Bone metastases from HCC are infrequent, with a poorer prognosis. However, the survival influencing factors are not yet well understood.Aim: The aim of the present study was to assess the clinical features and tumor characteristics of HCC patients with bone metastasis.Methods: A cohort of 170,576 adult patients with HCC was studied using the National Cancer Database (NCDB) spanning from 2010 to 2019, and within this group, 5285 patients (3.1%) were diagnosed with bone metastasis. We performed the Kaplan–Meier method to calculate the median overall survival (OS). We included demographics (age at diagnosis, gender, race, insurance status), comorbidity score, and treatment characteristics.Results: Of a total of 5285 HCC patients with bone metastasis, 86.2% were male and 61.2% were non-Hispanic white. Most patients (55.1%) were below 65, and 89% had a total Charlson-Deyo comorbidity score of under 3. Among patients with known tumor grade, 24.8% had well-differentiated tumors, and 36.1% had poorly differentiated tumors. Chemotherapy was administrated to 39.5% of patients. In univariate analysis, patients with well-differentiated tumors had better OS compared to poorly differentiated tumors (5.4 months vs 3.0 months, p = 0.001). Patients who received single or multiagent chemotherapy were significantly associated with improved OS compared to patients who did not receive chemotherapy (7.0 and 8.5 months vs 1.94 months, respectively). We also found mortality difference between age, comorbidity scores, facility types and race groups.Conclusion: In this cohort analysis of NCDB data, we found better OS in treatment receipt, lower tumor grade, younger age, non-Hispanic Black and Hispanic race, treatment at academic facility and lower comorbidity score in HCC patients with bone metastasis. The study results may have a consequential impact on the treatment decisions for HCC patients with bone metastasis.Keywords: hepatocellular carcinoma, HCC, bone metastasis

Abstract

Core-collapse supernovae are among the most magnificent events in the observable universe. They produce many of the chemical elements necessary for life to exist and their remnants -- neutron stars and black holes -- are interesting astrophysical objects in their own right. However, despite millennia of observations and almost a century of astrophysical study, the explosion mechanism of core-collapse supernovae is not yet well understood. Hyper-Kamiokande is a next-generation neutrino detector that will be able to observe the neutrino flux from the next galactic core-collapse supernova in unprecedented detail. We focus on the first 500 ms of the neutrino burst, corresponding to the accretion phase, and use a newly-developed, high-precision supernova event generator to simulate Hyper-Kamiokande's response to five different supernova models. We show that Hyper-Kamiokande will be able to distinguish between these models with high accuracy for a supernova at a distance of up to 100 kpc. Once the next galactic supernova happens, this ability will be a powerful tool for guiding simulations towards a precise reproduction of the explosion mechanism observed in nature., Comment: 21 pages, 7 figures. New Re-Os molybdenite geochronology is presented from the Ballachulish Igneous Complex (433.5 [+ or -] 1.8 Ma) and the Kilmelford Igneous Complex (425.8 [+ or -] 1.7 Ma) of the SW Scottish Highlands. The sulphide mineralization in the Ballachulish Igneous Complex is related to the latest phase of granite magmatism, and therefore the new Re-Os age provides a minimum crystallization age, c. 3.7 Ma earlier than previous crystallization age estimates. This Re-Os age overlaps U-Pb ages obtained from subduction-related granitic and appinitic magmatism north of the Great Glen Fault, and an origin related to active subduction rather than slab breakoff is proposed for the Ballachulish Igneous Complex. Molybdenite mineralization in the Kilmelford Igneous Complex is spatially and genetically associated with porphyry Cu mineralization, which is consistent with the rapid ascent of volatile-rich magma during early rebound following the breakoff of subducted oceanic lithosphere. The Kilmelford Igneous Complex and the coeval Lorn Lava Pile may represent the earliest of the igneous bodies predicted by slab breakoff, indicating that slab breakoff occurred at c. 426 Ma. doi: 10.1144/0016-76492009-077.

