1. Regional variation in urinary catheter use in the Netherlands from 2012 to 2021: a population-based cohort

Abstract

Objectives: Our aim was to evaluate trends and regional differences in the use of indwelling and intermittent urinary catheters in the community setting in the Netherlands from 2012 to 2021. Design and methods: For this population-based cohort study, data on catheter use was collected from the Drug and Medical Devices Information System of the National Healthcare Institute of the Netherlands. This database contains information on the Dutch insured population, which was 100% of the total population in 2018. Users were divided into 12 provinces according to the Nomenclature of Territorial Units for Statistics codes. The number of catheter users was adjusted for the total population of the provinces by sex and age, and was expressed by users per 100,000 people. Negative binomial regression (NBR) was used to test for differences in clean intermittent catheter (CIC) and indwelling catheter (IDC) users across Dutch provinces. Results: Between 2012 and 2021, IDC users increased by 44.6% from 41,619 to 60,172, and CIC users increased by 27.3% from 34,204 to 43,528. The greatest increases were mainly observed among IDC users over 85 years old and male CIC users over 65 years old. NBR showed significant differences for IDC and CIC users between the 12 provinces. CIC incidence was higher in Drenthe and Groningen (Northern Netherlands) compared to Zuid-Holland (Southern Netherlands). IDC incidence was higher in seven provinces dispersed throughout the Netherlands compared to Noord-Holland. Conclusion: CIC and IDC users have continued to increase in recent years; this was especially observed among older men. In addition, there were regional differences in the number of CIC and IDC users; CIC was more prominent in the northern region of the Netherlands, and IDC varied between multiple provinces. Practice variation in urinary catheterization may result from patient population differences or healthcare provider preferences and their alignment with guidelines.

IntroductionCatheter-associated urinary tract infection (CAUTI) is a common complication among patients with urinary catheters and is often treated with antibiotics. With increasing rates of antibiotic resistance, it is necessary to explore alternative treatment options for CAUTIs. The aims of this study were 1) to assess the efficacy and treatment satisfaction of bladder irrigation (BI) with tap water to prevent and treat CAUTIs, 2) and to evaluate the current use of BI for CAUTIs among Dutch clinicians.MethodsThe first part of this study consisted of a cross-sectional study among patients with intermittent or indwelling catheters who performed BI with tap water between March 2020 and May 2021. Efficacy, treatment satisfaction, and Patient Global Impression of Improvement (PGI-I) were assessed using questionnaires. Outcomes were compared between neurogenic lower urinary tract dysfunction (NLUTD) and non-NLUTD patients. Factors associated with overall treatment satisfaction were determined using logistic regression analysis. Second, a nationwide survey of Dutch clinicians was conducted to evaluate the current use of BI for CAUTIs.ResultsA total of 99 patients who were performing BI for at least three months were included. The median age was 61.9 years, 41.4% had NLUTD, and 72.2% performed BI >1 year. The majority of both NLUTD (65.9%) and non-NLUTD patients (68.4%) were (very) satisfied with BI. Women had higher odds of reporting higher satisfaction and each additional CAUTI decreased the odds. Most NLUTD (85.4%) and non-NLUTD (65.5%) patients reported an improvement on the PGI-I with a difference in favour of NLUTD patients (p=0.002). In addition, 40.4% of the patients had no CAUTI, and 59.6% reported 1.39 (SD 2.06) CAUTIs. Only half of these self-reported CAUTIs were treated with antibiotics. In addition, 33 (58.9%) clinicians used BI for CAUTIs, of which ten used tap water as irrigation agent.DiscussionThis study provides first evidence supporting the efficacy of BI with tap water in the treatment of CAUTIs and reducing the use of antibiotics. Patients are overall satisfied and experience improvement in their condition with BI. In addition, the majority of the surveyed Dutch clinicians use BI for CAUTIs. However, irrigation with tap water is still not widely used.

