Resumo O modelo linear de produção e consumo prejudica o meio ambiente e acelera a escassez de recursos naturais. Em contrapartida, a Economia Circular (EC) surge como modelo que visa manter produtos, componentes e materiais em seu mais alto nível de utilidade e valor o tempo todo. Esta pesquisa busca responder à seguinte questão-problema: como a literatura internacional aborda a relação entre Energias Renováveis e Economia Circular em um contexto industrial? Como objetivo geral, tem-se: identificar, na literatura internacional, como os termos Energias Renováveis e Economia Circular se relacionam no contexto industrial. A metodologia aplicada consistiu em uma revisão sistemática da literatura associada à bibliometria nas bases de dados da Scopus e Web of Science, aplicando os termos: “renewabl* energ*” AND “circular economy” AND “industr*”. Os indicadores bibliométricos foram analisados com o auxílio dos softwares MINITAB e VOSviewer. A partir da amostra final, de 88 artigos, os países que mais se destacaram em número de publicações foram a Itália, seguida pela China, Alemanha e Reino Unido. Notou-se que os anos com maior quantidade de publicações foram 2018, 2019 e 2020, sendo que a primeira publicação surgiu em 2012. Dentre os principais resultados, verificou-se que as pesquisas, em sua maioria, são estudos de casos que diagnosticam uma realidade e propõem soluções eficientes junto aos processos produtivos e a redução de impactos ambientais. Por fim, concluiu-se que a Economia Circular é um modelo progressista de produção e consumo, que respeita a limitação dos recursos naturais, ao passo que atende às necessidades humanas.

O objetivo geral desta tese é analisar os elementos críticos da Simbiose Industrial (SI) em direção a um modelo circular de produção. A tese é composta por três estudos interdependentes, realizados sequencialmente, na seguinte ordem: (1) análise dos processos de implementação da SI nos parques industriais de Kalundborg/Dinamarca, Ulsan/Coreia do Sul e Kwinana/Austrália. A partir dos dados levantados, um quadro teórico-analítico foi proposto com base nas convergências encontradas nos três casos. Os resultados mostram que as decisões relacionadas à implementação da SI foram moldadas por um conjunto semelhante de variáveis que incluem: a diversidade de indústrias; a viabilidade econômica das trocas; questões ambientais críticas como motivadores de práticas mais sustentáveis; acordos bilaterais como precursores do SI; engajamento coletivo por meio de fóruns, clubes, conselhos e associações; confiança para estabelecer relações de cooperação; estratégias de comunicação e compartilhamento de informações; uma estrutura regulatória alinhada nos níveis nacional, regional e local; congruência entre as ações do governo e da empresa para criar um ambiente cooperativo; e uma estrutura de governança que envolve o governo local, empresas, instituições de P&D e uma entidade coordenadora. (2) de modo a complementar os dados do primeiro estudo, evidências empíricas sobre a SI no Brasil foram levantadas a partir da análise do comportamento das organizações no processo de implementação da SI no Projeto Piloto de EC do distrito industrial de Sete Lagoas. A contribuição do estudo 2 para o objetivo geral da pesquisa está em apontar os elementos críticos do processo de SI a partir da realidade brasileira uma vez que o corpo teórico de SI se sustenta basicamente a partir de casos de implementação de SI em países desenvolvidos. (3) os dados empíricos oriundos dos estudos 1 e 2 evidenciaram que a compreensão a respeito da mudança de comportamento das organizações na transição para o modelo circular não se restringe apenas às dimensões técnicas e econômicas, o que expôs as fragilidades teóricas do campo quando se trata das dimensões sociais, culturais, institucionais e organizacionais. Nesse sentido, o estudo três integrou as abordagens teóricas do Neoinstitucionalismo e das Relações Interorganizacionais ao corpo teórico da SI de modo a contribuir na construção de um entendimento mais amplo. The general objective of this thesis is to analyze the critical elements of Industrial Symbiosis (IS) towards a circular model of production. The thesis is composed by three interdependent studies, carried out sequentially, in the following order: (1) analysis of the IS implementation processes in the industrial parks of