Abstract

A detailed field and microstructural investigation of mineral fabrics in the late-Caledonian Galway Granite Batholith (~ 400 Ma) provides insights into the relationship between emplacement-related deformation and crystallization state. These relationships are used to infer the regional instantaneous strain pattern at the time of intrusion. A Marginal Deformation Zone occurs in the granite along part of its northern sector, where planar fabrics are contact-parallel and dip steeply to the north. Within the Marginal Deformation Zone, the granite has similar patterns of pre- and post-RCMP (Rheologically Critical Melt Percentage) fabrics on either side of the NNE-trending Shannawona Fault Zone, which separates the Western and Central blocks of the batholith. Oblate pre-RCMP fabrics, which intensify towards the contact, are overprinted in a down-temperature continuum of deformation by co-axial post-RCMP fabrics that also become more intense towards the contact. At the southern edge of the Marginal Deformation Zone, deformation ceased before the granite reached its RCMP whereas, close to the contact, deformation ceased at ~ 500 [degrees]C. Within the Central Block, oblate fabrics also developed parallel with internal granite facies boundaries. Throughout the batholith, the fabrics formed by co-axial deformation as a result of lateral expansion operating in successive magma batches at the emplacement level. These intrusion-related fabrics are consistent with other evidence that indicates the Galway Granite was emplaced into a transtensional setting at the end of the Caledonian Orogeny. 28. New perspectives on the order and style of granite emplacement in the Galway Batholith, western Ireland

Abstract

The late Caledonian Galway Batholith is cut by two major faults which divide it into three separate areas: the western, central and eastern blocks. The upthrown and more deeply eroded central block is bounded by these faults, in the west by the north-northeast trending Shannawona Fault and in the east by the north-northwest trending Barna Fault. We present new granite field relations from part of the central block (Inveran sector) which are fundamental in establishing the order and style of emplacement for the granites of the central block and the batholith as a whole. Unequivocal field evidence from the Inveran sector indicates upward movement of early central block granites which then became the solid roof rocks to subsequent intrusions. In the case of the Knock Granite these earlier intrusions were block stoped. We use this field evidence to review the geology of the central block in a 200 [km.sup.2] area that incorporates the previously mapped Costelloe and Spiddal areas. Sharp intrusive contacts are a predominant feature of this sector of the central block and are in marked contrast to the gradational contacts recorded elsewhere in the batholith. Whereas juxtaposition of plutons in the western block occurred as the granites were partly crystallized, the central block reveals earlier, deeper level granites that were consolidated by the time they were intruded by late-stage higher level granites.

Abstract

The geological processes that create fluid storage capacity and connectivity in global fractured basement reservoirs are poorly understood compared to conventional hydrocarbon plays. Hosting potentially multibillion barrels of oil, the upfaulted Precambrian basement of the Rona Ridge, offshore west of Shetland, UK, gives key insights into how such reservoirs form. Oil presence is everywhere associated with sub-millimeter- to meter-thickness mineralized fracture systems cutting both basement and local preseal cover sequences. Mineral textures and fluid inclusion geothermometry suggest a low-temperature (90–220 °C), near-surface hydrothermal system, as does the preservation of clastic sediments in the same fractures. These fills act as permanent props holding fractures open, forming long-term fissures in the basement that permit oil ingress and storage. Calcite-fill U-Pb dating constrains the onset of mineralization and contemporaneous oil charge to the Late Cretaceous. The additional preservation of oil-stained injected sediment slurries and silica gels along basement faults suggests that rift-related seismogenic faulting initiated lateral oil migration from Jurassic source rocks into the adjacent upfaulted ridge. Subsidence below sea level in the latest Cretaceous sealed the ridge with shales, and buoyancy-driven migration of oil into the preexisting propped fracture systems continued long after the cessation of rifting. These new observations provide an explanation for the viability of sub-unconformity fractured basement reservoirs worldwide, and have wider implications for subsurface fluid migration processes generally.

