The second Gravitational-Wave Transient Catalog reported on 39 compact binary coalescences observed by the Advanced LIGO and Advanced Virgo detectors between 1 April 2019 15:00 UTC and 1 October 2019 15:00 UTC. We present GWTC-2.1, which reports on a deeper list of candidate events observed over the same period. We analyze the final version of the strain data over this period with improved calibration and better subtraction of excess noise, which has been publicly released. We employ three matched-filter search pipelines for candidate identification, and estimate the astrophysical probability for each candidate event. While GWTC-2 used a false alarm rate threshold of 2 per year, we include in GWTC-2.1, 1201 candidates that pass a false alarm rate threshold of 2 per day. We calculate the source properties of a subset of 44 high-significance candidates that have an astrophysical probability greater than 0.5. Of these candidates, 36 have been reported in GWTC-2. If the 8 additional high-significance candidates presented here are astrophysical, the mass range of events that are unambiguously identified as binary black holes (both objects $\geq 3M_\odot$) is increased compared to GWTC-2, with total masses from $\sim 14 M_\odot$ for GW190924_021846 to $\sim 182 M_\odot$ for GW190426_190642. The primary components of two new candidate events (GW190403_051519 and GW190426_190642) fall in the mass gap predicted by pair instability supernova theory. We also expand the population of binaries with significantly asymmetric mass ratios reported in GWTC-2 by an additional two events (the mass ratio is less than $0.65$ and $0.44$ at $90\%$ probability for GW190403_051519 and GW190917_114630 respectively), and find that 2 of the 8 new events have effective inspiral spins $\chi_\mathrm{eff} > 0$ (at $90\%$ credibility), while no binary is consistent with $\chi_\mathrm{eff} < 0$ at the same significance., Comment: 8 figures, 8 tables, including updates to parameter estimates of events from GWTC-1 and GWTC-2 in an Appendix

Background Since the emergence of SARS-CoV-2, evolutionary pressure has driven large increases in the transmissibility of the virus. However, with increasing levels of immunity through vaccination and natural infection the evolutionary pressure will switch towards immune escape. Genomic surveillance in regions of high immunity is crucial in detecting emerging variants that can more successfully navigate the immune landscape. Methods We present phylogenetic relationships and lineage dynamics within England (a country with high levels of immunity), as inferred from a random community sample of individuals who provided a self-administered throat and nose swab for rt-PCR testing as part of the REal-time Assessment of Community Transmission-1 (REACT-1) study. During round 14 (9 September–27 September 2021) and 15 (19 October–5 November 2021) lineages were determined for 1322 positive individuals, with 27.1% of those which reported their symptom status reporting no symptoms in the previous month. Results We identified 44 unique lineages, all of which were Delta or Delta sub-lineages, and found a reduction in their mutation rate over the study period. The proportion of the Delta sub-lineage AY.4.2 was increasing, with a reproduction number 15% (95% CI 8–23%) greater than the most prevalent lineage, AY.4. Further, AY.4.2 was less associated with the most predictive COVID-19 symptoms (p = 0.029) and had a reduced mutation rate (p = 0.050). Both AY.4.2 and AY.4 were found to be geographically clustered in September but this was no longer the case by late October/early November, with only the lineage AY.6 exhibiting clustering towards the South of England. Conclusions As SARS-CoV-2 moves towards endemicity and new variants emerge, genomic data obtained from random community samples can augment routine surveillance data without the potential biases introduced due to higher sampling rates of symptomatic individuals.

The spike protein of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is critical for virus infection through the engagement of the human ACE2 protein1 and is a major antibody target. Here we show that chronic infection with SARS-CoV-2 leads to viral evolution and reduced sensitivity to neutralizing antibodies in an immunosuppressed individual treated with convalescent plasma, by generating whole-genome ultra-deep sequences for 23 time points that span 101 days and using in vitro techniques to characterize the mutations revealed by sequencing. There was little change in the overall structure of the viral population after two courses of remdesivir during the first 57 days. However, after convalescent plasma therapy, we observed large, dynamic shifts in the viral population, with the emergence of a dominant viral strain that contained a substitution (D796H) in the S2 subunit and a deletion (ΔH69/ΔV70) in the S1 N-terminal domain of the spike protein. As passively transferred serum antibodies diminished, viruses with the escape genotype were reduced in frequency, before returning during a final, unsuccessful course of convalescent plasma treatment. In vitro, the spike double mutant bearing both ΔH69/ΔV70 and D796H conferred modestly decreased sensitivity to convalescent plasma, while maintaining infectivity levels that were similar to the wild-type virus.The spike substitution mutant D796H appeared to be the main contributor to the decreased susceptibility to neutralizing antibodies, but this mutation resulted in an infectivity defect. The spike deletion mutant ΔH69/ΔV70 had a twofold higher level of infectivity than wild-type SARS-CoV-2, possibly compensating for the reduced infectivity of the D796H mutation. These data reveal strong selection on SARS-CoV-2 during convalescent plasma therapy, which is associated with the emergence of viral variants that show evidence of reduced susceptibility to neutralizing antibodies in immunosuppressed individuals.

