1. On the Generalizability of Iterative Patch Selection for Memory-Efficient High-Resolution Image Classification

Abstract

Classifying large images with small or tiny regions of interest (ROI) is challenging due to computational and memory constraints. Weakly supervised memory-efficient patch selectors have achieved results comparable with strongly supervised methods. However, low signal-to-noise ratios and low entropy attention still cause overfitting. We explore these issues using a novel testbed on a memory-efficient cross-attention transformer with Iterative Patch Selection (IPS) as the patch selection module. Our testbed extends the megapixel MNIST benchmark to four smaller O2I (object-to-image) ratios ranging from 0.01% to 0.14% while keeping the canvas size fixed and introducing a noise generation component based on B\'ezier curves. Experimental results generalize the observations made on CNNs to IPS whereby the O2I threshold below which the classifier fails to generalize is affected by the training dataset size. We further observe that the magnitude of this interaction differs for each task of the Megapixel MNIST. For tasks "Maj" and "Top", the rate is at its highest, followed by tasks "Max" and "Multi" where in the latter, this rate is almost at 0. Moreover, results show that in a low data setting, tuning the patch size to be smaller relative to the ROI improves generalization, resulting in an improvement of + 15% for the megapixel MNIST and + 5% for the Swedish traffic signs dataset compared to the original object-to-patch ratios in IPS. 2. Foreign Language Education in Louisiana: A Cluster Analysis

Abstract

Louisiana is currently the only state in the United States that requires foreign language (FL) study for some, but not all of their high school students, and these requirements are undergoing seismic changes. Considering that "geographic," "economic," and "integration" factors contribute to whether a school can provide FL at all--and biased counseling dissuades minoritized students from taking FL--this study asks: "has FL education been equitably accessible to all Louisiana high school students who want to pursue it, regardless of race or economic background?" To address this question, this paper presents results from two analyses. First, program-internal equity variables (e.g., [dis]similarity between school-wide and FL student demographics) were clustered to produce "profiles" of FL programs. Next, a multinomial logistic regression using program-external factors (e.g., federal funding status, desegregation orders) was conducted to isolate the factors impacting equity. Federal funding and rurality were both found to be significant, with schools receiving federal Title I funds and rural schools being much more likely to exhibit inequitable access to FL courses. In (a) identifying schools with enrollment inequities and (b) the outside factors associated with greater inequity, this paper aims to provide policymakers with empirically based tools to address (in)equity in Louisiana high school FL education. These findings can be especially helpful in light of the state's recent (2023-2024 school year) expansion of FL requirements to accept computer science courses.

Abstract

Effective policy and intervention strategies to combat human trafficking for child sexual exploitation material (CSEM) production require accurate prevalence estimates. Traditional Network Scale Up Method (NSUM) models often necessitate standalone surveys for each geographic region, escalating costs and complexity. This study introduces a partially pooled NSUM model, using a hierarchical Bayesian framework that efficiently aggregates and utilizes data across multiple regions without increasing sample sizes. We developed this model for a novel national survey dataset from the Philippines and we demonstrate its ability to produce detailed municipal-level prevalence estimates of trafficking for CSEM production. Our results not only underscore the model's precision in estimating hidden populations but also highlight its potential for broader application in other areas of social science and public health research, offering significant implications for resource allocation and intervention planning.

4. Proca in an Expanding Universe

Abstract

The superradiant growth of massive vector fields in rotating black hole spacetimes has garnered significant attention in recent literature. However, the majority of these studies overlook the influence of a cosmological constant, which likely constitutes the primary energy content of our universe. In this paper, we extend recent research by incorporating a cosmological constant into the Einstein+Proca system and numerically evolving the resulting equations of motion. Utilizing the newly released GRBoondi numerical relativity code, designed specifically for the numerical evolution of (generalized) Proca fields, we discover that parameters causing a growing instability in the $\Lambda = 0$ scenario transition to a decaying state when $\Lambda > 0$. This results in a more intriguing phenomenology. These simulations pave the way for future full Einstein+Proca simulations to explore the secular decay of the resultant cloud from gravitational emission.

