Abstract

Oral-facial-digital syndromes (OFDS) gather rare genetic disorders characterised by facial, oral and digital abnormalities associated with a wide range of additional features (polycystic kidney disease, cerebral malformations and several others) to delineate a growing list of OFDS subtypes. The most frequent, OFD type I, is caused by a heterozygous mutation in the OFD1 gene encoding a centrosomal protein. The wide clinical heterogeneity of OFDS suggests the involvement of other ciliary genes. For 15 years, we have aimed to identify the molecular bases of OFDS. This effort has been greatly helped by the recent development of whole-exome sequencing (WES). Here, we present all our published and unpublished results for WES in 24 cases with OFDS. We identified causal variants in five new genes (C2CD3, TMEM107, INTU, KIAA0753 and IFT57) and related the clinical spectrum of four genes in other ciliopathies (C5orf42, TMEM138, TMEM231 and WDPCP) to OFDS. Mutations were also detected in two genes previously implicated in OFDS. Functional studies revealed the involvement of centriole elongation, transition zone and intraflagellar transport defects in OFDS, thus characterising three ciliary protein modules: the complex KIAA0753-FOPNL-OFD1, a regulator of centriole elongation; the Meckel-Gruber syndrome module, a major component of the transition zone; and the CPLANE complex necessary for IFT-A assembly. OFDS now appear to be a distinct subgroup of ciliopathies with wide heterogeneity, which makes the initial classification obsolete. A clinical classification restricted to the three frequent/well-delineated subtypes could be proposed, and for patients who do not fit one of these three main subtypes, a further classification could be based on the genotype.

Abstract

Abstract Background The incidence of childhood type 1 diabetes (T1D) incidence is rising in many countries, supposedly because of changing environmental factors, which are yet largely unknown. The purpose of the study was to unravel environmental markers associated with T1D. Methods Cases were children with T1D from the French Isis-Diab cohort. Controls were schoolmates or friends of the patients. Parents were asked to fill a 845-item questionnaire investigating the child’s environment before diagnosis. The analysis took into account the matching between cases and controls. A second analysis used propensity score methods. Results We found a negative association of several lifestyle variables, gastroenteritis episodes, dental hygiene, hazelnut cocoa spread consumption, wasp and bee stings with T1D, consumption of vegetables from a farm and death of a pet by old age. Conclusions The found statistical association of new environmental markers with T1D calls for replication in other cohorts and investigation of new environmental areas. Oral–facial–digital syndromes (OFDS) gather rare genetic disorders characterised by facial, oral and digital abnormalities associated with a wide range of additional features (polycystic kidney disease, cerebral malformations and several others) to delineate a growing list of OFDS subtypes. The most frequent, OFD type I, is caused by a heterozygous mutation in theOFD1gene encoding a centrosomal protein. The wide clinical heterogeneity of OFDS suggests the involvement of other ciliary genes. For 15 years, we have aimed to identify the molecular bases of OFDS. This effort has been greatly helped by the recent development of whole-exome sequencing (WES). Here, we present all our published and unpublished results for WES in 24 cases with OFDS. We identified causal variants in five new genes (C2CD3,TMEM107,INTU,KIAA0753andIFT57) and related the clinical spectrum of four genes in other ciliopathies (C5orf42,TMEM138,TMEM231andWDPCP) to OFDS. Mutations were also detected in two genes previously implicated in OFDS. Functional studies revealed the involvement of centriole elongation, transition zone and intraflagellar transport defects in OFDS, thus characterising three ciliary protein modules: the complex KIAA0753-FOPNL-OFD1, a regulator of centriole elongation; the Meckel-Gruber syndrome module, a major component of the transition zone; and the CPLANE complex necessary for IFT-A assembly. OFDS now appear to be a distinct subgroup of ciliopathies with wide heterogeneity, which makes the initial classification obsolete. A clinical classification restricted to the three frequent/well-delineated subtypes could be proposed, and for patients who do not fit one of these three main subtypes, a further classification could be based on the genotype.

Abstract

BackgroundThe incidence of childhood type 1 diabetes (T1D) incidence is rising in many countries, supposedly because of changing environmental factors, which are yet largely unknown.PurposeTo unravel environmental markers associated with T1D. Methods: Cases were children with T1D from the French Isis-Diab cohort. Controls were schoolmates or friends of the patients. Parents were asked to fill a 845-item questionnaire investigating the child’s environment before diagnosis. The analysis took into account the matching between cases and controls. A second analysis used propensity score methods.ResultsWe found a negative association of several lifestyle variables, gastroenteritis episodes, dental hygiene, hazelnut cocoa spread consumption, wasp and bee stings with T1D, consumption of vegetables from a farm and death of a pet by old age.ConclusionsThe found statistical association of new environmental markers with T1D calls for replication in other cohorts and investigation of new environmental areas.

