A search is presented for τ slepton pairs produced in proton–proton collisions at a center-of-mass energy of 13TeV. The search is carried out in events containing two τ leptons in the final state, on the assumption that each τ slepton decays primarily to a τ lepton and a neutralino. Events are considered in which each τ lepton decays to one or more hadrons and a neutrino, or in which one of the τ leptons decays instead to an electron or a muon and two neutrinos. The data, collected with the CMS detector in 2016 and 2017, correspond to an integrated luminosity of 77.2fb⁻¹. The observed data are consistent with the standard model background expectation. The results are used to set 95% confidence level upper limits on the cross section for τ slepton pair production in various models for τ slepton masses between 90 and 200GeV and neutralino masses of 1, 10, and 20GeV. In the case of purely left-handed τ slepton production and decay to a τ lepton and a neutralino with a mass of 1GeV, the strongest limit is obtained for a τ slepton mass of 125GeV at a factor of 1.14 larger than the theoretical cross section.

Measurements are presented of the triple-differential cross section for inclusive isolated-photon+jet events in pp collisions at s√=8 TeV as a function of photon transverse momentum (pγT), photon pseudorapidity (ηγ), and jet pseudorapidity (ηjet). The data correspond to an integrated luminosity of 19.7fb−1 that probe a broad range of the available phase space, for |ηγ| 25GeV. The measurements are compared to next-to-leading order perturbative quantum chromodynamics calculations, which reproduce the data within uncertainties.

Humans have always been exposed to different types of natural and cosmic radiation. At present we experience 83 % natural radiation, for example from granite marbles, building materials such as concrete, brick, granite, or drywall. However, although these are potential sources of radon, they are rarely the cause of a high level of pollution in a building.Ashlar is a material of volcanic origin comprising heterogeneous and solidified detrital pyroclastic materials such as rhyolite, sand, volcanic ash and pumice, among others. Chemically, ashlar is principally composed of elements such as sodium, magnesium, calcium, iron, aluminum, sulfur, and cobalt. In order to identify the presence of these elements in the ashlar, X-ray Fluorescence studies were carried out first, using modern XRD and XRF equipment, followed by quantitative analysis using a neutron activation technique, and finally, low-level gamma spectrometry to evaluate the presence of radioactive elements.

In this article, we examine the state-of-the-art and current applications of artificial intelligence (AI), specifically for human resources (HR). We study whether, due to the experimental state of the algorithms used and the nature of training and test samples, a further control and auditing in the research community is necessary to guarantee fair and accurate results. In particular, we identify the positive and negative consequences of the usage of video-interview analysis via AI in recruiting processes as well as the main machine learning techniques used and their degrees of efficiency. We focus on some controversial characteristics that could lead to ethical and legal consequences for candidates, companies and states regarding discrimination in the job market (e.g. gender and race). There is a lack of regulation and a need for external and neutral auditing for the type of analyses done in interviews. We present a multi-agent architecture that aims at total legal compliance and more effective HR processes management.

Los estudiantes japoneses de español como lengua extranjera suelen confundir el nombre de los países con sus nacionalidades, así como el sufijo derivativo correcto para la formación de estas últimas. En este artículo se examinan estos errores analizando los factores intraléxicos que influyen en el aprendizaje de una palabra en relación con los adjetivos deonomásticos de lugar y se propone una secuencia didáctica que incluye varias actividades específicas de morfología para subsanar este problema. Asimismo, se presentan los resultados de una prueba que se realizó para comprobar el impacto de estas actividades. De los datos obtenidos de dicha prueba se concluye que la creación de una conciencia morfológica en los estudiantes japoneses de ELE de nivel inicial favorece el aprendizaje de los adjetivos gentilicios y la distinción entre estos y sus bases toponímicas.

