Background: The moral harassment - a global problem rooted in medical culture - has a negative impact on medical formation. Resilience, conversely, is a protective competence against adverse situations, including harassment. Although they are relevant topics, there are few studies about moral harassment and resilience in medical formation.Objectives: The objective is to obtain data about resilience and moral harassment, which are part of the mental health of medical students and compound ethical-professional formation of them.Materials and Methods: Data collection was done during the enrollment of the students in the first year of the Universidade Federal de Su00e3o Paulo in February 2019. The data collection followed 4 questionnaires: 1) Sociodemographics; 2) Patient Health Questionnaire-4; 3) Wagnild-Young Resilience Scale- 25 itemsu00ae; 4) Questionnaire about moral harassment in previous moments of life.Results: All questionnaires had 109 responses, what represent a 90% of all first-year students. The main findings were: i) 91% of participants reported having suffered some type of moral harassment before; ii) 55% of participants are on the lower scores of the Resilience Scale: u201cVery lowu201d and u201cLowu201d; iii) 63% of students had non-normal scores at PHQ-4 tracking; iv) 49% of students had suggestive levels of depression, 19% suggestive levels of anxiety, and 19% suggestive levels of depression and anxiety at PHQ-4 tracking. Conclusions: These data provides some worrying notes: while the moments of moral harassment, depression and anxiety have high levels among the students, the resilience, a protective competence, have low levels. This reinforces the need for the implementation of preventive-interventive strategies about medical education.

A system of high performance liquid chromatography (HPLC) was used for the development and validation of efficient method for quantitative determination of three aminoacids involved in the inherited metabolic disease Branched-Chain Ketoaciduria (BCK), also called maple syrup urine disease. The analytical conditions were selected in order to obtain baseline separation profiles of the amino acids known to be altered in blood plasma of BCK patients, namely L-valine, L-isoleucine, and L-leucine. Most accurate data were obtained using HPLC/diode detector. As the analytes do not have chromophore groups, they were pre-derivatized with o-phthaldialdehyde (OPA), yielding an unsaturated adduct, making thus possible the detection of amino acids. The validation was conducted according to National Health Surveillance Agency (ANVISA) and Guidance for Industry (Bioanalytical Method Validation) United States Food and Drug Administration (U.S. FDA). The results were satisfactory, with high sensitivity, good linearity, precision and accuracy, limit of detection and quantification, all within the established parameters for bioanalytical methods, showing its applicability and low cost compared to other existing techniques such as sequential mass spectrometry. For the three amino acids, L-valine, L-isoleucine and L-leucine, the detection limits (LOD) found were: 1.61, 1.84 and 1.88 mmol L- 1 and the quantification limits (LOQ) 4.37, 6.13 and 6.27 mmol L- 1, respectively. Neste trabalho, um sistema de cromatografia líquida de alta eficiência (HPLC) foi usado para desenvolver e validar um eficiente método para determinar quantitativamente aminoácidos envolvidos na desordem rara conhecida como cetonúria de aminoácidos de cadeia ramificada ou doença do xarope de bordo. As condições analíticas foram desenvolvidas para obter os perfis dos aminoácidos de L-valina, L-isoleucina e L-leucina, sabidamente alterados no plasma sanguíneo dos pacientes. Empregou-se HPLC provido de um detector com arranjo de diodo. Os analitos não possuem grupos cromóforos e, por isso, foram pré-derivatizados com o-ftalaldeído (OPA) para tornar possível sua detecção. A validação foi conduzida de acordo com as normas da Agência Nacional de Vigilância Sanitária (ANVISA) (RDC No. 27, de 17 de maio de 2012) e secção de validação de bioanalítica da United States Food and Drug Administration (U.S. FDA). Os resultados foram satisfatórios, apresentando alta sensibilidade, boa linearidade, precisão e exatidão, limite de detecção e quantificação, todos parâmetros estabelecidos para métodos bioanalíticos, demonstrando a aplicabilidade e baixo custo do método comparado com outras técnicas como espectrometria de massas. Para os três aminoácidos, L-valina, L-isoleucina e L-leucina, os limites de detecção encontrados foram: 1,61, 1,84 e 1,88 mmol L–1 e limites de quantificação 4,37, 6,13 e 6,27 mmol L–1, respectivamente.

