INTRODUCTION: Copy-number variations (CNVs) impacting on small DNA stretches and associated with language deficits provide a unique window to the role played by specific genes in language function. METHODS: We report in detail on the cognitive, language, and genetic features of a girl bearing a small deletion (0.186 Mb) in the 2p16.3 region, arr[hg19] 2p16.3(50761778_50947729)×1, affecting exons 3–7 of NRXN1, a neurexin-coding gene previously related to schizophrenia, autism (ASD), attention deficit hyperactivity disorder (ADHD), mood disorder, and intellectual disability (ID). RESULTS: The proband exhibits many of the features commonly found in subjects with deletions of NRXN1, like ASD-like traits (including ritualized behaviors, disordered sensory aspects, social disturbances, and impaired theory of mind), ADHD symptoms, moderate ID, and impaired speech and language. Regarding this latter aspect, we observed altered speech production, underdeveloped phonological awareness, minimal syntax, serious shortage of active vocabulary, impaired receptive language, and inappropriate pragmatic behavior (including lack of metapragmatic awareness and communicative use of gaze). Microarray analyses point to the dysregulation of several genes important for language function in the girl compared to her healthy parents. DISCUSSION: Although some basic cognitive deficit − such as the impairment of executive function − might contribute to the language problems exhibited by the proband, molecular evidence suggests that they might result, to a great extent, from the abnormal expression of genes directly related to language.

Las cromosomopatías consisten en duplicaciones o deleciones de fragmentos cromosómicos que suelen conllevar alteraciones del lenguaje. Debido a su baja prevalencia, no se dispone aún de descripciones precisas del déficit lingüístico que llevan asociado ni de caracterizaciones funcionales del lenguaje de los afectados. En este trabajo proponemos una metodología pragmático-funcional para la valoración del déficit lingüístico en niños con cromosomopatías de baja prevalencia, que vincula las alteraciones detectadas en los componentes estructurales del lenguaje con las disfunciones comunicativas que generan. Discutimos la problemática que plantea la transcripción y codificación de la muestra con las herramientas del proyecto CHILDES y su etiquetado con PREP-CORP, ejemplificando nuestras reflexiones con datos del corpus CHROMOLANG. Concluimos que el análisis pragmático de muestras de lenguaje oral no elicitado complementa la evaluación mediante pruebas estandarizadas, contribuye al diagnóstico y permite orientar la intervención logopédica hacia el éxito comunicativo., Chromosomopathies consist of duplications or deletions of chromosomal fragments that usually produce language impairment. However, accurate descriptions of structural and functional linguistic deficits caused by chromosomopaties are not yet available due to the low prevalence of these alterations. Our aim in this paper is to put forward a pragmatic methodology in order to assess linguistic impairment in children with low prevalence chromosomopathies. This method links structural linguistic deficits together with the pragmatic dysfunctions they cause in communication. We discuss the transcription and coding of the data with the CHILDES tools and its labeling with PREP-CORP, and we illustrate our reflections with examples from the CHROMOLANG corpus. We conclude that the pragmatic analysis of natural oral language data complements the traditional assessment through standardized tests, contributes to diagnosis, and allows speech-language clinicians to focus on strategies to achieve communication success during the intervention., Les anomalies chromosomiques consistent en des duplications ou des délétions de fragments chromosomiques impliquant généralement des altérations du langage. En raison de sa faible prévalence, il n’existe toujours pas une description précise du déficit linguistique des individus affectés ni de caractérisation fonctionnelle de leur langage. Dans cet article, nous proposons une méthodologie pragmatique et fonctionnelle pour l’évaluation du déficit linguistique chez les enfants atteints d’anomalies chromosomiques à faible prévalence, qui relie les altérations détectées dans les composants structurels du langage avec les dysfonctionnements de la communication. Nous discutons les problèmes posés par la transcription et le codage de l'échantillon avec les outils du projet CHILDES et son étiquetage avec PREP-CORP, illustrant nos réflexions avec les données du corpus CHROMOLANG. Nous concluons que l'analyse pragmatique d'échantillons de langage oral naturel complète l'évaluation au moyen de tests standardisés, contribue au diagnostic et permet d'orienter l'intervention en orthophonie vers le succès de la communication.

En este trabajo se ofrece una adaptación del formato de transcripción y codificación CHAT (Codes for the Human Analysis of Transcripts) que ha sido específicamente desarrollada para posibilitar el trasvase del corpus PerLA de lenguaje patológico a la base de datos internacional TALKBANK. Para ello, se requiere la adopción de un formato estandarizado de transcripción y codificación compatible con el software CLAN (Computerized Language ANalysis) de análisis automatizado de lenguaje. Este formato admite, sin embargo, un grado considerable de variación en la elección de las convenciones empleadas, en función de la naturaleza de los datos, la perspectiva del estudio para el que el corpus haya sido elaborado, y los fenómenos que se considere relevante codificar. La adaptación que ofrecemos persigue resultar de utilidad para la codificación de muestras de lenguaje que reflejan un rango considerablemente amplio de déficits lingüísticos, con el fin de realizar estudios de naturaleza pragmática. The aim of this paper is to describe a version of the CHAT (Codes for the Human Analysis of Transcripts) transcription format developed with the purpose of enabling open-access to the PerLA corpus of language pathologies through the Talkbank International Database. In doing so, adapting PerLA to a transcription and coding format compatible with CLAN (Computerized Language ANalysis) software for automatic language analysis was required. However, CHAT conventions are not compulsory, but they allow for a wide range of variations depending on the nature of data, the scope of the study for which the data have been collected, and the kind of phenomena that are considered relevant for coding. The adaptation offered here has been envisaged to allow the transcription and coding of data showing a wide variety of language and communication disorders, in order to carry out further studies from a pragmatic perspective.