7. GRBoondi: A code for evolving Generalized Proca theories on arbitrary backgrounds

Abstract

While numerical simulations offer unparalleled precision and robustness in studying complex physical systems, their execution is often hindered by complexity, costliness, and time consumption due to the intricate equations involved. This challenge is already encountered in General Relativity, where non-flat spacetimes exacerbate the computational burden. This complexity is further intensified when dealing with additional degrees of freedom. To address this challenge head-on, we introduce GRBoondi, a groundbreaking fixed-background numerical relativity code designed to provide a unified interface for numerically solving Generalized Proca theories. GRBoondi grants users the ability to make arbitrary modifications to the Proca equations of motion on any background, providing a robust and versatile tool for exploring diverse classes of Generalized Proca theories. Abstract

The Compact Linear Accelerator for Research and Applications (CLARA) is a 250 MeV ultrabright electron beam test facility at STFC Daresbury Laboratory. A user beam line has been designed to maximise exploitation of CLARA in a variety of fields, including novel acceleration and new modalities of radiotherapy. In this paper we present the specification and design of this beam line for Full Energy Beam Exploitation (FEBE). We outline the key elements which provide users to access ultrashort, low emittance electron bunches in two large experiment chambers. The results of start-to-end simulations are reported which verify the expected beam parameters delivered to these chambers. Abstract

BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection during pregnancy increases the risk of adverse fetal and neonatal outcomes, but the contribution to severe maternal morbidity (SMM) has been less frequently documented. METHODS: We conducted a national cohort study of 93 624 deliveries occurring between 11 March 2020 and 1 July 2021 using medical claims information from the OptumLabs Data Warehouse. SARS-CoV-2 infection was identified from diagnostic and laboratory testing claims records. We identified 21 SMM conditions using International Classification of Diseases, Tenth Revision, Clinical Modification and procedure codes and compared SMM conditions by SARS-CoV-2 status using Poisson regression with robust variance, adjusting for maternal sociodemographic and health factors, onset of labor, and week of conception. RESULTS: Approximately 5% of deliveries had a record of SARS-CoV-2 infection: 27.0%

21. Independent Reading in the Exploratory Spanish Classroom: Who Is Missing from the Picture of 'Typical' Second Language Reading Development?

Abstract

In the United States, research on reading difficulties is predominantly carried out by scholars who project findings on L1 English reading difficulty to generalized difficulty in learning a second language (L2; e.g., Galuschka et al., 2020; Shaywitz & Shaywitz, 2020; cf. Sparks, 2023). This research extrapolation creates a negative feedback loop (Sparks, 2016; Wight, 2015) in which more students with reading and learning difficulties are excluded from L2 study (see Cioe-Pena, 2020; Ritz & Sherf, 2021) and are, as a result, understudied. This dissertation aims to address this issue by conducting a series of paired case studies with American L1 English early elementary learners of Spanish as they participated in an independent reading task tracking their L2 reading development over time within an Exploratory L2 program (Pufahl & Rhodes, 2008, 2011). Participants were aged seven to eight years old and received approximately 700 minutes of task-based (Ellis & Shintani, 2013) L2 instruction over the course of an L1 eight-week summer camp. Participants were video-recorded selecting and reading L2 books aloud (framed as a bilingual "storytime" task). Participants' reading sessions were coded for accuracy (word and syllable decoding) and each participant diagnosed with a learning difficulty (N = 4) was analyzed in comparison to a typically developing reader (N = 4) of comparable background to illustrate differences in L2 reading development over the course of the summer camp's Exploratory L2 Spanish program. Results for case participants were triangulated with academic reports to shed additional light on their L1 reading accuracy. An analysis of the participants' book selection behaviors indicated that those with learning difficulties selected books that were more--and more variably--difficult (Edmunds & Bauserman, 2006; Merga & Roni, 2017) and that L2 book selection strategy instruction may be needed. Though the participants with learning difficulties did not decode their selected L2 books with as much accuracy as their typically developing peers did, their decoding errors patterned in similar ways, indicating that their instructional needs manifest in the same focal areas as their typically developing peers. Pedagogical and research implications are discussed in relation to error loci to inform future work with young, beginner L2 learners with learning difficulties. Abstract

The University of Iowa (UI) is the state of Iowa's flagship institution; a public research one, division one university in the Big Ten Conference (B1G) with twelve colleges. One hidden gem in the accomplishments of the Office of the Registrar is UI's FERPA Team (UI-FT); a point of pride unknown to many but greatly appreciated by those who know the complexity of the regulation and its situation in the American higher education landscape. Even though to some it may feel like it has been here forever, FERPA, sometimes referred to as the Buckley Amendment, was signed into law in 1974; nearly every institution of higher education in the United States was impacted by it. UI-FT's mission is to support the campus community as an institutional resource for FERPA regulation training, guidance, policy interpretation, and protocol. It aims to provide consistent, reliable information and data while protecting student privacy and educational record security via accessible and timely service with integrity. UI-FT's structure supports a flexible framework with clear expectations to produce effective results.