Additionally, we determined whether the extent of RB-associated protein (pRb) expression is helpful in assessing the prognosis in RB patients. Methods: Conventional whole tissue section and tissue microarray immunohistochemistry for neuron-specific enolase, synaptophysin, and pRb were carried out in a series of 22 RBs. Results: Neuron-specific enolase and synaptophysin were expressed in 75%–100% of the tumor cells, and the staining intensity was strong. Two RBs expressed pRb in 75%–100% of the tumor cells, also with strong staining intensity. Concordance between the immunohistochemical outcomes for whole tissue staining and tissue microarray staining was 76.2% for neuron-specific enolase, 85.7% for synaptophysin, and 80.0% for pRb. Conclusion: Neuron-specific enolase and synaptophysin have the potential to be useful markers for the diagnosis of RBs. Extensive and strong pRb staining is not associated with less aggressive tumor behavior according to the pathologic classification of RBs. The CERN IT Storage Group ensures the symbiotic development and operations of storage and data transfer services for all CERN physics data, in particular the data generated by the four LHC experiments (ALICE, ATLAS, CMS and LHCb). In order to accomplish the objectives of the next run of the LHC (Run-3), the Storage Group has undertaken a thorough analysis of the experiments’ requirements, matching them to the appropriate storage and data transfer solutions, and undergoing a rigorous programme of testing to identify and solve any issues before the start of Run-3. In this paper, we present the main challenges presented by each of the four LHC experiments. We describe their workflows, in particular how they communicate with and use the key components provided by the Storage Group: the EOS disk storage system; its archival back-end, the CERN Tape Archive (CTA); and the File Transfer Service (FTS). We also describe the validation and commissioning tests that have been undertaken and challenges overcome: the ATLAS stress tests to push their DAQ system to its limits; the CMS migration from PhEDEx to Rucio, followed by large-scale tests between EOS and CTA with the new FTS “archive monitoring” feature; the LHCb Tier-0 to Tier-1 staging tests and XRootD Third Party Copy (TPC) validation; and the erasure coding performance in ALICE.

Cuando comenzó a percibirse las redes sociales como un factor importante que podía afectar al negocio empresarial muchas organizaciones la han tenido en cuenta para el desarrollo de su gestión. Esta utilización de las redes sociales es especialmente relevante en las empresas turísticas ya que dependen en gran medida de la opinión y la experiencia del cliente. Así, en este estudio se ha planteado conocer el uso y la importancia de las redes sociales en el sector hotelero a través de una investigación cualitativa mediante entrevistas a diez responsables de la gestión de redes sociales de establecimientos hoteleros. De esta manera se ha tenido en cuenta el marco teórico para obtener las categorías y realizar las preguntas de investigación. Posteriormente, mediante un análisis de contenido a través del software Nvivo 11, se obtuvo inductivamente subcategorías con características comunes que se dividieron en unidades de análisis mediante la codificación de los datos, facilitando así la interpretación de la información. En los resultados los gestores de redes sociales entrevistados destacaron la importancia que tienen los medios sociales en el sector hotelero para relacionarse e interactuar con los usuarios, de manera que consideran esencial el planteamiento de objetivos propios para este ámbito y el desarrollo de estrategias dependiendo de las redes sociales en las que vayan a intervenir. Así, las ventajas más destacadas son la visibilidad, la facilidad de acceder a la información y la posibilidad de tener una mejor comunicación, relación e interacción con los usuarios.

26. Managing the CERN Batch System with Kubernetes

The CERN Batch Service faces many challenges in order to get ready for the computing demands of future LHC runs. These challenges require that we look at all potential resources, assessing how efficiently we use them and that we explore different alternatives to exploit opportunistic resources in our infrastructure as well as outside of the CERN computing centre. Several projects, like BEER, Helix Nebula Science Cloud and the new OCRE project, have proven our ability to run batch workloads on a wide range of non-traditional resources. However, the challenge is not only to obtain the raw compute resources needed but how to define an operational model that is cost and time efficient, scalable and flexible enough to adapt to a heterogeneous infrastructure. In order to tackle both the provisioning and operational challenges it was decided to use Kubernetes. By using Kubernetes we benefit from a de-facto standard in containerised environments, available in nearly all cloud providers and surrounded by a vibrant ecosystem of open-source projects. Leveraging Kubernetes’ built-in functionality, and other open-source tools such as Helm, Terraform and GitLab CI, we have deployed a first cluster prototype which we discuss in detail. The effort has simplified many of the existing operational procedures we currently have, but has also made us rethink established procedures and assumptions that were only valid in a VM-based cloud environment. This contribution presents how we have adopted Kubernetes into the CERN Batch Service, the impact its adoption has in daily operations, a comparison on resource usage efficiency and the experience so far evolving our infrastructure towards this model.