We compared 19,207 cases of SARS-CoV-2 variant B.1.1.7/S gene target failure (SGTF), 436 B.1.351 and 352 P.1 to non-variant cases reported by seven European countries. COVID-19 cases with these variants had significantly higher adjusted odds ratios for hospitalisation (B.1.1.7/SGTF: 1.7, 95% confidence interval (CI): 1.0-2.9; B.1.351: 3.6, 95% CI: 2.1-6.2; P.1: 2.6, 95% CI: 1.4-4.8) and B.1.1.7/SGTF and P.1 cases also for intensive care admission (B.1.1.7/SGTF: 2.3, 95% CI: 1.4-3.5; P.1: 2.2, 95% CI: 1.7-2.8). Abstract Since 1994, Irish cattle have been exposed to greater risks of acquiring Mycobacterium avium subspecies paratuberculosis (MAP) infection as a consequence of the importation of over 70,000 animals from continental Europe. In recent years, there has been an increase in the number of reported clinical cases of paratuberculosis in Ireland. This study examines the prevalence of factors that promote the introduction and within-herd transmission of Mycobacterium avium subspecies paratuberculosis (MAP) on selected Irish dairy farms in the Cork region, and the association between these factors and the results of MAP screening tests on milk sock filter residue (MFR). A total of 59 dairy farms, selected using non-random methods but apparently free of endemic paratuberculosis, were enrolled into the study. A questionnaire was used to collect data about risk factors for MAP introduction and transmission. The MFR was assessed on six occasions over 24 months for the presence of MAP, using culture and immunomagnetic separation prior to polymerase chain reaction (IMS-PCR). Furthermore, blood samples from all entire male and female animals over one year of age in 20 herds were tested by ELISA. Eighteen (31%) farms had operated as closed herds since 1994, 28 (47%) had purchased from multiple sources and 14 (24%) had either direct or indirect (progeny) contact with imported animals. Milk and colostrum were mixed on 51% of farms, while 88% of farms fed pooled milk. Thirty (51%) herds tested negative to MFR culture and IMS-PCR, 12 (20%) were MFR culture positive, 26 (44%) were IMS-PCR positive and seven (12%) were both culture and IMS-PCR positive. The probability of a positive MFR culture was significantly associated with reduced attendance at calving, and with increased use of individual calf pens and increased (but not significantly) if mulitiple suckling was practised. There was poor agreement between MFR culture and MFR IMS-PCR results, but moderate agreement between MFR culture and ELISA test results. This study highlights a lack of awareness among Irish dairy farmers about the effect of inadequate biosecurity on MAP introduction. Furthermore, within-herd transmission will be facilitated by traditional calf rearing and waste management practices. The findings of viable MAP in the presence of known transmission factors in non-clinically affected herds could be a prelude to long-term problems for the Irish cattle and agri-business generally.

- Abstract
The evolution of the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) virus leads to new variants that warrant timely epidemiological characterization. Here we use the dense genomic surveillance data generated by the COVID-19 Genomics UK Consortium to reconstruct the dynamics of 71 different lineages in each of 315 English local authorities between September 2020 and June 2021. This analysis reveals a series of subepidemics that peaked in early autumn 2020, followed by a jump in transmissibility of the B.1.1.7/Alpha lineage. The Alpha variant grew when other lineages declined during the second national lockdown and regionally tiered restrictions between November and December 2020. A third more stringent national lockdown suppressed the Alpha variant and eliminated nearly all other lineages in early 2021. Yet a series of variants (most of which contained the spike E484K mutation) defied these trends and persisted at moderately increasing proportions. However, by accounting for sustained introductions, we found that the transmissibility of these variants is unlikely to have exceeded the transmissibility of the Alpha variant. Finally, B.1.617.2/Delta was repeatedly introduced in England and grew rapidly in early summer 2021, constituting approximately 98% of sampled SARS-CoV-2 genomes on 26 June 2021.

