ObjectivesWe examined the fecal virome and bacterial community composition of children with Crohn disease (CD), ulcerative colitis (UC), and healthy controls to test the hypothesis that unique patterns of viral organisms and/or presence of bacterial pathogens may be identified that could contribute to the pathogenesis of pediatric inflammatory bowel disease (IBD).MethodsFecal samples from 24 children (mean 12.2 years) with CD (n = 7) or UC (n = 5) and similar aged controls (n = 12) were processed to determine individual viromes. Viral sequences were identified through translated protein sequence similarity search. Bacterial microbiota were determined by sequencing of the V4 region of the 16S rRNA gene.ResultsOnly a few human viruses were detected, so virome analyses focused on bacterial viruses. The relative abundance of Caudovirales was greater than that of Microviridae phages in both IBD and healthy controls. Caudovirales phages were more abundant in CD (mean 80.8%) than UC (48.8%) (P = 0.05) but not controls. The richness of viral strains in Microviridae but not Caudovirales was higher in controls than CD (P = 0.05) but not UC cases. No other measure of phage abundance, richness, or Shannon diversity showed significant difference between the 2 IBD and control groups. Bacterial microbiota analysis revealed that IBD diagnosis, albumin, hemoglobin, erythrocyte sedimentation rate, and probiotic supplementation correlated to the composition of gut bacterial microbiota.ConclusionsMinor patterns in gut virome and bacterial community composition distinguish pediatric IBD patients from healthy controls. Probiotics are associated with bacterial microbiota composition. These exploratory results need confirmation in larger studies.

ObjectivesThe aim of the study was to investigate the value of microscopic findings in the classification of pediatric Crohn disease (CD) by determining whether classification of disease changes significantly with inclusion of histologic findings.MethodsSixty patients were randomly selected from a cohort of patients studied at the Pediatric Inflammatory Bowel Disease Clinic at the University of California, San Francisco Benioff Children's Hospital. Two physicians independently reviewed the electronic health records of the included patients to determine the Paris classification for each patient by adhering to present guidelines and then by including microscopic findings.ResultsMacroscopic and combined disease location classifications were discordant in 34 (56.6%), with no statistically significant differences between groups. Interobserver agreement was higher in the combined classification (κ = 0.73, 95% confidence interval 0.65-0.82) as opposed to when classification was limited to macroscopic findings (κ = 0.53, 95% confidence interval 0.40-0.58). When evaluating the proximal upper gastrointestinal tract (Paris L4a), the interobserver agreement was better in macroscopic compared with the combined classification.ConclusionsDisease extent classifications differed significantly when comparing isolated macroscopic findings (Paris classification) with the combined scheme that included microscopy. Further studies are needed to determine which scheme provides more accurate representation of disease extent.

- Abstract
Reversible self-association (RSA) of therapeutic proteins presents major challenges in the development of high-concentration formulations, especially those intended for subcutaneous administration. Understanding self-association mechanisms is therefore critical to the design and selection of candidates with acceptable developability to advance to clinical trials. The combination of experiments and in silico modeling presents a powerful tool to elucidate the interface of self-association. RSA of monoclonal antibodies has been studied extensively under different solution conditions and have been shown to involve interactions for both the antigen-binding fragment and the crystallizable fragment. Novel modalities such as bispecific antibodies, antigen-binding fragments, single-chain-variable fragments, and diabodies constitute a fast-growing class of antibody-based therapeutics that have unique physiochemical properties compared to monoclonal antibodies. In this study, the RSA interface of a diabody–interleukin 22 fusion protein (FP-1) was studied using hydrogen–deuterium exchange coupled with mass spectrometry (HDX-MS) in combination with in silico modeling. Taken together, the results show that a complex solution behavior underlies the self-association of FP-1 and that the interface thereof can be attributed to a specific segment in the variable light chain of the diabody. These findings also demonstrate that the combination of HDX-MS with in silico modeling is a powerful tool to guide the design and candidate selection of novel biotherapeutic modalities.