Abstract The objective of this study was to search for polymorphisms in the coding region of the estrogen receptors 1 and 2 (ESR1 and ESR2 )and to analyze the effects of these variants and the well known intronic ESR1 Pvu II polymorphism on litter size in a Chinese-European pig line. We identified five silent single nucleotide polymorphisms (SNP) in the ESR1 cDNA: c.669T > C (exon 3), c.1227C > T (exon 5), c.1452C > T (exon 7), c.1665T > C and c.1755A > G (exon 8). One pair of these SNP (c.1665T > C and c.1755A > G) co-segregated in the analyzed line, and the SNP c.669T > C showed the same segregation pattern as the Pvu II polymorphism. These polymorphisms were tested in this study, although the c.1452C > T SNP within exon 7 was not analyzed due to its low informativeness. In the ESR2 cDNA, one missense SNP was found within exon 5, which caused an amino acid substitution in the coded protein: "c.949G > A (p.Val317Met)" and was tested on sow litter size. Information on 1622 litter records from 408 genotyped sows was analyzed to determine whether these SNP influenced the total number of piglets born (TNB) or the number of born alive (NBA). The polymorphisms ESR1: [Pvu II; c.669T > C], ESR1: [c.1665T > C; c.1755A > G] and ESR2: c.949G > A showed no statistically significant association with litter size. However, the ESR1: c.1227T allele was significantly associated with TNB. The additive substitution effect was estimated to be 0.40 piglets born per litter (P < 0.03), and no dominance effects were observed. This SNP could be useful in assisted selection for litter size in some pig lines, as a new genetic marker in linkage disequilibrium with the causative mutation.

O estilo pessoal do terapeuta pode ser entendido como um dos componentes do processo terapêutico, referindo-se a um conjunto de funções que se mostram de maneira integrada e expressam disposições, riscos e atitudes que todo terapeuta evidencia em seu exercício profissional. Este estudo apresenta dados da versão brasileira da escala do estilo pessoal do terapeuta (EPT-C) de Fernández-Alvarez e Garcia. Os procedimentos de adaptação seguiram os passos de tradução do instrumento do espanhol para o português, versão avaliada por juízes, estudo piloto e versão final. A fidedignidade obtida nas classes Atenção, Comprometimento e Operativa foram 0, 70, 0,71, e 0,79, respectivamente. São apresentados dados descritivos referentes a 225 terapeutas brasileiros, comparando-se terapeutas menos e mais experientes, assim como desempenhos nas funções entre terapeutas cognitivos, psicanalíticos e integrativos. Conclui-se que a escala pode ser um elemento importante no treinamento de terapeuta e apresenta características psicométricas satisfatórias na versão brasileira

Se revisa el género Veronica para el territorio colombiano, representado por 7 taxones (5 especies y 2 variedades) de acuerdo con las colecciones estudiadas. En la mayoría de los casos (5), se trata de elementos exóticos naturalizados en nuestro territorio y asociados generalmente a cultivos y ambientes alterados. Se presenta una clave para las especies y variedades detectadas en Colombia y se documentan dos novedades coro lógicas para Colombia y para el norte de Suramérica.The genus Veronica, according to the revised coIlections in this work, has 7 taxa (5 species and 2 varieties) in Colombia. AIl (minus two) are exotic species naturalized in this territory, and are commonly asociated with cultures and disturbed places. A key for the species and varieties recognized in Colombia is presented and two new records for this country and Northern Southamerica are documented.

The Taccaceae (Liliidae) are known in Colombia only from an old and obscure collection of Tacca parkeri Seem. Two new localities from the department of Guainía are here reported. This variable species is widely distributed in the Guayana region and the Amazonian lowlands. Some comments about morphological variability, flowering time and distribution of this species are also included.Se documenta la presencia de la familia Taccaceae (Liliidae) en Colombia, conocida solo por un impreciso registro de Tacca parkeri Seem., con la adición de dos nuevas localidades provenientes del departamento del Guainía. La especie, muy variable, se encuentra ampliamente distribuida en la región Guayana y en zonas bajas de la cuenca amazónica. Se adiciona información sobre variabilidad morfológica, período de floración y distribución de esta especie.

