integral "A Gripe Espanhola de 1918" encontra-se disponível no RepositóriUM em: http://hdl.handle.net/1822/64699, A dimensão e os efeitos da pandemia de gripe, que grassou entre a população portuguesa a partir de 1918, carecem ainda de aprofundamento sobretudo na perspetiva regional. O presente artigo visa analisar os contextos e as consequências do fenómeno em quatro municípios da região do Alto Minho – Viana do Castelo (nas suas 2 freguesias urbanas), Caminha (19 freguesias), Vila Nova de Cerveira (15 freguesias) e Paredes de Coura (21 freguesias) - integrados no distrito de Viana do Castelo e localizados na área geográfica que estabelece fronteira com a Galiza. Considerando esta proximidade, mas também a clivagem entre meio urbano e rural ou a especificidade das comunidades marítimas relativamente à restante população, pretendemos analisar eventuais comportamentos diferenciais observados durante o período de maior exposição à epidemia.

Esta pesquisa tem por finalidade refletir a importância da arte na escola, com um enfoque maior nas artes visuais, e como objetivos específicos analisar as dificuldades enfrentadas pelos professores do 4º ano da Unidade Integrada Buriti Selvagem no processo de se trabalhar com a disciplina de Arte na referida instituição, bem como, identificar formas que possibilitem a transformação das dificuldades em respostas positivas no ensinar e aprender Arte. Fez-se uma pesquisa bibliográfica para que a abordagem do tema fosse feita de maneira coerente e embasada nas ideias de diversos pensadores que ampliassem e dessem suporte às ideias apresentadas. Procurou-se também entender os dilemas enfrentados pelos professores na escola, saber como a unidade de ensino trata a disciplina, se a mesma busca recurso e projetos que valorizem o ensino de arte na escola. Outro fator importante também destacado na pesquisa foi a realidade do aluno e saber se ela está sendo inserida nas aulas de artes visuais nas turmas do 4º ano. Os autores que fundamentaram esta pesquisa foram Ferreiro e Teberosy (2008), Buoro (2003), Barbosa (2011) e Selbach (2010), entre outros. A pesquisa foi realizada somente com os professores por meio de um questionário de perguntas abertas. Com as respostas obtidas foram analisados os dados e posteriormente lançadas algumas sugestões para que o ensino de arte seja mais significativo e relevante aos alunos podendo contribuir para uma melhoria do processo de ensino e aprendizagem na área artística. Portanto, o trabalho com artes visuais contribui significativamente para que o aluno consiga, por meio das formas, cores texturas e sons, expressar-se e comunicar-se com o mundo exterior, através das suas ideias e dos seus sentimentos.

O estudo da função vascular periférica tem beneficiado do desenvolvimento de diversas técnicas e metodologias de avaliação o as quais permitem caracterizar a função com um mínimo de invasibilidade. O estudo do significado de diversas variáveis transcutâneas tem ajudado a melhor compreender as complexas relações que regem a perfusão dos tecidos in vivo. No presente estudo procurámos avaliar o perfil de resposta de um grupo de indivíduos saudáveis jovens de ambos os sexos (n=23, 22,4 ± 2,2 anos), a uma manobra dinâmica (provocação) envolvendo a variação postural passiva do membro inferior (A: elevação da perna e B: decúbito dorsal) de modo a caracterizar a função vascular periférica por meio de variáveis transcutâneas (tc). A perfusão local medida por Fluxometria de Laser Doppler (LDF) , a Perda Trans-epidérmica de água (PTEA) as pressões parciais de oxigénio e dióxido de carbono transcutâneos e a oximetria de pulso foram escolhidas e, um intervalo de confiança de 95% adoptado. LDF, PTEA e tcpO 2 são, manifestamente, as variáveis que melhor permitem seguir o processo em cada fase experimental. Em A foram registadas, durante a provocação, diferenças significativas na perfusão sanguínea e na pressão transcutânea de oxigénio e PTEA, especialmente nas mulheres, enquanto que o perfil de resposta obtido em B foi idêntico em ambos os grupos e comparável B hiperémia reactiva. Qualquer dos protocolos permitiu evidenciar uma relação inversamente recíproca entre a PTEA e o LD. Os resultados obtidos confirmam a utilidade destas metodologias na avaliação da função vascular periférica, confirmando as relações entre PTEA e LDF as quais devem ser adequadamente aprofundadas., The study of the peripheral vascular function benefits from the development of several evaluation techniques and methodologies which allow for the characterization of the function without invasion of the tissue. The study of the meaning of several transcutaneous variables has helped to understand the complex relationships that rule the perfusion of the tissues in vivo. In the present study, we attempted to evaluate the profile of response of a group of healthy young individuals from both genders (n=23, 22,4 ± 2,2 years), to a dynamic maneuver (provocation) involving the postural passive change of the inferior member (A: elevation of the leg and B: dorsal decubitus), to perform the characterization of the peripheral vascular function through transcutaneous variables (tc). The local perfusion, measured by Laser Doppler flowmetry (LDF), the Trans Epidermal Water Loss (TEWL), the transcutaneous partial pressures of oxygen and carbon and the pulse oximetry were chosen and a confidence interval of 95% was adopted. LDF, TEWL and tcpO2 are, clearly, the variables that allow for a better follow up of the process in each experimental phase. In A, during the provocation, significant differences in the blood perfusion and in the oxygen transcutaneous pressure and TEWL were registered, especially in women. The profile response obtained in B was identical in both groups and comparable to the reactive hyperemia. Any of the protocols allowed to evidence an inversely reciprocal relationship between TEWL and LDF.