- Abstract
Objective: We aimed to identify features that allow differentiation of primary antiphospholipid syndrome (PAPS) patients that suffer recurrent thrombotic events (RTE) despite anticoagulation, from the other diagnosed PAPS patients. Methods: This was an exploratory study of anticoagulated PAPS patients attending an Autoimmune Diseases Unit (1998-2018). From 2016, anti-phospholipid antibodies and lupus anticoagulant were determined for each patient at consecutive visits, collected together with retrospective clinical characteristics, laboratory, and therapeutic markers and compared according to the occurrence of thrombotic events during follow-up. Results: Overall, two thirds of the patients were female, 93% were Caucasian, with a median age of 40 years at diagnosis, for a median time of 11.5 years in follow-up. Out of 54 patients, 10 were identified with RTE. There were no significant differences among the RTE and non-RTE patients as far as classical risk factors for clotting disorders. The RTE group was characterized by a higher proportion of younger patients, male sex and positivity for all laboratory markers, and initially and over follow-up as well as a sustained high-risk profile based on APS laboratory markers. Anticardiolipin IgG at onset was the only statistically significant marker of the RTE group. At the end of follow-up, consistent reversion to negative status was a rare event, observed in 20% of RTE vs. 25% of non-RTE patients. Conclusions: Despite therapy, we were able to identify features associated to thrombotic events in patients with PAPS. Prospectively regular clinical and laboratory monitoring might be warranted in order to treat APS more assertively.

O Teatro Mágico é um grupo musical criado em 2003 pelo músico, compositor e instrumentista Fernando Anitelli. A trupe, como também é conhecido o grupo, foi inspirada nos saraus e reúne várias expressões artísticas, como dança, circo, performances acrobáticas, poesia e literatura. Trouxe para o palco as vivências compartilhadas em encontros com diversos artistas que acreditaram na arte como forma de manifestação de ideias. O projeto, levando em consideração o redirecionamento da relação do público com a música, incorporou a ideia de música livre e, por consequência, a criação da sigla MPB (música para baixar). Este trabalho analisa três letras de canções do grupo musical O Teatro Mágico, que foram selecionadas em função de apresentarem, numa comparação com a totalidade de sua obra, características do grupo em dialogar com os campos político e social do Brasil nas últimas décadas. O objetivo central desta pesquisa é averiguar como as letras das músicas do grupo estabelecem relações dialógicas com o cenário social, político e econômico do país, a partir de conceitos criados pelo filósofo russo Mikhail Bakhtin, já que uma das características das canções é a presença de enunciados com um teor crítico, irônico e reflexivo. Para compor o objeto de pesquisa, foram eleitas três canções, que carregam a característica de crítica social mencionada: O mérito e o monstro, Este mundo não vale o mundo e O sol e a peneira. Para isso, foram elencadas contribuições conceituais bakhtinianas como as de gênero do discurso, ideologia, intertextualidade e interdiscursividade e enunciado, categorias que se mostram essenciais para a análise das composições já que, para Bakhtin, as relações dialógicas podem estar presentes em uma única palavra, se levarmos em consideração que a palavra em si perpassa seu significado existente no dicionário. As letras foram analisadas, a partir de uma análise qualitativa, fazendo referência à teoria e a conceitos bakhtinianos, levando em consideração o contexto histórico de produção das canções. Observou-se, por meio das análises das letras das músicas, a importância da arte e da cultura, como forma de produção de enunciados que dialogam com questões de tamanha complexidade que estão presentes em nosso contexto social e político. This work analyzes three song lyrics from the musical group O Teatro Mágico, which were selected due to their comparison with the totality of their work, presenting characteristics of the group in dialogue with the political and social fields of Brazil in recent decades. The main objective of this research is to find out how the lyrics of Teatro Mágico's music establish dialogical relations with the country's social, political and economic scenario, based on concepts created by the Russian philosopher Mikhail Bakhtin, since one of the characteristics of the songs is the presence of statements with a critical, ironic and reflective content. To compose the object of research, three songs were chosen, which carry the aforementioned social criticism characteristic: O mérito e o monstro, Este mundo não vale o mundo and O sol e a peneira. For this, Bakhtinian conceptual contributions were listed, such as those of discourse genre, ideology, intertextuality and interdiscursivity and enunciation, categories that are essential for the analysis of the compositions since, for Bakhtin, dialogical relations can be present in a single word, if we take into account that the word itself pervades its existing meaning in the dictionary. The lyrics were analyzed, through a qualitative analysis, making reference to the Bakhtinian theory and concepts, taking into account the historical context of the songs production. Through the analysis of the lyrics of the songs, the importance of art and culture was observed, as a way of producing statements that dialogue with issues of such complexity that are present in our social and political context. Despite the availability of substantial clinical trial evidence, there is a lack of real-life data. The aim of this study was to assess disease status and quality of life in an outpatient population treated with biological disease-modifying anti-rheumatic drugs. Material and Methods: Cross-sectional study recalling all patients ever treated in our unit with biological disease-modifying anti-rheumatic drugs. Clinical and demographic data, compliance, disease activity, functional status, joint deformities, and comorbidities were documented, and patients queried on occupational status, education, marital status and generic health related quality of life questionnaires. Results: Recall was attended by 77 of the original 94 patients. At recall, median age was 63 years old, 82% of the patients were female and the median disease duration was 12 years. Biological therapy was started at a median of four years following disease onset. According to the disease activity score (DAS28), the percentage of patients with high, moderate, low disease activity or remission changed from 50, 45, 0 and 5 (pre-therapy) to 11, 37, 25 and 26 at recall, respectively; functional status was significantly improved. Seventy-five per cent of the patients retained the original treatment with good compliance. Lower Short Form-36 domain scores accompanied a low EQ-5D-3L score. Deceased patients (n = 6) had a lower estimated 10-year survival rate. In this group, biological therapy was discontinued at a higher frequency during follow-up. Discussion: A high disease activity and a high HAQ disability index characterized most patients at pre-bDMARD onset. Conclusion: Despite therapy switches and regular follow-up, a significant percentage of patients still presented with moderate disease activity, functional impairment and a poor health-related quality of life.