OBJETIVO: Avaliar a prevalência de infecção por Chlamydia trachomatis e Neisseria gonorrhoeae em mulheres submetidas à reprodução assistida em um serviço público de referência da região Centro-Oeste do Brasil.MÉTODOS: Estudo transversal com 340 mulheres com idade entre 20 e 47 anos, histórico de infertilidade, submetidas às técnicas de reprodução assistida. Foram analisadas as infecções por Chlamydia trachomatis e Neisseria gonorrhoeae detectadas em amostras de urina pela técnica de PCR e o perfil da infertilidade. Utilizou-se o teste do χ2 ou o teste exato de Fisher para avaliar a associação entre a infecção e as variáveis.RESULTADOS: Observou-se prevalência de 10,9% das mulheres com infecção por Chlamydia trachomatis, sendo que houve coinfecção por Neisseria gonorrhoeaeem 2 casos. Estima-se que entre 60 e 80 milhões de pessoas em todo o mundo em algum momento de suas vidas enfrentem dificuldades para se reproduzirem e calcula-se que este índice atinja aproximadamente 20% dos casais em idade reprodutiva. A gonorréia é uma infecção bacteriana frequente, causada por Neisseria gonorrhoeae (NG), transmitida quase que exclusivamente por contato sexual ou perianal. Nas mulheres a infecção genital pode ser assintomática e quando se descobre pode haver complicações sérias como a doença inflamatória pélvica (DIP), uma das principais causas da infertilidade feminina e da gravidez ectópica. As técnicas diagnósticas moleculares são grandes aliadas no rastreamento de NG e outras doenças sexualmente transmissíveis (DST). Objetivos: realizar uma investigação na literatura científica no formato de uma Revisão Sistemática de Literatura (RSL) sobre a prevalência de NG em mulheres assintomáticas, observando a faixa etária, as técnicas moleculares diagnósticas e as amostras utilizadas na detecção dessa bactéria; e, uma revisão de prontuários dos últimos 4 anos para detectar a prevalência da infecção gonocócica entre outros diagnósticos como fator causal da infertilidade em mulheres atendidas no Laboratório de Reprodução Humana – Hospital das Clínicas/UFG, Goiânia, Goiás. Na RSL foram encontrados 720 artigos dentre os quais 694 foram excluídos por critérios previamente estabelecidos e 26 incluídos nesta revisão. A população predominante foi de jovens 7 (27%) e mulheres profissionais do sexo 6 (23%). A idade variou de 12 a 59 anos. O material biológico mais frequente foi esfregaço endocervical 10 (38,46%), esfregaço vaginal 8 (30,76%), urina 4(15,38%) e a associação dos mesmos 3(11,53%). A técnica PCR convencional foi a técnica de escolha em 12 estudos para diagnóstico de NG. Nos outros 14 estudos foram utilizados distintos kits diagnósticos comerciais. Cada vez mais se observa o uso de amostras biológicas coletadas de forma menos invasiva para o diagnóstico de DST. As técnicas moleculares encontradas no presente estudo são as mais empregadas em pacientes assintomáticos, pois apresentam maior eficiência na detecção de NG, cujo padrão ouro só se aplica em pacientes sintomáticos. Na revisão de prontuários entre as 340 mulheres estudadas, a prevalência de NG de 0,6% e coinfecção com Chlamydia trachomatis (CT) foi 0,6%. DIP foi presente em 2,1% dos casos estudados sendo 2 casos por coinfecção de NG e CT e a média de idade foi 33 anos. As infecções por CT e NG tiveram baixa prevalência na amostra estudada e são necessários estudos em outros centros de reprodução do país para confirmar a prevalência de infecções nesse grupo particular de mulheres com problemas de infertilidade. It is estimated that between 60 and 80 million people worldwide enfretem difficulties to reproduce at some point in their lives and it is estimated that this ratio reaches approximately 20% of couples of reproductive age. Gonorrhea is a common bacterial infection caused by Neisseria gonorrhoeae (NG), transmitted almost exclusively by sexual or perianal contact. In women genital infection can be asymptomatic and when it discovers there may be serious complications such as pelvic inflammatory disease, and ectopic pregnancy, a leading cause of female infertility. The molecular diagnostic techniques are powerful allies in the tracking of NG and other sexually transmitted diseases (STD). Therefore, an investigation was conducted in the scientific literature in the form of a Systematic Literature Review (SLR) on the prevalence of NG, molecular diagnostic techniques and the samples used in the detection