Kalundborg/Denmark, Ulsan/South Korea and Kwinana/Australia. From the data collected, a theoretical-analytical framework was proposed with basis on the convergences found in the three cases. The results show that decisions related to IS implementation were molded by a similar set of variables that include: the diversity of industries; the economic viability of the exchanges; critical environmental issues as motivators for more sustainable practices; bilateral agreements as precursors to IS; collective engagement through forums, clubs, councils, and associations; trust to establish cooperative relationships; communication and information sharing strategies; an aligned regulatory framework at the national, regional, and local levels; congruence between government and company actions to create a cooperative environment; and a governance structure that involves local government, companies, R&D institutions, and a coordinating entity. (2) In order to complement the data from the first study, empirical evidence on IS in Brazil was raised from the analysis of the behavior of organizations in the IS implementation process in the CE Pilot Project of the Sete Lagoas industrial district. The contribution of study 2 to the general research objective is in pointing out the critical elements of the IS process from the Brazilian reality since the IS theoretical body is basically sustained from IS implementation cases in developed countries. (3) The empirical data from studies 1 and 2 showed that the understanding about the change in behavior of organizations in the transition to the circular model is not restricted only to the technical and economic dimensions, which exposed the theoretical weaknesses of the field when it comes to the social, cultural, institutional and organizational dimensions. In this sense, the study three integrated the theoretical approaches of Neoinstitutionalism and Interorganizational Relations to the IS theoretical body in order to contribute to the construction of a broader understanding.

A anafilaxia é uma reação sistémica grave que pode ocorrer em qualquer momento do ato anestésico. A anafilaxia perioperatória é uma das causas mais frequentes de anafilaxia fatal na idade adulta. Neste artigo de revisão serão abordados os principais aspetos do conhecimento atual sobre anafilaxia perioperatória (APO), destacando-se as manifestações clínicas e fatores de risco, os principais agentes anestésicos envolvidos, o algoritmo diagnóstico e as medidas preventivas. Realça-se a alta eficácia dos testes cutâneos no diagnóstico da alergia, quando aplicada uma metodologia rigorosa, que passa pela realização de testes a todos os anestésicos e outras substâncias a que o doente esteve exposto, utilizando concentrações determinadas. São ainda abordadas as particularidades de situações que exibem uma abordagem diferenciada: outros procedimentos médicos sob anestesia geral, a idade pediátrica, o asmático e a anestesia de urgência. Destaca-se a importância da parceria entre a anestesia e a imunoalergologia no rastreio dos doentes em risco de APO e da orientação precoce nas suspeitas de sensibilização a agentes anestésicos ou outras substâncias usadas no perioperatório, de modo a minimizar os casos de APO e complicações cirúrgicas. Perioperative anaphylaxis is a frequent cause of fatal anaphylaxis in a hospital environment in adulthood. We herein review the current knowledge about perioperative anaphylaxis (POA). We focus on the clinical manifestations associated with the main anesthetic agents, the diagnostic algorithm, the risk factors and the preventive measures of POA. Skin tests are efficacious for the diagnosis of allergy to perioperative agents. A rigorous methodology is required; all anesthetics and other substances to which the patient was exposed need to be tested and standard concentrations must be used. We also discuss the particularities of situations that require a differentiated approach, namely, other medical procedures under general anesthesia, the pediatric age, the asthmatic patient and the emergent anesthesia. The importance of the implementation of protocols between anesthesia and immunoallergology is emphasized in the early investigation of cases of suspected sensitization to anesthetic agents or other substances used in the perioperative period, in order to minimize cases of POA and medical or surgical complications.