A new species of Phragmotheca (Bombacaceae) is described and illustrated. This represents the first record of this genus in premontane forest (subandean belt) and is also the first locality for Phragmotheca in the Eastern Cordillera of Colombia. The genus was previously known only from the Chocó biogeographic region and the Amazonian lowlands.Se describe e ilustra una nueva especie de Phragmotheca (Bombacaceae), que representa el primer registro del género en bosques premontanos (franja subandina) y en la Cordillera Oriental de Colombia. Phragmotheca era conocido hasta ahora, sólo de la región biogeográfica chocoana y de la planicie amazónica.

OBJECTIVE: The assessment of an easy to prepare and low cost control material for Hematology, available for manual and automated methods. MATERIAL AND METHOD: Aliquots of stabilized whole blood were prepared by partial fixation with aldehydes; the stability at different temperatures (4. 20 and 37 °C) during periods of up to 8-9 weeks and aliquot variability with both methods were controlled. RESULTS: Aliquot variability with automated methods at day 1, expressed as CV% (coefficient of variation) was: white blood cells (WBC) 2.7, red blood cells (RBC) 0.7, hemoglobin (Hb) 0.6, hematocrit (Hct) 0.7, mean cell volume (MCV) 0.3, mean cell hemoglobin (MCH) 0.6, mean cell hemoglobin concentration (MCHC) 0.7, and platelets (PLT) 4.6. The CV (coefficient of variation) percentages obtained with manual methods in one of the batches were: WBC 23, Hct 2.8, Hb 4.5, MCHC 5.9, PLT 41. Samples stored at 4ºC and 20ºC showed good stability, only a very low initial hemolysis being observed, whereas those stored at 37ºC deteriobed a rapidly (metahemoglobin formation, aggregation of WBC and platelets, as well as alteration of erythrocyte indexes). CONCLUSIONS: It was confirmed that, as long as there is no exposure to high temperatures during distribution, this material is stable, allowing assessment, both esternal and internal, for control purposes, with acceptable reproductivity, both for manual and auttomatic methods.

37. Diversidad florística en dos bosques subandinos del sur de Colombia

The floristic diversity in two plots of 0.1 ha of two subandean forest, in Amazonian (La Campucana, Putumayo) and Pacific (Ñarnbí, Nariño) Andean slopes at southwestern Colombia, was studied. The La Campucana forest was more diverse (269 species, 69 families and 158 genera) than Ñambí forest (205 species, 59 families and 122 genera). Rubiaceae was the most diverse family in both localities, followed byAraceae and Melastomataceae in Ñambí and Lauraceae and Sapotaceae in La Campucana. The distribution of life forms ts as follows: there were 32% trees, 28% treelets in Ñambí, and herbs and climbers are structurally more important than shrubs, lianes and hemiepiphytes. In La Campucana there were 49% of trees, 22% treelets; herbs and shrubs were less important while the lianas were more important. Species with dbh > 2.5 cm were more abundant in La Campucana than in Ñambí. The number of individuals was higher in La Campucana (1444) than in Ñambí (1349). At the specific level the similarity between regions was low. The diversity and dominance indexes showed that the two sampled forests were highly diversified.Se realizó un estudio comparativo de la diversidad florística en dos localidades entre 1350 y 1450 m de altitud, en las vertientes amazónica (La Campucana, Putumayo) y pacífica (Ñambí, Nariño) al sur de Colombia. En 0.1 ha se muestrearon todos los individuos con DAP mayor o igual a 1 cm. El número de especies fue mayor en el bosque de La Campucana (269 especies, 69 familias y 158 géneros), que en Ñambí (205 especies, 59 familias y 122 géneros). Rubiaceae fue la familia con mayor diversidad específica en las dos localidades, seguida por Araceae y Melastomataceae en Ñambí, y Lauraceae y Sapotaceae en La Campucana. En Ñambí el 32 % de las especies fueron árboles, el 28 % arbolitos; las hierbas y trepadoras herbáceas fueron más importantes que los arbustos, bejucos y hemiepífitos. En La Campucana, el 49 % fueron árboles, el 22 % arbolitos, los bejucos tuvieron mayor importancia estructural y las hierbas y arbustos estuvieron en muy baja proporción. En La Campucana el número de especies con diámetros mayores de 2.5 cm fue mayor, mientras que en Ñambí hay más especies con diámetros menores. En Ñambí se encontraron 1347 individuos y en La Campucana 1444. Los índices de similitud entre las dos localidades son muy bajos. Los índices de diversidad indican que los dos bosques estudiados son muy diversos.