Abstract

Flare frequency distributions represent a key approach to addressing one of the largest problems in solar and stellar physics: determining the mechanism that counter-intuitively heats coronae to temperatures that are orders of magnitude hotter than the corresponding photospheres. It is widely accepted that the magnetic field is responsible for the heating, but there are two competing mechanisms that could explain it: nanoflares or Alfv\'en waves. To date, neither can be directly observed. Nanoflares are, by definition, extremely small, but their aggregate energy release could represent a substantial heating mechanism, presuming they are sufficiently abundant. One way to test this presumption is via the flare frequency distribution, which describes how often flares of various energies occur. If the slope of the power law fitting the flare frequency distribution is above a critical threshold, $\alpha=2$ as established in prior literature, then there should be a sufficient abundance of nanoflares to explain coronal heating. We performed $>$600 case studies of solar flares, made possible by an unprecedented number of data analysts via three semesters of an undergraduate physics laboratory course. This allowed us to include two crucial, but nontrivial, analysis methods: pre-flare baseline subtraction and computation of the flare energy, which requires determining flare start and stop times. We aggregated the results of these analyses into a statistical study to determine that $\alpha = 1.63 \pm 0.03$. Abstract

The advent of gravitational wave astronomy has seen a huge influx of new predictions for potential discoveries of beyond the Standard Model fields. The coupling of all fundamental fields to gravity, together with its dominance on large scales, makes gravitational physics a rich laboratory to study fundamental physics. This holds especially true for the search for the elusive dark photon, a promising dark matter candidate. The dark photon is predicted to generate instabilities in a rotating black hole spacetime, birthing a macroscopic Bose-Einstein condensate. These condensates can especially form around super massive black holes, modifying the dynamical inspiralling process. This then opens another window to leverage future space-borne gravitational wave antennas to join the hunt for the elusive dark matter particle. This study builds a preliminary model for the gravitational waveform emitted by such a dressed extreme mass-ratio inspiral. Comparing these waveforms to the vacuum scenario allows projections to the potential constrainability on the dark photon mass by space-borne gravitational wave antennas. The superradiant instability of a massive vector field on a Kerr background is calculated and, under reasonable approximations, the backreaction on the orbit of an inspiralling solar mass-scale compact object due to the secular evolution of the resulting boson cloud is determined. The end result is the projection that the LISA mission should be able to constrain the dark photon mass using extreme mass ratio inspirals in the range $[1.8 \times 10^{-17}, 4.47 \times 10^{-16}]$ eV., Comment: 21 Pages, 8 Figures

Abstract

Disruption of host-associated microbial communities can have detrimental impacts on host health. However, the capacity of individual host-associated microbial communities to resist disturbance has not been well defined. Using a novel fecal sampling method for honey bees (Apis mellifera), we examined the resistance of the honey bee gut microbiome to disruption from a low dose of the antibiotic, tetracycline (4.5 μg). Prior to the experiment, bacterial communities from fecal samples were compared to communities from dissected whole guts of the same individuals to ensure fecal samples accurately represented the gut microbiome. Fecal samples were collected from lab-caged honey bees prior to, and five days after, tetracycline exposure to assess how antibiotic disturbance affected the communities of individuals. We used metrics of alpha and beta diversity calculated from 16S rRNA gene amplicon sequences to compare gut community structure. Low dose tetracycline exposure did not consistently change honey bee gut microbiome structure, but there was individual variation in response to exposure and specific taxa (one ASV assigned to Lactobacillus kunkeei and one ASV in the genus Bombella) were differentially abundant following tetracycline treatment. To assess whether individual variation could be influenced by the presence of tetracycline resistance genes, we quantified the abundance of tet(B) and tet(M) with qPCR. The abundance of tet(M) prior to tetracycline treatment was negatively correlated with change in community membership, assessed by difference in Jaccard dissimilarity over the five-day experiment. Our results suggest that the honey bee gut microbiome has some ability to resist or recover from antibiotic-induced change, specific taxa may vary in their susceptibility to tetracycline exposure, and antibiotic resistance genes may contribute to gut microbiome resistance.

