1. How Computer Literacy and Socioeconomic Status Affect Attitudes Toward a Web-Based Cohort: Results From the NutriNet-Santé Study

Abstract

BackgroundIn spite of the growing literature in the field of e-epidemiology, clear evidence about computer literacy or attitudes toward respondent burden among e-cohort participants is largely lacking. ObjectiveWe assessed the computer and Internet skills of participants in the NutriNet-Santé Web-based cohort. We then explored attitudes toward the study demands/respondent burden according to levels of computer literacy and sociodemographic status. MethodsSelf-reported data from 43,028 e-cohort participants were collected in 2013 via a Web-based questionnaire. We employed unconditional logistic and linear regression analyses. ResultsApproximately one-quarter of participants (23.79%, 10,235/43,028) reported being inexperienced in terms of computer use. Regarding attitudes toward participant burden, women tended to be more favorable (eg, “The overall website use is easy”) than were men (OR 0.65, 95% CI 0.59-0.71, P12 years of schooling) were less likely to accept the demands associated with participation (eg, “I receive questionnaires too often”) compared to their less educated counterparts (OR 1.62, 95% CI 1.48-1.76, P

Abstract

Background Medical workers are the largest group of workers occupationally exposed to low doses of ionizing radiation (IR) worldwide. Aims This review presents all the cohorts of medical workers exposed in the world and summarizes cancer risks associated with radiation exposure in this population. Methods Epidemiological studies on health professionals exposed to IR published from 1975 to 2019 were reviewed. Strength of evidence was assessed according to the modified Royal College of General Practitioners three-star system. Results Among the 16 epidemiological studies focusing on cancers from 11 cohorts among medical staff exposed to radiation, higher risks of cancer were observed for pre-1950 exposure and for medical workers who performed fluoroscopically guided interventional procedures or radionuclides procedures compared to those who did not. However, strength of evidence supporting the associations remains moderate as several methodological limits including the lack of dosimetry data, lifestyle factors and recent updates may obscure the link between medical occupational exposure and cancer occurrence. Conclusions Excess risk of cancer is suspected for high and former exposures. The more highly exposed professions, i.e. interventional physicians and nuclear medicine workers, should be monitored carefully. Collaborative projects aiming to increase the quantity and quality of information of the studied populations would be a key point

14. Vigorous Physical Activity and Cognitive Trajectory Later in Life: Prospective Association and Interaction by Apolipoprotein E e4 in the Nurses' Health Study.

Abstract

Background: The apolipoprotein E (APOE) e4 allele is a well-established genetic risk factor of brain aging. Vigorous physical activity may be particularly important in APOE-e4 carriers, but data have been inconsistent, likely due to differences in the timing of the physical activity assessment, definition of cognitive decline, and/or sample size.Methods: We prospectively evaluated the association between vigorous physical activity and cognition assessed at least 9 years later, according to APOE-e4 carrier status. Biennially from 1986, Nurses' Health Study participants reported their leisure-time physical activities. Starting in 1995-2001 and through 2008, participants (aged 70+ years) underwent up to 4 repeated cognitive telephone assessments (6 tasks averaged together using z-scores).Results: Among 7252 women, latent process mixed models identified 3 major patterns of cognitive change over 6 years: high-stable, medium-stable, and decline. Taking the high-stable cognitive trajectory as the outcome reference in multinomial logistic regressions, highest tertile of vigorous physical activity (≥5.9 metabolic-equivalent [MET]-hours/wk) compared to lowest tertile (≤0.9 MET-hours/wk) was significantly associated with subsequent lower likelihood of the medium-stable trajectory in the global score (odds ratio [OR] [95% CI] = 0.72 [0.63, 0.82]), verbal memory (OR [95% CI] = 0.78 [0.68-0.89]), and telephone interview of cognitive status score (OR [95% CI] = 0.81 [0.70-0.94]). Vigorous physical activity was also associated with lower likelihood of decline in category fluency (OR [95% CI] = 0.72 [0.56, 0.92]). We observed some evidence (p-interaction = .07 for the global score) that the association was stronger among APOE-e4 carriers than noncarriers (OR [95% CI] = 0.60 [0.39, 0.92] vs 0.82 [0.59, 1.16]).Conclusion: Midlife vigorous physical activity was associated with better cognitive trajectories in women in their seventies, with suggestions of stronger associations among APOE-e4 carriers. Abstract