Background: Anaphylaxis is increasing at pediatric age; however, its characterization is hampered by underdiagnosis and underreporting. The aim of this study was to identify the causes of anaphylaxis in children and adolescents in Portugal, thus contributing to a better knowledge of its etiology, clinical manifestations, and management. Methods: During a 10‐year period, a nationwide notification system for anaphylaxis was implemented, with voluntary reporting by allergists. Data on 533 patients under 18 years of age with anaphylaxis were included. Results: Mean age was 8.5 ± 4.9 years, 61% were male; 45% had asthma. Mean age at the first anaphylaxis episode was 5.3 ± 4.7 years (ranging from 1 month to 17 years of age), 63% at pre‐school age. Most reactions occurred at home (57%). Food‐induced anaphylaxis was the leading cause (77%). The main culprit foods were cow's milk (32%), tree nuts (16%), shellfish (13%), egg (12%), fresh fruits (11%), fish (8%), and peanut (8%). Other causes included drugs (11%), insect sting (5%), cold‐induced anaphylaxis (4%), exercise‐induced anaphylaxis (2%), latex (1%), and idiopathic anaphylaxis (1%). Most patients (83%) were admitted to the emergency department; only 46% received adrenaline treatment. Recurrence of anaphylaxis occurred in 41% of the patients (3 or more episodes in 21%). An adrenaline autoinjector was used in 9% of the patients. Conclusions: In the Portuguese pediatric population, food is the leading cause of anaphylaxis. Undertreatment with adrenaline and high recurrence of anaphylaxis highlight the need to improve both the diagnosis and the therapeutic management of this life‐threatening entity. Introdução: A asma é uma das doenças crónicas não transmissíveis mais frequentes afetando cerca de 700 000 portugueses atualmente. Os rastreios constituem uma oportunidade de prestar esclarecimentos sobre a doença à população, alertar a população para a importância do controlo da asma e reconhecimento dos seus sintomas. Objetivos, material e métodos: Caracterização sociodemográfica e descritiva dos resultados da campanha “Vencer a Asma 2017”. Estudo transversal em 8 cidades de Portugal continental numa iniciativa pública, gratuita e de inclusão voluntária da população a propósito do Dia Mundial da Asma. Aplicação de questões adaptadas do questionário GA2LEN e realização de espirometria simples. Resultados: 1086 participantes (idade 52,5±19,7 anos; 62,4 % do género feminino; 20,5 % fumadores e 62,6 % observados na região Norte). Dos participantes, 299 (28,0 %; IC95 % 25,3-30,7) declararam ter asma atual ou no passado (autorreportada) e, entre estes, 80 (26,8 %; IC95 % 21,7-31,8) reportaram pelo menos uma hospitalização por asma, 181 (60,5 %; IC95 % 55,0-66,1) afirmaram estar medicados para a asma; 498 (46,6%; IC95% 43,6-49,6) mencionaram ter tido um episódio de sibilância nos últimos 12 meses. Durante o rastreio, 549 participantes (50,6%; IC95 % 47,6-53,5) realizaram espirometria simples e, destes, 186 (34,1%; IC95 % 30,1-38,1) apresentaram alterações dos parâmetros espirométricos avaliados. A análise de subgrupos revelou uma prevalência de “asma atual” de 19,9% (IC95%; 17,5-22,2). Conclusões: O rastreio permitiu, apesar das suas limitações, identificar um grande número de indivíduos na comunidade num curto espaço de tempo. No futuro, será útil que estas iniciativas incluam questionários de resultados reportados pelo doente, testes cutâneos por picada, prova de broncodilatação ou fração exalada de óxido nítrico, que permitam uma melhor caracterização da população estudada bem como a perceção de necessidade de avaliação clínica posterior nos casos em que não haja um adequado acompanhamento médico. The Portuguese Severe Asthma Registry (Registo de Asma Grave Portugal, RAG) was developed by an open collaborative network of asthma specialists. RAG collects data from adults and pediatric severe asthma patients that despite treatment optimization and adequate management of comorbidities require step 4/5 treatment according to GINA recommendations. In this paper, we describe the development and implementation of RAG, its features, and data sharing policies. The contents and structure of RAG were defined in a multistep consensus process. A pilot version was pretested and iteratively improved. The selection of data elements for RAG considered other severe asthma registries, aiming at characterizing the patient's clinical status whilst avoiding overloading the standard workflow of the clinical appointment. Features of RAG include automatic assessment of eligibility, easy data input, and exportable data in natural language that can be pasted directly in patients' electronic health record and security features to enable data sharing (among researchers and with other international databases) without compromising patients' confidentiality. RAG is a national web-based disease registry of severe asthma patients, available at asmagrave.pt. It allows prospective clinical data collection, promotes standardized care and collaborative clinical research, and may contribute to inform evidence-based healthcare policies for severe asthma. publishersversion published

