Objectives The aim of this clinical observational study was to assess the efficacy of L-PRF as a hemostatic agent in patients under treatment with vitamin K antagonists (VKAs) or direct oral anticoagulants (DOACs). Materials and methods Patients under oral anticoagulant therapy (VKA or DOACs) who needed a single simple tooth extraction were enrolled. L-PRF plug was positioned inside the alveolus and secured with non-absorbable sutures. Surgical time, pain-VAS, paracetamol intake, intra-operative, post-operative biological complications, and bleeding events have been registered. Results A total of 112 patients (59 patients for DOAC and 53 for VKA group) were enrolled. Post-operative bleeding was recorded in nine patients (17%) for VKA group and nine patients (15.3%) for DOACs group. None of the patients needed a medical support for managing of bleeding. Seven days after surgery, no cases of post-extractive complications occurred. Conclusions The use of L-PRF resulted in limited mild late post-operative bleedings without the need of medical intervention. Clinical relevance The use of L-PRF can be adopted for an uneventful post-operative curse in anticoagulated patients without chasing their therapy for single tooth extraction.

Background: Peri-implantitis is widely recognized as a major cause of late implant failure, both in pristine and regenerated bone. The present study aims to evaluate the prevalence of peri-implantitis in implants inserted in augmented maxillary sinuses and to analyze possible risk factors. Methods: A cross-sectional study was conducted in four centers including patients who underwent lateral or transcrestal sinus augmentation and received dental implants. Clinical and anamnestic data were collected using a standardized form. Univariate and multivariate logistic regression analyses have been performed for both implant-level and patient-level variables. Subsequently, a multilevel logistic mixed-effect model was built to analyze variables correlated with the occurrence of peri-implantitis. Results: A total of 156 patients (61 males and 95 females; mean age: 60.9 ± 11.6 years) with 315 implants inserted into augmented maxillary sinuses with a follow-up ranging from 1 to 18 years were evaluated. Seven implants in seven patients were previously lost for peri-implantitis (2.2% and 4.5% at implant- and patient-level, respectively); 250 implants showed no signs of peri-implant diseases (79.4%), 34 implants presented mucositis (10.8%), and 24 implants exhibited peri-implantitis (7.6%). Corresponding data evaluated at patient-level were 125 (80.1%), 17 (10.9%), and 14 (9.0%), respectively. A doença renal em estágio terminal (DRT) requer terapia de substituição renal como diálise peritoneal, hemodiálise ou transplante. Os pacientes afetados pela DRT ou em hemodiálise correm risco de desenvolver uma série de comorbidades, incluindo hipertensão, anemia, risco de sangramento, suscetibilidade à infecção, efeitos colaterais dos medicamentos e manifestações orais associadas à própria doença e ao tratamento em hemodiálise. Nesse contexto, as doenças bucais representam uma causa potencial e evitável de maus resultados de saúde em pessoas com DRT devido à sua relação com infecção, inflamação e desnutrição.

Objectives: the aim of this ex vivo report was to evaluate, on a microscopical analysis, the presence of microbrush remnants on in the adhesive surface in extracted teeth.Methods: Twenty extracted teeth were divided into four groups. Half of the teeth were prepared as Class I cavities, whereas the other half as Class II cavities, according to Black classification. The teeth were conditioned with primer and bonding, both applied with microbrushes. Each of these groups was divided into halves, and the two sub-groups received a polymerization process or not, respectively. The teeth were then analyzed by scanning electron microscopy working in environmental mode.Results: All of the analyzed surfaces (100%) showed the presence of residual bristles on the adhesion surface.Conclusions: Microscopical analysis showed the presence of residual bristles in the 100% of the surfaces treated with Black Classes I and II cavities. Further studies are necessary to evaluate the influence of this factor on the adhesion strength and capacity. Clinical Significance: Clinician should be aware of the realistic possibility of the presence of bristles belonging to disposable applicators in the adhesive interface of Black's class I and II cavities. The impact of these remnants has still to be ascertained.