Abstract

Abstract Purpose Local anesthesia is the most used anesthetic technique for inguinal hernia repair, despite its unpredictability. Selective spinal anesthesia with a short-term local anesthetic guarantees rapid recovery, predictable duration and low incidence of side effects. We tried to assess the efficacy of this neuraxial technique in ambulatory setting. Methods One hundred thirty-two ASA I–III, aged > 18 patients scheduled for inguinal hernia repair have been randomized into two groups receiving unilateral spinal anesthesia with 40 mg of hyperbaric prilocaine (group A) or local anesthesia with mepivacaine (group B). Primary endpoint: intraoperative and post-operative NRS. Other outcomes: sensory block onset, need for opiates and deep sedation, surgery duration, and time to discharge. Results Group A: intraoperative NRS was 0 in 100% of patients; post-operative maximum NRS was > 3 in 12.12% of patients. Group B: mean intraoperative NRS was 4; mean post-operative NRS was 2.5. Spinal anesthesia resulted superior in controlling both intraoperative and post-operative pain (p < 0.00001; p = 0.008). Mean time of the motor block resolution in group A was 98 ± 2 min. Mean time to discharge was not significantly different between groups. Surgical time was significantly different between the two groups (mean time of 37 ± 3.2 min group A; 54 ± 6 min group B—p < 0.00001). Conclusion Spinal anesthesia group patients had significantly less pain than local anesthesia group, both intraoperatively and post-operatively, without differences in time to discharge, incidence of complications and with improvement of surgical time. Abstract

Background: Videolaryngoscopy (VLS) is recommended by international guidelines for the management of difficult airways. We conducted an observational prospective pilot study to assess the efficacy of the new VL3 videolaryngoscope for routine tracheal intubation (TI) in adults; in terms of success rate, the number of attempts, and maneuver duration, including both normal and difficult airways. Methods: This prospective observational pilot study comprised a sample of 56 adult patients undergoing elective general anesthesia. For each patient, we performed VLS by VL3 recording the following data: successful TI rate, number of attempts, time of intubation, time to glottis visualization, Cormack-Lehane grade (CL), need for external laryngeal pressure, and presence of post-laryngoscopy side effects. Results: TI was successfully carried out in the totality of patients. In only 4 out of 56 cases, the VL3 offered a CL II. The first attempt intubation was achieved in 48 patients (85.7%). In one case, external laryngeal pressure was needed. No CL III or CL IV were observed. We did not find any significant difference between the predicted difficult airways sample and the rest of the population. Conclusion: VL3 videolaryngoscope showed to be an effective and safe device for routine TI, even in those patients with predicted difficult airway. More studies are needed to confirm our findings and verify its efficacy even in other settings.

Abstract

Introduction: Spinal anesthesia is a technique performed since more than a century and the introduction of hyperbaric anesthetics allowed the anesthesiologists to be more selective when using this technique. The aim of this study is to show the in vitro flow patterns of a hyperbaric dye solution through 27 G Quincke and Sprotte spinal needles, injected at different speeds, in a lower-density fluid. Methods: A simulator was made using a gummy-like sponge and a disposable plastic urine glass, filled with saline solution, which has a similar density to cerebrospinal fluid (CSF). A hyperbaric dye solution was composed by mixing 3 ml of plain methylene blue with 1 ml of glucose 33%. We used both 27 G Quincke and Sprotte spinal needles to perform a bevel up and a bevel down injection with both slow (15 s) and fast (4 s) injection speed of 0.5 mL hyperbaric dye solution. All the injections were performed using a preset syringe pump and recorded by a camera. Results: The least selectivity was observed after a bevel up-fast injection through the 27 G Sprotte needle, followed by both bevel up and down fast injections through the 27 G Quincke needle. On the contrary, the best selectivity was observed after a bevel down-slow injection through the 27 G Sprotte needle, followed by both bevel up and down slow injections through the 27 G Quincke needle. Conclusion: When a 27 G Sprotte needle is used to inject a hyperbaric solution in a lower-density fluid-like CSF, the spread depends on both the bevel direction and the injection speed.

