The School Health Program aims to build healthy schools. This qualitative case study aimed to identify and understand how professionals involved in the School Health Program behave in the integration based on their inclusion in the recommended activities. The consensus was reached with experts to develop a matrix of indicators after building the Program’s logical model, resulting in a semi-structured roadmap. Twenty-five interviews were held in the Basic Health Units, Elementary Schools, and the Regional Health and Education Administrations. The practices were multisectoral, with no combined efforts and effective co-participation, planned by a single sector or professional, the existing gaps are recognized as much as the importance of the program, and the practices are implemented with a low bond between teams. The fragmented perception of students’ problems hinders this integration.

RESUMO O Programa Saúde na Escola tem como meta a construção de escolas saudáveis. Estudo de caso, de abordagem qualitativa com intenção de identificar e compreender como se comportam, no esforço de integração, profissionais envolvidos no Programa Saúde na Escola, a partir de sua inserção nas atividades preconizadas. Após construção do modelo lógico do programa, foi realizado consenso com experts para elaboração de matriz de indicadores, originando roteiro semiestruturado. Foram realizadas 25 entrevistas, nas Unidades Básicas de Saúde e Escolas de Ensino Fundamental, além das Gerências Regionais da Saúde e da Educação. As práticas foram consideradas multissetoriais, sem soma de esforços e coparticipação efetiva, sendo planejadas por um único setor ou profissional; os gaps existentes são reconhecidos tanto quanto a importância do programa; as práticas acontecem com baixo vínculo entre equipes. A forma fragmentada de perceber os problemas dos escolares dificulta essa integração. ABSTRACT The School Health Program aims to build healthy schools. Objective: To compare the performance of anthropometric indicators that identify excess body fat (BF) in adolescents. Methods: This is a methodological study that used probability cluster sampling through school and class draws. Data collection included sociodemographic characteristics and anthropometric measures. Body mass index (BMI), waist-to-height ratio (WtHR), conicity index (C index), and waist circumference (WC) were calculated. Body fat percentage (BF%) was calculated from skinfold thickness and used as the gold standard. To analyze the data, descriptive statistics, Student’s t-test, Receiver Operating Characteristic (ROC) curve, and Youden’s index were used, in addition to correlation coefficient calculation between the indicators and BF%. Results: A total of 997 adolescents enrolled in municipal secondary schools participated in the study. By calculating the BMI, we found that 10.6% of adolescents were overweight, and 4.7% were obese. BMI, WC, and WtHR had the highest accuracy to predict body fatness. All the anthropometric indicators had higher specificity than sensitivity to diagnose excess BF in males. WC had the highest sensitivity in both genders. C index had the smallest area under the ROC curve and the lowest sensitivity in both genders, but its specificity was equivalent to that of the other indicators. Conclusions: BMI, WtHR, and WC were the best anthropometric indicators to predict excess BF in adolescents and had the best correlation coefficients. These tools can be considered in the screening to detect excess BF in adolescents. RESUMO Objetivo: Comparar o desempenho de indicadores antropométricos para identificar o excesso de gordura corporal (GC) em adolescentes. Métodos: Estudo metodológico com amostragem probabilística por conglomerados, por sorteio das escolas e turmas. A coleta de dados incluiu características sociodemográficas e medidas antropométricas. O índice de massa corporal (IMC), a relação cintura-estatura (RCE), o índice de conicidade (IC) e a circunferência da cintura (CC) foram calculados. O percentual de gordura corporal (%GC) foi calculado com base na espessura das dobras cutâneas e utilizado como padrão-ouro. Para analisar os dados, foram utilizadas estatísticas descritivas, teste t de Student, curva Receiver Operating Characteristic (ROC) e índice de Youden. Foi calculado o coeficiente de correlação entre os indicadores e o %GC. Resultados: Participaram do estudo 997 adolescentes de escolas municipais. Identificou-se, pelo IMC, que 10,6% dos adolescentes apresentavam sobrepeso e 4,7%, obesidade. O IMC, a CC e a RCE apresentaram a maior acurácia para predizer a gordura corporal. Todos os indicadores antropométricos apresentaram especificidade superior à sensibilidade para diagnosticar excesso de GC no sexo masculino. A CC apresentou a maior sensibilidade em ambos os sexos. O IC apresentou a menor área sob a curva ROC e a menor sensibilidade em ambos os sexos, mas especificidade equivalente aos demais indicadores. Conclusões: IMC, RCE e CC apresentaram a melhor capacidade de predizer excesso de GC em adolescentes e os melhores coeficientes de correlação. Essas ferramentas podem ser consideradas formas de rastreamento na identificação do excesso de GC em adolescentes.