Gingival crevicular fluid (GCF) is an interesting biofluid reflecting the physiological and pathological states of a single dental element. Due to this unique feature, in recent years, metabolomic analysis of GCF has gained attention as a biometric tool for the diagnosis and therapy of periodontal disease. Traditional methods are, however, too slow, cumbersome and expensive for a health-care routine. Surface enhanced Raman scattering (SERS) can offer rapid and label-free detailed molecular fingerprints that can be used for biofluid analysis. Here we report the first SERS characterization of GCF using an easy and quick sample preparation. The dominant features in the SERS spectrum of GCF are ascribed to very few metabolites, in particular to uric acid, hypoxanthine, glutathione and ergothioneine. Additionally, we succeeded in differentiating between the SERS signal of GCF collected from healthy volunteers and the one collected from patients with periodontal disease.

Electrospun polycaprolactone (PCL) membranes have been widely explored in the literature as a solution for several applications in tissue engineering and regenerative medicine. PCL hydrophobicity and its lack of bioactivity drastically limit its use in the medical field. To overcome these drawbacks, many promising strategies have been developed and proposed in the literature. In order to increase the bioactivity of electrospun PCL membranes designed for guided bone and tissue regeneration purposes, in the present work, the membranes were functionalized with a coating of bioactive lactose-modified chitosan (CTL). Since CTL can be used for the synthesis and stabilization of silver nanoparticles, a coating of this compound was employed here to provide antibacterial properties to the membranes. Scanning electron microscopy imaging revealed that the electrospinning process adopted here allowed us to obtain membranes with homogeneous fibers and without defects. Also, PCL membranes retained their mechanical properties after several weeks of aging in simulated body fluid, representing a valid support for cell growth and tissue development. CTL adsorption on membranes was investigated by fluorescence microscopy using fluorescein-labeled CTL, resulting in a homogeneous and slow release over time. Inductively coupled plasma-mass spectrometry was used to analyze the release of silver, which was shown to be stably bonded to the CTL coating and to be slowly released over time. The CTL coating improved MG63 osteoblast adhesion and proliferation on membranes. On the other hand, the presence of silver nanoparticles discouraged biofilm formation by Pseudomonas aeruginosa and Staphylococcus aureus without being cytotoxic. Overall, the stability and the biological and antibacterial properties make these membranes a valid and versatile material for applications in guided tissue regeneration and in other biomedical fields like wound healing.

Statement of problem The dimensional stability of alginate dental impressions is a key factor for the reliability of delayed gypsum pouring and digital scanning. However, studies of the dimensional stability of alginates with conventional methods that consider the dimensional variations of large impressions are lacking. Purpose The purpose of this in vitro study was to investigate and compare 2 digital methods for the analysis of dimensional stability of large impressions made with 5 different extended-pour alginates and to assess dimensional stability up to 5 days. Material and methods Impressions of a simplified master maxillary model were made with Alginoplast, Blueprint, Hydrogum 5, Orthoprint, and Phase Plus and then analyzed at different time points. Digital scans of the alginate impression surfaces were obtained with a desktop scanner and analyzed by evaluating the linear measurements between reference points and by using a novel method that consists of the analysis of the entire scanned surface to evaluate the expansion and contraction of the impressions. Results The first method revealed that the dimensional changes did not exceed 0.5%, with the exception of Phase Plus at day 3 (-0.6 ±0.7%), and the average dimensional variation was always lower than or equal to 0.2 mm. Blueprint was the most stable material (-0.2 ±0.6%). The second method revealed dimensional variations always lower than 0.03 mm and confirmed Blueprint as the best performing material (0.001 ±0.006 mm) and Phase Plus the worst (-0.019 ±0.006 mm). Conclusions Both the methods used to evaluate alginate stability showed that the analyzed materials remain stable over time; the dimensional variations showed a similar trend, with differences in the absolute values depending on the applied method. Linear measurements are affected by the operator and choice of reference points; however, by evaluating the average variations of the entire structure surfaces, local variations should be minimized. The evaluation of the average variations with the second method offers the advantage of a rapid visual representation of these variations.