26. Understanding and Minimizing Perimeter Losses of Silicon-Based Monolithic Tandem Solar Cells – A Simulation Study

The vast majority of monolithic perovskite-silicon-based tandem cells to date is manufactured in R&D environments using small scale perovskite top cells (~1-4 cm²), typically placed on a larger silicon bottom cell. The high perimeter-to-area ratio leads to significant perimeter losses, which limit the maximum achievable efficiency and complicates loss analysis for further design optimization. An understanding of perimeter loss mechanisms and design rules for their minimization is therefore important for effectively progressing high efficient tandem solar cells in R&D. In this work we perform a 3D device simulation study of perimeter losses in an exemplary perovskite-silicon tandem solar cell. The impact of structuring the various laterally conducting layers, i.e. at the front and rear side as well as between the sub cells, on the perimeter efficiency loss is systematically investigated. We thereby identify the fundamental mechanisms of carrier transport into the perimeter, and clarify the impact of the silicon and perovskite absorber, the various laterally conducting layers, as well the electrical connection of the sub cells. We find that in each sub cell at least one carrier type must be hindered to be conducted into the perimeter. This is however complicated by the fact that this parasitic lateral transport can also be provided by “bordering” conductive layers connected via the tunnelling or recombination junction. Promising structuring variants and general design rules to achieve low perimeter losses with low structuring effort are then derived from the results. The losses are quantified to range from ~0.2 – 4%abs for a 1 cm2 top cell size, with the lowest values representing an unavoidable loss from the Si wafer conductance which is present also in champion cells with high structuring effort.

27. Improving and expanding Medical Subject Headings (MeSH) during times of social change

Abstract

In the summer of 2020, the Latinx Caucus of the Medical Library Association focused on addressing bias in medical subject heading terminologies by establishing the Hispanic/Latinx Inclusive Terminologies Project. Two project teams were formed. The Medical Subject Headings (MeSH) Project Team’s objective was to recommend inclusive and expanded language within existing medical subject headings to better represent Hispanic/Latinx populations in the United States. From January 2021 to June 2021, the MeSH Project Team conducted a scoping assessment of Hispanic/Latinx terminologies from various sources. The scoping assessment included a review of federal sources, biomedical and health databases, style guides, social media, news articles, and academic literature to review the historical use of Hispanic and Latino and usage trends of gender-neutral terms like Latinx and Latine. In reviewing the scoping assessment and existing MeSH structure for Hispanic and Mexican Americans, the MeSH Project Team recommended two new MeSH terms, Latin Americans and Brazilian Americans, and additional entry terms for existing headings. Team recommendations were submitted to the United States National Library of Medicine (NLM) between June and October 2021. The team also contacted NLM's MeSH Division for guidance on the terms creation and submission process. By systematically evaluating various resources using a scoping assessment as the primary tool and reviewing gaps within medical subject headings, the MeSH Project Team members could propose four recommendations for enhancing Hispanic/Latinx terminologies and influencing changes to existing terms.

Abstract

ABSTRACTThis scoping review examines the role of digital solutions in active, participant-centered surveillance of adverse events following initial release of COVID-19 vaccines. The goals of this paper were to examine the existing literature surrounding digital solutions and technology used for active, participant centered, AEFI surveillance of novel COVID-19 vaccines approved by WHO. This paper also aimed to identify gaps in literature surrounding digital, active, participant centered AEFI surveillance systems and to identify and describe the core components of active, participant centered, digital surveillance systems being used for post-market AEFI surveillance of WHO approved COVID-19 vaccines, with a focus on the digital solutions and technology being used, the type of AEFI detected, and the populations under surveillance. The findings highlight the need for customized surveillance systems based on local contexts and the lessons learned to improve future vaccine monitoring and pandemic preparedness.

Abstract

Since 2011, the Global Energy and Water cycle Exchanges (GEWEX) Water Vapor Assessment (G-VAP) has provided performance analyses for state-of-the-art reanalysis and satellite water vapour products to the GEWEX Data and Analysis Panel (GDAP) and the user community in general. A significant component of the work undertaken by G-VAP is to characterise the quality and uncertainty of these water vapour records to (i) ensure full exploitation and (ii) avoid incorrect use or interpretation of results. This study presents results from the second phase of G-VAP, where we have extended and expanded our analysis of total column water vapour (TCWV) from phase 1, in conjunction with updating the G-VAP archive. For version 2 of the archive, we consider 28 freely available and mature satellite and reanalysis data products, remapped to a regular longitude–latitude grid of 2° × 2° and on monthly time steps between January 1979 and December 2019. We first analysed all records for a “common” short period of 5 years (2005–2009), focusing on variability (spatial and seasonal) and deviation from the ensemble mean. We observed that clear-sky daytime-only satellite products were generally drier than the ensemble mean, and seasonal variability/disparity in several regions up to 12 kg m−2 related to original spatial resolution and temporal sampling. For 11 of the 28 data records, further analysis was undertaken between 1988–2014. Within this “long period”, key results show (i) trends between −1.18 ± 0.68 to 3.82 ± 3.94 kg m−2 per decade and −0.39 ± 0.27 to 1.24 ± 0.85 kg m−2 per decade were found over ice-free global oceans and land surfaces, respectively, and (ii) regression coefficients of TCWV against surface temperatures of 6.17 ± 0.24 to 27.02 ± 0.51 % K−1 over oceans (using sea surface temperature) and 3.00 ± 0.17 to 7.77 ± 0.16 % K−1 over land (using surface air temperature). It is important to note that trends estimated within G-VAP are used to identify issues in the data records rather than analyse climate change. Additionally, breakpoints have been identified and characterised for both land and ocean surfaces within this period. Finally, we present a spatial analysis of correlations to six climate indices within the long period, highlighting regional areas of significant positive and negative correlation and the level of agreement among records.