Systemic Lupus Erythematosus (SLE) is an autoimmune disease of unknown etiology that often affects women during childbearing age. Pregnant women with SLE are considered high-risk patients, with pregnancy outcomes being complicated by high maternal and fetal mortality and morbidity. Obstetric morbidity includes preterm birth, fetal growth restriction (FGR), and neonatal lupus syndromes. Active SLE during conception is a strong predictor of adverse pregnancy outcomes and exacerbations of disease can occur more frequently during gestation. Therefore, management of maternal SLE should include preventive strategies to minimize disease activity and to reduce adverse pregnancy outcomes. Patients with active disease at time of conception have increased risk of flares, like lupus nephritis, imposing a careful differential diagnosis of pre-eclampsia, keeping in mind that physiological changes of pregnancy may mimic a lupus flare. Major complications arise when anti-phospholipid antibodies are present, like recurrent pregnancy loss, stillbirth, FGR, and thrombosis in the mother. A multidisciplinary approach is hence crucial and should be initiated to all women with SLE at childbearing age with an adequate preconception counseling with assessment of risk factors for adverse maternal and fetal outcomes with a tight pregnancy monitoring plan. Although treatment choices are limited during pregnancy, prophylactic anti-aggregation and anticoagulation agents have proven beneficial in reducing thrombotic events and pre-eclampsia related morbidity. Pharmacological therapy should be tailored, allowing better outcomes for both the mother and the baby. Immunosuppressive and immunomodulators, must be effective in controlling disease activity and safe during pregnancy. Hydroxychloroquine is the main therapy for SLE due to its anti-inflammatory and immunomodulatory effects recommended before and during pregnancy and other immunosuppressive drugs (e.g. azathioprine and calcineurin inhibitors) are used to control disease activity in order to improve obstetrical outcomes. Managing a maternal SLE is a challenging task, but an early approach with multidisciplinary team with close monitoring is essential and can improve maternal and fetal outcomes.