n France, the interest of the public and cancer patients for the practice of fasting is increasing. The systematic review of the scientific literature conducted by the NACRe expert group shows that currently, there is no evidence in humans of a beneficial effect of fasting on the occurrence of cancer and during cancer treatments. Since fasting can be a risk of worsening undernutrition and muscle loss, it is therefore essential that health professionals inform their patients.; En France, l’engouement du public et des patients atteints de cancer pour la pratique du jeûne est en plein essor. La revue systématique de la littérature scientifique réalisée par le groupe d’experts du réseau National Alimentation Cancer Recherche (NACRe) montre qu’actuellement il n’y a pas de preuve chez l’homme d’un effet bénéfique du jeûne sur la survenue de cancer et au cours des traitements du cancer. Cette pratique pouvant présenter un risque d’aggravation de la dénutrition et de perte musculaire, il est essentiel que les professionnels de santé en informent leurs patients.

18. Red and processed meat intake and cancer risk: Results from the prospective NutriNet-Santé cohort study

Abstract

The International Agency for Research on Cancer (WHO-IARC) classified red meat and processed meat as probably carcinogenic and carcino genic for humans, respectively. These conclusions were mainly based on studies concerning colorectal cancer, but scientific evidence is still limited for other cancer locations. In this study, we investigated the prospective associations between red and processed meat intakes and overall, breast, and prostate cancer risk. This prospective study included 61,476 men and women of the French NutriNet Santé cohort (2009-2015) aged ≥35y and who completed at least 3 24h dietary records during the first year of follow-up. The risk of developing cancer was compared acro ss sex specific quintiles of red and processed meat intakes by multivariable Cox models. 1,609 first primary incident cancer cases were diagnosed during follow up, among which 544 breast cancers and 222 prostate cancers. Red meat intake was associated with increased risk of overall cancers (HR Q5vs.Q1 =1.31 (1.10–1.55), P trend =0.01) and breast cancer (HR Q5vs.Q1 =1.83 (1.33–2.51), P trend =0.002). The latter association was observed in both premenopausal (HR Q5vs.Q1 =2.04 (1.03 4.06)) and postmenopausal women (HR Q5vs.Q1 =1.79 (1.26 2.55)). No association was observed bet ween red meat intake and prostate cancer risk. Processed meat intake was relatively l ow in this study (cut offs for the 5 th quintile=46g/d in men and 29g/d in women) and was n ot associated with overall, breast or prostate cancer risk. This large cohort study suggested that red meat may be involved carcinogenesis at several cancer locations (other than colon rectum), in particular breast cancer. These results are consistent with mechanistic evidence from experimental studies.

