Abstract

BackgroundeConsulta is a tele-consultation service involving doctors and patients, and is part of Catalonia's public health information technology system. The service has been in operation since the end of 2015 as an adjunct to face-to-face consultations. A key factor in understanding the barriers and facilitators to the acceptance of the tool is understanding the sociodemographic characteristics of general practitioners who determine its use. ObjectiveThis study aimed to analyze the sociodemographic factors that affect the likelihood of doctors using eConsulta. MethodsA retrospective cross-sectional analysis of administrative data was used to perform a multivariate logistic regression analysis on the use of eConsulta in relation to sociodemographic variables. ResultsThe model shows that the doctors who use eConsulta are 45-54 years of age, score higher than the 80th percentile on the quality of care index, have a high degree of accessibility, are involved in teaching, and work on a health team in a high socioeconomic urban setting. ConclusionsThe results suggest that certain sociodemographic characteristics associated with general practitioners determine whether they use eConsulta. These results must be taken into account if its deployment is to be encouraged in the context of a public health system.

Abstract

Deep learning as a means to inferencing has proliferated thanks to its versatility and ability to approach or exceed human-level accuracy. These computational models have seemingly insatiable appetites for computational resources not only while training, but also when deployed at scales ranging from data centers all the way down to embedded devices. As such, increasing consideration is being made to maximize the computational efficiency given limited hardware and energy resources and, as a result, inferencing with reduced precision has emerged as a viable alternative to the IEEE 754 Standard for Floating-Point Arithmetic. We propose a quantization scheme that allows inferencing to be carried out using arithmetic that is fundamentally more efficient when compared to even half-precision floating-point. Our quantization procedure is significant in that we determine our quantization scheme parameters by calibrating against its reference floating-point model using a single inference batch rather than (re)training and achieve end-to-end post quantization accuracies comparable to the reference model.

Abstract

Heart transplantation (HT) is the gold standard therapy for advanced heart failure (ADHF), and LVADs as destination therapy are an option in non-HT candidates. Most patients with ADHF never receive HT or an LVAD, so alternative strategies are needed. Intermittent levosimendan can reduce HF hospitalizations in ADHF patients in the short term. It is uncertain whether the results of the comparison of inotropes with older-generation LVADs would have the same outcomes in the current era of ADHF patients treated with levosimendan, who are less sick but older. In this paper, we compare the use of two therapeutic strategies for end-stage HF in patients who are not candidates for HT: repetitive intermittent levosimendan vs. LVAD as destination therapy. To do so, we compare two multicenter cohorts of real-life patients from Spain: the LEVO-D registry and the REGALAD registry. In total, 715 patients coming from the two registries were found: 403 from LEVO-D and 312 from REGALAD. Non-adjusted median survival was shorter for LEVO-D patients, with the benefit for the LVADs seen only after the first year of therapy. The survival advantage for the LVAD cohort was also true after analysis of the matched cohort but, as in the non-matched analysis, the survival benefit was mainly shown after one year of follow-up. We conclude that in elderly ADHF non-HT candidates, LVAD therapy offers significantly better long-term outcomes when compared to intermittent levosimendan; thus, it should be considered in carefully selected candidates. On the other hand, in poor LVAD candidates or highly comorbid patients, intermittent inotropic support with levosimendan could be a reasonable alternative to LVAD, as 1-year outcomes are similar. Abstract

Interfaces based on projection screens have become increasingly more popular in recent years, mainly due to the large screen size and resolution that they provide, as well as their stereo-vision capabilities. This work shows a local method for real-time detection of non-stationary photometric perturbations in projected images by means of computer vision techniques. The method is based on the computation of differences between the images in the projector's frame buffer and the corresponding images on the projection screen observed by the camera. It is robust under spatial variations in the intensity of light emitted by the projector on the projection surface and also robust under stationary photometric perturbations caused by external factors. 48. Reduction of nonholonomic systems in two stages and Hamiltonization

Abstract

In this paper we study the reduction of a nonholonomic system by a group of symmetries in two steps. Using the so-called 'vertical-symmetry' condition, we first perform a 'compression' of the nonholonomic system leading to an almost symplectic manifold. Second, we perform an (almost) symplectic reduction, relying on the existence of a momentum map. In this case, we verify that the resulting (almost) symplectic reduced spaces are the leaves of the characteristic foliation of the reduced nonholonomic bracket. On each leaf we study the (Lagrangian) equations of motion, obtaining a nonholonomic version of the Lagrange-Routh equations, and we analyze the existence of a conformal factor for the reduced bracket in terms of the existence of conformal factors for the almost symplectic leaves. We also relate the conditions for the existence of a momentum map for the compressed system with gauge transformations by 2-forms.