Introdução: A mielinólise centropôntica e extrapôntica (MCPEP) é uma síndrome desmielinizante rara. A MCPEP é mais prevalente em receptores de transplante hepático tendo um prognóstico desfavorável associado. A correção rápida de hiponatremia e agentes imunossupressores, como os inibidores da calcineurina, foram identificados como causas possíveis para o desenvolvimento desta patologia. Métodos: Os autores descrevem uma revisão casuística com cinco casos apresentados em tabela simples de doentes que no decorrer do internamento na unidade de cuidados intensivos após transplantação hepática, desenvolveram sinais e sintomas neurológicos concomitantemente com alterações imagiológicas identificadas através de ressonância magnética crânio encefálica compatível com MCPEP. Resultados: O consumo de etanol em excesso, síndrome de encefalopatia hepática e hiponatremia foram os 3 pontos mais comummente identificados no período pré-cirúrgico. Quatro dos cinco doentes apresentaram, durante o período intraoperatório, variação dos níveis séricos de sódio superior aos valores de referência. O objetivo do estudo foi verificar quais fatores levam a fidelização dos alunos idosos sob a percepção dos gestores de academias, com intuito de idealizar se estruturas e meios satisfazem as necessidades dos alunos atendidos, e também objetivou a formulação de um estudo piloto, para que se torne fonte de pesquisa para melhor atender essa clientela. A verificação quanto à fidelização dos alunos idosos na prática de exercícios físicos foi obtida através de 34 questões objetivas, respondidas por 44 gestores de academias. Os resultados apontaram que Limpeza, Cumprimento dos Horários, Equipamentos Conservados, Avaliação Física, Serviço de Fisioterapia, Funcionários atenciosos e qualificados, Planos e Promoções, tornam os alunos idosos fidedignos. Logo, conclui-se embasados nas opiniões dos gestores, que o idoso quer atenção e qualidade, e que fidelidade não se compra, se conquista ao longo do prazo através de atitudes que transmitam confiança, respeito, cuidado e atenção com o outro.

A variedade da alimentação e dos hábitos de consumo mundiais dependiam de muitos fatores: posição social, riqueza, localização e clima. Atualmente, a comida denominada como fast food tem permeado quase todos os níveis da sociedade e culturas. Os restaurantes de fast food surgiram nos Estados Unidos da América (EUA), em 1950, e junto com um enorme número de restaurantes de fast food, cafés/bares, franchisings de motéis que abriram ao longo do sistema interestadual de autoestradas. Os franchisings mais conhecidos incluem Kentucky Fried Chicken (1930), Dunkin Donuts (1950), Burger King (1954) e McDonald’s (1955) (Dant, 2011). A população tinha uma vida atarefada, por conveniência comiam fora de casa e compravam comida-para-levar com frequência. No contexto de um mercado interno maduro e de alta competitividade, as cadeias de fast food expandiram as operações para países estrangeiros. Habitualmente, as cadeias multinacionais de fast food personalizam o formato e menu que oferecem de acordo com as preferências, tradições e costumes da população da região/país onde pretendem implementar os restaurantes. De acordo com a Euromonitor, espera-se que mercados mais desenvolvidos de fast food sigam direções diferentes. Uma parte dos Clientes de fast food procura experiências de consumo/serviço e alimentos com maior qualidade, mais frescos e opções flexíveis - mesmo pagando mais. A outra parte valoriza as características que tornaram a comida fast food popular – rapidez, comodidade, preços baixos. Esta tese de mestrado foca-se na pesquisa e avaliação das possibilidades de internacionalização da cadeia de restaurantes portuguesa h3™ para Londres. In recent times, in Europe as in other continents, eating and drinking habits varied tremendously depending upon many determinants including social standing, wealth, geographical position, climate, among others. Nowadays, fast food has permeated almost every level of society and cultures. Fast food restaurants emerged in United States of America (USA), in the 1950s, as a massive numbers of fast food restaurants, diners and motel franchises opened up along the newly constructed interstate highway infrastructures. Some of the well-known franchises (and their inaugural years) include Kentucky Fried Chicken (1930), Dunkin Donuts (1950), Burger King (1954), and McDonald’s (1955) (Dant, 2011). People would eat out frequently, and have food-on-the-go because of their busy life-style. With such a maturing and highly competitive domestic market these fast food chains expanded operations to many foreign countries. Multinational fast food chains have to customize their format and menu offerings according to the population preferences, tastes and customs from the region/country where they are implementing the restaurants. According to a Euromonitor report, the more mature fast food markets headed in two different directions. One part of the fast food Customers look for higher quality and foodservice experiences, fresher food and flexible options – even if it means pay a bit more. The other part of the Customers were more focused on what made fast food popular, speed, convenience and low prices. The scope of this master thesis is to investigate and evaluate the possibilities for the internationalization of the Portuguese restaurant chain h3™ to London.

