Summary The large and increasing burden of stroke in Latin American countries, and the need to meet the UN and WHO requirements for reducing the burden from non-communicable disorders (including stroke), brought together stroke experts and representatives of the Ministries of Health of 13 Latin American countries for the 1st Latin American Stroke Ministerial meeting in Gramado, Brazil, to discuss the problem and identify ways of cooperating to reduce the burden of stroke in the region. Discussions were focused on the regional and country-specific activities associated with stroke prevention and treatment, including public stroke awareness, prevention strategies, delivery and organisation of care, clinical practice gaps, and unmet needs. The meeting culminated with the adoption of the special Gramado Declaration, signed by all Ministerial officials who attended the meeting. With agreed priorities for stroke prevention, treatment, and research, an opportunity now exists to translate this Declaration into an action plan to reduce the burden of stroke.

Introdução. Há evidências de que a desigualdade social pode influenciar o prognóstico do câncer quando foram comparadas as taxas de sobrevida segundo o status socioeconômico, entretanto isso ainda não foi estudado no Brasil. Objetivo. Analisar a desigualdade social na determinação da sobrevida específica em um, três e cinco anos, de pacientes diagnosticados com câncer de boca e orofaringe em São Paulo. Metodologia. Utilizou-se a coorte de base hospitalar do grupo de pesquisa Genoma Clínico do Câncer de Cabeça e Pescoço (GENCAPO), à qual foram aplicados novos critérios de exclusão. Assim, foram analisados dados demográficos, socioeconômicos, comportamentais, sintomas autorreferidos e de condições clínicas de 1.154 pacientes, com diagnóstico de malignidade nas regiões de boca (C00.3-C06) e orofaringe (C09-C10), arrolados de 2001 a 2009 e acompanhados por cinco anos. A desigualdade social foi aferida pela escolaridade e a ocupação. As probabilidades acumuladas de sobrevida específica em um, três e cinco anos foram calculadas pelo método de Kaplan-Meier e as diferenças entre as curvas de sobrevida, pelo teste de Log-rank. A regressão de Cox univariada e múltipla possibilitou a análise dos fatores associados à sobrevida - Hazard Ratio (HR) com IC95%. Resultados. As probabilidades acumuladas de sobrevida específica em um, três e cinco anos para câncer de boca e orofaringe, foram de 74,72%, 49,86 e 42,46%, respectivamente. Baixa escolaridade (HR=1,25; IC95%=1,03-1,51) e realização de trabalhos manuais (HR=1,37; IC95%=1,05-1,78) foram associados à menor probabilidade de sobrevivência em cinco anos na análise não ajustada. A cor da pele preta/parda, tumor de tamanho T3 ou T4 e comprometimento de linfonodos foram fatores independentemente associados à menor sobrevida específica por câncer de boca e orofaringe em um, três e cinco anos. Disfagia e dificuldade respiratória foram associados à menor sobrevida específica em um ano. Em três anos, observou-se também a otalgia e em cinco anos, a dificuldade de deglutição. Conclusões. Foi identificada associação entre a desigualdade social, menor escolaridade e trabalho manual, à menor sobrevida em 5 anos. Esses fatores, na análise ajustada pelas características demográficas, tiveram sua associação potencialmente explicada pela cor da pele preta/parda. Introduction. There is evidence that social inequality may influence the prognosis of cancer when comparing survival rates according to socioeconomic status, but this has not been studied in Brazil. Aim. To evaluate social inequality in the determination of one, three and five years specific survival of patients diagnosed with oral and oropharyngeal cancer in São Paulo. Methodology. A hospital-based cohort of the research group \"Clinical Genome of Head and Neck Cancer\" (GENCAPO) was used, applying new exclusion criteria. Demographic, socioeconomic, behavioral, selfreported symptoms and clinical conditions characteristics of 1,154 patients, diagnosed with malignancy in the mouth (C00.3-C06) and oropharynx (C09-C10) regions, included from 2001 to 2009 and followed up for five years, were analyzed. Social inequality was measured by schooling and occupation. The cumulative probabilities of one, three, and five years specific survival were calculated by the Kaplan-Meier method and differences between the survival curves by the log-rank test. Univariate and Multiple Cox Regression allowed to analyze the factors associated with survival - Hazard Ratio (HR) with 95% CI. Results. The accumulated probabilities of one, three and five years specific survival for oral and oropharyngeal cancer were 74.72%, 49.86 and 42.46%, respectively. Low schooling (HR = 1.25, 95% CI = 1.03-1.51) and manual work (HR = 1.37, 95% CI = 1.05-1.78) were associated with a lower survival probability at five years in the unadjusted analysis. Dysphagia and breathing difficulties were associated with lower one year specific survival. At three years, it was also observed earache, and at five years, swallowing difficulty. Conclusions. An association among social inequality, lower schooling and manual labor, with the lowest survival at 5 years, was identified. These results were potentially explained by black/brown skin color in the adjusted analysis for demographic characteristics.