Abstract

This work was conducted in the framework of the French network for Nutrition And Cancer Research (NACRe network), www.inra.fr/nacre.; International audience; Circadian disruption has been classified as probably carcinogenic to humans by the World Health Organization's International Agency for Research on Cancer. The circadian clock is subject to environmental factors, particularly light exposure and food intake rhythms. However, the association between nutritional circadian behaviours and cancer is not well understood. We investigated the longitudinal associations between number of eating episodes, night-time fasting duration, time of first and last eating episodes, as well as nutritional quality of last eating episode, respectively, with breast and prostate cancer risks, the two main cancer locations in women and men respectively. This prospective study included 41,389 day-working adults in the French NutriNet-Santé cohort (2009-2016) who completed at least three 24 h dietary records during the first 2 years of follow-up. Multivariable Cox models were computed. 1,732 first primary incident cancer cases were diagnosed during the follow-up, among which 428 breast and 179 prostate cancers. After adjustment for covariates including sleep duration, late eaters (last eating episode after 9:30 pm) had an increased risk of breast (Hazard ratio [HR] = 1.48 [1.02-2.17], p = 0.03) and prostate (HR = 2.20 [1.28-3.78], p = 0.004) cancers. However, no association was observed between cancer risk and number of eating episodes, night-time fasting duration, time of first eating episode or macronutrient composition of the last eating episode. This large cohort study suggests that circadian perturbations resulting from late time of last food intake may be involved in carcinogenesis at different locations. Beyond nutritional quality of food intake, nutritional circadian regulation should be further investigated in the context of cancer prevention.

Un score reflétant une alimentation plus riche en produits végétaux et moins riche en produits animaux a récemment été développé et a été associé à une diminution de la mortalité globale chez des sujets à risque cardiovasculaire. Cependant, à notre connaissance, l’association entre ce score et le risque de cancers n’a jamais été explorée. L’objectif de ce travail était d’étudier les associations entre ce score « pro-végétal » et le risque de cancers. Méthodes : Cette étude prospective a inclus 42 544 hommes et femmes, âgés de 45 ans et plus, issus de la cohorte NutriNet-Santé (2009-2016), qui ont complété au moins 3 enregistrements alimentaires de 24h pendant la première année de suivi. Le risque de développer un premier cancer a été estimé par un modèle de Cox multivariable par tertile sexe-spécifique du score « pro-végétal ». Résultats : 1 591 premiers cas de cancers incidents ont été diagnostiqués au cours du suivi, parmi lesquels 487 cas de cancers du sein, 243 cas de cancers de la prostate, 198 cas de cancers digestifs et 68 cas de cancers du poumon. Un score « pro-végétal » plus élevé était associé à une diminution du risque de cancers au global (RRt3vs.t1=0,85; IC 95 % 0,76, 0,97; ptendance=0,01), de cancers digestifs (RRt3vs.t1=0,68; IC 95 % 0,47; 0,99; ptendance =0,04) et de cancers du poumon (RRt3vs.t1=0,47; IC 95 % 0,25, 0,90; ptendance =0,02). Conclusions et perspectives : Cette étude suggère qu’un régime alimentaire plus riche en produits végétaux et moins riche en produits animaux contribuerait à la prévention des cancers.

Abstract

IF 7.36; International audience; Nutrition is often used by cancer survivors as a lever to take charge of their own health. However, some dietary behaviors are not currently recommended for patients without medical supervision. Our study aimed at evaluating weight-loss restrictive diets and fasting practices among cancer survivors of the NutriNet-Santé cohort, as well as related socio-demographic and lifestyle factors. In October 2016, 2,741 cancer survivors had completed a specific questionnaire about their practices. Fasting and non-fasting patients (respectively dieting and non-dieting) were compared using logistic regression models. Analyses were weighted according to the age, gender, and cancer location distribution of French cancer cases. 13.8% had already practiced weight-loss restrictive diet as their diagnosis. They were more likely to be women, professionally active, overweight/obese, to use dietary supplements and to have breast cancer (all p < 0.05). 6.0% had already fasted, 3.5% as their diagnosis. They were more likely to be younger, with higher educational level, higher incomes, professionally active, to have a healthy weight, and to use dietary supplements (all p < 0.05). Fasting was associated with the opinion that such practice could improve cancer prognosis (p < 0.0001). Patients who received nutritional information from health care professionals were less likely to practice fasting or weight-loss restrictive diet (0.42[0.27-0.66], p < 0.0001 and 0.49[0.38-0.64], p < 0.0001 respectively). Our study provided original results suggesting that weight-loss restrictive diets are widely practiced by cancer survivors. Fasting was less common in our study though non negligible. Sources of nutritional information received as cancer diagnosis seemed to be a key determinant of these practices.