Relatório de estágio de mestrado em Ciências da Comunicação (área de especialização em Publicidade e Relações Públicas), Com base na unidade curricular de estágio, no 2º Ciclo de Ciências da Comunicação, especialização em Publicidade e Relações Públicas, fiz o meu estágio na Amazing Ideas, onde ganhei alguma experiência profissional no mercado de trabalho. O relatório de estágio apresentado tem como principal objectivo descrever os projetos e atividades desenvolvidos aquando da passagem pela empresa Amazing Ideias, assim como refletir sobre os desafios de uma empresa portuguesa de calçado no mercado internacional. A Amazing Ideas é uma agência que presta serviços na gestão de projetos na área do marketing, comunicação, relações públicas e organização de eventos. Numa altura em que o calçado é exemplo de reindustrializar e inovar, qualificar e rejuvenescer, internacionalizar e comunicar, parece relevante estudar as estratégias de marketing e posicionamento internacional, nomeadamente, as que integram ações de publicidade. As empresas que pretendem entrar no mercado internacional têm que ter em conta os países estratégicos onde vão tentar comercializar os seus produtos; definir as estratégias de marketing para cada país e delinear estruturas e métodos de organização próprios para assegurar uma atividade internacional. Neste sentido, tendo em conta a experiência profissionalizante que o estágio conferiu, o presente relatório procurará refletir sobre os desafios de uma empresa portuguesa de calçado no mercado internacional, tendo como ponto fulcral o posicionamento internacional da mesma nos seguintes países estratégicos: Ucrânia, Rússia, Marrocos, França, Suíça, Luxemburgo, Estados Unidos, Austrália e Brasil. Assim, com base no caso My Maria Guimarães, trabalhado durante o estágio, serão analisados conceitos tais como marketing internacional, segmentação e posicionamento e o seu enquadramento e simbiose com noções de inovação, competitividade, brand equity, publicidade e tendências de comunicação., Companies seeking to enter the international market must take into account the differences between specific countries where they try to market their products. This means defining marketing strategies for each country and being aware of the structures and methods of organizing themselves to ensure good levels of sales and visibility. Amazing Ideas is an agency that provides services in project management, marketing, communications, public relations and various promotional events. I elected to join an internship with Amazing Ideas, part way through my course in Communications Sciences, as part of my Advertising and Public Relations module. The particular project I was involved in was helping to define certain strategies for a Protuguese footware company, “My Maria Guimarães”, in particular, focusing on international positioning within the following strategic countries: Ukraine, Russia, Morocco, France, Switzerland, Luxembourg, United States, Australia and Brazil. This included many concepts such as international marketing, segmentation and positioning, environmental issues and symbiosis, and notions of innovation, competitiveness, brand equity, advertising and communication trends analysis. Footwear is a good example where brands can be globally recognized but where targeted marketing is required. I was able to gain a great deal of professional experience in how to qualify and rejuvenate brands and also internationalise products and see how different countries communicate. I was also able to study marketing strategies and international positioning and integrated advertising. My final report described the projects and activities developed by me during my time at Amazing ideas, and reflected on the challenges of a typical Portuguese shoe company in the international market.

Syftet med studien är att analysera hur engagemang och underkastelse upprätthålls hos Jehovas vittnens medlemmar. Studien visar hur värvningsprocessen påverkar upprätthållelsen och belyser hur organisationens sanktioner och sociala kontroll påverkar medlemmarna. Studien belyser även hur Jehovas vittnen förhåller sig till uteslutna medlemmar. Studien utgår från ett organisationsteoretiskt perspektiv för att förklara hur Jehovas vittnen lyckas ha ett starkt inflytande över sina medlemmar. Studien genomfördes med semistrukturerade intervjuer där sju avhoppare från Jehovas vittnen medverkade. Studiens resultat visar att då organisationen byråkratiserats fanns det möjlighet för den att upprätthålla ett engagemang hos sina medlemmar. Värvningen är en central del för att fånga nya medlemmar men är också en central del till att bibehålla kontroll och inflytande över existerande medlemmar. Den sociala kontrollen inom Jehovas vittnen sker exempelvis genom angiveri, där medlemmarna kontrollerar och övervakar enskilda medlemmars engagemang. Ett annat sätt för Jehovas vittnen att upprätthålla sin organisation kan vara att utesluta tvivlarna för att de går emot de regler och normer som uppkommit i byråkratiseringen.