of this bacterium. Simultaneously, we developed a chart review in the last 4 years to detect the prevalence of gonococcal infection among other diagnoses as the cause of infertility in women seen at the Laboratory of Human Reproduction - Clinical Hospital, Goiânia, Goiás. SLR was found in 720 articles of which 694 were excluded due to previously established criteria and 26 were included in this review. The population was predominantly young 7 (27%) and female sex workers 6 (23%). The age ranged from 12 to 59 years. The more frequent biological material was endocervical swab 10 (38.46%), vaginal swabs 8 (30.76%), urine 4 (15.38%) and the association of these 3 (11.53%). The in-house PCR was the choice of 12 trials for the diagnosis of NG. In the other 14 studies different commercial diagnostic kits were used. Gradually more it is observed the use of biological samples collected from a less invasive diagnosis of STD. Molecular techniques found in this study are the most used in asymptomatic patients, since they have greater efficiency in the detection of NG, whose gold standard applies only in symptomatic patients. In reviewing the records among the 340 women studied, the prevalence of NG 0.6% and co-infection with Chlamydia trachomatis (CT) was 0.6%. DIP was present in 2.1% of cases and 2 cases of coinfection of NG and CT and the mean age was 33 years. The CT and NG infections had low prevalence in the studied sample and studies are needed in other breeding centers in the country to confirm the prevalence of infections in this particular group of women with infertility problems. Orientador: Marcelo Nicolas Muscari Dissertação (mestrado) - Universidade Estadual de Campinas, Faculdade de Ciencias Medicas Resumo: A esclerose múltipla (EM) é uma doença inflamatória crônica do sistema nervoso central (SNC), caracterizada pela infiltração de células do sistema imune neste compartimento, com destruição de mielina e perda de oligodendrócitos. O emprego do modelo experimental, a encefalomielite autoimune (alérgica) experimental (EAE), tem trazido grandes contribuições para a compreensão dos mecanismos envolvidos e o esclarecimento da patogênese de doenças autoimunes, tais como a EM. Apesar do modelo EAE ter sido estabelecido em 1940, os únicos (e últimos) trabalhos nos quais foi estudada a resposta cardiovascular destes animais foram publicados entre 1966 e 1972 (quando ainda não se tinha conhecimento da ocorrência do NO em sistemas biológicos). Atualmente sabemos que há alterações na produção de NO na EAE e da importância deste mediador no controle do tônus vascular. Os objetivos deste estudo foram investigar a reatividade vascular in vitro de aorta e artéria pulmonar provenientes de ratos portadores de EAE; bem como verificar a importância da produção endógena de NO, assim como a participação de produtos derivados da ciclooxigenase sobre a resposta vascular nos animais portadores da EAE e avaliar a evolução temporal de parâmetros hemodinâmicos (pressão arterial média e freqüência cardíaca), ao longo do desenvolvimento da doença EAE. O tratamento de ratos com o inibidor de iNOS aminoguanidina (AG) ou com o inibidor de COX-2 etoricoxibe (ETO) resultou em retardo do aparecimento dos sinais clínicos da EAE, sem modificar o perfil de perda ponderal que se segue à imunização com o antígeno de mielina. Não há diferenças significativas nos parâmetros hemodinâmicos, mesmo quando tratados com AG ou ETO. Tanto anéis de aorta como de artéria pulmonar de animais com EAE mostraram uma resposta diminuída a agonistas adrenérgicos vasoconstritores, os quais de alguma forma foram revertidas quando tratados com AG. Anéis de aorta de animais com EAE mostraram uma resposta aumentada à acetilcolina (ACh), a qual pode ser revertida pelo tratamento com AG ou ETO. Por outro lado as respostas dos tecidos de animais controle ou com EAE ao nitroprussiato de sódio (SNP) não foram diferentes. As ações vasorelaxantes do isoproterenol (ISO) não foram diferentes nos anéis de aorta provenientes dos diversos grupos experimentais, apesar da aparente maior potência do agonista nos tecidos dos animais com EAE, e a reversão pelo tratamento com AG. Contrariamente ao observado nos anéis de aorta, a resposta a ACh mostrou-se muito deprimida nos anéis de artéria pulmonar de ratos com EAE em relação aos animais controle. Neste tecido, a resposta máxima parece normalizar-se nos animais com