Objectives The present systematic review aimed to perform an in‐depth analysis of the different features of retracted publications in the dental field. Material and methods This review has been recorded in the PROSPERO database (CRD42017075634). Two independent reviewers performed an electronic search (Pubmed, Retraction Watch) for retracted articles in dental literature up to December 31, 2018. Results 180 retracted papers were identified, the first published in 2001. Retractions increased by 47% in the last four‐year period (2014–2018), when compared with 2009–2013 (94 and 64 retracted publications, respectively). Author misconduct was the most common reason for retraction (65.0%), followed by honest scientific errors (12.2%) and publisher‐related issues (10.6%). The majority of retracted research was conducted in Asia (55.6%), with 49 papers written in India (27.2%). 552 researchers (89%) are listed as authors in only one retracted article, while 10 researchers (1.6%) are present in five or more retracted publications. Retracted articles were cited 530 times after retraction: the great majority of these citations (89.6%) did not consider the existence of the retraction notice and treated data from retracted articles as reliable. Conclusions Retractions in dental literature have constantly increased in recent years, with the majority of them due to misconduct and fraud. The publication of unreliable research has many negative consequences. Studies derived from such material are designed on potentially incorrect bases, waste funds and resources, and most importantly, increase risk of incorrect treatment for patients. Citation of retracted papers represents a major issue for the scientific community.

Background:Different approaches were proposed in the literature for the treatment of malpositioned ankylosed teeth. The present case report describes a modification of Orthodontic Bone Stretching Technique (OBS) for the repositioning of ankylosed teeth, consisting of dentoalveolar segmental osteotomies performed with piezoelectric instruments followed by orthodontic and orthopedic traction.Case Report:A 22-year-old female in good general health was referred by her orthodontist due to an infraoccluded and ankylosed maxillary upper left canine. Attempts of conventional and corticotomy-assisted orthodontic alignment of the tooth were previously performed with no success. After elevating a full-thickness flap, three osteotomies were performed by using piezoelectric inserts (Piezotome Cube, Acteon, Merignac, France). Mesial and distal cuts were full-thickness osteotomies, parallel to the long axis of the tooth, through the buccal and palatal cortical plates. The apical osseous incision was a horizontal corticotomy, involving only the buccal plate, and connecting the vertical osteotomies two millimetres over the apex of the tooth. Heavy orthodontic forces were immediately applied by using both dental and skeletal anchorage. The initial movement of the dentoalveolar segment was observed three weeks after surgery and case finishing has been completed in two months. At a one-year follow-up, the repositioned canine showed good periodontal conditions, no discoloration and positive pulp response to the electric test.Conclusion:Modified OBS technique was effective in repositioning an infraoccluded and ankylosed maxillary canine, providing satisfactory function and esthetics with short treatment time.

Obesity is a systemic disease, associated with an increased risk of cardiovascular disorders, type 2 diabetes, cancer, asthma, and osteoarthritis. Overweight and obesity have been suggested to be associated with periodontitis as published in studies and narrative summaries. Obesity and periodontal diseases are very prevalent in the world, and both can lead to severe chronic health conditions and impair people’s life quality. Knowledge of how immune mechanisms and inflammatory responses are regulated is critical for understanding the pathogenesis of complex diseases, such as periodontitis. In conditions of overweight, it has been demonstrated that approximately 70–80% of individuals present an adipose tissue turnover that is both structurally and functionally causing of the systemic inflammatory reaction. The objective of this review is to explore the influence of lipoinflammation. The effects of lipoinflammation and obesity on development of periodontal disease are reported together with the exploration of the mechanisms of interaction between these two diseases.

