Background. Granulomatous dermatoses, particularly on facial skin, pose a diagnostic challenge, as similar histologic patterns can be produced by different causes. Aim. To evaluate the correlation between clinical suspicion and histopathological findings in various facial granulomatous dermatoses. Materials and Methods. This retrospective, cross-sectional study included all patients with the histopathological diagnosis of facial granulomatous dermatoses from the years 2016 to 2021 in an academic hospital. Demographic, clinical, and histopathologic features were reviewed and analyzed. Results. In this study, 150 histopathological records with the diagnosis of facial granulomatous dermatoses from the years 2016 to 2021 were reviewed. The most common clinical diagnosis was rosacea 34 (23.6%), followed by sarcoidosis 27 (18.8%), leishmaniasis 15 (10.4%), and granulomatous rosacea 10 (6.9%). The frequency of clinical diagnosis of rosacea (70.6), sarcoidosis (66.7), foreign body G (62.5), TB (75), pseudolymphoma (75), acne agminata (66.7), and granulomatous rosacea (70) in female patients was higher than that in males (P value = 0.03). The effect of age on the type of both clinical and histopathological diagnosis was statistically significant (P value = 0.0001 and 0.004, respectively). Conclusion. Our study contributed significantly to the understanding of the clinicopathological aspects of facial granulomatous dermatoses and advocated for a multidisciplinary approach to the diagnosis and management of these complex skin conditions. A 69-year-old male presented to the ED with a thoraco-abdominal aortic dissection (AD) (DeBakey type IIIB/Stanford classification B) and an intramural thoracic wall hematoma, detected during CT imaging. He arrived with hypertension (BP 160/92, heart rate 81) but was asymptomatic. Physical examination revealed palpable pulses. Stage IV anorectal mucosal melanoma treated with ipilimumab/nivolumab, nivolumab+axitinib, radiation, currently on nivolumab+relatlimab. Prior superior mesenteric and right external artery dissection, right common iliac artery aneurysm, central adrenal insufficiency, hypothyroidism and recent hypertension-related syncopal episodes, leading to discontinuation of Losartan. Lab work was normal, EKG showed normal sinus rhythm. Echocardiogram was normal with ejection fraction 60-65%. The patient received IV labetalol and esmolol, following which BP improved. CT indicated chronicity with proximal thrombosis, and surgical intervention was not recommended. Cardiology started coreg 3.125 mg twice daily and a low-dose statin. He was discharged with oral anti-hypertensive meds for outpatient follow-up. Hypertension, aging, atherosclerosis, connective tissue diseases like Marfan and Ehlers-Danlos syndromes, hereditary factors like Turner syndrome and bicuspid aortic valve, coarctation of the aorta, previous cardiac surgery, smoking, cocaine use, and iatrogenic factors such as VEGFR and TK inhibitors. AD results from arterial wall tearing, causing a false lumen due to blood leakage through aortic media [2]. Stress or degeneration factors contribute [2]. Axatinib, a tyrosine kinase inhibitor blocking the TK activities of VEGFR, can lead to hypertension and cardiac dysfunction linked to AD [3]. In recent years, a dramatic breakthrough in cancer treatment has arisen with the advent of immune-checkpoint inhibitor (ICI) therapy. However, these therapies are accompanied by multiple unique immune-related adverse effects that can present as an oncologic emergency to emergency department (ED). Here we describe the characteristics of cancer patients with no previous diagnosis of adrenal insufficiency who present to the emergency with low adrenocorticotropic hormone and cortisol levels following ICI therapy. Using data from a comprehensive cancer center, we conducted a retrospective cohort study of patients 18 years and older who received ICI between April 1 st, 2018, and December 31st, 2022, and presented to the ED with low ACTH and total cortisol levels. The billing database was used to exclude patients with prior diagnosis of adrenal insufficiency based on the ICD-10 codes. Demographics, clinical and cancerrelated data were collected from the institution's data warehouse. Descriptive statistics were used to analyze the data. A total of 101 cancer patients were identified based on the eligibility criteria, with a median age of 63 years (interquartile range= 58-72 years), who were mostly White race (83.2 %) and Not Hispanic or Latino ethnic group (86.1%). Melanoma, lung, renal and head and neck cancer were the most common cancer types for these patients. The median Charlson comorbidity index was 8 (interquartile range: 7–9). Most (98%) of the patients presented as level 2 or 3 on the emergency severity index. Almost all patients (94%) were admitted, of these, 26 patients (25.7%) were placed in the observation unit. Of the patients who got admitted, around 10% were placed in the intensive care unit during their hospital stay. The clinical use of ICI is continually broadening and certain ICI-induced toxicities including endocrinopathies requires immediate medical attention. We found that most patients presenting to the ED with low ACTH and cortisol levels with no previous diagnosis of adrenal insufficiency after ICI therapy require admission and close monitoring. Further studies are needed to better evaluate the incidence and outcomes of ICI-induced adrenal insufficiency among cancer patients after ICI therapy. [ABSTRACT FROM AUTHOR]
