1. An investigation of the sustainability of file folders used by the campus community

Abstract

With the development of the sustainability concept and greener manufacture processes, there are more sustainable products on the market. This project has the purpose to analyze file folders used by the UBC community and determine if the “green” file folders are actually more sustainable than the “non-green” similar items and also compare these products to determine if some are better than others. A list of 11 file folders is taken from the most commonly used office supplies purchased by UBC to be analyzed. This process is done by using a Triple Bottom Line (TBL) assessment approach in which social, environmental and financial impacts are evaluated. In order to complete this analysis, an assessment tool combining 15 questions each divided equally between those three factors. Each item is evaluated based on the same criteria and is given a score for each question. After assessing all products, the total score can help compare all products against each other. This assessment using the TBL approach allows a fair and equitable comparison against similar items by making sure the three factors (social, environmental and financial) are weighted equally. This analysis shows that the products branded as “green” actually show some more sustainable characteristics that “non-green” products do not have. With that being said, all the green marketed products do not stand at the same level on the “sustainability ladder”. In fact, their final score varies from 45% to 87% based on our evaluation. As for the non-green products, they have low scores of 32% and 45%.The major differences found between the high score and the low score products are the portion of recycled content, the location of the manufacturing facility and the presence of a variety of materials involved in the manufacture making them hard to fully recycle at the end of their life. There are many differing opinions regarding recycling paper fibers, however, it is a deep and complicated subject and beyond the scope of this paper. Abstract

The efficacy and safety of prolonging prophylaxis for venous thromboembolism in medically ill patients beyond hospital discharge remain uncertain. We hypothesized that extended prophylaxis with apixaban would be safe and more effective than short-term prophylaxis with enoxaparin. METHODS: In this double-blind, double-dummy, placebo-controlled trial, we randomly assigned acutely ill patients who had congestive heart failure or respiratory failure or other medical disorders and at least one additional risk factor for venous thromboembolism and who were hospitalized with an expected stay of at least 3 days to receive apixaban, administered orally at a dose of 2.5 mg twice daily for 30 days, or enoxaparin, administered subcutaneously at a dose of 40 mg once daily for 6 to 14 days. The primary efficacy outcome was the 30-day composite of death related to venous thromboembolism, pulmonary embolism, symptomatic deep-vein thrombosis, or asymptomatic proximal-leg deep-vein thrombosis, as detected with the use of systematic bilateral compression ultrasonography on day 30. The primary safety outcome was bleeding. All efficacy and safety outcomes were independently adjudicated. RESULTS: A total of 6528 subjects underwent randomization, 4495 of whom could be evaluated for the primary efficacy outcome - 2211 in the apixaban group and 2284 in the enoxaparin group. Among the patients who could be evaluated, 2.71% in the apixaban group (60 patients) and 3.06% in the enoxaparin group (70 patients) met the criteria for the primary efficacy outcome (relative risk with apixaban, 0.87; 95% confidence interval [CI], 0.62 to 1.23; P = 0.44). By day 30, major bleeding had occurred in 0.47% of the patients in the apixaban group (15 of 3184 patients) and in 0.19% of the patients in the enoxaparin group (6 of 3217 patients) (relative risk, 2.58; 95% CI, 1.02 to 7.24; P = 0.04). CONCLUSIONS: In medically ill patients, an extended course of thromboprophylaxis with apixaban was not superior to a shorter course with enoxaparin. 3. Ventricular dyssynchrony and risk markers of ventricular arrhythmias in nonischemic dilated cardiomyopathy: a study with phase analysis of angioscintigraphy.