Abstract

While cancer survivors are at increased risk for negative conditions as second cancers, other comorbidities, and functional decline ; lifestyle factors, such as a healthy diet, regular exercise and weight control, may contribute to prevent these conditions and improve survivors' quality of life. In this context, aims of this thesis were, among cancer survivors from the French prospective NutriNet-Santé study, to 1) investigate nutritional variations between before and after cancer diagnosis as well as dietary supplements use, and 2) evaluate opinions relating to some diet factors and to weight-loss restrictive diets and fasting practices, and to link them on the one hand to their real practices, and, on the other hand, to their sources of nutritional information.In the first part, our results highlight some healthy behaviors such as a decrease in alcohol and sweetened drinks consumption, but also less favorable trends, such as a decrease in vegetable consumption and in many vitamin and mineral intakes. We also observed a decline in overall and vigorous physical activity after diagnosis, especially in prostate and skin cancers, in men and professionally inactive patients. Concomitantly, we observed an increase in sedentary behaviors, especially in women, older subjects and professionally inactive patients. We also observed that while weight loss was reported in many colorectal cancer patients, a substantial proportion of breast cancer patients gained weight. Sociodemographic and economic factors appeared as important determinants of weight gain, illustrating social inequalities in health (higher risk among patients with lower income and lower education). Our results suggest that dietary supplements use was widespread among cancer survivors, a large amount of which being used without any medical supervision, including a non-negligible proportion of patients having dietary supplement practices which can be considered as “at risk”. In a second part, we observed that opinions from cancer survivors regarding some nutritional factors seemed to impact their dietary practices and were themselves impacted by sources of nutritional information. In particular, opinions regarding alcohol consumption were concerning, with an important proportion of cancer survivors who thought that alcohol consumption (and even more regarding red wine) had a positive impact on their disease. Weight-loss restrictive diets were practiced by a large number of cancer survivors since their diagnosis, while fasting was less practiced, but was far from being an isolated phenomenon.; Alors que les survivants du cancer sont à risque plus élevé de second cancer, d’autres pathologies et de déclin fonctionnel ; les facteurs de mode de vie, comme une alimentation équilibrée, une pratique d’activité physique et le contrôle d’un poids sain, peuvent contribuer à prévenir ces risques et améliorer la qualité de vie des survivants du cancer. Dans ce contexte, les objectifs de cette thèse étaient, parmi les survivants du cancer de la cohorte française prospective NutriNet-Santé, 1) d’investiguer les variations du comportement nutritionnel entre avant et après diagnostic de cancer ainsi que la consommation de compléments alimentaires, et 2) d’évaluer les opinions vis-à-vis de certains facteurs nutritionnels, et de les mettre en regard de leurs pratiques alimentaires réelles d’une part, et de leurs sources d’information nutritionnelles d’autre part. Dans un premier temps, nous avons observé des tendances favorables à la santé comme une diminution de la consommation d’alcool et des boissons sucrées non alcoolisées, et d’autres moins favorables comme une diminution de la consommation de légumes et de plusieurs apports en vitamines et minéraux. Un déclin de l’activité physique globale et intense était observé. De manière cohérente, nous avons observé une augmentation des comportements sédentaires, particulièrement chez les femmes, les sujets plus âgés et les patients qui n’étaient pas actif professionnellement. Nous avons observé une perte de poids chez certains cas de cancers colorectaux, alors qu’une prise de poids était observée chez les cas de cancers du sein et de la peau. Les facteurs sociodémographiques et économiques semblaient influencer le risque de prise de poids, suggérant des inégalités sociales de santé (risque plus élevé chez les patients ayant des revenus et un niveau d’étude plus faible). Nos résultats suggéraient que la consommation de compléments alimentaires était largement répandue chez les survivants du cancer, en grande partie sans suivi médical, incluant une proportion non négligeable de patients ayant des pratiques de consommation de compléments alimentaires pouvant être considérées comme « à risque ».Dans un second temps, nous avons observé que les opinions des survivants du cancer concernant certains facteurs nutritionnels semblaient impacter leurs pratiques alimentaires et étaient eux-mêmes impactés par les sources d’informations auxquelles les survivants du cancer avaient eu accès depuis leur diagnostic de cancer. En particulier, les opinions concernant la consommation d’alcool étaient préoccupantes, avec une proportion importante de survivants du cancer qui pensaient que la consommation d’alcool (et plus encore concernant le vin rouge) avait un impact positif sur leur maladie. Les régimes restrictifs pour perdre du poids étaient pratiqués par une grande partie de notre échantillon ; le jeûne était moins pratiqué, mais était loin d’être un phénomène isolé.