OBJECTIVE: The aim of this multicenter prospective study was to analyze clinically and histologically the influence of sinus cavity dimensions on new bone formation after transcrestal sinus floor elevation (tSFE). Dear Sir/Madame, The SARS-CoV-2 outbreak left all the scientific and medical community with tremendous doubts and difficulties in managing both symptomatic and asymptomatic patients, especially in case of seronegativity.1,2 In fact, in patients with mild symptoms, detectable IgG may not develop or progressively decrease.3 The present letter aims to highlight an interesting finding, never mentioned before, that may help clinicians and epidemiologists in detecting previous SARS-CoV-2 infection. Learning from archaeological studies,4 we hypothesized that dental calculus, a mineralized oral plaque biofilm that contains biomolecules such as nucleic acids, could have preserved SARS-CoV-2 RNA after infection, regardless of the development and severity of symptoms. Here we present the results from a pilot study including 5 individuals with previous diagnosis of SARS-CoV-2 infection obtained by nasopharyngeal swab testing and subsequently healed within 2 months prior the study (POS), 2 individuals contact with a positive who developed mild symptoms and were negative at nasopharyngeal swab testing (SUSP), and 5 healthy individuals who denied contacts with positive subjects and had negative nasopharyngeal swabs (CTRL). All subjects underwent dental calculus sampling performed with sterile curettes. Samples were analyzed performing real-time reverse transcription polymerase chain reaction assays after nucleic acid extraction and amplification. The RdRp Institut Pasteur IP2 assay was used, as previous studies demonstrated its high sensitivity and specificity.5 The results showed that SARS-CoV-2 RNA was detected in all POS subjects, in the two SUSP subjects and one among 5 CTRL subjects. Ct values are reported in the Table. Furthermore, one of the POS subjects resulted negative at a serologic assay while the other 4 subjects developed specific IgG antibodies. Samples were further analyzed using the CDC N1 assay, confirming the presence of SARS-CoV-2 RNA in one POS subject. The presence of amplified SARS-CoV-2 RNA was confirmed by genomic sequencing. Sanger sequencing of the samples was run at Eurofins Genomics facilities (Eurofins Genomics Germany GmbH, Anzinger Str. 7a, 85560 Ebersberg, Germany). Dental calculus has been previously analyzed, mostly for microbiome definition in archaeological human samples and serving as milieu for the preservation of genomic information. Similarly, dental calculus may be helpful to detect SARS-CoV-2 traces in previously infected and possibly also in asymptomatic or mild symptomatic patients that may not develop detectable antibodies, therefore lacking a method to confirm the occurred infection. Further studies are needed to define method's reliability, cost/effectiveness, and suitability for large-scale epidemiological studies or post-mortem analysis. Although our study has some limitations due to the small number of samples, we believe that its interest resides in the fact that it is the first describing the presence of SARS-CoV-2 RNA in dental calculus.

The aim is to reduce the aerosol spread during dental treatments. The system is made of an already existing structure: a dirty towel basket. The structure has to be put directly above patient’s head. A transparent cellophane has to cover the structure, to provide a laboratory hood-like structure. The efficacy of the “dental-hood” was tested with a semi-quantitative colorimetric assay. A room dedicated for a dental unit was covered by absorbent paper. An air-polishing system and a staining powder served for the purpose. Standardized photographs were acquired one hour after dental treatment with and without dental-hood. The treatment without the dental-hood produced staining until two meters from the dental chair, with a contamination of the floor and the furnishings sensibly higher compared with the dental-hood. According to the critical situation we are living, the dental-hood rep resents a simple, inexpensive and effective way to reduce aerosol spread during dental treatments.

The purpose of this investigation is to evaluate 2 different methods for reducing cortical wall thickness in sinus floor augmentation surgery. A manual bone scraper was compared in terms of efficacy, speed, and safety to an ultrasonic insert for osteoplasty, in a randomized controlled clinical trial with a split-mouth design. Twenty-five patients with severe posterior maxillary atrophy were treated with bilateral sinus floor elevation with lateral approach. Antrostomies were randomly performed by eroding the cortical wall with a manual bone scraper (test site) or with an ultrasonic insert (control site) until the membrane was visible under a thin layer of bone, before outlining the window with a piezoelectric device. Occurrence of membrane perforation, laceration of vascular branches, and surgical time were recorded. Mean surgical time of the antrostomy in the test sites was 9'18", while in the control sites was 9'47". No significant differences were found in terms of surgical time, incidence of membrane perforation during antrostomy (4.3% in both groups), or other intraoperative complications between the 2 techniques. Both surgical approaches represent effective options for performing lateral antrostomies during sinus floor elevation procedures in a safe and predictable way.

Objectives: The aim of this prospective comparative clinical study was to evaluate the effect of oral anticoagulants on peri- and post-operative bleeding during simple single tooth extractions, comparing patients in treatment with vitamin K antagonists (VKAs) and patients assuming direct oral anticoagulants (DOACs). Materials and methods: Patients under oral anticoagulant therapy needing dental extraction were eligible for entering the study; patients were enrolled following inclusion and exclusion criteria and divided into VKAs and DOAC group according to the anticoagulation therapy. Included patients underwent a simple single dental extraction with elevators and forceps with a maximum surgical time of 15 minutes, without anticoagulation therapy discontinuation. All participants were assessed pre-operatively, during surgery, 30 min minutes and 7 days after surgery. Biological complications were registered and post-extraction bleeding was clinically defined according to Iwabuchi classification. Parametric and non-parametric tests were used to evaluate the variables between the groups. Results: Sixty-five patients per group were enrolled and 130 teeth were extracted. The two groups were comparable for pre-, peri-, and post-operative variables. Only 1 patient of DOAC group and 2 patients for VKA group needed medical evaluation for post-extractive bleeding. No statistically significant difference resulted in post-operative bleeding events between the groups (p = 0.425). Conclusions: DOAC and VKA patients showed the same incidence of bleeding complications after simple single tooth extraction. Bleeding events were not statistically significant and not clinically relevant. Clinical relevance: Patients assuming DOACs can be treated similarly to patients in VKAs therapy with INR index between 2 and 3. Non-ceasing of DOAC therapy seems to be appropriate for simple single dental extractions. © 2018, Springer-Verlag GmbH Germany, part of Springer Nature.

