Background: Work ability consists in the balance between employee’s capabilities and work demands. It can be measured by the Work Ability Index, a validated questionnaire that has been used in Europe since 1980s. This study is the first evaluating work ability among dock workers, a workforce which carries out complex activities requiring physical and mental engagement (night shifts, irregular shifts, constant demand for attention and concentration and significant physical effort). Objective: It was hypothized that the high strain level affecting this peculiar job may lead to a decrease in work ability. The aim of the study was to assess work ability and investigate if and how much not work-related factors, such as psycho-emotional state, lifestyle, health conditions and socio-demographic aspects, can interfere with it. Psycho-diagnostic protocol consisted of 6 questionnaires, chosen because of fast and effective data collection aiming to worker compliance and simple interpretation of results. Method: A total of 105 workers was engaged in different companies involved in port services. A protocol consisting of 6 questionnaires has been applied individually before the work shift: Work Ability Index (WAI), Effort-reward imbalance questionnaire (ERI), Beck Depression Inventory-II (BDI-II), Hamilton Rating Scale for Anxiety (HAM-A), Epworth Sleepiness Scale (ESS), Pittsburgh Sleep Quality Index (PSQI). Results: Work ability resulted excellent in 60 subjects and good in 37. No significant correlation between individual characteristics and work ability was found. Our results indicated that work ability was negatively related with emotional states, particularly with anxiety. Univariate and multivariate linear regression model for work ability, have shown a statistically significant correlation with BDI, HAM-A and ERI questionnaires’ results. Conclusion: Our results showed that WAI is a useful tool for routine occupational health surveillance. The association of work ability with psychosocial factors suggests that actions at the workplace adopted by occupational physicians in order to prevent a reduction in working capacity, should have a multidimensional approach evaluating not only individual lifestyle, but also mood states., Mediterranean Journal of Clinical Psychology, Vol 9, No 2 (2021)

The association between genetic variations in the cytochrome P450 (CYP) family genes and pathological conditions related to long-term exposure to organochlorine compounds (OCs) deserves further elucidation. OCs are persistent organic pollutants with bioaccumulative and lipophilic characteristics. They can act as endocrine disruptors and perturb cellular mechanisms. Prolonged exposure to OCs has been associated with different pathological manifestations. CYP genes are responsible for transcribing enzymes essential in xenobiotic metabolism. Therefore, polymorphisms in these genetic sequences a. alter the metabolic pathways, b. induce false cellular responses, and c. may provoke pathological conditions. The main aim of this review is to define the interaction between parameters a, b and c at a mechanistic/molecular level, with references in clinical cases. © 2017 The Authors

In real life, consumers are exposed to complex mixtures of chemicals via food, water and commercial products consumption. Since risk assessment usually focuses on individual compounds, the current regulatory approach doesn't assess the overall risk of chemicals present in a mixture. This study will evaluate the cumulative toxicity of mixtures of different classes of pesticides and mixtures of different classes of pesticides together with food additives (FAs) and common consumer product chemicals using realistic doses after long-term exposure. Groups of Sprague Dawley (CD-SD) rats (20 males and 20 females) will be treated with mixtures of pesticides or mixtures of pesticides together with FAs and common consumer product chemicals in 0.0, 0.25 x acceptable daily intake (ADI)/tolerable daily intake (TDI), ADI/TDI and 5 x ADI/TDI doses for 104 weeks. All animals will be examined every day for signs of morbidity and mortality. Clinical chemistry hematological parameters, serum hormone levels, biomarkers of oxidative stress, cardiotoxicity, genotoxicity, urinalysis and echocardiographic tests will be assessed periodically at 6 month intervals. At 3-month intervals, ophthalmological examination, test for sensory reactivity to different types of stimuli, together with assessment of learning abilities and memory performance of the adult and ageing animals will be conducted. After 24 months, animals will be necropsied, and internal organs will be histopathologically examined. If the hypothesis of an increased risk or a new hazard not currently identified from cumulative exposure to multiple chemicals was observed, this will provide further information to public authorities and research communities supporting the need of replacing current single-compound risk assessment by a more robust cumulative risk assessment paradigm.