Abstract

International audience; PURPOSE: While many cancer patients are affected by weight loss, others tend to gain weight, which may impact prognosis and risk of recurrence and of second cancer. The aim of this prospective study was to investigate weight variation between before and after cancer diagnosis and socio-demographic, economic, lifestyle and clinical factors associated with moderate-to-severe weight gain.METHODS: 1051 incident cases of first primary cancer were diagnosed in the NutriNet-Santé cohort between 2009 and 2015. Weight was prospectively collected every 6 months since subjects' inclusion (i.e. an average of 2y before diagnosis). Mean weights before and after cancer diagnosis were compared with paired Student's t-test. Factors associated with moderate-to-severe weight gain (≥5% of initial weight) were investigated by age and sex-adjusted logistic regression.RESULTS: Weight loss was observed in men (-3.54±4.39kg in those who lost weight, p=0.0002) and in colorectal cancer patients (-3.94±4.40kg, p=0.001). Abstract

The International Agency for Research on Cancer (WHO-IARC) classified red meat and processed meat as probably carcinogenic and carcino genic for humans, respectively. These conclusions were mainly based on studies concerning colorectal cancer, but scientific evidence is still limited for other cancer locations. In this study, we investigated the prospective associations between red and processed meat intakes and overall, breast, and prostate cancer risk. This prospective study included 61,476 men and women of the French NutriNet Santé cohort (2009-2015) aged ≥35y and who completed at least 3 24h dietary records during the first year of follow-up. The risk of developing cancer was compared acro ss sex specific quintiles of red and processed meat intakes by multivariable Cox models. 1,609 first primary incident cancer cases were diagnosed during follow up, among which 544 breast cancers and 222 prostate cancers. Red meat intake was associated with increased risk of overall cancers (HR Q5vs.Q1 =1.31 (1.10–1.55), P trend =0.01) and breast cancer (HR Q5vs.Q1 =1.83 (1.33–2.51), P trend =0.002). The latter association was observed in both premenopausal (HR Q5vs.Q1 =2.04 (1.03 4.06)) and postmenopausal women (HR Q5vs.Q1 =1.79 (1.26 2.55)). No association was observed bet ween red meat intake and prostate cancer risk. Processed meat intake was relatively l ow in this study (cut offs for the 5 th quintile=46g/d in men and 29g/d in women) and was n ot associated with overall, breast or prostate cancer risk. This large cohort study suggested that red meat may be involved carcinogenesis at several cancer locations (other than colon rectum), in particular breast cancer. These results are consistent with mechanistic evidence from experimental studies.

