12. Presencia e influencia de sintomatología sugestiva de trastorno por déficit de atención e hiperactividad en adultos con un trastorno de la conducta alimentaria

Abstract

Fundamento. La coexistencia de trastornos de la conducta alimentaria (TCA) y del trastorno por déficit de atención e hiperactividad (TDAH) es frecuente. El objetivo del estudio fue cuantificar la presencia de sintomatología TDAH en pacientes adultos con TCA y evaluar posibles correlaciones entre ambas sintomatologías. Material y métodos. Estudio transversal en pacientes con TCA. Se cumplimentaron los cuestionarios Eating Attitudes Test de 40 ítems (EAT-40), Eating Disorder Inventory 3ª edición (EDI-3) y la versión abreviada del Adult ADHD Self-Report Scale (ASRS v1.1). Aquellos pacientes con ≥12 puntos en ASRS v1.1 completaron el cuestionario Attention Déficit Hyperactivity Disorder Rating Scale 4ª edición (ADHD-RS IV). Resultados. El 42,6% de los 108 pacientes incluidos puntuaron ≥12 en el ASRS v1.1. Estos sujetos puntuaron más en bulimia y en algunas escalas de desajuste psicológico del EDI-3, y aquellos con conductas de atracón y/o purga puntuaron más en Trastornos psicobiológicos del EAT-40 y en Insatisfacción corporal y Riesgo de TCA del EDI-3. No hubo diferencias en ADHD-RS IV según fueran cuadros restrictivos puros (n=13) o compulsivo-purgativos (n=33). La sintomatología TDAH correlacionó más con la clínica alimentaria y con el desajuste psicológico general en los casos compulsivo-purgativos. Conclusiones. En población clínica con TCA son frecuentes los síntomas sugestivos de TDAH, más en cuadros con clínica compulsivo-purgativa pero no necesariamente más intensos. La correlación entre TDAH y TCA es mayor en cuadros compulsivo-purgativos. Los síntomas de TDAH conllevan mayor desajuste psicológico y mayor gravedad a nivel alimentario en los cuadros con sintomatología compulsivo-purgativa. Background. The coexistence of eating disorders (ED) and attention deficit hyperactivity disorder (ADHD) is common. The aim of the study was to quantify the presence of ADHD symptoms in adult patients with ED and evaluate any possible correlation between both symptomatologies. Methods. Cross-sectional study of patients with ED. The following questionnaires were completed: the Eating Attitudes Test - 40 items (EAT-40), the Eating Disorder Inventory 3rd edition (EDI-3) and the abbreviated version of the Adult ADHD Self-Report Scale (ASRS v1.1). The patients who scored ≥12 points in ASRS v1.1 completed the Attention Deficit Hyperactivity Disorder Rating Scale (4th edition, ADHD-RS IV). Results. 42.6% of the 108 patients scored ≥12 in the ASRS v1.1. These subjects scored higher in bulimia and in some scales of psychological maladjustment of the EDI-3, and those with binging and/or purging behaviours scored higher in Psychobiological disorders of the EAT-40 and in Body dissatisfaction and Eating disorder risk composite scales of the EDI-3. There were no differences in ADHD-RS IV between pure restrictive syndromes (n=13) and those with binging and/or purging behaviours (n=33). We found that, in cases with binge/purge symptoms, ADHD symptoms correlated higher with ED symptoms and with general psychological maladjustment. Conclusion. ADHD symptoms are common in the clinical population with ED, and more in groups with binge/purge symptoms although they are not necessarily more intense. The correlation between ADHD symptoms and ED is higher in cases with binging and/ or purging behaviours. ADHD symptoms involve more psychological maladjustment and more serious ED symptomatology in cases with binge/purge symptoms.

Abstract

Many countries have seen a two-wave pattern in reported cases of coronavirus disease-19 during the 2020 pandemic, with a first wave during spring followed by the current second wave in late summer and autumn. Empirical data show that the characteristics of the effects of the virus do vary between the two periods. Differences in age range and severity of the disease have been reported, although the comparative characteristics of the two waves still remain largely unknown. Those characteristics are compared in this study using data from two equal periods of 3 and a half months. The first period, between 15(th) March and 30(th) June, corresponding to the entire first wave, and the second, between 1(st) July and 15(th) October, corresponding to part of the second wave, still present at the time of writing this article. Two hundred and four patients were hospitalized during the first period, and 264 during the second period. Patients in the second wave were younger and the duration of hospitalization and case fatality rate were lower than those in the first wave. In the second wave, there were more children, and pregnant and post-partum women. The most frequent signs and symptoms in both waves were fever, dyspnea, pneumonia, and cough, and the most relevant comorbidities were cardiovascular diseases, type 2 diabetes mellitus, and chronic neurological diseases. Patients from the second wave more frequently presented renal and gastrointestinal symptoms, were more often treated with non-invasive mechanical ventilation and corticoids, and less often with invasive mechanical ventilation, conventional oxygen therapy and anticoagulants. Abstract

The application of artificial intelligence to chemistry usually focuses on the identification of good correlations between descriptors and a given property of interest. The descriptors often come from arbitrary sets, with the implicit assumption that the evaluation of a sufficiently wide range of descriptors will lead to a satisfactory choice. Recent work in our group has focused on applying statistical analysis to large amounts of DFT results with the goal of finding optimal descriptor sets for a given property, which we label as hidden descriptors. This article briefly discusses this treatment and the chemical knowledge that has been gained through its application in two different domains: metal-ligand bond strength in transition metal complexes, and energy barriers in bimolecular nucleophilic substitution reactions.