Abstract

Abstract A search for leptoquark pair production decaying into $$te^- \bar{t}e^+$$ t e - t ¯ e + or $$t\mu ^- \bar{t}\mu ^+$$ t μ - t ¯ μ + in final states with multiple leptons is presented. The search is based on a dataset of pp collisions at $$\sqrt{s}=13~\text {TeV} $$ s = 13 TeV recorded with the ATLAS detector during Run 2 of the Large Hadron Collider, corresponding to an integrated luminosity of 139 fb $$^{-1}$$ - 1 . Four signal regions, with the requirement of at least three light leptons (electron or muon) and at least two jets out of which at least one jet is identified as coming from a b-hadron, are considered based on the number of leptons of a given flavour. The main background processes are estimated using dedicated control regions in a simultaneous fit with the signal regions to data. No excess above the Standard Model background prediction is observed and 95% confidence level limits on the production cross section times branching ratio are derived as a function of the leptoquark mass. Under the assumption of exclusive decays into $$te^{-}$$ t e - ( $$t\mu ^{-}$$ t μ - ), the corresponding lower limit on the scalar mixed-generation leptoquark mass $$m_{\textrm{LQ}_{\textrm{mix}}^{\textrm{d}}}$$ m LQ mix d is at 1.58 (1.59) TeV and on the vector leptoquark mass $$m_{{\tilde{U}}_1}$$ m U ~ 1 at 1.67 (1.67) TeV in the minimal coupling scenario and at 1.95 (1.95) TeV in the Yang–Mills scenario.

35. Development of ion irradiation experimental capabilities and the irradiation response of ZrC

Abstract

The work presented in this thesis describes how an in-situ ion beam with Transmission Electron Microscope (TEM) system was upgraded to allow for the undertaking of experiments that will further the understandings of material science. In order to achieve more accurate experimental results, the MIAMI-1 (Microscopes and Ion Accelerators for Materials Investigations) in situ TEM ion beam line at Huddersfield University was upgraded. The beamline now has direct line of sight with the TEM reducing maintenance and improving alignment. The ion beam line can produce ions of various species. The energies of the ions run from 2-100 keV. This produces fluxes in the range of 1010-1014 ions/cm²/s. A new system for measuring dosimetry has been introduced ensuring that experimental results are accurate and can be compared with the wider literature. Accompanying this was the development of pre-experimental calculation techniques to allow for more accurate experimental parameter setup for use in spherical and cylindrical nanoparticle experiments. These calculations have already been used in the publications of papers and it is hoped that they will continue to be useful for future experiments. The characteristics of ZrC once it had undergone helium ion implantation was also reported on. The formation and density of bubbles in the material were explored. Amorphisation analysis was performed with it being discovered that ZrC amorphises at 600°C under helium ion irradiation at 1 DPA a new discovery for the scientific community.

Abstract

Este artigo teve como objetivo investigar a inserção da Educação Física na área de Linguagens, a partir da análise da produção científica relacionada ao tema. Trata-se de uma revisão bibliográfica, na qual foram analisados 14 artigos selecionados nas bases do Portal de Periódicos Capes e do Google Acadêmico, oriundos de pesquisas bibliográficas/documentais ou da realidade escolar. A partir da Análise Textual, emergiram três categorias: Base Nacional Comum Curricular e a Educação Física na área de linguagens: entre avanços e retrocessos; a busca por pressupostos para sustentar a Educação Física enquanto linguagem, e a Educação Física na área de linguagens: práticas pedagógicas escolares e relatos de professores. Constatamos a necessidade de reavaliar o papel da Educação Física na Área de Linguagens, reconhecendo sua capacidade de enriquecer a educação com perspectivas históricas, culturais e críticas, na construção da cidadania e da consciência social. Quanto aos pressupostos teóricos para a compreensão da Educação Física enquanto linguagem, podemos destacar a busca por sustentação na semiótica, em Bakhtin, na hermenêutica de Gadamer e de Mauro Betti do campo da Educação Física. Apesar desses esforços, as pesquisas no contexto escolar indicam dificuldades na efetivação da Educação Física neste novo espaço curricular, devido à hierarquização de outros componentes, considerados adequados à lógica empregatícia. Portanto, há necessidade de uma abordagem crítica e interdisciplinar, na qual teorias da linguagem e da compreensão de cultura corporal de movimento entrelacem-se, transcendendo a compreensão do corpo para além dos limites biológicos e estendendo-se à expressividade e às diversas manifestações culturais.