The economic evaluation has gained popularity in recent years but, to be useful, it must be conducted and reported accurately. The aim of this study was to perform a mapping review identifying published articles about economic evaluation of school health programmes, characterizing and qualifying what has been published, and discussing how these studies were conducted. The review was carried out in PubMed for studies published in the past decade. The search strategy included "cost", "economic evaluation", "school health services", "school health", "school health promotion" and "school health program”. Thirty-five studies were assessed. The studies addressed ten different countries and cost-effectiveness was the most economic evaluation used. The narrative synthesis gathered the selected studies in nine groups, demonstrating a high heterogeneity of methods and results in the economic analyzes performed. There is evidence that school health programmes can bring benefits to the target population and society. The gathered information in this article can contribute to performing economic evaluation studies and improving programmes

Aim: Although bulk fill composites have been widely used as restorative material, there is no consensus regarding the best clinical protocol in terms of composite technique and adhesive system. Therefore, this clinical trial evaluated the clinical performance of bulk fill composites for class I restorations under different protocols. Methods: A randomized clinical trial including 155 class I restorations was conducted using different adhesive systems: conventional technique (phosphoric acid + conventional three-step adhesive system) (Group 1, 2 and 3); or self-etching adhesive system (Groups 4, 5 and 6). Control groups 1 and 4 were restored with conventional composite; groups 2 and 5 with low viscosity bulk fill and conventional composite as occlusal coverage; groups 3 and 6 with high viscosity bulk fill. The FDI criteria was used for clinical evaluation at baseline and after 6 months. Results: All groups showed good clinical performance. At baseline, the adhesive system did not affect postoperative hypersensitivity. After 6 months, group 5 showed a significant reduction in color and translucency; group 6 a reduction in terms of anatomical form and for postoperative sensitivity and an improvement in patient satisfaction (p

Background: The prevalence of metabolic syndrome (MS) varies according to the different definitions available for clinical practice. The study aimed to estimate and compare the prevalence of MS according to the definitions of the American Heart Association (AHA)/National Heart, Lung, and Blood Institute (NHBI), International Diabetes Federation (IDF), and Joint Interim Statement (JIS) in a sample of climacteric women in Brazil. Methods: A population-based cross-sectional study, using a probabilistic sampling by clusters, involved 874 climacteric women. Clinical and anthropometric variables were acquired while fasting, and MS was defined according to three recommendations. To define the agreement between different definitions on MS, we calculated the sensitivity and specificity for the diagnosis of MS based on AHA/NHBI and IDF in relationship to the JIS definition. Results: The prevalence of MS was 56.9% according to the AHA/NHBI definition, 61.6% by the IDF, and 64.8% by the JIS definition, without statistical significance. When prevalence of MS was stratified by climacteric stages, the ability of the different definitions identifying MS was also similar among groups. It was observed that the MS definition proposed by AHA/NHBI identified 87.4% of MS cases from the total cases diagnosed by JIS, with agreement levels by the Kappa index of 0.835. The IDF detected 95.3% of MS cases compared to the JIS definition and the agreement level was 0.934. Conclusions: The study indicates a high prevalence of MS in climacteric women regardless of the definition used (AHA/NHBI, IDF e JIS). We did not observe significant divergences between definitions.