OBJETIVO: Investigar os fatores associados à desigualdade no uso de serviços de saúde bucal da Estratégia Saúde da Família (ESF)medida pelo indicador realização de exodontia. MÉTODOS: Realizou-se um inquérito populacional de base domiciliar em dois municípios do Estado da Bahia, Brasil, com 100% de cobertura da ESF. Foram sorteados 10 setores censitários. Os domicílios foram definidos de forma sistemática. Para estimar a utilização dos serviços públicos, consideraram-se prevalência de 50%, erro amostral de 4% e intervalo de confiança de 95%. Agentes comunitários de saúde treinados entrevistaram informantes-chave, que responderam a questões sociodemográficas e de utilização dos serviços odontológicos em nome de todos os moradores do domicílio com 15 anos ou mais. Considerou-se a exodontia como desfecho principal. A razão de prevalência foi usada para aferir a associação entre exodontia e as variáveis preditoras. A razão de chances (odds ratio, OR) foi usada na análise de regressão logística multivariada. RESULTADOS: Foram coletados dados de 2 539 indivíduos, dos quais 682 (26,86%) haviam utilizado o serviço de saúde bucal da atenção básica nos últimos 12 meses. A exodontia foi realizada em 218 (31,96%) dos 682 pacientes da atenção básica. Receber benefício social (OR = 1,43; IC95%: 1,02 a 2,01), ter mais de 35 anos (OR = 1,59; IC95%: 1,12 a 2,27) e menor nível de escolaridade (OR = 1,81; IC95%: 1,27 a 2,56) foram variáveis associadas independentemente ao desfecho. CONCLUSÕES: Os resultados apontam para a permanência de desigualdades históricas na utilização dos serviços de saúde bucal. Ações intersetoriais podem ser importantes para a melhoria da saúde bucal.