38. Should face-to-face in-person therapy be preserved for some clients with anxiety? Evaluation of Anxiety UK's psychological therapy services before and during the COVID-19 pandemic

Abstract

Background The COVID-19 pandemic initiated a mass switch to psychological therapy being delivered remotely, including at Anxiety UK, a national mental health charity. Understanding the impact of this forced switch could raise implications for the provision of psychological therapies going forwards. Aims To understand whether the forced switch to remote therapy had any impact on outcomes, and if certain groups should continue to be routinely offered certain delivery modalities in future. Method Data were available for 2323 individuals who accessed Anxiety UK services between January 2019 and October 2021. Demographic data, baseline and discharge anxiety and depression symptoms, and mode of therapy delivery were available. Regression models were built to model (a) the mode of therapy delivery received pre-pandemic using logistic regression, and (b) outcomes pre- and post-pandemic onset within demographic groups. Results No statistically significant changes in baseline anxiety symptoms, demographics or outcomes were observed before and after the onset of the COVID-19 pandemic. Pre-pandemic, males were more likely to receive online video therapy than telephone therapy (Relative Risk Ratio (RRR) 1.42, [1.01, 1.99]), while older clients were less likely to receive online video therapy (RRR 0.98, [0.97, 0.99]). However, no differences in outcomes were observed post-pandemic onset within these groups, with only the number of sessions of therapy being a significant predictor of outcomes. Conclusions Anxiety UK services remained effective throughout the pandemic. We observed no evidence that any demographic group had worse outcomes following the forced switch to remote therapy.

Abstract

Summary: Background: Group B Streptococcus (GBS) significantly contributes to neonatal sepsis and meningitis, with varying disease rates reported globally and limited population-based data. We estimated infant GBS disease burden in Ontario, Canada and assessed the association of maternal GBS screening (35–37 weeks' gestation) and intrapartum antibiotic prophylaxis (IAP) provision with infant disease rates. Methods: Our population-based cohort study included pregnant individuals and their offspring from April 2012 to March 2018, utilising the provincial birth registry linked to health administrative data. GBS cases were ascertained through culture results and diagnostic codes. We calculated incidence rates for early-onset disease (EOD: 0–6 days), late-onset disease (LOD: 7–89 days), and ultra-LOD (ULOD: 90–365 days). Adjusted incidence rate ratios (aIRR) were derived via log-binomial regression to compare infant GBS rates according to screening and IAP-receipt. Findings: Among 776,148 liveborn infants, we identified 803 with GBS, with multiples exhibiting a threefold incidence increase. Incidence rates of EOD, LOD and ULOD were 0.49, 0.46 and 0.07 per 1000 livebirths, respectively. Of eligible pregnancies, 94% were screened; 23% screened positive, and 81% of them received IAP. Nearly 12% of term EOD infants had mothers who missed IAP despite screening positive. Maternal screening was associated with lower rates of any infant GBS disease (aIRR: 0.60; 95% CI: 0.45, 0.80). Among screen-positive births, IAP-receipt was associated with reduced rates of EOD (aIRR: 0.72, 95% CI: 0.48, 1.29) and LOD/ULOD (aIRR: 0.69; 95% CI: 0.46, 1.05), but confidence intervals included 1.0. Interpretation: Our study, the largest Canadian investigation into infant GBS disease, highlights both widespread adoption and ongoing challenges of the current prevention strategy. Abstract

Purpose The Akrivia Health cohort was created to extract data from electronic health records in secondary mental health and dementia care services in England and Wales. The data are anonymised, structured and harmonised from the source electronic health records across a range of information technology systems, enabling for unified, privacy-preserving access for research purposes.Participants The cohort contains data from electronic health records for over 4.6 million patients in England and Wales, as of January 2024. The data are refreshed with regularity, and the dataset expands whenever a new healthcare provider joins the Akrivia network. 13% of the database are patients under 18 years old (n=590 160), 56% are adults 18–65 years old (n=2 631 690) and 31% are older people (n=1 422 609). About 11.5% are deceased (n=538 371).Findings to date Structured data include patient demographics and service pathways. Akrivia Health also uses a bespoke natural language processing model to further extract the research-relevant information from free-text progress notes, including diagnoses, medications and clinical symptoms. This allows for an in-depth longitudinal description of patient journeys.Future plans The anonymised data can be accessed in collaboration with Akrivia Health, following the National Health Service guidelines and without requiring a separate ethics application. There is no planned end date for data collection.