O Programa Saúde na Escola é uma política intersetorial, resultado do trabalho entre o Ministério da Saúde e o Ministério da Educação, cuja finalidade é ampliar as ações de saúde aos escolares da rede pública. As escolas participantes devem incluir no projeto político pedagógico escolar os temas das atividades em saúde desenvolvidas no Programa, de modo a atender às expectativas dos professores e escolares. Os temas são abordados para facilitar o aprendizado e devem ser adequados às fases do desenvolvimento do escolar. Estudo qualitativo, realizado em 2016, com o objetivo de investigar as percepções dos escolares em relação às atividades desenvolvidas pelo Programa Saúde na Escola em Belo Horizonte, Brasil. A coleta de dados foi realizada utilizado desenho acompanhado de narrativa oral, com participantes na faixa etária de seis a dez anos. Foi possível demonstrar que os escolares puderam receber informações e compreendê-las, a ponto de poder reproduzi-las.

This study aimed to evaluate the validity and precision of the International Physical Activity Questionnaire (IPAQ) for climacteric women using computational intelligence techniques. The instrument was applied to 873 women aged between 40 and 65 years. Considering the proposal to regroup the set of data related to the level of physical activity of climacteric women using the IPAQ, we used 2 algorithms: Kohonen and k-means, and, to evaluate the validity of these clusters, 3 indexes were used: Silhouette, PBM and Dunn. The questionnaire was tested for validity (factor analysis) and precision (Cronbach's alpha). The Random Forests technique was used to assess the importance of the variables that make up the IPAQ. To classify these variables, we used 3 algorithms: Suport Vector Machine, Artificial Neural Network and Decision Tree. The results of the tests to evaluate the clusters suggested that what is recommended for IPAQ, when applied to climacteric women, is to categorize the results into two groups. The factor analysis resulted in three factors, with factor 1 being composed of variables 3 to 6; factor 2 for variables 7 and 8; and factor 3 for variables 1 and 2. Regarding the reliability estimate, the results of the standardized Cronbach's alpha test showed values between 0.63 to 0.85, being considered acceptable for the construction of the construct. In the test of importance of the variables that make up the instrument, the results showed that variables 1 and 8 presented a lesser degree of importance and by the analysis of Accuracy, Recall, Precision and area under the ROC curve, there was no variation when the results were analyzed with all IPAQ variables but variables 1 and 8. Through this analysis, we concluded that the IPAQ, short version, has adequate measurement properties for the investigated population.

Este artigo descreve a vivência de estudantes de medicina em intervenções promotoras do envelhecimento saudável em uma instituição de longa permanência (ILPI) para idosos em Minas Gerais. Trata-se de relato de experiência sobre intervenções para promoção da saúde e implementação de estratégias mais efetivas junto a idosas residentes em uma ILPI. Realizou-se um diagnóstico situacional por estimativa rápida, mediado pela observação e escuta, para levantar os principais problemas do serviço. As ações desenvolvidas envolveram grupos de musicoterapia, atividades cognitivas, caminhada e ioga/otago e ocorreram em 2017 e 2018. Constatou-se adesão das idosas às atividades propostas. Vale destacar que as atividades cognitivas proporcionaram aumento da capacidade de socialização. O trabalho em equipe e a sensibilidade para ouvir e cativar o outro, além da exposição a vulnerabilidades das mais diversas formas são algumas das reflexões proporcionadas por essa experiência. As atividades demonstraram potencial para desenvolver as habilidades sociais e motoras das idosas residentes na instituição em estudo, contribuindo para um envelhecimento mais ativo.

