Abstract

Despite the occurrence of wildfires quadrupling over the past four decades, the health effects associated with wildfire smoke exposures during pregnancy remains unknown. Particulate matter less than 2.5 μms (PM2.5) is among the major pollutants emitted in wildfire smoke. Previous studies found PM2.5 associated with lower birthweight, however, the relationship between wildfire-specific PM2.5 and birthweight is uncertain. Our study of 7923 singleton births in San Francisco between January 1, 2017 and March 12, 2020 examines associations between wildfire smoke exposure during pregnancy and birthweight. We linked daily estimates of wildfire-specific PM2.5 to maternal residence at the ZIP code level. We used linear and log-binomial regression to examine the relationship between wildfire smoke exposure by trimester and birthweight and adjusted for gestational age, maternal age, race/ethnicity, and educational attainment. We stratified by infant sex to examine potential effect modification. Exposure to wildfire-specific PM2.5 during the second trimester of pregnancy was positively associated with increased risk of large for gestational age (OR = 1.13; 95% CI: 1.03, 1.24), as was the number of days of wildfire-specific PM2.5 above 5 μg m-3 in the second trimester (OR = 1.03; 95% CI: 1.01, 1.06). We found consistent results with wildfire smoke exposure in the second trimester and increased continuous birthweight-for-gestational age z-score. Differences by infant sex were not consistent. Counter to our hypothesis, results suggest that wildfire smoke exposures are associated with increased risk for higher birthweight. We observed strongest associations during the second trimester. These investigations should be expanded to other populations exposed to wildfire smoke and aim to identify vulnerable communities. Additional research is needed to clarify the biological mechanisms in this relationship between wildfire smoke exposure and adverse birth outcomes.

Abstract

The University Library at the Technical University of Catalonia (UPC) currently manages several collections with different automated systems. These collections include the bibliographic collections of libraries, the Digital Library (which includes commercial products, such as books, journals and databases) and the documents generated by the University (such as dissertations, teaching materials, research results, projects by students, digital videos, etc.). The production processes of the digital materials that are generated by the University, which are highly specific and complex, follow an established workflow that traditional management systems are unprepared for. Increasingly, the University requests the Library to actively participate not only in the management of the final documents (within the library) but also during the entire life cycle of the documents generated. We should keep in mind that digital documentation has its own specific publication and editing requirements, which are sometimes very sophsticated, and therefore the management of different kinds of materials with one overall system is difficult. The University Library has developed different systems to suit the characteristics of each collection, which has also spawned different support products that meet specific needs to be developed for the management of these collections. These products are therefore very efficient; however, they have led to a disparity in the procedures for document retrieval and, furthermore, the system’s development and maintenance has become very expensive. aced with this situation, the University Library is analysing the advantages and disadvantages of using a unique management system or, alternatively, of continuing to employ different systems, as the previous stage to the design of a new strategy for the management of university production.

Abstract

The definition of the new European space of university education introduces the possibility of reframing as they will be the knowledge and abilities that the university students will have to acquire to not only to become good professionals but also in people able to form throughout all the life. The student will have to learn to learn, capacity for which is necessary to have a good base in the use and access to the information. This new educative frame appears like an excellent opportunity to librarians to collaborate with the educational staff. The innovation and the quality of the universities will be based on good part in the capacity to transmit these new informational abilities.

Abstract

Purpose The main characteristics of mechanically ventilated ARDS patients affected with COVID-19, and the adherence to lung-protective ventilation strategies are not well known. We describe characteristics and outcomes of confirmed ARDS in COVID-19 patients managed with invasive mechanical ventilation (MV). Methods This is a multicenter, prospective, observational study in consecutive, mechanically ventilated patients with ARDS (as defined by the Berlin criteria) affected with with COVID-19 (confirmed SARS-CoV-2 infection in nasal or pharyngeal swab specimens), admitted to a network of 36 Spanish and Andorran intensive care units (ICUs) between March 12 and June 1, 2020. We examined the clinical features, ventilatory management, and clinical outcomes of COVID-19 ARDS patients, and compared some results with other relevant studies in non-COVID-19 ARDS patients. Results A total of 742 patients were analysed with complete 28-day outcome data: 128 (17.1%) with mild, 331 (44.6%) with moderate, and 283 (38.1%) with severe ARDS. At baseline, defined as the first day on invasive MV, median (IQR) values were: tidal volume 6.9 (6.3–7.8) ml/kg predicted body weight, positive end-expiratory pressure 12 (11–14) cmH2O. Values of respiratory system compliance 35 (27–45) ml/cmH2O, plateau pressure 25 (22–29) cmH2O, and driving pressure 12 (10–16) cmH2O were similar cto values from non-COVID-19 ARDS observed in other studies. Recruitment maneuvers, prone position and neuromuscular blocking agents were used in 79%, 76% and 72% of patients, respectively. The risk of 28-day mortality was lower in mild ARDS [hazard ratio (RR) 0.56 (95% CI 0.33–0.93), p = 0.026] and moderate ARDS [hazard ratio (RR) 0.69 (95% CI 0.47–0.97), p = 0.035] when compared to severe ARDS. The 28-day mortality was similar to other observational studies in non-COVID-19 ARDS patients. Conclusions In this large series, COVID-19 ARDS patients have features similar to other causes of ARDS, compliance with lung-protective ventilation was high, and the risk of 28-day mortality increased with the degree of ARDS severity. Abstract

Background. Homelessness is a serious public health problem that affects more than 31.000 people in Spain. It is a population that is aging, with important consequences for their health, causing among other things a greater prevalence of chronic diseases. Also, within this aging homeless population, there is a high proportion of people who use drugs, they are called "aging drug users". This consumption adds a risk to an already vulnerable population. Aim. Describe the population of aging problematic drug users regarding the age of onset of the first chronic diseases and compare it with the rest of the homeless population depending on drug use, both those who have a non-problematic consumption and those who do not use drugs. Methods. A retrospective cohort study was conducted in the homeless population served in the city of Girona from 2006 to 2016.The information includes clinical data from January 1, 2000 to December 31, 2016, collected thanks to electronic records of primary care and the mental health centre of Girona. Data included sociodemographic, chronic and infectious diseases, substance misuse disorder. The average healthy months and years the person passes until the onset of the first chronic diseases is compared in the 3 different groups of consumption using the Kruskal- Walls test. The Kaplan-Meier test was used to determine the probability of being healthy over time in the 3 consumption groups. Results. The sample consisted of 492 participants. Homeless men were accounted for 84.5% of the sample. The mean age of the cohort was 49.7 years (SD 11,09) for men and 49.29 years (SD 13,6) for women. In relation to drug use, 59.1% of the sample has no substance misuse problem, 31,9% had a non-problematic drug use, and 18,1% of the total were problem drug users. About chronic diseases that were recorded in the electronic registry of the PCC, 152 people had al least one. We focus now only on those people over 40, who represent 80% of the sample. Within this older group, non-consumers still are the larger group, representing 44.9% of the people over 40. Aging problematic drug users represent 14% of the sample and those people over 40 who use other drugs represent 21,1% of the sample. Within this aging group, we have the date of diagnosis of the different chronic diseases registered of 115 people. The age of onset of the first chronic disease for each group is; 42,2 years for aging problematic drug users, 52,25 years for other drug use group and 51,25 years for no substance misuse group. Conclusions. We can conclude that there are differences in the time of appearance of chronic diseases in individuals experiencing homelessness classified depending on their drug consumption, being the "aging problematic drug users" those exposed to an earlier onset of them. However, we cannot assure the association between consumption and the age of appearance