It is well-known that occupational and environmental exposure to several factors, including benzene, heavy metals, chemicals and mineral fibers, is associated with the risk of developing a great number of diseases. Numerous studies have been carried out in order to investigate the mechanisms of toxicity of these substances, with particular regard to the possible toxic effects on the immune system. However, little is known about the influence of heavy metals, such as lead, on the immune system in human populations. Lead is a heavy metal still used in many industrial activities. Human exposure to lead can induce various biological effects depending upon the level and duration of exposure, such as toxic effects on haematological, cardiovascular, nervous and reproductive systems. Several studies demonstrated that exposure to lead is associated to toxic effects also on the immune system, thus increasing the incidence of allergy, infectious disease, autoimmunity or cancer. However, the effects of lead exposure on the human immune system are not conclusive, mostly in occupationally exposed subjects; nevertheless some immunotoxic abnormalities induced by lead have been suggested. In particular, in vivo, in vitro and ex vivo lead is able to improve T helper 2 (Th2) cell development affecting Th1 cell proliferation. Further studies are required to better understand the mechanisms of lead immunotoxicity and the ability of lead to affect preferentially one type of immune response.

Sulla base di tre casi di infortunio mortale verificatisi nell'ambito dell'edilizia, gli autori hanno analizzato le problematiche connesse al mancato impiego dei DPI quali fattori favorenti il determinismo nonchè quale elemento di individuazione di possibile responsabilità per le figure coinvolte.

5-FU cytotoxicity mechanism has been assigned both to the miss-incorporation of fluoronucleotides into RNA and DNA and to the inhibition of thymidylate synthase. 5-FU is one of the most widely used chemotherapeutic drugs, although it has severe side effects that may vary between patients. Pharmacogenetic studies related to 5-FU have been traditionally focused on the rate-limiting catabolic enzyme, dihydropyrimidine dehydrogenase that breaks 80-85% of 5-FU into its inactive metabolite. Choosing the right dosing scheme and chemotherapy strategy for each individual patient remains challenging for personalized chemotherapy management. In the general effort toward reduction of colorectal cancer mortality, in vitro screening studies play a very important role. To accelerate translation research, increasing interest has been focused on using in vivo-like models such as three-dimensional spheroids. The development of higher throughput assays to quantify phenotypic changes in spheroids is an active research area. Consequently, in this study we used the microarray technology to reveal the HT-29 colorectal adenocarcinoma cells gene expression signature as response to 5-FU/OXP/FA treatment in a state of the art 3D culture system. We report here an increased reactive oxygen species production under treatment, correlated with a decrease in cell viability and proliferation potential. With respect to the HT-29 cells gene expression under the treatment with 5-FU/OXP/FA, we found 15.247 genes that were significantly differentially expressed (p < 0.05) with a fold change higher that two-fold. Among these, 7136 genes were upregulated and 8111 genes were downregulated under experimental conditions as compared to untreated cells. The most relevant and statistic significant (p < 0.01) pathways in the experiment are associated with the genes that displayed significant differential expression and are related to intracellular signaling, oxidative stress, apoptosis, and cancer. © 2016 Negrei, Hudita, Ginghina, Galateanu, Voicu, Stan, Costache, Fenga, Drakoulis and Tsatsakis.

Summary Introduction. Since it is still controversial whether-low-to moderate long-term lead below current threshold values causes neurobehavioural deficits in adults. Methods. Forty lead-exposed workers subjects with a mean blood lead (PbB) level of 56.4 μg/dL and 40 non-lead-exposed aged matched subjects (PbB: 15.4 μg/dL) with the same socio-economic background were investigated. Participants were administered a neuropsychological tests consisting of BAMT (Branches Alternate Movements Task), FT (Finger Tapping Speed), DS (Digit Span) POMS (Profile of Mood States). Results. Authors noted a significant relationship between the exposed and the referent groups in tests mainly involving executive functions, short time memory and psycho-emotional variables. In addition, Poisson regression test performed on single psychoemotional factors (POMS), has allowed to evidence a significant influence of Pb e ZPP levels on tension, anxiety and depression. Conclusions. The present study showed that lead exposure among adults at levels previously considered safe, results in impairment of certain cognitive abilities.

Gli autori dopo aver ripercorso l'iter normativo che ha introdotto in Italia l'Istituto della Mediazione/Conciliazione, delineano la figura del mediatore e le sue competenze nell'ambito della valutazione del danno da responsabilità civile, soffermandosi inoltre sulle specifiche competenze necessarie per il mediatore chiamato ad assolvere il suo ruolo in que casi ove la proposta conciliativa deve prevdere una valutazione della pretesa stessa, l'accertamento del nesso causale ed al contempo del quantum risarcitorio.