Este estudo de corte transversal descreve as principais características de acesso e utilização de serviços odontológicos básicos e especializados em dois municípios de médio porte da Bahia com 100% de cobertura da Estratégia de Saúde da Família. Realizou-se um inquérito com 2.539 indivíduos de idade acima de 15 anos em 952 domicílios. As principais variáveis analisadas foram a necessidade de saúde bucal percebida, a procura pelo serviço, a barreira de acesso organizacional e o tipo de serviço e o procedimento utilizado. A utilização de serviços públicos odontológicos especializados foi de 11,7% e básicos de 26%. No município com maior utilização de serviços públicos odontológicos, foi menor o uso de serviços privados. A principal barreira de acesso situou-se na AP (Atenção Primária), entre 5,0% e 15,2%. Observou-se pouca interface da atenção secundária com a AP já que apenas 16,6% dos usuários retornaram a esse nível de atenção. Pode-se concluir que a principal barreira de acesso, em municípios com realidades organizacionais e geográficas específicas parece se situar na AP. Ações preventivas individuais foram pouco relatadas. Recomenda-se a redução de barreiras de acesso na AP, bem como a instituição de um protocolo que estimule a longitudinalidade do cuidado nesse nível.This cross-sectional study sought to describe the main characteristics of access and utilization of primary and specialized public dental services in two medium-sized cities in Bahia with 100% coverage of the Family Health Program. A survey of 952 households and 2.539 individuals aged over 15 years was conducted. The main variables analyzed were: perceived oral health needs, demand for the service, barriers of organizational access and the type of service and procedure utilized. The use of specialized public dental services was of 11.7% and primary care was 26%. In the city where there was greater use of public dental services, there was less use of private services. The main barrier to access remained in primary care (from 5.0% to 15.2%). There was little interface between secondary care a primary care, as only 16.6% of users returned to this level of care. In conclusion, the main barrier to access in cities with specific organizational and geographic realities appears to be in primary dental care. Individual preventive actions were little reported. It is recommended that barriers to access in primary care be eliminated, and also that a protocol (clinical guides) be established to foster the continuity and longitudinality of primary dental care.

Tooth bleaching technique has presented a significant evolution, promoting higher satisfaction and comfort to the patients. Therefore, the aim of this study was to present the bleaching agents and the techniques, discussing advantages and disadvantages of each one, and the effect of these agents in the oral environment. The main agents used in the bleaching technique are the hydrogen peroxide and the carbamide peroxide, promoting the bleaching effect through oxidation of organic compounds. The application of these agents can be made at home or at a doctor office. During treatment, it may occur some adverse effects, such as tooth sensibility, increasing of dental porosity, and some interactions with the restorative material. However, these adverse effects can be eliminated or controlled when the treatment is executed under professional orientation. When the bleaching technique is well indicated and correctly conducted, it is associated with significantly positive results.

The aim of this study was to identify factors associated with the use of primary and specialized public dental health services and private services. A population-based household survey was conducted in two cities of Bahia State, Brazil. Key informants provided data on socioeconomic variables and use of dental health services. Organization of the local public dental health service was ranked as worse versus better. Univariate and multivariate polytomous logistic regression was performed. Of the total of 1,290 individuals, 38.76% used private services, 33.80% used public primary care, and 17.29% used both primary care and the Center for Dental Specialties. Less use of both primary care and specialized public services was associated with lower education (OR = 1.47; 95%CI: 1.03-2.10) and worse organization of services (OR = 1.74; 95%CI: 1.22-2.48), when compared to the exclusive use of primary care. The study showed inequality in the use of dental services, even when comparing more homogeneous groups, namely users of public services.

To investigate the factors associated with inequality in the use of oral health services in the Brazilian Family Health Program (Estratégia Saúde da Família, ESF) as measured by dental extractions.A population-based household survey was carried out in two municipalities with full coverage by the ESF in the state of Bahia, Brazil. Ten census tracts were randomly selected. Households were selected by systematic sampling. To assess the use of public services by the population, a prevalence of 50%, sampling error of 4%, and confidence interval of 95% were considered. Trained community health workers conducted key informant interviews on use of oral health services and collected demographic data for all household members aged 15 years or older. Dental extraction was the main outcome. The association between dental extraction and predictive variables was assessed using prevalence ratio, and multivar logistic regression analysis was performed using odds ratio (OR).Data from 2,539 people was collected. Of these, 682 (26.86%) had used primary oral health care services in the previous 12 months. Dental extraction was performed in 218 (31.96%) out of 682 primary care patients. Being assisted by social programs (OR = 1.43; IC95%: 1.02-2.01), being older than 35 years (OR = 1.59; IC95%: 1.12-2.27), and having lower education levels (OR = 1.81; IC95%: 1.27-2.56) were independently associated with the outcome.The present results suggest that historical inequalities of access to oral health services persist. Intersectoral actions may play an important role in improving oral health conditions.
