Abstract

Background: A new method for the stimulation of the organ of equilibrium by means of a broad-scale and monochromatic near infrared emission was developed. This method should be examined within the framework of a pilot study, evaluated and its clinical possible applications examined. Patients and Methods: Healthy probands (n = 15), patients with a radical cave of the ear (n = 5), patients with a defect of the tympanic membrane (n = 5) and spontaneous nystagmus (n = 5) were examined. In healthy probands an irritation with broad-scale as well as monochromatic near infrared (NIR) was performed and compared with a water irrigation (44 degrees, 50 ml in 30 seconds). The subjective, local feelings during the application and the appearance of giddiness according to irritation were corded and the nystagmus was registered by means of videonystagmography. In patients with radical cave of the ear and tympanic membrane defects, a broad-scale NIR-irritation before a comparative irritation with warm air (44 degrees) was performed exclusively. Results: In all healthy probands, a nystagmus reaction could be seen with broad-scale and monochromatic NIR. Compared to the hot water irritation slow phase velocity (SPV) was decreased however registrable by means of Frenzel glasses and elektronystagmography during the culmination stage. In patients with radical cave (n = 4) and tympanic membrane defects (n = 3) showing paradoxical nystagmus reaction during hot air irritation, a nystagmus to the site of stimulation resulted by means of light calorisation. In patients with a spontaneous nystagmus an attenuation (n = 1) or inversion (n = 2) could be achieved by NIR-radiation. Conclusions: The method of the NIR-radiation is suitable in clinical practice for the caloric test proofing warm reaction. Vaporization cold does not occur. The application of heat charm is better proportionable and steerable than during air irritation. The procedure is sterile, noiseless and non-contact. Difficulties in interpretation of results of vestibular tests because of evaporation coldness do not occur.

Abstract

Konvektion und Warmeleitung werden als Hauptfaktoren fur die thermische Reaktion angesehen. Warmestrahlung wird als weiterer Faktor diskutiert. Fragestellung war, ob Warmestrahlung wesentlich an der thermischen Antwort beteiligt ist, ob eine thermische Reizung der Bogengange mittels Warmestrahlung emittierender Strahlungsquellen unterschiedlichen Spektrums im Nahinfrarotbereich (NIR) moglich ist und ob sich bei Probanden mit beiden Strahlungsquellen ein Nystagmus generieren lasst. Warmestrahlung (NIR) durchdringt Knochengewebe. Die Erwarmung durch beruhrungslose Applikation bei mazerierten und feuchten Knochenpraparaten tritt fast verzogerungslos ein. Mit hohen Reizstarken werden am Praparat spezifische Temperaturmaxima durch fokale, selektive Reizung der 3 Bogengange mit monochromatischem und breitbandigem NIR nachgewiesen. Ein Nystagmus wird bei Probanden (n=5) mit beiden NIR-Lichtquellen generiert. Warmestrahlung (NIR) ist am Zustandekommen von Temperaturgradienten und des Nystagmus beteiligt. Sowohl mit einer lichtleitergekoppelten breitbandigen als auch mit einer monochromatischen NIR-Strahlungsquelle ist eine Nystagmusreaktion auslosbar. Die NIR-Reizung ist beruhrungs- und gerauschlos sowie schmerzfrei.

18. Some effects of a topical collagen-based matrix on the microcirculation and wound healing in patients with chronic venous leg ulcers: preliminary observations.

Abstract

The microcirculation of the wound bed is a key parameter for improving granulation tissue formation and, hence, wound healing. The aim of this study was to determine whether a wound dressing comprising collagen/oxidized regenerated cellulose has effects over a short term on wound healing. Wounds were evaluated using a clinical wound score; pain associated with wounds was measured using a visual analogue scale. Wound microcirculation was evaluated using a technique based on noncontact remission spectroscopy. A prospective trial was performed in 40 patients with chronic venous leg ulcers (mean age 74 years; range, 43-93 years; 25 females and 15 males). Patients in group A were treated with PROMOGRAN Matrix (Johnson& Johnson, New Brunswick, NJ) combined with 'good' ulcer care for 2 weeks. Control group B consisted of 10 patients who received only good ulcer care. The authors measured a favorable clinical response in 76.9% (group A) versus 66.7% (group B). The mean reduction of ulcer area was statistically significant in group A (P < .05). The wound score improved in group A from 2.28 +/- 1.24 (before treatment) increasing to 3.72 +/- 1.57 (after 1 week; P < .00023) and 4.92 +/- 1.68 (after 2 weeks; P < .000027). In group B, the score improved from 1.44 +/- 1.33 (before treatment) to 3.22 +/- 1.30 (after 1 week; P < .0077). The mean visual analogue pain score before treatment was 8.72 (group A) and 7.88 (group B) (ns, P > .05). After 1 week of treatment, the score dropped to 5.76 (group A) and 6.66 (group B). In the second week, group A patients had a mean pain score of 3.84 compared with the pain score before treatment (P < .05). After 1 week of treatment, in group A there was a decrease in remission spectroscopy, which is considered to reflect an improvement in microcirculation. [ABSTRACT FROM AUTHOR]

25. Fluorescence lifetimes of adsorbed polymethines.

Abstract

Polymethine dyes are used as colourants and as spectral sensitizers. Thiacarbocyanine and thiadicarbocyanine dyes were studied in solution, on electronically inert (CaF) and active surfaces (AgCl, ZnO), using steady state and time resolved fluorescence spectroscopy. Fluorescence lifetimes are strongly dependent on the environment. Deactivation processes such as nonradiative decay and fluorescence are discussed. The widely accepted model for the S deactivation of polymethines is torsion of the polymethine chain. In the adsorbed state, the quantum yield of fluorescence increases and shows a strong concentration dependence. We studied 3,3′-diethylthiacarbocyanine iodide (DTCC) and 3,3′-diethylthiadicarbocyanine chloride (DTDC) in different solvents and on CaF. The adsorption isotherms of DTCC and DTDC on CaF are of non-Langmuir type. The fluorescence lifetimes of adsorbed polymethines are not constant and start to decrease even at surface concentrations which are much less than one monolayer. The quenching was observed at concentrations lower than those at which dimer formation could be spectroscopically detected. Förster energy transfer between the adsorbed molecules was therefore taken into account. On active surfaces, electron transfer and Förster energy transfer were investigated as competing processes.