A maioria dos asmáticos apresenta doença controlada sob corticoterapia inalada, associada ou não a broncodilatadores. No entanto, cerca de 5 % dos asmáticos apresenta asma grave refratária à terapêutica convencional com grande impacto na qualidade de vida. Este subgrupo de doentes é responsável por grande parte dos internamentos, idas ao serviço de urgência, recurso a consultas não programadas e mortes por asma. Nos últimos anos, foram desenvolvidos vários anticorpos monoclonais para o tratamento da asma refratária. Anticorpos monoclonais anti-IL5 têm mostrado eficácia na asma eosinofílica e biológicos promissores estão a ser desenvolvidos tendo como alvo as citocinas IL-4, IL-13 e TSLP. Esta terapêutica dirigida depende do conhecimento da fisiopatologia dos subtipos de doença. A caracterização dos endótipos da asma e seus biomarcadores é fundamental para determinar, de uma forma personalizada, os doentes que podem beneficiar destas terapias específicas Most of asthmatic patients maintain controlled symptoms with inhaled glucocorticoid therapy with or without bronchodilators. However, about 5% of asthma patients presents severe asthma, refractory to conventional therapy, with a great impact in quality of life. This subgroup of asthmatics accounts for the majority of hospital admissions, emergency visits, non-programmed consultations and asthma-related deaths. Recently, several biologic therapeutics were developed for the treatment of severe refractory asthma. Anti-IL-5 monoclonal antibodies have shown efficacy in eosinophilic asthma. Promising therapeutics targeting IL-4, IL-13 or TSLP are currently under development. The use of specific targeting treatments requires a deep knowledge of the underlying physiopathology of the disease. Therefore, it is of the utmost importance that asthma endotypes are well characterized and that relevant biomarkers are available in order to determine, in a personalized manner, which specific therapy is appropriate for each asthmatic patient

Diante de um cenário de implementação de processos avaliativos no Ensino Superior desde a década de 90, a análise deste estudo baseou-se na possibilidade de utilização dos dados extraídos do Questionário Socioeconômico do ENADE, exame nacional de desempenho estudantil, que avalia o rendimento dos alunos da educação superior. A pesquisa teve por objetivo principal a identificação dos fatores determinantes do desempenho dos discentes dos cursos de Administração a fim de que esses apontamentos pudessem servir de subsídio para o direcionamento das políticas públicas de Educação. Para tanto, fez-se o uso dos microdados do ENADE 2012 dos cursos de Administração do Distrito Federal disponibilizados em sítio eletrônico do INEP. A pesquisa tem características de análise predominantemente quantitativa, baseada em uso de técnica estatística descritiva e multivariada. De início foi realizada a análise descritiva e exploratória de dados para cada uma das variáveis do estudo a fim de identificar o perfil dos alunos dos cursos de Administração do DF. Em seguida, procedeu-se a análise fatorial exploratória do instrumento de pesquisa com o objetivo de definir a estrutura subjacente à matriz de dados. Na sequência, a técnica multivariada de regressão múltipla foi utilizada para verificar se as características próprias do estudante, como aspectos pessoais e socioeconômicos, as características da instituição, a organização didático-pedagógico seriam variáveis significativas na explicação do resultado do ENADE 2012. Os resultados da regressão confirmaram as seguintes hipóteses: (H1) a renda familiar tem influência positiva no desempenho do aluno; (H2) a variável escolaridade dos pais está positivamente relacionada ao desempenho dos alunos; (H4) a qualificação do corpo docente está positivamente relacionada ao desempenho do aluno e (H5) a infraestrutura da instituição está positivamente relacionada ao desempenho do aluno. Os resultados da regressão apontaram que os fatores relacionados ao aluno que apresentaram significância na predição da variabilidade da variável dependente (desempenho) foram: renda familiar, sexo masculino, escolaridade da mãe, escolaridade do pai, Ensino Médio cursado maior parte em escola privada, Ensino Médio metade cursado em escola pública metade em escola privada, Ensino Médio cursado maior parte em escola pública, estado civil casado e quantidade de livros lidos. Adicionalmente, os fatores relacionados à Instituição foram: tipo de organização acadêmica - Universidade, horas de estudos por semana, plano de ensino apresentado pelo docente, a utilização de manuais ou materiais elaborados pelos docentes, contribuição do curso para a formação teórica, exigência de língua estrangeira, atendimento docente extraclasse e utilização de artigo e periódicos. Diante dos resultados obtidos no estudo, foi possível constatar que a relação entre os resultados do ENADE e as respostas do questionário socioeconômico mostra claramente o peso do capital econômico, social e cultural das