A coleta de dados foi realizada com escolares de onze a quatorze anos, e, como instrumento de coleta, foi utilizado o relato escrito (redação). A partir da análise dos relatos, foram identificados dois temas: 1. O Programa Saúde na Escola como cuidado, e 2. O Programa Saúde na Escola como benesse? Para alguns, as atividades significam a possibilidade do cuidado na área de saúde, para identificar os problemas e encaminhar as soluções, ter a possibilidade de ter informações e algum aprendizado que poderá ser benéfico na manutenção da saúde. No entanto, para uns, foi uma oferta recebida passivamente. Para outros uma benesse, um favor, para o qual mostram a sua gratidão. Os escolares se mostraram passivos e receptivos sem, no entanto, o protagonismo necessário à produção da própria saúde. A corresponsabilização parece ser uma semente ainda germinando. This qualitative study was carried out in 2016 to investigate Brazilian schoolchildren’s perceptions of activities under the Health at School Programme. Data were collected from schoolchildren eleven to fourteen years of age using the written report (essay) as the instrument. Analysis of the reports highlighted two themes: 1. the Health at School Programme as care, and 2. the Health at School Programme as benevolence. For some, the activities meant the possibility of health care, of identifying problems and arranging solutions, and the opportunity for gaining information and learning that may be beneficial in maintaining health – the offer was received passively, however; for others, they were a blessing, a favour, for which they showed their gratitude. The schoolchildren were passive and receptive, without being proactive enough to produce their own health. Co-responsibility seems to be a seed that is still germinating.

This study aims to investigate the prevalence and factors associated with abdominal obesity in climacteric women assisted at Family Health Strategy units of the city of Montes Claros, State of Minas Gerais, Brazil. It is a cross-sectional analytical study. The women were selected by probabilistic sampling from August 2014 to August 2015. A questionnaire containing information referring to sociodemographic and economic characteristics, behavioral characteristics and clinical data was used. To estimate abdominal obesity, the measure of circumference ≥ 88 cm was considered. To analyze the association between abdominal obesity and the independent variables, a bivariate analysis was performed by means of Pearson’s chi-square test (p≤0.25). Este estudo teve por objetivo identificar as políticas públicas de diagnóstico precoce e rastreamento do câncer bucal no Brasil. Trata-se de revisão integrativa da literatura realizada nas bases Scientific Electronic Library Online (SciELO), Literatura Latino-Americana e do Caribe em Ciências da Saúde (LILACS) e Biblioteca Virtual em Saúde (BVS). Realizou-se cruzamento dos seguintes descritores: a) Sistema Único de Saúde (SUS); b) atenção primária em saúde; c) políticas públicas de saúde; d) programas de rastreamento; e) detecção precoce do câncer; e f) câncer bucal. Foram incluídos estudos nos idiomas português e inglês, publicados no período de 2008 a 2018, disponíveis em texto integral e acesso livre e gratuito. Os 12 artigos selecionados possibilitaram a identificação de 5 estratégias de prevenção para detecção precoce e rastreamento do câncer bucal: a) exame visual; b) levantamento da população de risco; c) autoexame bucal; d) vigilância em saúde; e e) fluxos assistenciais fortes. Constatou-se a necessidade de qualificar os profissionais da saúde para o atendimento, além de aumentar a oferta de serviços odontológicos de qualidade.

This study aims to investigate the association between health conditions and overweight in climacteric women assisted by primary care professionals. It is a cross-sectional study conducted with 874 women from 40 to 65 years of age, selected by probabilistic sampling between August 2014 and August 2015. In addition to the outcome variable, other variables such as overweight/obesity, sociodemographic, reproductive, clinical, eating and behavioural factors were evaluated. Descriptive analyses of the variables investigated through their frequency distributions were performed. Then, bivariate analyses were performed through Poisson regression. For the multiple analyses, the hierarchical Poisson regression was used to identify factors associated with overweight/obesity in the climacteric period. The prevalence of overweight/obesity was 74%. Attending public school (PR: 1.30 - 95% CI 1.14 - 1.50), low schooling (PR: 1.11 - 95% CI 1.01 - 1.23), gout (PR: 1.18 - 95% CI 1.16-1.44), kidney disease (PR: 1.18 - 95% CI 1.05 - 1.32), metabolic syndrome - MS (PR: 1.19 - 95% CI 1.05 - 1.34) and fat intake (PR: 1.12 - 95% CI 1.02 - 1.23) were considered risk factors for overweight. Having the first birth after 18 years (PR: 0.89 - 95% CI 0.82 to 0.97) was shown to be a protective factor for overweight and obesity. The presence of overweight and obesity is associated with socio-demographic, reproductive, clinical and eating habits.