During the past, a more comprehensive knowledge of mechanisms implicated in bone resorption processes has driven researchers to develop a compound library of many small molecules that specifically interfere with the genesis of osteoclast precursors cells. Natural compounds that suppress osteoclast commitment may have therapeutic value in treating pathologies associated with bone resorption like osteoporosis, rheumatoid arthritis, bone metastasis, and periodontal disease. The present review is focused on the current knowledge on the polyphenols derived from plants that could be efficacious in suppressing osteoclast differentiation and bone resorption.

Nowadays platelet concentrates (PCs) show interesting potential in oral surgery for soft tissue healing promotion. However, inductive properties for bone formation remains still controversial. Recently, numerous studies explored the merging of two different tissue healing inducing agents: PCs with osteoinducing agents or scaffolds or antiresorptive drugs. Research trend looks towards the amelioration of the inductive properties of PCs, mainly towards bone regeneration. This paper aims to evaluate i) the durability of the pristine L-PRF membranes in dissolution assay; ii) the opportunity of coupling the obtained membranes with an osteoconductive molecule, such as nanohydroxyapatites (nHAp). Thus, the durability of pristine membrane in SBF was tested. At each time point, one sample was analyzed with SEM; image processing revealed an average fiber diameter of 0.103 ± 0.05µm without any statistically significant differences during time. Degradation assay showed a two-folds increase of the weight related to the SBF absorption in the first 2 days. From the third day a constant degradation was observed. In the time frame of this experiment, the dimensional stability of the fibrin structure up to day 7 suggested that PRF membranes may also be used uncovered in the oral cavity. Subsequently, the effects of the nHAp addition during the forming process of PRF (thus during centrifugation) were investigated.

Background Fungus ball (FB) typically affects healthy adults, and Aspergillus fumigatus is the most frequent etiologic agent: iatrogenic factors represent an important issue in FB pathogenesis. Moreover, a recent study suggested a significant association between the use of anorganic bovine bone as sinus grafting material and subsequent development of FB. The aim of the present investigation is to evaluate in vitro eventual differences in the ability of Aspergillus fumigatus to colonize different bone grafting materials and grow on them as biofilm. Findings Five different bone substitutes (demineralized bone matrix, anorganic bovine bone, ß-tricalcium phosphate, synthetic nano-hydroxyapatite, and synthetic hydroxyapatite), commonly used in sinus floor augmentation procedures, were inoculated with conidia suspensions of A. fumigatus and incubated at 37 °C for 4 and 8 h, in standardized conditions. Biofilm bound to the different materials underwent quantitative and qualitative analysis by confocal and scanning electron microscopy. A. fumigatus proved to be able to adhere and form biofilm on all the tested bone substitutes. The surface plot representation of the samples displayed some differences in the density of the superficial layer, due to the physical characteristics of the biomaterials. Nevertheless, Kruskal–Wallis test showed no significant differences in biomass amount among the five bone substitutes (p = 0.236 and p = 0.55 after 4 and 8 h adhesion, respectively). Conclusions All the bone substitutes normally used in sinus floor augmentation represent a favorable substrate for fungal growth, due to their physical and chemical characteristics. During sinus floor elevation procedures, Schneiderian membrane integrity should be maintained in order to avoid the exposure of the grafting material at the respiratory environment, with potential risks of fungal colonization.