41. Mind wandering, poor sleep, and negative affect: a threefold vicious cycle?

Abstract

Mind wandering (MW) is intricately linked to sleep and affect, bearing clinical relevance for various psychiatric conditions, notably attention deficit/hyperactivity disorder, depression, and anxiety disorders. Most reviews concur that the relationship between disturbed sleep and negative affect is bidirectional. The directional relationships between MW propensity and disturbed sleep, as well as MW propensity and negative affect, are less clear. Therefore, this brief review aims to examine the limited studies that have directly explored temporally sequential relationships. These studies provide clear evidence for an impact of affect on MW and of MW on sleep, along with less unequivocal evidence for an influence of MW on affect and sleep on MW. Collectively, these individual reinforcement loops may constitute a threefold vicious cycle, which may contribute to the development and perpetuation of psychiatric disorders. Available data convincingly suggest an impact cycle in the direction “MW propensity → disturbed sleep → negative affect → MW propensity,” while evidence for the inverse impact cycle is less pronounced.

42. Occupational exposure and new-onset asthma in the population-based Telemark study: a 5-year follow-up

Abstract

Objectives This study aimed to estimate the incidence of asthma and assess the association between job exposure matrix (N-JEM) assigned occupational exposure, self-reported occupational exposure to vapour, gas, dust and fumes (VGDF), mould, damages from moisture and cold, and new-onset asthma. We also aimed to assess the corresponding population attributable fraction (PAF) for ever exposure to VGDF.Design Longitudinal population-based respiratory health study.Setting Responders from the baseline Telemark Study in south-eastern Norway were followed up from 2013 to 2018.Participants 7120 participants, aged 16–55, were followed during a 5-year period.Main outcome measures New-onset asthma and its association with self-reported occupational exposure to VGDF, data from the N-JEM and self-reported workplace conditions were assessed using logistic regression adjusted for gender, age, smoking and body mass index. The PAF was calculated using the PUNAF command in STATA.Results There were 266 (3.7%) cases of new-onset asthma and an incidence density of 7.5 cases per 1000 person-years. A statistically significant association was found for ever exposed to VGDF with an OR of 1.49 (95% CI 1.15 to 1.94), weekly OR 2.00 (95% CI 1.29 to 3.11) and daily OR 2.46 (95% CI 1.39 to 4.35) exposure to VGDF. The corresponding PAF for ever exposed to VGDF was 17% (95% CI 5.4% to 27.8%) and the risk of asthma onset increased with frequent VGDF exposure, indicating a possible exposure–response relationship (p=0.002 for trend). The N-JEM exposure group, accidental peak exposure to irritants had an increased risk of new-onset asthma, OR 2.43 (95% CI 1.21 to 4.90). A significant association was also found for self-reported exposure to visible damages due to moisture 1.51 (95% CI 1.08 to 2.11), visible and smell of mould 1.88 (95% CI 1.32 to 2.68), 1.55 (95% CI 1.12 to 2.16) and cold environment 1.41 (95% CI 1.07 to 1.86).Conclusion Participants had elevated ORs for asthma associated with self-reported and N-JEM-assigned exposures. A PAF of 17% indicates that work-related asthma is still common. The possible exposure–response relationship suggests that reducing occupational VGDF exposure frequency could prevent the onset of asthma.

Abstract

A pandemia causada pelo novo coronavírus afetou diretamente a saúde mundial. Nesse cenário de enfrentamento da COVID-19, mudanças significativas foram vivenciadas na realidade do trabalho, em especial dos profissionais de saúde atuantes em Unidades de Terapia Intensiva (UTIs) COVID. OBJETIVO: analisar o perfil sociodemográfico e de saúde de profissionais de saúde atuantes em UTIs COVID e suas percepções acerca da pandemia. METODOLOGIA: trata-se de um estudo transversal, analítico, quantitativo e multicêntrico, que foi realizado com profissionais de saúde que atuam nas UTIs COVID da região Missioneira do Rio Grande do Sul. Foi utilizado como instrumento de coleta de dados o questionário sociodemográfico, laboral e clínico, elaborado pelas pesquisadoras e foi realizado análise descritiva dos dados. RESULTADOS: participaram do estudo 205 profissionais de saúde. Desses, 76,6% são do sexo feminino. A faixa etária predominante está entre os 18 a 30 anos (46,3%). Em relação ao estado civil, 105 (51,2%) declararam ter companheiros e, 52,7% têm filhos. CONCLUSÃO: a Enfermagem representa maior percentual de trabalhadores nas Unidades estudadas e, embora atue a pouco tempo na profissão, possuem especialização na área de atuação. Apesar da alta vigilância, do medo de contrair a doença e dos cuidados preventivos, muitos profissionais foram infectados pela COVID-19.