Background Health education actions are strategic actions aimed at health promotion. Moreover, better health education practices have been linked to comprehensive care developed by primary health services. Aim To understand the perception of mothers about the health education developed by the Primary Health Care (PHC) services. Design We adopted a qualitative research approach, conducted using semi-structured interviews, and assuming, as the theoretical reference, the attributes derived from PHC. Pregnant women and mothers of children under 1 year old attended by Brazilian Family Health teams were included. Results Eight pregnant women and twelve mothers of children under 1 year of age participated in the study. These mothers perceive educational actions as necessary only for primiparous mothers. They value the guidance of informal networks and maternity hospitals to the detriment of primary services, and they do not adhere to care that conflicts with their previous knowledge. The evidence shows that the educational actions of the primary health services do not adequately respond to the attributes of community orientation, family focus, and cultural competence. Conclusion It is necessary to modify the educational practices in order to incorporate and value the existing knowledge in the territory, thereby adapting the care guidelines to the local context.

O Programa Saúde na Escola (PSE), política intersetorial da saúde e educação tem como meta a construção de escolas saudáveis. O objetivo deste estudo foi realizar uma avaliação do Programa Saúde na Escola (PSE). Foi realizado um estudo de caso em Belo Horizonte em duas Escolas de Ensino Fundamental (EEF) e Unidades Básicas de Saúde (UBS) de referência, e desenvolvido em cinco etapas: 1. Análise estratégica por meio de um estudo de avaliabilidade do PSE, baseado nas seis linhas de orientação sobre o processo em que se deve pautar esse tipo de estudo e utilizando os parâmetros de qualidade do Joint Committee on Standards for Educational Evaluation (JCEE). O PSE em Belo Horizonte foi considerado um programa avaliável; 2. Mapeamento da literatura sobre avaliações econômicas de programas de saúde escolar, realizado no PubMed/Medline para estudos publicados na última década, utilizando como estratégia de busca os termos "cost", "economic evaluation", "school health services", "school health", "school health promotion" e "school health program. Os resultados, após a análise de 35 artigos mostraram uma relação positiva entre os programas de saúde escolar, custos e benefícios para os escolares e as comunidades; 3. Avaliação normativa do programa considerando dados secundários, avaliando a frequência com que as atividades foram realizadas. A análise apresentou desempenho medio em avaliações de saúde (visual, 54,78%; saúde bucal, 53,20%; situação vacinal, 55%; avaliação antropométrica, 83,32%). Ações de prevenção de agravos mais realizadas pelas escolas foram combate a dengue e vacinação contra o Papiloma Vírus Humano, assim como, ações de promoção de saúde relacionadas à segurança alimentar e promoção de alimentação saudável. Não foram encontrados registros de capacitações realizadas aos profissionais da saúde e da educação. Algumas ações de prevenção de agravos que parecem ser de domínio dos executores foram feitas com maior frequência. As ações de promoção da saúde apresentaram os piores resultados apesar de sua importância. Os sistemas de monitoramento possuem falta de regularidade de alimentação e a falta de capacitação para o preenchimento; 4. Avaliação do contexto por meio da análise com foco na integração entre a saúde e a educação no PSE, um estudo de caso, com abordagem qualitativa, que se iniciou com uma matriz de indicadores, a partir do modelo teórico e lógico desenvolvido no estudo de avaliabilidade, que foi consensuada por experts utilizando a Técnica Delphi, possibilitando a contrução de um roteiro de entrevista semiestruturado. Como resultado, o PSE em Belo Horizonte apresentou uma característica multissetorial, onde a maioria das ações e serviços prestados foi planejada por um único setor e aplicados junto aos outros, com baixo ou nenhum vínculo entre as equipes, sendo as ações pontuais, pouco efetivas e sem