Nowadays implant-prosthetic rehabilitations are one of the best solutions for rehabilitating our patients. These allow obtaining rehabilitations with optimal functional and aesthetic performances. Often patients, who undergo implant-prosthetic therapy, have the conditions of edentulias, single or multiple, that have been going on for some time. This, according to the literature, produces resorption of the alveolar bone, a process that is complicated in the posterior area of the upper arch by a pneumatization of the maxillary sinuses. The loss of vertical height, so that the implant fixtures can be inserted, requires a maneuver called maxillary sinus floor elevation. This procedure, now safely performed with piezoelectric instruments, allows increasing bone height through bone grafting. In this study, the tissue obtained from a patient, after 15 years from the intervention, was evaluated by histological and SEM analyses. The bone healing in the patient has led to a perfect integration between the patient&rsquo, s bone and the fresh frozen allograft used, however still present and detectable after 15 years.

Nanofibre-based membranes or scaffolds exhibit high surface-to-volume ratio, which allows an improved cell adhesion, representing an attractive subgroup of biomaterials due to their unique properties. Among several techniques of nanofiber production, electrospinning is a cost-effective technique that has been, to date, attractive for several medical applications. Among these, guided bone regeneration is a surgical procedure in which bone regeneration, due to bone atrophy following tooth loss, is “guided” by an occlusive barrier. The membrane should protect the initial blood clot from any compression, shielding the bone matrix during maturation from infiltration of soft tissues cells. This review will focus its attention on the application of electrospinning (ELS) in oral surgery bone regeneration. Despite the abundance of published papers related to the electrospinning technique applied in the field of bone regeneration of the jaws, to the authors’ knowledge, no articles report clinical application of these structures. Moreover, only a few records can be found with in vivo application. Therefore, no human studies have to date been detectable. New approaches such as multifunctional multilayering and coupling with bone promoting factors or antimicrobial agents, makes this technology very attractive. However, greater efforts should be made by researchers and companies to turn these results into clinical practice.

Objective The replacement of an irremediably compromised tooth requires an implant rehabilitation or a traditional fixed partial denture. In well-selected cases, a further therapeutic possibility is represented by tooth autotransplantation. Although dental transplants are poorly understood and practiced, the international literature agrees that it is considered the first choice when applicable. The advantages of this technique are numerous: use of an autologous element, maintenance of tissue trophism, aesthetic and functional restoration, costs reduction. Although autotransplantation is often performed with immature teeth, even mature teeth with fully formed apex can be used as donors. The aim of the present work was to analyze consecutive cases of completely formed donor teeth autotransplantations performed from 2005 to 2011 in 21 patients for evaluating the survival and success rate. Materials and methods The medical records of patients who underwent transplantation in a specialized center in Rimini (Italy) from 2005 to 2011 were checked. Only transplants of mature donor molars were considered. Patients were called up to evaluate the survival rate and success rate. Results The mean age at the time of the surgery was 33,6 ± 7,4; mean follow up was 11,9 years ±1,9. Success rate at the time of latest recall visit was 80 % and survival 95 % of the analyzed cases. Conclusions The survival and success rate are in complete agreement with the most recent literature and confirm that the technique of autotransplantation is reliable when indications and protocols are rigidly followed, also using mature teeth as donors.

Severe dental erosion could be one of the complications of gastroesophageal reflux disease and food disorders such as bulimia nervosa. The aim of the present in vitro study was to evaluate the remineralization efficiency and the erosion prevention capability obtained by combining the use of Elmex Erosion Protection (Elm-EP) and GC Tooth Mousse (GC-TM) in cases of strongly eroded enamel surfaces. Twenty-four specimens of bovine tooth were superficially treated with hydrochloric acid to mimic severe erosion conditions. The specimens were divided into 3 groups accordingly to a different remineralizing treatment for 15 days: immersion in artificial remineralizing saliva, brushing with Elm-EP and brushing with Elm-EP in association with a daily application of GC-TM paste. After the remineralization procedures, the specimens were exposed to a treatment with acid. The effectiveness of the treatments were assessed by surface profilometric analysis and scanning electron microscopy at four different steps. Furthermore, the quantity of the Ca2+ lost during erosion was determined. The combined action of Elm-EP and GC-TM led to a 50% roughness decrease of critically eroded dental surfaces. The tandem use of Elmex Erosion Protection and GC Tooth Mousse resulted a promising protecting strategy for the prevention of the dental mineral loss.