famílias e dos estudantes na aprendizagem e na sua trajetória acadêmica. Ao se considerar as condições das IES foi possível observar também que as unidades que possuem melhor infraestrutura e corpo docente mais qualificado tendem a proporcionar aos seus alunos melhores condições, consequentemente, melhor desempenho. As contribuições do estudo estão relacionadas a sistematização da literatura nacional e internacional recente a respeito dos fatores relacionados ao desempenho acadêmico, especificamente na educação superior; no que diz respeito à parte metodológica, o estudo contribuiu ao fazer uso científico de uma ampla base de dados produzida periodicamente pelo INEP e MEC acerca dos cursos da educação superior. Quanto às implicações práticas da pesquisa, entende-se como principal contribuição o fornecimento de subsídios para a formulação de políticas públicas e privadas relacionadas à oferta desse nível de educação, ou seja, conhecer o que influencia o desempenho acadêmico pode auxiliar na elaboração de políticas públicas coerentes, dando subsídios para que o SINAES reestruture o sistema de avaliação, inserindo variáveis apontadas pela literatura e estudos empíricos como determinantes do desempenho acadêmico. Facing the scenario of evaluative processes implementation in Higher Education since the 90's, the analysis of this study was based on the possibility of using the data extracted from the Socio-Economic Questionnaire of ENADE, a national student performance exam, which evaluates the students income in Higher Education. The research main objective was to identify the determinants of Management students performance in such a way it could be used as a subsidy to guide the directions of educational policies. For this purpose, it was used the ENADE 2012’s micro data related to Management students of Distrito Federal, available at INEP’s website. The research characteristic was predominantly quantitative, based on the use of descriptive and multivariate statistical techniques. Inicially, it was made a descriptive and exploratory analysis for each variables of the study in order to identify the Management Students profile. Then, the exploratory factor analysis of the research instrument was carried out in order to define the underlying structure of the data matrix. The multivariate multiple regression technique was used to verify the extent to which student's own characteristics, such as personal and socioeconomic aspects, as well as, institutions characteristics and the learning strategies could be significant variables in predicting the ENADE 2012 results. The regression results confirmed the following hypotheses: (H1) family income has a positive influence on student performance; (H2) the variable parents 'schooling is positively related to the students' performance; (H4) faculty qualification is positively related to student performance (H5) the institution's infrastructure is positively related to student performance. The factors related to students with significance power in predicting the variability of the dependent variable (performance) were: family income, male sex, mother's schooling, father's schooling, half enrolled in public school half in private school, high school attended mostly in public school, married civil status and quantity of books read. Addicionally, the factors related to institution were: type of academic organization - University, hours of study per week, teaching plan presented by the teacher, use of manuals or materials prepared by teachers, contribution of the course to theoretical training, foreign language requirement, teaching extraclass and use of articles and journals. Based on the research results, it can be concluded that the associations between the ENADE results and the responses of the socioeconomic questionnaire clearly shows the impact of economic, social and cultural capital of families and students in learning process and in their academic trajectory. Besides, when considering the institutions conditions, it was showned that colleges which have the best infrastructure and the most qualified teaching staff tend to provide better conditions to their students, consequently, they would performance better. The study contributions are related to the systematization of recent national and international literature regarding factors related to academic performance, specifically in higher education; With regard to the methodological part, the study contributed to make scientific use of a broad database periodically produced by INEP and MEC on higher education courses. Regarding the practical implications of the research, it is understood as the main contribution the provision of subsidies for the formulation of public and private policies related to the offer of this level of education, that is, knowing what influences academic performance can help in the elaboration of coherent public policies, giving subsidies for the SINAES to restructure the evaluation system, inserting variables pointed out in the literature and empirical studies as determinants of academic performance.

