Abstract

In this paper, we provide a concise description of the six nuclear reactor concepts that are under development in the framework of the Generation-IV International Forum. After a brief introduction on the world energy needs, its plausible evolution during the next fifty years, and the constraints imposed by the necessity to address the climate challenges we are facing today, we will present the main features of the innovative nuclear energy systems that hold the promise to produce almost-zero-carbon-emission electricity, heat for chemistry and industrial manufacturing, hydrogen to be used as energy vector, and affordable freshwater. Potential advantages over currently available nuclear systems in terms of increased safety, reduced proliferation risks, economical affordability, sustainability of the fuel cycle, and management of the waste inventory will be critically discussed against the technical challenges that remain to be overcome.

Abstract

Background: The prevalence of hypothyroidism among older patients hospitalized for COVID-19 and its association with mortality is unclear. This study aims to investigate the prevalence of hypothyroidism in older COVID-19 inpatients and verify if this comorbidity is associated with a specific pattern of onset symptoms and a worse prognosis. Methods: COVID-19 inpatients aged ≥ 60 years, participating in the GeroCovid acute wards cohort, were included. The history of hypothyroidism was derived from medical records and the use of thyroid hormones. Sociodemographic data, comorbidities, symptoms/signs at the disease onset and inflammatory markers at ward admission were compared between people with vs without history of hypothyroidism. The association between hypothyroidism and in-hospital mortality was tested through Cox regression. Results: Of the 1245 patients included, 8.5% had a history of hypothyroidism. These patients were more likely to present arterial hypertension and obesity compared with those without an history of hypothyroidism. Concerning COVID-19 clinical presentation, patients with hypothyroidism had less frequently low oxygen saturation and anorexia but reported muscle pain and loss of smell more commonly than those without hypothyroidism. Among the inflammatory markers, patients with hypothyroidism had higher lymphocytes values. At Cox regression, hypothyroidism was associated with reduced in-hospital mortality only in the univariable model (HR = 0.66, 95% CI 0.45–0.96, p = 0.03); conversely, no significant result were observed after adjusting for potential confounders (HR = 0.69, 95% CI 0.47–1.03, p = 0.07). Conclusions: Hypothyroidism does not seem to substantially influence the prognosis of COVID-19 in older people, although it may be associated with peculiar clinical and biochemical features at the disease onset.

Abstract

Background and objectives: Depression is highly prevalent in older adults, especially in those with dementia. Trazodone, an antidepressant, has shown to be effective in older patients with moderate anxiolytic and hypnotic activity; and a common off-label use is rising for managing behavioral and psychological symptoms of dementia (BPSD). The aim of the study is to comparatively assess the clinical profiles of older patients treated with trazodone or other antidepressants. Methods: This cross-sectional study involved adults aged ≥ 60 years at risk of or affected with COVID-19 enrolled in the GeroCovid Observational study from acute wards, geriatric and dementia-specific outpatient clinics, as well as long-term care facilities (LTCF). Participants were grouped according to the use of trazodone, other antidepressants, or no antidepressant use. Results: Of the 3396 study participants (mean age 80.6 ± 9.1 years; 57.1% females), 10.8% used trazodone and 8.5% others antidepressants. Individuals treated with trazodone were older, more functionally dependent, and had a higher prevalence of dementia and BPSD than those using other antidepressants or no antidepressant use. Logistic regression analyses found that the presence of BPSD was associated with trazodone use (odds ratio (OR) 28.4, 95% confidence interval (CI) 18-44.7 for the outcome trazodone vs no antidepressants use, among participants without depression; OR 2.17, 95% CI 1.05-4.49 for the outcome trazodone vs no antidepressants use, among participants with depression). A cluster analysis of trazodone use identified three clusters: cluster 1 included mainly women, living at home with assistance, multimorbidity, dementia, BPSD, and depression; cluster 2 included mainly institutionalized women, with disabilities, depression, and dementia; cluster 3 included mostly men, often living at home unassisted, with better mobility performance, fewer chronic diseases, dementia, BPSD, and depression. Abstract

In this paper, we describe a pedagogical approach to elastic body movement based on measurements of the contact times between a metallic rod and small bodies colliding with it and on modelling of the experimental results by using a microcomputer-based laboratory and simulation tools. The experiments and modelling activities have been built in the context of the laboratory of mechanical wave propagation of the two-year graduate teacher education programme of Palermo's University. Some considerations about observed modifications in trainee teachers' attitudes in utilizing experiments and modelling are discussed.