Abstract

The management of old trees suffering from precarious health state and biomechanical issues represents a very common task for many cities in Italy. It is not possible to determine when or whether trees are going to fall, but renewal can be planned to prevent expensive management and monitoring practices in the face of poor biological, ornamental and architectural results, as well as of high levels of risk. To be precise, while the ornamental and functional services exhaust, the emotional value gets fuelled and surges over time by citizens' affection. Given this, renewal plans for tree-lined avenues are not welcomed and the lack of an effective, scientific-based communication could exacerbate the problem. The search for a shared method represents an objective starting line to assess and make far-sighted decisions. The work illustrates a method for evaluating and managing old and/or dangerous trees in urban environment so that citizens are involved too. Economic and environmental aspects are analyzed by comparing the safeguard of an old tree line both to simultaneous or gradual renewal plans. Here we focus on the extent of the environmental services brought in urban areas by the diversely managed tree lines, estimating their leaf area indexes along 10-year span. Basing on results, Public Administrations can show citizens clear and objective data making management choices and strategies easier to share and agree with. Even though necessary and fundamental, the research for a comprehensive assessment of economic, social and environmental aspects related to tree lines is poorly explored.

27. Linezolid Intoxication with Extreme Lactate Blood Levels Successfully Treated with Dialytic Treatment in ICU: A Case Report

Abstract

Introduction: Lactic acidosis is a rare but life-threatening complication associated with pro-longed linezolid therapy. No specific treatment is suggested, except for antibiotic therapy interruption. Case Report: A 70-years-old woman faced severe linezolid intoxication after antibiotics therapy initi-ation for infection of a surgical sternal wound. The patient suffered from a severe increment of blood lactate and thrombocytopenia. She was admitted to ICU twice, and due to dialytic treatment, linezolid and lactate serum levels came back to normality. Conclusion: More studies should be conducted to evaluate the human tissue storage sites of linezolid and the influence of various factors on its clearance and plasma concentrations in critically ill patients.

Abstract

Background and aims: To validate a set of indicators for monitoring the quality of care of patients with diabetes in ‘real-life’ practice through its relationship with measurable clinical outcomes and healthcare costs. Methods and results: A population-based cohort study was carried out by including the 20,635 patients, residents in the Lombardy Region (Italy), who in the year 2012 were newly taken-in-care for diabetes. Adherence with clinical recommendations (i.e., controls for glycated haemoglobin, lipid profile, urine albumin excretion and serum creatinine) was recorded during the first year after the patient was taken-in-care, and categorized according whether he/she complied with none or almost none (0 or 1), just some (2) or all or almost all (3 or 4) the recommendations, respectively denoted as poor, intermediate and high adherence. Short- and long-term complications of diabetes, and healthcare cost incurred by the National Health Service, were assessed during follow-up. Compared with patients with poor adherence, those with intermediate and high adherence respectively showed (i) a delay in outcome occurrence of 13 days (95% CI, −2 to 27) and 23 days (9–38), and (ii) a lower healthcare cost of 54 € and 77 €. In average, a gain of 18 Euros and 15 Euros for each day free from diabetic complication by increasing adherence respectively from poor to intermediate and from poor to high were observed. Conclusion: Close control of patients with diabetes through regular clinical examinations must be considered the cornerstone of national guidance, national audits, and quality improvement incentive schemes.

37. Influence of adherence with guideline-driven recommendations on survival in women operated for breast cancer: Real-life evidence from Italy