The systematic position of Phragmotheca in the tribe quararibeae is discussed and a key for the Bombacaceae with simple or unifoliolate leaves occurring in northern South America is included. The diagnostic utility of some characters in the segregation of species in Phragmotheca, is reviewed and a diagnostic key is presented. Four new species and two new infraspecific taxa are described and illustrated: P.rubriflora, P.lemniscata, P.hydra, P.siderosa subsp. megacarpa and P. amazonica varo leticiana from Colombia and P.sidereotricha from Perú.Se comenta la posición sistemática de Phragmotheca en relación con la tribu Quararibeae y se incluye una clave para la separación de los géneros de bombacáceas de hoja entera o unifoliolada del norte de Suramérica. Se revisa la utilidad de algunos caracteres en la separación de las especies y se presenta una clave diagnóstica. Se describen e ilustran 4 nuevas especies y dos taxones infraespecíficos P. rubriflora, P. lemniscata, P. hydra, P. siderosa subsp. megacarpa, P. amazonica varo leticiana de Colombia y P. sidereotricha de Perú.

According with the historical context of the Real Expedición Botánica del Nuevo Reino de Granada (1783-1816), the nomenclatural history of the generic name Escallonia Mutis ex L. fiI. (Grossulariaceae) is discussed. The extant misinterpretation concerning the name in the manuscript documentation of the Expedition, is brigthened; four species of diferent families appear associated to this name in the manuscripts. Type specimens of two species, Escallonia myrtilloides L. fiI. and Dichondra evolvulacea (L. fiI.) Britton are located or selected. Dentro del contexto histórico de la Real Expedición Botánica del Nuevo Reino de Granada (1783-1816), se realiza el seguimiento del nombre genérico Escallonia Mutis ex L. fiI. (Grossulariaceae). Se aclara la confusión existente acerca de este nombre en la documentación manuscrita de la Expedición (Archivos de Mutis y Linneo), donde se encuentra asociado a descripciones originales de cuatro especies de diferentes familias. Se localiza y selecciona material tipo de dos de ellas: Escallonia myrtilloides L. fil. Y Dichondra evolvulacea (L. IiI.) Britton.

Se describe una nueva especie del género Aragoa H.B.K. (Scrophulariaceae). A. romeroi Fernández Alonso, propia de zonas subparamunas de la Sierra de Perijá, Cordillera Oriental, Colombia. Se subdivide el género en tres "grupos" de especies, comentándose su relación morfológica y fitogeográfica. Paralelamente se discute la ubicación de la nueva especie dentro del género. A new species of Aragoa H.B.K. (Scrophulariaceae), A. romeroi Fernández Alonso, characteristic of the subparamo zone of the Sierra de Perijá, Eastern Cordillera, Colombia is described. A division of the genus in three groups is made and their morphological and phytogeographical relationships are commented. Simultaneously, the location of the new species within the genus is discussed.

Our purpose was twofold: (1) to evaluate the effect of a computer-based, Spanish-language CRC screening decision aid on screening knowledge, intent to obtain screening, and screening self-efficacy in a community sample of Latinos with limited English proficiency (LEP); and (2) to survey these decision aid viewers at four months to determine their rates of CRC discussions with a health care provider as well as their rates of screening test completion. Methods We recruited 50-75 year old Latinos with LEP who were not current with CRC. Participants screening viewed a 14 minute multimedia decision aid that addresses CRC screening rationale, recommendations, and options. We conducted an uncontrolled (pre-post) study in which we assessed screening knowledge, self-efficacy, and intent at baseline and immediately after decision aid viewing. We also conducted a follow-up telephone survey of participants at four months to examine rates of patient-provider screening discussions and test completion. Results Among n = 80 participants, knowledge scores increased from 20% (before) to 72% (after) decision aid viewing (absolute difference [95%CI]: 52% [46, 59]). The proportion with high screening self-efficacy increased from 67% to 92% (25% [13, 37]); the proportion with high screening intent increased from 63% to 95% (32% [21, 44]). We reached 68 (85%) of 80 participants eligible for the follow-up survey. Of these 36 (53%) reported discussing screening with a provider and 13 (19%) completed a test. Conclusion Viewing a Spanish-language decision aid increased CRC screening knowledge, self-efficacy, and intent among Latinos with LEP. Decision aid viewing appeared to promote both CRC screening discussions with health care providers and test completion. The decision aid may be an effective tool for promoting CRC screening and reducing screening disparities in this population.