Abstract

This paper addresses the question of how to develop prospective teachers' pedagogical content knowledge (PCK) in science teacher education. The main focus is on the knowledge transformation process and on the cognitive strategies used to shift prospective teachers' explanations within the domain of modelling thermal physical phenomena. This study investigates the development of PCK within a group of 28 pre-service physics teachers during the first semester of their two-year post-graduate teacher education program. It focuses on the central issue of the relationships between observable phenomena, like macroscopic thermal properties of matter and their interpretation and/or explanation in terms of corpuscular characteristics and/or thermodynamics theory. The strategy is based on the consideration that knowledge transformation is not a one-way process from subject matter knowledge to pedagogical content knowledge, as literature suggests, but a bidirectional process involving deepening of subject matter knowledge and increasing awareness of pedagogical issues.

Abstract

Unstable particles rarely feature in conjunction with integrability in 1+1D quantum field theory. However, the family of homogenous sine-Gordon models provides a rare example where both stable and unstable bound states are present in the spectrum whilst the scattering matrix is diagonal and solves the usual bootstrap equations. In the standard scattering picture, unstable particles result from complex poles of the $S$-matrix located in the unphysical sheet of rapidity space. Since they are not part of the asymptotic spectrum, their presence is only felt through the effect they have on physical quantities associated either to the theory as a whole (i.e.~scaling functions, correlation functions) or to the stable particles themselves (i.e.~energy/particle density). In two recent publications, the effect of unstable particles in different out-of-equilibrium settings has been studied. It has been shown that their presence is associated with specific signatures in many quantities of physical interest. A good way to select those quantities is to adopt the generalised hydrodynamic approach and to consider the effective velocities and particle densities of the stable particles in the theory. For an initial state given by a spacial gaussian profile of temperatures peaked at the origin, time evolution gives rise to particle and spectral particle densities that exhibit hallmarks of the creation and decay of unstable particles. While these signatures have been observed numerically elsewhere, this paper explores their quantitative and qualitative dependence on the parameters of the problem. Abstract

In January 2019, Italy banned the sale of plastic cotton buds, which is one of the most abundant litter items entering the sea and then washing ashore. However, since the ban came into force, no studies have been carried out to assess whether the measure has actually led to the reduction of plastic cotton buds accumulating on Italian coasts. Here we aim at evaluating the effectiveness of the ban in reducing the amount of cotton buds reaching sandy beaches of the Tyrrhenian coast. Specifically, we monitored the accumulation of beach litter for one year since the ban came into force. By surveying eight coastal sites from winter 2019 to winter 2020, we collected a total of 52,824 items mostly constituted by plastic debris (97.6%). We found that cotton buds were the most abundant item (42.3% of total litter), followed by plastic (28.5%) and polystyrene (5.43%) fragments. Our preliminary assessment suggests that the ban has so far not led to a sensible reduction in the amount of cotton buds entering the marine ecosystem. This was to be expected since implementation strategies are still lacking (i.e. no economic sanctions can be imposed in case of non-compliance) and bans are differently implemented among countries facing the Mediterranean Sea, calling for law enforcement and implementation at the national and international levels.