Transmission of SARS-CoV-2 is uncontrolled in many parts of the world; control is compounded in some areas by the higher transmission potential of the B.1.1.7 variant1, which has now been reported in 94 countries. It is unclear whether the response of the virus to vaccines against SARS-CoV-2 on the basis of the prototypic strain will be affected by the mutations found in B.1.1.7. Here we assess the immune responses of individuals after vaccination with the mRNA-based vaccine BNT162b22. We measured neutralizing antibody responses after the first and second immunizations using pseudoviruses that expressed the wild-type spike protein or a mutated spike protein that contained the eight amino acid changes found in the B.1.1.7 variant. The sera from individuals who received the vaccine exhibited a broad range of neutralizing titres against the wild-type pseudoviruses that were modestly reduced against the B.1.1.7 variant. This reduction was also evident in sera from some patients who had recovered from COVID-19. Decreased neutralization of the B.1.1.7 variant was also observed for monoclonal antibodies that target the N-terminal domain (9 out of 10) and the receptor-binding motif (5 out of 31), but not for monoclonal antibodies that recognize the receptor-binding domain that bind outside the receptor-binding motif. Introduction of the mutation that encodes the E484K substitution in the B.1.1.7 background to reflect a newly emerged variant of concern (VOC 202102/02) led to a more-substantial loss of neutralizing activity by vaccine-elicited antibodies and monoclonal antibodies (19 out of 31) compared with the loss of neutralizing activity conferred by the mutations in B.1.1.7 alone. The emergence of the E484K substitution in a B.1.1.7 background represents a threat to the efficacy of the BNT162b2 vaccine.

44. Treatment-refractory checkpoint inhibitor-induced colitis requiring colectomy: A case series review.

- Abstract
Introduction: Immune checkpoint inhibitor (ICI)-induced colitis is the most common adverse effect of ICI therapy. A proportion of patients develop severe colitis requiring biological therapy. There is a paucity of data in the literature about how to treat ICI-induced colitis when biological therapy fails. We present three similar cases of severe ICI-induced colitis that was treated with total colectomy after corticosteroids and biological therapy failed. Case reports: Three men (aged 47, 63, and 47 years) who had metastatic melanoma developed colitis with a history of inflammatory bowel disease. Patient 1 was treated with ipilimumab and nivolumab, Patient 2 with pembrolizumab, and Patient 3 with nivolumab (all standard dosing regimens). Patients 2 and 3 went into remission with their ICI therapy before colitis developed. Patient 1 had disease progression. They were treated for 1, 9, and 6 months, respectively, before developing ICI-induced colitis. All three patients presented with Common Terminology Criteria for Adverse Events (CTCAE) 3 symptoms, with computed tomography evidence of colitis and markedly raised levels of inflammatory markers. They had negative stool cultures. Flexible sigmoidoscopy showed features consistent with Mayo 2-3 colitis in all three patients. Histology showed classical features of ICI-induced colitis, including neutrophilic crypt micro-abscesses, cryptitis, and increased crypt epithelial cell apoptosis. High-dose intravenous corticosteroids (>=80 mg methylprednisolone daily) failed in all three patients, and they received rescue infliximab as salvage therapy on initial presentation. They partially responded to this, with decrease in stool frequency, absence of bloody stools, and improvement of albumin and C-reactive protein levels. They all underwent dose frequency and dose-escalated infliximab regimens (compared with standard 5 mg/kg at 0, 2, and 6 weeks), as well as cessation of ICI therapy. Background and Aim: There are very few studies addressing the safety and diagnostic efficacy of pediatric endoscopy performed by adult-trained endoscopists. This has resulted in debate within the endoscopy community as to whether pediatric endoscopy should be performed solely by pediatric-trained endoscopists. Limiting pediatric endoscopy to centralized specialist pediatric endoscopy units across Australia would likely result in many children traveling long distances or facing significant delays to access endoscopic procedures. Our aims were to assess whether pediatric endoscopy is safely performed by adult-trained endoscopists and to evaluate the detection of disease in the pediatric population when performed by adult-trained endoscopists. Method(s): Ethics approval was obtained from the University of Tasmania ethics committee. All pediatric endoscopic procedures performed by adult-trained endoscopists in Tasmanian public hospitals from July 2010 to June 2020 were reviewed. Sites included the Launceston General Hospital (LGH), Royal Hobart Hospital (RHH), North West Regional Hospital (NWRH), and Mersey Community Hospital (MCH). Data collection was facilitated by retrospective review of hospital medical records, and pathology and endoscopy databases. A pediatric patient was defined as a child aged 13 years or younger, in line with the recognized Conjoint Committee for the Recognition of Training in Gastrointestinal Endoscopy definition. Result(s): A total of 700 procedures were performed, with 26 excluded due to insufficient medical records. RHH did 388 procedures (58%), LGH did 269 (40%), MCH did 12 (2%), and NWRH did five (<1%). The median age was 8 years (IQR, 6; range, 0-13), 350 patients (52%) were male, and 322 (48%) were female. Common indications included suspected celiac disease (20%), foreign body ingestion/food bolus (15%), abdominal pain (13%), suspected inflammatory bowel disease (IBD) (11%), and rectal bleeding (5%).