Abstract

Background: A set of indicators to assess the quality of care for women operated for breast cancer was developed by an expert working group of the Italian Health Ministry in order to compare the Italian regions. A study to validate these indicators through their relationship with survival was carried out. Methods: The 16,753 women who were residents in three Italian regions (Lombardy, Emilia-Romagna and Lazio) and hospitalized for breast cancer surgery during 2011 entered the cohort and were followed until 2016. Adherence to selected recommendations (i.e., surgery timeliness, medical therapy timeliness, appropriateness of complementary radiotherapy and mammographic follow-up) was assessed. Multivariable proportional hazards models were fitted to estimate hazard ratios for the association between adherence with recommendations and the risk of all-cause mortality. Results: Adherence to recommendations was 53% for medical therapy timeliness, 73% for appropriateness of mammographic follow-up, 74% for surgery timeliness and 82% for appropriateness of complementary radiotherapy. Risk reductions of 26%, 62% and 56% were observed for adherence to recommendations on medical therapy timeliness, appropriateness of complementary radiotherapy and mammographic follow-up, respectively. There was no evidence that mortality was affected by surgery timeliness. Conclusions: Clinical benefits are expected from improvements in adherence to the considered recommendations. Close control of women operated for breast cancer through medical care timeliness and appropriateness of radiotherapy and mammographic monitoring must be considered the cornerstone of national guidance, national audits, and quality improvement incentive schemes.

In the Enhanced Recovery After Surgery era, parasternal intercostal nerve block has been proposed to improve pain control and reduce opioid use in patients undergoing cardiac surgery. However current literature has reported conflicting evidence about the effect of this multimodal pain management, as procedural variations might pose a significant bias on outcomes evaluation. In this setting, the infiltration of the parasternal plane into 2 intercostal spaces, second and fifth, with a local anesthetic spread under or above the costal plane with ultrasound guidance, seem to be standardized in theory, but significant differences might be observed in clinical practice. This narrative review summarizes and defines the optimal techniques for parasternal plane blocks in patients undergoing cardiac surgery with full median sternotomy, considering both pectointercostal fascial block and transversus thoracic plane block. A total of 10 randomized trials have been published, in adjunct to observational studies, which are heterogeneous in terms of techniques, methods, and outcomes. Parasternal block has been shown to reduce perioperative opioid consumption and provide a more favorable analgesic profile, with reduced postoperative opioid-related side effects. A trend toward reduced intensive care unit stay or duration of mechanical ventilation should be confirmed by adequately powered randomized trials or registry studies. Differences in operative technique might impact outcomes and, therefore, standardization of the procedure plays a pivotal role before reporting specific outcomes. Parasternal plane blocks might significantly improve outcomes of cardiac surgery with full median sternotomy, and should be introduced comprehensively in Enhanced Recovery After Surgery protocols.

39. Multilingualism and language choice in the imperial Roman army

Abstract

This thesis is a sociolinguistic study of language choice in the Roman army, focused on the period between the first and the third centuries AD. Its main aims are to revisit our understanding of language choice in military administration, and to identify patterns of language choice in communication between soldiers and their associates. These themes are approached through a tripartite framework inspired by discourse analysis, which includes the concepts of audience, identity, and practicality. The case studies examined in the thesis show that Latin was not the only language of military administration, but played a vital role in written contexts where the army acted in its institutional function. This “institutional” use of Latin is reflected at the interpersonal level, where it enabled soldiers and their associates to express belonging to the military experience. Latin and other languages appear also as lingue franche and as markers of different types of identities, often connected with questions of prestige. Overall, this thesis enables us to refine our understanding of the reasons that prompted soldiers and their associates to use one language instead of another when a choice was available, and to renew our appreciation for cross-linguistic communication in Roman military environments.

Abstract

Abstract Objectives To assess the incidences of Herpes Simplex-1 and 2 (HSV-1, HSV-2), Cytomegalovirus (CMV), Epstein–Barr Virus (EBV) reactivations in critically ill COVID-19 patients. To determine the association between viral reactivation and in-hospital mortality, Intensive Care Unit Bloodstream infection (ICU–BSI), ventilator-associated pneumonia (VAP). Design Observational retrospective cohort study. Setting COVID-19 Intensive Care Unit. Patients From November 2020 to May 2021, one hundred and twenty patients with COVID-19 severe pneumonia were enrolled and tested for HSV-1, HSV-2, CMV and EBV at the admission in ICU and weekly until discharge or death. The presence of VAP and ICU–BSI was evaluated according to clinical judgement and specific diagnostic criteria. Measurements and main results One hundred and twenty patients were enrolled. Multiple reactivations occurred in 75/120 (63%) patients, single reactivation in 27/120 patients (23%). The most reactivated Herpesvirus was EBV, found in 78/120 (65%) patients. The multivariate analysis demonstrated that viral reactivation is a strong independent risk factor for in-hospital mortality (OR = 2.46, 95% CI 1.02–5.89), ICU–BSI (OR = 2.37, 95% CI 1.06–5.29) and VAP (OR = 2.64, 95% CI 1.20–5.82). Conclusions Human Herpesviruses reactivations in critically ill patients with COVID-19 severe Pneumonia are associated with mortality and with a higher risk to develop both VAP and ICU–BSI.