Abstract

Hyperammonemia (HA) is a potential side-effect of valproate (VPA) treatment, which has been described during long-term administration. The aim of this study was to evaluate the incidence, the impact and the risk factors of HA in critically ill patients.We reviewed the data of all adult patients treated in our mixed 35-bed Department of Intensive Care over a 12-year period (2004-2015) who: a) were treated with VPA for more than 72 h and b) had at least one measurement of ammonium and VPA levels during the ICU stay; patients with Child-Pugh C liver cirrhosis were excluded. HA was defined as ammonium levels above 60 μg/dl.Of a total of 2640 patients treated with VPA, 319 patients met the inclusion criteria (median age 64 years; male gender 55%); 78% of them were admitted for neurological reasons and ICU mortality was 30%. Median ammonium levels were 88 [63-118] µg/dl. HA was found in 245 (77%) patients. For those patients with HA, median time from start of VPA therapy to HA was 3 [2-5] days. In a multivariable analysis, high VPA serum levels, mechanical ventilation and sepsis were independently associated with HA during VPA therapy. In 98/243 (40%) of HA patients, VPA was interrupted; VPA interruption was more frequent in patients with ammonium levels 100 μg/dl than others (p = 0.001). HA was not an independent predictor of ICU mortality or poor neurological outcome.In this study, HA was a common finding during treatment with VPA in acutely ill patients. VPA levels, sepsis and mechanical ventilation were risk factors for HA. Hyperammonemia did not influence patients' outcome.

Abstract

Background: The NIBIT-MESO-1 study demonstrated the activity and safety of tremelimumab combined with durvalumab in unresectable mesothelioma (Calabrò L et al. Lancet Resp Med 2018; 6:451-460). We report four-year survival and retreatment outcomes from this study, and the role of tumor mutational burden (TMB) in identifying patients with a better outcome to combined therapy. Methods: Patients with disease progression following initial clinical benefit were eligible for retreatment and received intravenous tremelimumab (1 mg/kg) and durvalumab (20 mg/kg) every four weeks (Q4W) for 4 doses, then maintenance durvalumab (20 mg/kg) Q4W for nine doses. Tumour response per immune-related (ir)-modified RECIST criteria, overall survival, safety, and immuno-biologic correlates of clinical activity were evaluated. This study is registered with ClinicalTrials.gov, number NCT0258813. Findings: At a median follow-up of 52 months (IQR: 49-53), median overall survival of the 40 patients enrolled in the study was 16·5 months (IQR: 9·4-29·3); 3- and 4-year survival rates were 20% and 14·6%, respectively. Seventeen (42·5%) patients were re-treated; 7/17 (41·2%) achieved ir-stable disease. At a median follow-up of 24 months (IQR: 22-25), median overall survival was 12·5 months (IQR: 5·8-20·5), and 2-year survival was 23·5%. There were no grade 3-4 ir-AEs. Patients with a TMB above the median value of 8·3 mutations/Mb had the highest median overall survival. Interpretation: Tremelimumab combined with durvalumab induced long-term survival in mesothelioma patients. Retreatment was clinically meaningful and safe, and merits further investigation. TMB at baseline identified patients with more favourable outcome to therapy.

Abstract

Hyperinvasive strains of Neisseria meningitidis serogroup C have caused outbreaks of severe disease in Italy. Here, we report the analysis of the migration patterns of C:P1.5-1,10-8:F3-6:ST-11(cc11) meningococcal strains from different Italian regions collected between 2012 and 2017.N. meningitidis genomes were sequenced through the whole genome sequencing (WGS) method and were analyzed using the BIGSdb Genome Comparator tool. The phylogeography was performed using BEAST. The gene flows in Italy were tested by using MacClade.The C:P1.5-1,10-8:F3-6:ST-11(cc11) hyperinvasive meningococcal strain, for the data available at the time of the analysis, from UK reached at first Emilia Romagna region, and then, in 2012, was detected in the outbreak occurred in the port of Livorno. The "Tuscany-outbreak strain" was likely introduced in Italy between 2013 and 2014. Most of the observed gene flow events occurred from the Center to Northern part of Italy.The phylogeographic analysis allowed to track the dissemination of C:P1.5-1,10-8:F3-6:ST-11(cc11) strains in the country.