Abstract Background Vaginitis is a common complaint in primary care. In uncomplicated candidal vaginitis, there are no differences in effectiveness between oral or vaginal treatment. Some studies describe that the preferred treatment is the oral one, but a Cochrane's review points out inconsistencies associated with the report of the preferred way that limit the use of such data. Risk factors associated with recurrent vulvovaginal candidiasis still remain controversial. Methods/Design This work describes a protocol of a multicentric prospective observational study with one year follow up, to describe the women's reasons and preferences to choose the way of administration (oral vs topical) in the treatment of not complicated candidal vaginitis. The number of women required is 765, they are chosen by consecutive sampling. All of whom are aged 16 and over with vaginal discharge and/or vaginal pruritus, diagnosed with not complicated vulvovaginitis in Primary Care in Madrid. The main outcome variable is the preferences of the patients in treatment choice; secondary outcome variables are time to symptoms relief and adverse reactions and the frequency of recurrent vulvovaginitis and the risk factors. In the statistical analysis, for the main objective will be descriptive for each of the variables, bivariant analysis and multivariate analysis (logistic regression).. The dependent variable being the type of treatment chosen (oral or topical) and the independent, the variables that after bivariant analysis, have been associated to the treatment preference. Discussion Clinical decisions, recommendations, and practice guidelines must not only attend to the best available evidence, but also to the values and preferences of the informed patient.

Abstract Background The aim of this work was to study the effects on litter size of variants of the porcine genes RBP4, ESR1 and IGF2, currently used in genetic tests for different purposes. Moreover, we investigated a possible effect of the interaction between RBP4-MspI and ESR1-PvuII polymorphisms. The IGF2-intron3-G3072A polymorphism is actually used to select lean growth, but other possible effects of this polymorphism on reproductive traits need to be evaluated. Methods Detection of polymorphisms in the genomic and cDNA sequences of RBP4 gene was carried out. RBP4-MspI and IGF2-intron3-G3072A were genotyped in a hyperprolific Chinese-European line (Tai-Zumu) and three new RBP4 polymorphisms were genotyped in different pig breeds. A bivariate animal model was implemented in association analyses considering the number of piglets born alive at early (NBA12) and later parities (NBA3+ ) as different traits. A joint analysis of RBP4-MspI and ESR1-PvuII was performed to test their possible interaction. In the IGF2 analysis, paternal or maternal imprinting effects were also considered. Results Four different RBP4 haplotypes were detected (TGAC, GGAG, GAAG and GATG) in different pig breeds and wild boars. A significant interaction effect between RBP4-MspI and ESR1-PvuII polymorphisms of 0.61 ± 0.29 piglets was detected on NBA3+. The IGF2 analysis revealed a significant increase on NBA3+ of 0.74 ± 0.37 piglets for the paternally inherited allele A. Conclusions All the analyzed pig and wild boar populations shared one of the four detected RBP4 haplotypes. This suggests an ancestral origin of the quoted haplotype. The joint use of RBP4-MspI and ESR1-PvuII polymorphisms could be implemented to select for higher prolificacy in the Tai-Zumu line. In this population, the paternal allele IGF2-intron3-3072A increased litter size from the third parity. The non-additive effects on litter size reported here should be tested before implementation in other pig breeding schemes.