43. Regional variation in urinary catheter use in the Netherlands from 2012 to 2021: a population-based cohort.

Abstract

Objectives: Our aim was to evaluate trends and regional differences in the use of indwelling and intermittent urinary catheters in the community setting in the Netherlands from 2012 to 2021. Design and methods: For this population-based cohort study, data on catheter use was collected from the Drug and Medical Devices Information System of the National Healthcare Institute of the Netherlands. This database contains information on the Dutch insured population, which was 100% of the total population in 2018. Users were divided into 12 provinces according to the Nomenclature of Territorial Units for Statistics codes. The number of catheter users was adjusted for the total population of the provinces by sex and age, and was expressed by users per 100,000 people. Negative binomial regression (NBR) was used to test for differences in clean intermittent catheter (CIC) and indwelling catheter (IDC) users across Dutch provinces. Results: Between 2012 and 2021, IDC users increased by 44.6% from 41,619 to 60,172, and CIC users increased by 27.3% from 34,204 to 43,528. The greatest increases were mainly observed among IDC users over 85years old and male CIC users over 65years old. NBR showed significant differences for IDC and CIC users between the 12 provinces. CIC incidence was higher in Drenthe and Groningen (Northern Netherlands) compared to Zuid-Holland (Southern Netherlands). IDC incidence was higher in seven provinces dispersed throughout the Netherlands compared to Noord-Holland. Conclusion: CIC and IDC users have continued to increase in recent years; this was especially observed among older men. In addition, there were regional differences in the number of CIC and IDC users; CIC was more prominent in the northern region of the Netherlands, and IDC varied between multiple provinces. Practice variation in urinary catheterization may result from patient population differences or healthcare provider preferences and their alignment with guidelines. [ABSTRACT FROM AUTHOR]

Abstract

Purpose Evidence of an increased diagnostic pressure on thyroid has emerged over the past decades. This study aimed to provide estimates of a wide spectrum of surveillance indicators for thyroid dysfunctions and diseases in Italy. Methods A population-based study was conducted in North-eastern Italy, including 11.7 million residents (20% of the total Italian population). Prescriptions for TSH testing, neck ultrasound or thyroid fine needle aspiration (FNA), surgical procedures, and drugs for hypo- or hyperthyroidism were extracted from regional health databases. Proportions and rates of selected examinations were calculated from 2010 to 2017, overall and by sex, calendar years, age, and region. Results Between 2010 and 2017 in North-eastern Italy, 24.5% of women and 9.8% of men received at least one TSH test yearly. In 2017, 7.1% of women and 1.5% of men were prescribed drugs for thyroid dysfunction, 94.6% of whom for hypothyroidism. Neck ultrasound examinations were performed yearly in 6.9% of women and 4.6% of men, with a nearly two-fold variation between areas. Thyroid FNA and thyroidectomies were three-fold more frequent in women (394 and 85 per 100,000) than in men (128 and 29 per 100,000) with a marked variation between areas. Both procedures decreased consistently after 2013. Conclusions The results of this population-based study describe recent variations over time and between surrounding areas of indicators of ‘diagnostic pressure’ on thyroid in North-eastern Italy. These results emphasize the need to harmonize practices and to reduce some procedures (e.g., neck ultrasound and total thyroidectomies) in certain areas.