A alergia ao látex representa um importante problema de saúde em grupos populacionais seleccionados, denominados de risco. A identificação precoce e a instituição de medidas de prevenção e tratamento são essenciais, para assegurar a abordagem correcta do doente alérgico ao látex, produto ubiquitário a nível dos serviços de saúde. Background: Nonsteroidal anti-inflammatory drugs (NSAIDS) are among the most common causes of drug hypersensitivity (HS) reactions. The diagnosis is based on a careful clinical history, and provocation tests are considered the gold standard for diagnosis. Skin tests have some value to study reactions to pyrazolones. Laboratory investigations are mostly used for research purposes. Different phenotypes have been described. Objective and Methods: Our aim was to describe the most common clinical manifestations of NSAID HS in a large population of adult patients, the drugs involved, the association with previously described risk factors, and the outcome of diagnostic procedures. The classification of reactions proposed by the European Academy of Allergy and Clinical Immunology (EAACI) Drug Allergy Interest Group was adopted. Results: Acetylsalicylic acid was the drug most often involved in reactions (34%), isolated cutaneous symptoms were the most reported (60%), and immediate reactions (58%) were the most common. There was an overall female predominance (64%) and 35% of the patients were atopic. HS to NSAIDs was confirmed in 21% of the patients. The most common phenotypes encountered among HS patients were NSAID-induced urticaria/angioedema and single-NSAID-induced urticaria/angioedema or anaphylaxis. Logistic regression analysis showed that gender and atopy were not significant risk factors for HS confirmation, but diagnosis depended on the number of previous reactions, the type of reaction, and the time interval between drug intake and reaction. Conclusion: Only 21% of suspected HS reactions were confirmed after diagnostic workup. Patients describing >1 previous reaction and suffering immediate reactions had a higher probability of a positive investigation. [ABSTRACT FROM AUTHOR]

Trabalho final de mestrado integrado em Medicina área científica de Pneumologia, apresentado á Faculdade de Medicina da Universidade de Coimbra Introdução A asma representa uma das doenças mais prevalente a nível global e o seu padrão de incidência tem aumentado exponencialmente, sobretudo em crianças. Esta realidade repercute-se numa elevada taxa de morbilidade e impacto socioeconómico constituindo um sério problema de saúde pública. Atendendo a esta problemática atual, surge uma emergente necessidade de compreender os mecanismos patofisiológicos e etiológicos desta doença com o intuito de criar estratégias de abordagem e terapêuticas mais eficazes de modo a controlar e reverter o atual padrão de incidência desta patologia. O papel da infeção na génese da asma tem sido alvo de muita discussão e controvérsia. Nesta vertente, esta revisão tentou apurar quais os consensos, de modo a clarificar e perceber que microrganismos e mecanismos que caracterizam o binómio infeção e asma. Objetivos Neste trabalho, pretende-se reunir os mais recentes conhecimentos no âmbito dos cenários microbiológicos da asma, através de uma pesquisa detalhada da Literatura consagrada e indexada, com recurso a bases de dados e artigos de referência. Desenvolvimento A asma constitui uma doença de natureza multifatorial, de expressão clínica heterogénea cujos mecanismos envolvidos na sua génese não se encontram totalmente explicados. Vários estudos têm sido feitos nos últimos anos com o intuito de perceber a interação entre os microrganismos e o sistema imunitário humano. Atualmente reconhece-se que a flora microbiota das vias aéreas é muito mais extensa do que outrora se conhecia e que esta varia consoante o estado de saúde e doença. A infeção por certos microrganismos tem a capacidade de modular a reação da imunidade inata de modo a promover inflamação alérgica das vias aéreas que a longo prazo ocasiona alterações estruturais e inflamatórias compatíveis com asma. Reconhece-se o papel da infeção na exacerbação das crises asmáticas contudo ainda não se apura consenso quanto ao seu relevo na génese da asma. Dentro das discrepâncias existe um uníssono referente às infeções por rinovírus e vírus respiratório sincícial no início da vida como sendo um forte preditor de asma sobretudo quando existe contexto de atopia. Conclusões A evidência atual ainda não é capaz de provar uma relação de casualidade entre a infeção e a génese da asma. Vários mecanismos hipotéticos são sugeridos no entanto ainda é necessário desenvolver estudos que melhor caracterizam a ligação da infeção à asma. Contudo os estudos já feitos abriram caminho para repensar novas abordagens sobre este tema Introduction Asthma is one of the most prevalent diseases globally and its pattern of incidence has increased exponentially, especially in children. This reality reflects a