Abstract Background Coronary heart disease (CHD) is the leading cause of death in the United States. Previous research examining physicians ability to estimate cardiovascular risk has shown that physicians' generally overestimate the absolute risk of CHD events. This question has, however, only studied risk prediction for a limited number of patient care scenarios. The aim of this study is to measure the ability of physicians to estimate the risk of CHD events in patients with no previous history of coronary heart disease. Methods Twelve primary prevention scenarios with a 5-year risk of CHD events were developed. This questionnaire was surveyed at 3 university teaching hospitals where the participants were a convenience sample of internal medicine residents and fellows or attending physicians in general internal medicine or cardiology. For each scenario, physicians were asked to estimate the baseline 5-year risk of a coronary heart disease event and the revised risk if the patient were to receive lipid-lowering drug therapy. Estimates of the baseline 5-year risk were compared with values calculated from Framingham risk equations. Inaccurate responses were defined as those with a ratio of estimated to actual risk of more than 1.5 or less than 0.67. Physicians' estimates of the relative risk reduction with therapy were considered to be accurate if they were between 25% and 40%. Results 79 physicians (53 residents, 8 fellows, 18 attending physicians) completed the survey. Only 24% of physicians' risk estimates were accurate. In most cases, physicians overestimated the absolute risk of cardiovascular events without therapy (proportion overestimating ranged from 32–92% for the 12 individual scenarios). Physicians made larger errors in patient scenarios involving patients with high total or LDL cholesterol levels. Physicians' estimates of the relative risk reduction from treatment were more accurate: 43% of estimates were between 25 and 40%. Over 85% of physicians recommended treatment in 10 of 12 scenarios. Conclusions Physicians overestimate the absolute risk of CHD events and the potential absolute benefit of drug therapy.

Background: Carbohydrate antigen 125 (CA125) is a proteolytic fragment of MUC-16 that is increased in heart failure (HF) and associated with inflammation, fluid overload, and worse adverse events. Our main objective was to study the expression of CA125 on epicardium and its association with inflammation, adipogenesis, and fibrosis., Methods: Epicardial fat biopsies and blood were obtained from 151 non-selected patients undergoing open heart surgery. Immunohistochemistry, ELISA, or real-time PCR were used for analyzing protein or mRNA expression levels of CA125 and markers of inflammatory cells, fibroblasts, and adipocytes. Epithelial or stromal cells from epicardium were isolated and cultured to identify CA125 and its association with the adipogenesis and fibrosis pathways, respectively., Results: The median age was 71 (63-74) years, 106 patients (70%) were male, and 62 (41%) had an established diagnosis of HF before surgery. The slice of epicardial fat biopsy determined a positive and colorimetric staining on the epithelial layer after incubating with the CA125 M11 antibody, providing the first description of CA125 expression in the human epicardium. Epicardial CA125 showed a strong and positive correlation with markers of inflammation and fibrosis in the epicardial fat tissue while exhibiting a negative correlation with markers of the adipogenesis pathway. This relationship remained significant after adjusting for potential confounders such as a prior HF diagnosis and plasma CA125 levels., Conclusion: Epicardial cells express CA125, which is positively associated with inflammatory and fibroblast markers in epicardial adipose tissue. These results suggest that CA125 may be biologically involved in HF progression (transition from adipogenesis to fibrosis)., (© 2024. The Author(s).)

50. End-of-Life Care for Newborn Infants: A Multicenter Real-Life Prospective Study.

Introduction: Most neonatal deaths in industrialized countries follow a process of redirection of care. The objectives of this study were to describe how neonates die in a middle-income country, whether there was redirection of care, and the reason for this decision., Methods: This was a prospective, multicenter, cross-sectional study. Neonates who died in the delivery room or in the neonatal intensive care unit in 97 hospitals over a 6-month period were included. After each neonatal death, one investigator interviewed a member of the healthcare team who had been involved in the end-of-life care process. Perinatal data, conditions that led to death, whether there was redirection of care, and details of the end-of-life process were recorded., Results: Data from 697 neonatal deaths were analyzed, which represent 80% of the total deaths occurring in Argentina in that period. The main causes of death were complications of prematurity (47%) and congenital anomalies (27%). Overall, 32% of neonates died after a process of redirection of care, and this was less frequent in the neonatal intensive care unit (28%) than in the delivery room (70%, p < 0.001). The reasons for withholding/withdrawing care were inevitable death (75%) and severe compromise of expected quality of life (25%). Redirection of care consisted in withholding therapies in 66% and withdrawal in 34%. A diagnosis of a major congenital anomaly increased the odds of redirection of care (OR 5.45; 95% CI: 3.59-8.27)., Conclusion: Most neonates who die in Argentina do so while receiving full support.