BACKGROUND: The longitudinal aspect of dental caries has not been previously reported for the Italian population. The primary object of the present study is to collect information of the prevalence of dental decay among the schoolchildren of primary school of Gradisca d'Isonzo (GO) and to analyze the tendency of caries among the students followed since the first year of school. METHODS: Subsequent examinations hold from 2011 to 2015 has been conducted by two calibrated examiners. Oral hygiene instruction and motivation followed the visits. According to WHO principals DMFT and dmft were recorded. The children in the survey were divided into 5 groups according to their ages (6, 7, 8 and 9 years), and these groups were considered separately. Descriptive and statistical analysis of the data was conducted. RESULTS: More than 400 pupils were recruited, resulting in almost 900 examinations during 5 years. Overall, the %dft ≥1 children range from 18.9% (10 years, 2013) to 53.5% (8 years, 2011) across the different age groups. Overall, the %DFT≥1 children range from 8.3% (6 years, 2011) to 44.1% (10 years, 2012) across the different age groups. CONCLUSIONS: Within the limits of the present study, the WHO goals are still not met, among the population in exam. Our results show a trend of decay diminution that enhances in the cohort of 10-aged children suggesting the importance of dental education. Furthermore, the lack of pediatric initiatives of oral hygiene may be overcome by a national intensive educational program, supported by further scientific evidence.

Chronic renal failure is a progressive disease characterized by a gradual destruction of nephrons and a consequent reduction of kidney function. End-stage renal disease (ESRD) necessitates renal replacement therapy as peritoneal dialysis, hemodialysis, or transplantation. Patients affected by ESRD or in hemodialysis are at risk for developing a number of comorbidities including hypertension, anemia, risk of bleeding, susceptibility to infection, medication side effects, and oral manifestations associated with the disease itself and with hemodialysis treatment. In this context, oral diseases represent a potential and preventable cause of poor health outcomes in people with ESRD due to their relation to infection, inflammation, and malnutrition. The aim of this article was to review ESRD and hemodialysis-associated manifestations and to describe the dental operative protocols for patients awaiting kidney transplantation in light of the most recent literature.

Conjunctival chemosis is non-specific oedema, which may be caused by many systemic and local factors, including allergies; bacterial, viral, and mycotic infections; autoimmune diseases; thyroid disorders; and neoplasms. The orbit is susceptible to the contiguous spread of infections from the maxillary sinus as they are separated only by a thin plate of bone, which could also have congenital bony dehiscences

Purpose To analyze the occurrence of intraoperative complications during sinus floor elevation with a lateral approach and their correlations with the technique adopted by surgeons. Materials and methods Electronic and manual searches resulted in 4,417 records on sinus floor elevation. Twenty-one randomized clinical trials (RCTs) and 11 prospective controlled clinical trials (CCTs) reporting occurrence of intraoperative complications were included. Risk of bias was assessed according to the Cochrane tool and a modified Downs and Black quality analysis for RCTs and CCTs, respectively. Results Sinus membrane perforation and hemorrhagic events following vascular lesions were the only intraoperative complications reported by the selected studies with overall occurrences of 15.7% and 0.4%, respectively. Three different surgical devices (rotary instruments, piezoelectric osteotomes, and manual bone scrapers) were used to perform the lateral antrostomy. Ultrasonic devices and bone scrapers showed a lower incidence (10.9% and 6.0%, respectively) of membrane perforation compared with that of rotary instruments (20.1%). Among the different ultrasonic procedures, erosion of the lateral antral wall showed the lowest membrane perforations (4.7% incidence). Hemorrhagic complications seemed to be extremely infrequent with any surgical technique. Conclusion Sinus membrane perforation was the most frequently described intraoperative complication during sinus floor elevation with a lateral approach. Thinning the lateral wall of the sinus before performing the antrostomy (either with ultrasonic devices or manual bone scrapers) seemed to be an important factor in preventing membrane perforation during sinus surgery. Further high-quality RCTs specifically investigating intraoperative complication occurrence are needed.

Purpose Graft maturation in the maxillary sinus requires adequate angiogenesis and osteoprogenitor cells migration from the surrounding bony walls: the aim of this study was to analyze the correlation between sinus cavity dimensions and new bone formation after transcrestal sinus floor elevation (tSFE). Methods Patients needing maxillary sinus augmentation (residual crest height ≤ 4 mm) were treated with tSFE using xenogeneic granules. Six months later, bone-core biopsies were retrieved for histological analysis in the implant insertion sites. Buccopalatal sinus width (SW) was evaluated on cone beam computed tomography, and correlations between histomorphometric and anatomical parameters were quantified by means of linear regression analysis. Results Eight consecutive patients underwent tSFE procedures: at 6 months, average percentage of newly formed bone resulted 24.2% ± 7.9%. Statistical analysis showed a strong inverse correlation between SW and new bone formation (R = 0.88), and a strong direct correlation between the number of exposed bone walls and new bone formation (R = 0.82). Conclusion Within the limitations of this proof-of-concept study, in which a restricted number of patients were analyzed, tSFE showed more predictable results in narrow than in large sinuses, in terms of new bone formation.