Abstract

Obsessive-compulsive disorder (OCD) has been associated with a wide range of biological and neurocognitive findings, which could assist in the search for biomarkers. We conducted an umbrella review of systematic reviews and meta-analyses to assess and grade the strength of the evidence of the association between OCD and several potential diagnostic biomarkers while controlling for several potential biases. Twenty-four systematic reviews and meta-analyses were included, comprising 352 individual studies, more than 10,000 individuals with OCD, and covering 73 potential biomarkers. OCD was significantly associated with several neurocognitive biomarkers, with varying degrees of evidence, ranging from weak to convincing. A number of biochemical, neurophysiological, and neuroimaging biomarkers also showed statistically significant, albeit weak, associations with OCD. Analyses in unmedicated samples (123 studies) weakened the strength of the evidence for most biomarkers or rendered them non-significant. None of the biomarkers seem to have sufficient sensitivity and specificity to become a diagnostic biomarker. A more promising avenue for future biomarker research in OCD might be the prediction of clinical outcomes rather than diagnosis.

19. Relationship between effortful control and facets of mindfulness in meditators, non-meditators and individuals with borderline personality disorder

As dimensions of effortful control (EC), activation control, attentional control, and inhibitory control could mediate the relationship between mindfulness meditation practice and the facets of mindfulness (i.e., observing, describing, acting with awareness, non-judging and non-reacting to inner experience). In this study, we tested whether participant status: meditators (n=330), healthy non-meditators (n=254) and individuals with borderline personality disorder (BPD) diagnosis (n=46) predicted the facets of mindfulness and if these potential effects were mediated through the three effortful control (EC) dimensions (activation, attentional and inhibitory control). Meditators scored higher than non-meditators on attentional and inhibitory control and on the facets of mindfulness with attentional and inhibitory control partially mediating this relationship between meditation status and mindfulness facets. Participants with BPD showed lower scores on EC along with lower scores on the facets of mindfulness compared with healthy non-meditators. All three aspects of EC partially mediated the relationship between BPD and mindfulness facets. Copyright (c) 2018 John Wiley & Sons, Ltd.

20. Exposure induced changes in EEG phase synchrony and entropy: A snake phobia case report

En este estudio de caso único se registró la actividad electroencefalográfica (EEG) de una paciente fóbica a las serpientes una semana antes del tratamiento, una semana después de una exitosa terapia de exposición en una sesión y un año después. El EEG se obtuvo en situación de reposo y durante la presentación de imágenes de serpientes, imágenes con un valor de activación equivalente e imágenes emocionalmente neutras, todas ellas pertenecientes al Sistema Internacional de Imágenes Afectivas. Se midió la entropía (SampEn) de cada señal/canal y la sincronización de fase entre pares de canales EEG. Los resultados mostraron cambios pronunciados una semana después de la intervención: la entropía aumentó en todas las localizaciones y la sincronía de fase decreció en todos los pares de canales analizados. En el seguimiento, la paciente no presentaba ningún miedo pero los patrones de cambio en EEG eran similares a los observados antes de la intervención. A pesar de las limitaciones metodológicas, estos resultados sugieren que los cambios inducidos por la exposición en la entropía y sincronización del EEG son importantes pero pasajeros. El aumento de la flexibilidad cerebral podría ser uno de los mecanismos neuropsicológicos de la terapia de exposición.

21. HDL-c levels predict the presence of pleural effusion and the clinical outcome of community-acquired pneumonia

Objectives: To investigate if HDL cholesterol (HDL-c) could be a biomarker of the degree of severity according to prognostic prediction scores in community-acquired pneumonia (CAP) or the development of clinical complications such as pleural effusion. Methods: We included in a retrospective study 107 patients admitted to the hospital that fulfilled diagnostic criteria for CAP between the 30th October 2011 and 1st September 2012. HDL-c levels at admission, CAP prognosis scores (PSI and CURB65) and clinical outcomes were recorded for the study. Results: Basal HDL-c levels were not statistically different according to prognostics scores neither PSI nor CURB-65. Significantly lower levels of HDL-c were also associated to the development of septic shock and admission to the intensive care unit. HDL-c were inversely correlated with acute phase reactants CRP (r = −0.585, P < 0.001), ESR (r = −0.477, P < 0.001), and leukocytes cell count (r = −0.254, P < 0.009). Patients with pleural effusion showed significant lower levels of HDL-c [28.9 (15.5) mg/dl vs. 44.6 (21.1) mg/dl]; P = 0.007. HDL-c is a good predictor of the presence of pleural effusion in multivariate analyses and using ROC analyses [AUC = 0.712 (0.591–0.834), P = 0.006]. HDL-c levels of 10 mg/dl showed a sensitivity of 97.6 % and a specificity of 82.4 % for the presence of pleural effusion. Conclusion: Monitoring HDL-c in CAP is an useful serum marker of acute phase response, clinical outcome and the presence of pleural effusion.

Arts és una revista sociocultural que es fa des del Cercle de Belles Arts de Lleida amb la vocació de ser una eina de difusió i de reflexió al servei d'Allò Artístic. Així de clar i, alhora, així d'indeterminat. Perquè la precisió és límit i Arts està renyida amb els límits. No ens limitem ideològicament perquè el fet artístic és massa complex per anar delimitant els marges i massa lliure per anar col·locant barreres mentals que acaben esdevenint modes. Tampoc ens limitem geogràficament, perquè menyspreem tant el provincianisme atemorit i carrincló de botigueta com l'universalisme uniformitzador i amorf amb que es vesteixen els cosmopolites de cafè. No renunciem a entendre el món de l'art des del ponent català, el nostre localisme ens globalitza. Us oferim, doncs, un projecte obert però, al mateix temps, planificat, en el qual contingut, imatge i difusió han de funcionar com un sol ens, únic i indivisible, i fer d'aquesta revista un veritable instrument de comunicació (sense matisacions de cap mena).