Abstract

Microthermometric and vitrinite reflectance data from syntectonic quartz veins in Namurian rocks reveal the high level of organic maturation of the rocks in west Clare, Ireland. Organic maturation levels are due to palaeotemperatures of 340-370 degrees Celsius in late Carboniferous time, which coincides with the Variscan deformation. Temperature and salinity of fluids, which precipitate quartz, are examined for the thermal history of the region. Absence of vitrinate reflectance gradient in the Doonbeg No. 1 indicate the possibility of advective heat transfer.

Abstract

The occurrence of organic matter in terrestrial impact craters is important to astrobiology, as it may offer insight into possible relationships between impact events and the genesis, distribution and preservation of biologically relevant materials on planets. In particular, the processing and mobilization of preexisting organic material in planetary targets is of interest for studies of pathways to chemical complexity. Observations in old (Palaeozoic, Precambrian) craters indicate that organic carbon can survive large impacts. However, limited exposure and superimposed geological events can make the detailed history of organic matter in old craters difficult to decipher. We present here the first identification of hydrocarbons in the young (23 Ma, Miocene) Haughton impact structure.

Abstract

The Loch Shin Line is a geological–geophysical lineament associated with a zone of mantle-derived appinites, granites and strike-slip faulting that runs NW–SE across the Moine Nappe, northern Scotland. U–Pb zircon and Re–Os molybdenite dating of the Loch Shin and Grudie plutons, which lie immediately SW of the NW–SE Loch Shin–Strath Fleet fault system, yield c. 427–430 Ma ages that overlap within error. They also coincide with previously obtained U–Pb zircon ages for the Rogart pluton, which lies along-strike to the SE. Field and microstructural observations confirm the similarity and contemporaneous nature of the plutons and associated sulphide mineralization. Fluid inclusion analyses place further constraints on the P–T–X conditions during regional late Caledonian exhumation of the Moine Nappe. Synchronous to slightly younger brittle dextral strike-slip faulting along the WNW–ESE Loch Shin–Strath Fleet Fault System was probably antithetic to sinistral movements along the nearby Great Glen Fault Zone. Our findings support the hypothesis that the Loch Shin Line acted as a deep crustal channelway controlling the ascent and emplacement of Silurian magmas into the overlying Moine Nappe. We propose that this deep structure corresponds to the southeastern continuation of the Precambrian-age Laxford Front shear zone in the buried Lewisian autochthon.

Abstract

Since poor compliance with antimalarial therapy is often suspected but difficult to prove, this study attempted to establish a model for predicting the plasma concentration of phenobarbital (given in low doses in conjunction with the drug) as an indicator of compliance. Phenobarbital was chosen because its value had been demonstrated as a marker of compliance in long-course therapies, any significant departure from steady-state concentrations (achieved with full compliance) indicating one or more missed doses. Therapy for uncomplicated malaria varies from 5 days with artesunate to 7 days with quinine + tetracycline. Volunteers with confirmed falciparum malaria were randomized into 5 groups and given malaria therapy as well as phenobarbital daily for 3-7 days. Plasma samples for determination of phenobarbital concentrations were taken just prior to the daily dose of phenobarbital. Although there was a clear and predictable individual pattern of blood concentrations following each dose of phenobarbital, inter-individual variation in blood levels was significant and reduced their predictive value beyond the second day's dose. The cause of the variations is not clear; it could be attributable to different sources of the drug, previous intake of phenobarbital by the patient, or differences in drug absorption and disposition in malaria patients. Results for the 5-day artesunate regimen suggest that phenobarbital may be useful as a marker of compliance if the patient stops medication after 3 days; clear differences were evident at the end of the course of treatment between plasma phenobarbital concentrations in individuals completing the 5-day course and those who stopped after 3 days. For the quinine-tetracycline regimen, results suggest that it may be possible to discriminate between subjects where there is a 3-day difference in treatment. Phenobarbital is a better discriminant when dosing is every 24 hours as with artesunate, rather than the 8-hourly regimen for quinine-tetracycline. When measuring compliance for malaria treatment, if it is important to know what proportion of patients reach 3, 5 or 7 days of compliance, then phenobarbital might have a role to play in this assessment, but further investigations in more patients would be required.