Results: Weight loss was observed in men (-3.54kg in those who lost weight, p=0.0002) and in colorectal cancer patients (-3.94kg, p=0.0012). Weight gain was observed in breast and skin cancers (2.83kg, p=0.047, and 2.96kg, p=0.03 respectively). Women (OR=1.99[1.18-3.35]), younger patients (OR=1.78[1.05-3.03]), those with lower education (OR=2.17[1.07-4.37]), those with excess weight before diagnosis (OR=1.53[1.02-2.30]) and those who stopped smoking after diagnosis (OR=4.60[2.06-10.25]) were more likely to experience moderate-to-severe weight gain. In breast cancer patients, induced menopause was associated with weight gain (OR=4.12[1.76-9.67]), but no association was detected for tumor characteristics or treatments. Conclusion: This large prospective cohort provided original results on weight variation between before and after cancer diagnosis, highlighting different weight trajectories. Socio-demographic and economic factors appeared to strongly influence the risk of weight gain, illustrating social inequalities in health.

Abstract

Background: The Food Standards Agency Nutrient Profiling System (FSA-NPS) constitutes the basis for the Five-Colour Nutrition Label suggested in France to be put on the front-of-pack of food products. At the individual level, a dietary index (FSA-NPS DI) has been derived and validated and corresponds to a weighted mean of all FSA-NPS scores of foods usually consumed by the individual, reflecting the nutritional quality of his/her diet. Our aim was to investigate the association between the FSA-NPS DI and breast cancer risk in a large cohort. Methods: This prospective study included 46,864 women aged over 35y from the NutriNet-Santé cohort (2009-2015) who completed at least three 24h dietary records during the first 2y of follow-up (median follow-up: 4.0y). 555 incident breast cancers were diagnosed. FSA-NPS DI was computed for each subject using the following nutrient content for 100g of each foods and beverages consumed: energy, total sugar, saturated fatty acid, sodium, fruits and vegetables (%), fibres and proteins. Higher values of the FSA-NPS DI correspond to a lower nutritional quality of the diet. Associations were characterized by multivariate Cox proportional hazards models. Results: The FSA-NPS DI was directly associated with breast cancer risk (HR1-point increment=1.06 (1.02-1.11), P-trend=0.005; HRQ5vs.Q1=1.52 (1.11-2.08), P-trend=0.002). These associations were similar after the exclusion of cases diagnosed during the first year of follow-up. Conclusions: In this prospective study, a higher FSA-NPS individual score was associated with an increased breast cancer risk. Abstract

National audience; Purpose: While many cancer patients are affected by weight loss, others tend to gain weight, which may impact prognosis and risk of recurrence and of second primary cancer. The aim of this prospective study was to investigate weight variation between before and after cancer diagnosis and socio-demographic, economic, lifestyle and clinical factors associated with moderate-to-severe weight gain. Materials and methods: 1051 incident cases of first primary cancer were diagnosed in the NutriNet-Santé cohort between 2009 and 2015. Weight was prospectively collected every 6 months since subjects’ inclusion (i.e. an average of 2y before diagnosis). Mean weights before and after cancer diagnosis were compared with paired Student's t-test. Factors associated with moderate-to-severe weight gain (≥5% of initial weight) were investigated by multivariable logistic regression. Results: Weight loss was observed in men (-3.54kg in those who lost weight, p=0.0002) and in colorectal cancer patients (-3.94kg, p=0.0012). Weight gain was observed in breast and skin cancers (2.83kg, p=0.047, and 2.96kg, p=0.03 respectively). Women (OR=1.99[1.18-3.35]), younger patients (OR=1.78[1.05-3.03]), those with lower education (OR=2.17[1.07-4.37]), those with excess weight before diagnosis (OR=1.53[1.02- 2.30]) and those who stopped smoking after diagnosis (OR=4.60[2.06-10.25]) were more likely to experience moderate-to-severe weight gain. In breast cancer patients, induced menopause was associated with weight gain (OR=4.12[1.76-9.67]), but no association was detected for tumor characteristics or treatments. Conclusion: This large prospective cohort provided original results on weight variation between before and after cancer diagnosis, highlighting different weight trajectories. Socio-demographic and economic factors appeared to strongly influence the risk of weight gain, illustrating social inequalities in health.