Toothpaste-Induced Oral Mucosal Desquamation

PURPOSE: To compare the efficacy of two systems in cleaning used healing abutments (HAs). MATERIALS AND METHODS: A total of 60 used HAs were randomized into two groups: one treated with an automatic cleaning system, and the other with conventional decontamination procedures. After sterilization and staining, the HAs were microscopically analyzed and underwent a cellular adhesion in vitro assay. RESULTS: Contaminated areas were observed with different frequencies in the two groups (3.6% test; 78.2% control; P < .001). In vitro assay showed a uniform cell distribution in test HAs, while areas of debris without adhering cells were a common finding in the control HAs. CONCLUSION: Further studies investigating the chemical composition and clinical influence of biologic remnants are necessary before considering reusing HAs.

Early marginal bone loss (MBL) is a non-infective remodeling process of variable entity occurring within the first year after implant placement. It has a multifactorial etiology, being influenced by both surgical and prosthetic factors. Their impact remains a matter of debate, and controversial information is available, particularly regarding implants placed subcrestally. The present multicenter prospective clinical study aimed to correlate marginal bone loss around platform-switched implants with conical connection inserted subcrestally to general and local factors. Fifty-five patients were enrolled according to strict inclusion/exclusion criteria by four clinical centers. Single or multiple implants (AnyRidge, MegaGen, South Korea) were inserted in the posterior mandible with a one-stage protocol. Impressions were taken after two months of healing (T1), screwed metal-ceramic restorations were delivered three months after implant insertion (T2), and patients were recalled after six months (T3) and twelve months (T4) of prosthetic loading. Periapical radiographs were acquired at each time point. Bone levels were measured at each time point on both mesial and distal aspects of implants. Linear mixed models were fitted to the data to identify predictors associated with MBL. Fifty patients (25 male, 25 female, mean age 58.0 &plusmn, 12.8) with a total of 83 implants were included in the final analysis. The mean subcrestal position of the implant shoulder at baseline was 1.24 &plusmn, 0.57 mm, while at T4, it was 0.46 &plusmn, 0.59 mm under the bone level. Early marginal bone remodeling was significantly influenced by implant insertion depth and factors related to biological width establishment (vertical mucosal thickness, healing, and prosthetic abutment height). Deep implant insertion, thin peri-implant mucosa, and short abutments were associated with greater marginal bone loss up to six months after prosthetic loading. Peri-implant bone levels tended to stabilize after this time, and no further marginal bone resorption was recorded at twelve months after implant loading.

Objectives The purpose of this review was to evaluate whether history of periodontitis and smoking habits could represent a risk factor for peri-implantitis and implant loss. Material and Methods This systematic review followed PRISMA guidelines and was registered at the PROSPERO database [registration numbers CRD42016034160 (effect of history of periodontitis) and CRD42016033676 (effect of smoking)]. Broad electronic (MEDLINE) and manual searches were conducted among articles published from January 1st 1990 up to December 31st 2015, resulting in 49332 records for history of periodontitis and 3199 for smoking habits. Selection criteria included prospective studies comparing two cohorts of patients, with and without the investigated risk factor, with a minimum follow-up period of three years, and reporting data on peri-implantitis and implant loss occurrence. Considering that only prospective studies were included, dichotomous data were expressed as risk ratios and 95% confidence intervals. Results Three studies evaluating history of periodontitis (on which quantitative analysis was performed) and one study on smoking effect were included. Both implant and patient-based meta-analyses revealed a significantly higher risk of developing peri-implantitis in patients with a history of periodontitis compared with periodontally healthy subjects, but not a statistically significant increased risk for implant loss. Conclusions The outcomes of this systematic review indicate history of periodontitis as a possible risk factor for peri-implantitis, while insufficient data are present in literature to evaluate the role of smoking. However, available evidence is still weak and immature, and sound epidemiological studies are needed to analyse the specific contribution of these potential risk factors.