high rate of morbidity and socioeconomic impact which constitutes a serious public health problem. Given this current issue, there is an emerging need to understand the pathophysiological mechanisms and etiology of this disease in order to create strategies of approach and more effective therapeutic measures to control and reverse the current pattern of incidence of this pathology. The role of infection in the genesis of asthma has been the subject of much discussion and controversy and this review tried to determine which consensus exists to clarify and realize the microorganisms and mechanisms that characterize the infection and asthma binomial. Objective This work is intended to bring together the latest knowledge about the microbiological scenarios of asthma, through a detailed survey of the devoted and indexed literature using databases and reference articles. Development Asthma is a multifactorial disease with heterogeneous clinical expression, which mechanisms involved in its genesis are not fully explained. In last decade, several studies have been made in order to understand the interaction between microorganisms and the human immune system. Currently it is recognized that the microbial flora of the airways is much more extensive than was known previously and it is variable depending on the health and disease state. Infection by certain microorganisms has the ability to modulate innate immunity response and promote allergic inflammation of the airways that leads, in long term, to inflammatory and structural changes consistent with asthma. The role of infection in the asthma exacerbation is recognized, but consensus isn´t yet acquired concerning its importance in the asthma genesis. Among the discrepancies there is an accordance related to infections by rhinovirus and respiratory syncytial virus in early life as a strong predictor of asthma especially when there is an atopy context. Conclusion Current evidence is not yet able to prove a relationship of causality between infection and asthma genesis. Several hypothetical mechanisms are suggested however it is still necessary to develop studies to better characterize the binding of infection to asthma. However the studies already made pioneered new approaches to rethink on this topic.

Background: Urticaria is a common disorder, estimated to affect 2.1 to 6.7% of children and adolescents, and is a frequent cause of emergency department (ED) admissions., Methods: The aim of this study was to retrospectively characterize the clinical features of children and adolescents with a diagnosis of urticaria, evaluated in a tertiary care pediatric ED between 2015 and 2019. Statistical analysis was performed using IBM SPSS Statistics
® , version 27.0., Results: A total of 2254 episodes of urticaria were counted with 98.1% corresponding to acute urticaria (AU). A suspected trigger factor was identified in 51.6% of the episodes, namely infections (27.8%), drugs (9.9%) and food (7.6%). From these episodes, excluding infections, only 59.2% were referred to an Allergy Consultation for further study, with only 18.8% (drug) and 28.3% (food) confirmed as the AU trigger. Of the 43 episodes of chronic urticaria (CU), 79% were referred to consultation, with 23 being diagnosed with chronic spontaneous urticaria, 8 with inducible urticaria and 3 with both entities. Older age (p < 0.001), personal history of atopy (p = 0.019) and angioedema (p = 0.003) were factors associated with CU, while the presence of other accompanying symptoms (p = 0.007) was associated with AU. Older age (OR = 1.2; p < 0.001) and the presence of angioedema (OR = 2.7; p = 0.007) were identified as independent factors for CU., Conclusion: The majority of episodes corresponded to AU. Infections were the main suspected trigger, followed by drugs and food, with an overall confirmation rate ranging from 18 to 30%, highlighting the importance of an allergologic follow-up evaluation.- Published

- Abstract
Chronic Granulomatous Disease (CGD) is a primary immunodeficiency disorder characterized by recurrent purulent infections of the skin, lungs, and reticuloendothelial organs, primarily due to staphylococci, enteric bacteria, fungi, and occasionally mycobacteria. More than two thirds of all cases are X-linked and result from defects in the CYBB gene that encodes the gp91-phox subunit of NADPH oxidase. The authors present a case of a three month old child admitted with a metacarpic steomyelitis by Serratia marcescens. Studies confirmed an abnormal respiratory burst in activated neutrophils and absence of gp91-phox expression on patient and a brother (with previous Nocardia infection). Both hemizygous for a pathogenic mutation detected in exon 3 of CYBB gene (c.252 G>A, p.Ala84Ala), a variant that affects the splicing. At two years of age he is still on prophylaxis with cotrimoxazol and itraconazol, without relevant complications. CGD is rare but must be evocated in cases of uncommon or atypical infections.