Abstract

Item does not contain fulltext, Oral-facial-digital syndromes (OFDS) gather rare genetic disorders characterised by facial, oral and digital abnormalities associated with a wide range of additional features (polycystic kidney disease, cerebral malformations and several others) to delineate a growing list of OFDS subtypes. The most frequent, OFD type I, is caused by a heterozygous mutation in the OFD1 gene encoding a centrosomal protein. The wide clinical heterogeneity of OFDS suggests the involvement of other ciliary genes. For 15 years, we have aimed to identify the molecular bases of OFDS. This effort has been greatly helped by the recent development of whole-exome sequencing (WES). Here, we present all our published and unpublished results for WES in 24 cases with OFDS. We identified causal variants in five new genes (C2CD3, TMEM107, INTU, KIAA0753 and IFT57) and related the clinical spectrum of four genes in other ciliopathies (C5orf42, TMEM138, TMEM231 and WDPCP) to OFDS. Mutations were also detected in two genes previously implicated in OFDS. Functional studies revealed the involvement of centriole elongation, transition zone and intraflagellar transport defects in OFDS, thus characterising three ciliary protein modules: the complex KIAA0753-FOPNL-OFD1, a regulator of centriole elongation; the Meckel-Gruber syndrome module, a major component of the transition zone; and the CPLANE complex necessary for IFT-A assembly. OFDS now appear to be a distinct subgroup of ciliopathies with wide heterogeneity, which makes the initial classification obsolete. A clinical classification restricted to the three frequent/well-delineated subtypes could be proposed, and for patients who do not fit one of these three main subtypes, a further classification could be based on the genotype.

Resume L’objectif de l’etude est de definir les valeurs d’IgE specifiques pour confirmer le diagnostic d’allergie a l’œuf de poule avec une probabilite proche de 100 %. L’etude est conduite chez 185 enfants suspects d’allergie alimentaire vus consecutivement a l’hopital des enfants de Toulouse. Un premier groupe est compose de 100 enfants dont l’allergie a l’œuf est prouvee par un test de provocation par voie orale positif. Ces enfants sont compares a un deuxieme groupe de 85 enfants non allergiques a l’œuf mais allergiques a un autre aliment, le plus souvent l’arachide. Tous les enfants ont beneficie d’une exploration similaire : analyse clinique, prick test natif pour l’œuf de poule (blanc et jaune), dosage des IgE seriques specifiques pour le blanc et le jaune d’œuf avec la technique Pharmacia Cap System, et test de provocation par voie orale. L’âge moyen des enfants est de 2,1 ans avec des extremes de 8 mois a 15 ans. Le diametre moyen du prick test blanc d’œuf est de 11,3 mm (extremes 0 a 25 mm) pour le groupe 1 et de 5,5 mm pour le groupe 2, p = 0,00001. La mediane des IgE specifiques du blanc d’œuf est plus elevee chez les allergiques a l’œuf (22,5 kUA/L vs 0,76, p = 0,000001), comme la mediane des IgE jaune d’œuf (6 kUA/L vs 0,35, p = 0,0001). Une concentration d’IgE blanc d’œuf superieure ou egale a 7,5 kUA/L est associee a une valeur predictive positive de 100 % (valeur predictive negative 68 %). Une concentration d’IgE jaune d’œuf superieure ou egale a 5,5 kUA/L est associee a une valeur predictive positive de 100 % (valeur predictive negative 52 %). Conclusion. – Notre etude objective une meilleure sensibilite des IgE blanc d’œuf et une meilleure specificite des IgE jaune d’œuf. Les deux mesures sont utiles pour le diagnostic de l’allergie a l’œuf de poule chez l’enfant. Des IgE blanc d’œuf superieures ou egales a 7,5 kUA/L et des IgE jaune d’œuf superieures ou egales a 5,5 kUA/L permettent de porter le diagnostic d’allergie a l’œuf avec une probabilite proche de 100 %.

12. Quantification of egg white- and yolk-specific IgE antibodies in children with egg allergy

Abstract

The aim of this study was to define levels of egg white- and yolk-specific IgE antibodies that would confirm the diagnosis of egg allergy with a positive predictive value (PPV) of 100%. The study was carried out on 185 children with suspected food allergy recruited consecutively at the Children’s Hospital. One group included 100 children with a positive hen’s egg oral food challenge (OFC) test. They were compared with 85 children with peanut allergy but not egg allergy (negative egg OFC). We analyzed the children’s clinical symptoms, family history of atopic diseases, skin prick test results (SPT) with raw egg white and yolk, serum samples for specific IgE antibodies to egg white and yolk using the Pharmacia Cap system FEIA method, open controlled challenge tests with whole raw egg or double-blind controlled food challenges with peanut. Median age was 2.1 years (range 0.8–15 years). The median skin reaction to the egg extract was 11.3 mm (range 0–25 mm) in the first group and 5.5 mm in the second (P = 0.00001). We found a higher level of egg white-specific IgE antibodies in the egg allergy group (22.5 vs. 0.76 kUA/l, P = 0.000001) and also a higher level of yolk-specific IgE antibodies (6 vs. 0.35 kUA/l, P = 0.0001). For the egg white antibodies, a threshold value equal to or greater than 7.5 kUA/l resulted in a PPV of 100% and a negative predictive value (NPV) of 68%. For the yolk-specific antibodies, a threshold value equal to or greater than 5.5 kUA/l resulted in a PPV of 100% and a NPV of 52%.Conclusion. – Our study demonstrated better sensitivity for egg white-specific IgE antibodies and better specificity for egg yolk-specific IgE antibodies. Both measurements are useful for the diagnosis of egg allergy in children. In addition, with an egg white-specific IgE antibody level equal to or greater than 7.5 kUA/l and a yolk-specific IgE antibody level equal to or greater than 5.5 kUA/l, the patient can be said to be egg-allergic with nearly 100% certainty.