sustentabilidade; 5. Avaliação da efetividade por meio da análise da percepção dos estudantes sobre o PSE, estudo qualitativo, com coleta de dados realizada com escolares de onze a quatorze anos de idade e como instrumento de coleta foi utilizado narrativa escrita. A partir da análise das narrativas escritas foram identificados três temas: Facilidade de atendimento e cuidado; Aprendizagem e cuidado; e Bondade, gratuidade ou direito? Os escolares perceberam as atividades assistenciais desenvolvidas pelo programa, mas o mesmo não ocorreu com as atividades preventivas e de promoção de saúde. O programa é percebido como ajuda e favor, sujeito a gratidão. Os escolares se mostraram passivos e receptivos sem, no entanto, o protagonismo necessário à produção da própria saúde. A corresponsabilização parece ser uma semente ainda germinando. The School Health Program (SHP), an intersectoral health and education policy, aims to build healthy schools. The aim of this study was to carry out an evaluation of the School Health Program. The case study was carried out in Belo Horizonte in two Elementery Schools (ES) and Basic Unit Health (BUH), and was developed in five stages: 1. Strategic analysis through a SHP Evaluatability Study, based on the six guidelines on the process in which this type of study should be guided and using the quality parameters of the Joint Committee on Standards for Educational Evaluation (JCEE). The SHP in Belo Horizonte was considered an evaluable program; 2. Mapping of the literature on economic evaluations of school health programs, carried out in PubMed / Medline for studies published in the last decade, using as search strategy the terms "cost", "economic evaluation", "school health services", "school. The results, after the analysis of 35 articles, showed a positive relationship between school health programs, costs and benefits for schoolchildren and communities; 3. Normative considering secondary data, evaluating the frequency with which the activities were carried out. The analysis presented average performance in health assessments (visual, 54.78%, oral health, 53.20%, vaccine situation, 55%, anthropometric evaluation, 83.32%). Actions to prevent more serious diseases in schools have been to combat dengue and vaccination against human papillomavirus, as well as health promotion actions related to food safety and promotion of healthy eating. No education records were found for health and education professionals. Some actions to prevent injuries that appear to be in the hands of the executors were made more frequently. The actions of health promotion presented the worst results despite their importance. Monitoring systems lack proper regularity of food and capacity to fulfilneut; 4. Evaluation of the context through the analysis focusing on the integration between health and education in the SHP, a case study with a qualitative approach, which used a matrix of indicators, based on the theoretical and logical model developed in the study of assessability, which was agreed by experts using the Delphi Technique, enabling the construction of a semi-structured interview script. As a result, the SHP in Belo Horizonte presented a multisectoral characteristic, in which most of the actions and services provided were planned by a single sector and applied to others, with low or no link between the teams, being the punctual actions not very effective and without sustainability; 5. Evaluation of effectiveness through the student perceptions about the SHP, qualitative study, with data collection carried out with schoolchildren from eleven to fourteen years old and as instrument of data collection was used written narrative. From the analysis of written narratives, three themes were identified: Ease of care and care; Learning and care; and Kindness, gratuity and help?. The schoolchildren perceived the assistance activities developed by the program, but the same did not occur with preventive and health promotion activities. The program is perceived as help and favor, subject to gratitude. The schoolchildren were passive and receptive without, however, the protagonism necessary to the production of their own health. Co-responsibility seems to be a seed still germinating.