EAE tratados com AG ou ETO. Com base nestes resultados, nós propomos que: o NO (derivado da iNOS) e/ou prostanóides (derivados da COX-2) podem estar envolvidos nos primeiros sinais clínicos da EAE, porém, a participação destes mediadores parece não ser relevante no equilíbrio hemodinâmico geral; há alteração na regulação simpática nos animais portadores da EAE e o NO derivado de iNOS parece mediar esta resposta contrátil deficiente; a resposta vasodilatadora simpática mediada por receptores ß-adrenérgicos parece não estar substancialmente alterada; há ocorrência de resposta relaxante dependente de endotélio alterada na EAE; aorta e artéria pulmonar mostraram alterações em sentidos opostos que podem ser normalizados através da inibição de iNOS ou COX-2; os efeitos do NO derivado de iNOS ou dos prostanóides sintetizados pela COX-2 são dependentes da função, fisiologia e/ou tipo particular de leito vascular Abstract: Multiple sclerosis (MS) is a chronic inflammatory disease of the central nervous system (CNS), characterized by the infiltration of immune system cells in this compartment, with destruction of myelin and loss of oligodendrocites. Experimental autoimmune (or allergic) encephalomyelitis (EAE) is a useful animal model for the study of human multiple sclerosis that shows characteristics similar to those shown by patients with MS. Last works in which was studied the cardiovascular reply of EAE animals was published between 1966 and 1972 (when it was unknown the occurrence of Nitric Oxide (NO) in the biological systems). Currently we know that exist alterations in the production of NO in the EAE and your importance in the control of tonus vascular. The purpose of the present study is to investigation of vascular reactivity in vitro of aorta and pulmonary artery from rats with EAE; as well as verifying the importance of the endogenous production, the participation of derived products from cyclooxygenase on the vascular response in the animals with EAE and evaluating hemodynamic parameters (average arterial pressure and cardiac frequency), to the long one of the development of disease EAE. The treatment of rats with aminoguanidine (AG) inhibitor of iNOS or with etoricoxibe (ETO) inhibitor of Cox-2, resulted in retarding showing of the clinical signals of the EAE, without modifying the profile that follows to the immunization with the myelin antigen. There is not significant differences in the hemodynamic parameters, when treated with AG or ETO. The rings of aorta, pulmonary artery from animals with EAE has a diminished response for adrenergic vasoconstrictor agonists, the response to revert when treated with AG. Rings of aorta from animals with EAE had shown a increase response to acetilcolina (ACh), which can to be revert by the treatment with AG or ETO. On the other hand the responses of blood vessel of animal control or with EAE to the nitroprussiato of sodium (SNP) had not differences. The vasodilation actions of isoproterenol (ISO) had not differences significant in rings of aorta from the diverse experimental groups, despite apparent potency and to revert the response of vasodilation when treated with AG. In contrast to the observed one in rings of aorta, the response for ACh it was decreased in rings of pulmonary artery of rats with EAE, when compared to the control animals. In this blood vessel, the maximum reply it seems to be normalized in the animals with EAE when treated with AG or ETO. These results suggest that: the nitric oxide (derived from iNOS) and/or prostanoids (derived from the Cox-2) can be involved in the first clinical signals of the EAE, however, the participation of these mediators seems not to be important in the general hemodynamic equilibrium; there is alteration in the sympathetic regulation in the animals with EAE, NO derivative of iNOS seems to mediate this deficient contrátil response; the vasodilator response mediated by ß-adrenergic receptor it seems not to be substantially modified; the response of dependent relaxation of endothelium it was altered in the animals with EAE; aorta and pulmonary artery showed alterations in opposed directions that can be normalized through the inhibition of iNOS or Cox-2; the effects of NO derivative of iNOS or prostanoids synthesis for the Cox-2 are dependents of the function, physiology and/or particular type of blood vessel Mestrado Farmacologia Mestre em Farmacologia