Abstract

Biventricular pacing is a new form of treatment for patients with dilated cardiomyopathy and ventricular dyssynchrony. Limited information is available regarding the relationship between ventricular dyssynchrony and risk markers of ventricular arrhythmias in idiopathic dilated cardiomyopathy (IDC). In 103 patients with IDC, Fourier phase analysis of both ventricles was performed from equilibrium radionuclide angiography (ERNA). The difference between the mean phase of the LV and RV was a measure of interventricular dyssynchrony, and the standard deviations of the mean phases in each ventricle measured intraventricular dyssynchrony. There were no significant differences in inter- and intraventricular dyssynchrony between patients with versus without histories of sustained VT or VF, nonsustained VT, abnormal signal-averaged ECG, or induced sustained monomorphic VT. Dyssynchrony was not related to decreased heart rate variability (HRV). LV and interventricular dyssynchrony were weakly related to QT duration and QT dispersion. During a follow-up of 27 +/- 23 months, 21 patients had major adverse cardiac events (MACE), including 7 cardiac deaths, 11 progression of heart failure leading to cardiac transplantation, and 3 sustained VT/VF. The only independent predictors of MACE were an increased standard deviation of LV mean phase (P = 0.003), a decreased HRV (standard deviation of normal-to-normal intervals, P = 0.004), and histories of previous VT/VF (P = 0.03) or nonsustained VT (P = 0.04). In conclusion, left intraventricular dyssynchrony evaluated with ERNA was an independent predictor of MACE in IDC and was not related to usual risk markers of ventricular arrhythmias. This may have implications for resynchronization therapy and/or the use of implantable cardioverter defibrillators in IDC. [ABSTRACT FROM AUTHOR]

4. Evaluating the impact of substrate addition for anaerobic co-digestion on biogas production and digestate quality: The case of deinking sludge.

Abstract

To reduce greenhouse gas emissions from organic waste, anaerobic digestion has created new opportunities for energy and nutrient recovery from these wastes. However, the use of certain organic wastes in anaerobic digestion is limited due to their atypical physicochemical characteristics (e.g. unbalanced carbon to nitrogen ratio, high ash concentration). Deinking sludge is a residue from the paper recycling industry and is one of such substrates. This study aims at evaluating the impact of deinking sludge (DS) addition into a conventional co-digestion mixture on methane production and digestate quality. To this end, an integrated method was proposed, combining the analysis of physicochemical and biodegradability characteristics with parsimonious modeling using the SYS-Metha tool. The measured characteristics of the deinking sludge showed that its potential use in mono-digestion conditions is very limited. When co-digested with food waste and municipal sludge, no significant synergies or antagonisms were found. Based on these experiments, model simulations were executed to determine the optimal conditions for co-digestion with food waste and municipal sludge. A maximum of 22% of deinking sludge on a fresh mass basis can be added into a co-digestion mixture to achieve proper wet anaerobic digestion conditions. Regarding digestate quality, the addition of DS reduced nutrient and contaminants concentrations, which have an impact on digestate management, particularly for land application. Overall, the proposed methodology in this study allows determining optimal co-digestion mixtures and highlighted the limits needing further investigation under pilot/real conditions., (Copyright © 2022 Elsevier Ltd. Abstract

Documenting patterns and outcomes of venous thromboembolism (VTE) management and degree of adherence by clinicians to treatment guidelines could help identify remediable gaps in patient care. Prospective, clinical practice-based data from Canadian outpatient settings on management of VTE, degree of adherence with treatment guidelines and frequency of recurrent VTE and bleeding during follow-up was obtained in a multicentre, prospective observational study. From 12 Canadian centres, we assessed 868 outpatients with acute symptomatic VTE who received the low-molecular-weight heparin (LMWH) enoxaparin alone or with vitamin K antagonists (VKA), at baseline and at six months (or at the end of treatment, whichever came first). Index VTE was limb deep venous thrombosis (DVT) in 583 (67.2%) patients, pulmonary embolism (PE) with or without DVT in 262 (30.2%) patients, and unusual site DVT in 23 (2.6%) patients. VTE was unprovoked in 399 (46.0%) patients, associated with cancer in 74 (8.5%) patients, transient risk factors in 327 (37.7%) patients and hormonal factors in 68 (7.8%) patients.With regard to guideline adherence, 58 (7.3%) patients received <5 days LMWH and 114 (14.5%) had overlap <1 day. Among patients with cancer-related VTE, 59.5% were prescribed LMWH monotherapy and 43.2% received such treatment for >3 months. Only 38.1% of patients with transient VTE risk factors had received thromboprophylaxis. Our study provides useful information on clinical presentation, management and related outcomes in Canadian outpatients with VTE. Our results suggest there may be important gaps in use of thromboprophylaxis to prevent VTE and use of LMWH monotherapy to treat cancer-related VTE.