Abstract

Summary:. The incomplete unilateral cleft lip shares many of the same phenotypic characteristics as a complete unilateral cleft lip, but key differences include an intact nasal sill, a relative excess of skin in the lateral lip element, and favorable skeletal support. Surgical techniques for incomplete unilateral cleft lips should be tailored to the specific anatomical characteristics, be simple and reliable to perform, and minimize the risk of secondary deformity. Here we describe the “preservation technique” for incomplete unilateral cleft lip reconstruction. The modified design from the anatomical subunit approximation technique uses a nasal sill flap and obviates the need for nasal sill and lateral lip excision. Tissues of the lip are opened out and reorientated to maximize the anatomical advantage.

Abstract

Richter syndrome (RS) arising from chronic lymphocytic leukemia (CLL) exemplifies an aggressive malignancy that develops from an indolent neoplasm. To decipher the genetics underlying this transformation, we computationally deconvoluted admixtures of CLL and RS cells from 52 patients with RS, evaluating paired CLL-RS whole-exome sequencing data. We discovered RS-specific somatic driver mutations (including IRF2BP2, SRSF1, B2M, DNMT3A and CCND3), recurrent copy-number alterations beyond del(9p21)(CDKN2A/B), whole-genome duplication and chromothripsis, which were confirmed in 45 independent RS cases and in an external set of RS whole genomes. Through unsupervised clustering, clonally related RS was largely distinct from diffuse large B cell lymphoma. We distinguished pathways that were dysregulated in RS versus CLL, and detected clonal evolution of transformation at single-cell resolution, identifying intermediate cell states. Our study defines distinct molecular subtypes of RS and highlights cell-free DNA analysis as a potential tool for early diagnosis and monitoring.

Abstract

BackgroundKlebsiella pneumoniae contains an endogenous isobutanol synthesis pathway. The ipdC gene annotated as an indole-3-pyruvate decarboxylase (Kp-IpdC), was identified to catalyze the formation of isobutyraldehyde from 2-ketoisovalerate.ResultsCompared with 2-ketoisovalerate decarboxylase from Lactococcus lactis (KivD), a decarboxylase commonly used in artificial isobutanol synthesis pathways, Kp-IpdC has an 2.8-fold lower Km for 2-ketoisovalerate, leading to higher isobutanol production without induction. However, expression of ipdC by IPTG induction resulted in a low isobutanol titer. In vitro enzymatic reactions showed that Kp-IpdC exhibits promiscuous pyruvate decarboxylase activity, which adversely consume the available pyruvate precursor for isobutanol synthesis. To address this, we have engineered Kp-IpdC to reduce pyruvate decarboxylase activity. From computational modeling, we identified 10 amino acid residues surrounding the active site for mutagenesis. Ten designs consisting of eight single-point mutants and two double-point mutants were selected for exploration. Mutants L546W and T290L that showed only 5.1% and 22.1% of catalytic efficiency on pyruvate compared to Kp-IpdC, were then expressed in K. pneumoniae for in vivo testing. Isobutanol production by K. pneumoniae T290L was 25% higher than that of the control strain, and a final titer of 5.5 g/L isobutanol was obtained with a substrate conversion ratio of 0.16 mol/mol glucose.ConclusionsThis research provides a new way to improve the efficiency of the biological route of isobutanol production.

Abstract

Abstract A search for CP violation in the decay kinematics and vector-boson fusion production of the Higgs boson is performed in the H → ZZ * → 4ℓ (ℓ = e, μ) decay channel. The results are based on proton-proton collision data produced at the LHC at a centre-of-mass energy of 13 TeV and recorded by the ATLAS detector from 2015 to 2018, corresponding to an integrated luminosity of 139 fb −1. Matrix element-based optimal observables are used to constrain CP-odd couplings beyond the Standard Model in the framework of Standard Model effective field theory expressed in the Warsaw and Higgs bases. Differential fiducial cross-section measurements of the optimal observables are also performed, and a new fiducial cross-section measurement for vector-boson-fusion production is provided. All measurements are in agreement with the Standard Model prediction of a CP-even Higgs boson.
